1.Clinical efficacy of the traditional Mongolian medicine Zadi-5 pills combined with escitalopram oxalate in the treatment of first-episode depression

BackgroundThe global prevalence of depressive disorders is high, with a significant treatment gap remaining. Escitalopram oxalate, as a commonly used antidepressant in clinical practice, has a single target and is prone to cause adverse reactions such as gastrointestinal discomfort. The traditional Mongolian medicine Zadi-5 pills has a multi-target anti-depression mechanism and unique advantages such as improving the intestinal flora. However, there is currently a lack of randomized controlled trial evidence for the combined treatment of first-episode depression, along with insufficient research on its metabolic safety and potential drug interactions. ObjectiveTo explore the efficacy and safety of Zadi-5 pills combined with escitalopram oxalate in the treatment of the first-episode depression, so as to provide references for Mongolian medicine in depression management. MethodsA randomized controlled trial was conducted. A total of 103 patients with first-episode depression who were treated in the outpatient or inpatient departments of the Inner Mongolia Autonomous Region Mental Health Center from January 2023 to December 2024 and met the diagnostic criteria for depressive episode as defined in International Classification of Diseases, tenth edition (ICD-10) were included. Random numbers were generated using the RAND function in Excel software, and the patients were divided into the study group (n=52) and the control group (n=51). Both groups received escitalopram oxalate treatment, starting from 5 mg/d and gradually increasing to 20 mg/d. The study group additionally received Zadi-5 pills, 13 pills per day. The treatment duration for both groups was 6 weeks. Before treatment and at the 2nd, 4th, and 6th weeks of treatment, the severity of depressive symptoms was evaluated using the Hamilton Depression Scale-24 item (HAMD-24). At these time points, blood was collected on an empty stomach at 7∶00 for the detection of liver function (aspartate aminotransferase, alanine aminotransferase), triglycerides, and the blood concentration of escitalopram oxalate. ResultsA total of 100 patients completed the study, with 50 cases in the study group and the control group. The results of the repeated measures ANOVA showed that the interaction effect between group and time in the HAMD-24 score was statistically significant (F interaction= 18.070, P<0.01). At the end of the 4th and 6th weeks of treatment, the HAMD-24 score of the study group was lower than those of the control group (P<0.05). At each time point after treatment, the reduction rate of the HAMD-24 score in the study group was higher than those of the control group, and the differences were all statistically significant (P<0.05). The interaction effect between the triglyceride level groups and time was statistically significant (Finteraction=3.381, P<0.05).After treatment, the triglyceride levels in the study group showed a downward trend, while those in the control group showed an upward trend. There was no statistically significant interaction effect between the groups and time for liver function (aspartate aminotransferase and alanine aminotransferase) and the blood concentration of escitalopram oxalate (P>0.05). ConclusionCompared with escitalopram oxalate treatment alone, the combination of Zadi-5 pills and escitalopram oxalate is more effective in treating the first-episode depression, without increasing the risk of liver damage. 2.A retrospective study of the effects of different surgical procedures on the mandibular nerve canal involved by odontogenic keratocyst.

OBJECTIVES:
This study aims to evaluate the changes in the mandibular canal following the treatment of large odontogenic keratocysts through decompression and curettage, providing a theoretical basis for sequential treatment.
METHODS:
Twenty patients were selected for each decompression and curettage treatment of large odontogenic keratocysts in the mandible. Postoperative follow-up with was conducted every three months, during which cone beam computed tomography (CBCT) scans were performed. Then, the data were imported into MIMICS software to observe 3D changes in the position and structure of the mandibular nerve canal, followed by a comparative analysis.
RESULTS:
The total displacement of the mandibular canal was (1.89±0.21) mm on the decompression side and (0.80±0.19) mm on the curettage side. Vertically, the displacement range of the mandibular canal on the decompression side (M=1.03, SD=0.17) was larger than on the curettage side (M=0.52, SD=0.010) within nine months post-operation. In the buccal-lingual direction, the ratio of the thickness of the buccal plate to the lingual plate gradually increased with time. The amount of bone reconstruction at the part of the mandibular nerve canal closest to the cyst was (1.75±0.15) mm on the decompression side and (1.45±0.09) mm on the curettage side after nine months.
CONCLUSIONS
The mandibular nerve canal showed varying degrees of recovery and "relocation" after two surgical procedures. Osteogenesis around the mandibular nerve canal was more remarkable after decompression than after curettage. Therefore, for large odontogenic keratocyst, decompression is recommended as the initial treatment, followed by secondary curettage nine months later.

3.Research and clinical application progress of foot lengthening surgery.

OBJECTIVE:
To summarize the research and clinical application progress of foot lengthening surgery.
METHODS:
Relevant research literature on foot lengthening surgery in recent years at home and abroad was reviewed, and a summary was made from aspects such as the types of lengthening surgery, the types of foot diseases treated by clinical application, effectiveness, and complications.
RESULTS:
Bone defects and shortening deformities of the foot are relatively common clinically. As an innovative treatment method, foot lengthening surgery has gradually attracted attention, mainly including the Ilizarov technique and one-stage bone grafting lengthening surgery. The former promotes bone regeneration based on the tension-stress principle and is widely used in the treatment of calcaneal defects and congenital metatarsal brachymetatarsia, achieving good curative effects. However, there are also complications such as pin-tract infection, joint stiffness and contracture, non-union and delayed union of bone, re-fracture, and alignment deviation. The latter has a short treatment cycle, but the lengthening length is limited. Bone graft resorption and soft tissue complications are its main complications.
CONCLUSION
Foot lengthening surgery will develop towards the direction of personalization, intelligence, and precision. With the help of multi-center research, biological materials, and intelligent technologies, the effectiveness and safety will be further improved to better restore the function and appearance of the foot.

4.Recent advance in synergistic pathogenesis of complement and antibody in Guillain-Barré syndrome and its immunotherapy

Guillain-Barré syndrome (GBS) is an autoimmune-mediated acute polyradiculoneuropathy. Its key feature is focal segmental demyelination along with inflammatory cell infiltration and axonal damage. In clinical practice, anti-ganglioside antibodies in the body fluid of GBS patients show heterogeneity, and conventional immunotherapy is ineffective in some GBS subtypes. Drawing on treatment options for other neuroimmune diseases, targeted regulation of antibody-complement system has now become an important direction in GBS treatment research. This paper examines the immune response mechanisms involving complement, antibodies and related inflammatory factors, explores new strategy in GBS immunotherapy, so as to provide theoretical basis and research entry points for GBS individualized treatment.
5.Natural course of renal angiomyolipoma and risk factors for its progression

Objective:To clarify the natural course of renal angiomyolipoma and the risk factors for its progression.Methods:This was a retrospective case-control study that included 401 patients diagnosed several times by ultrasound examination in the hospital physical examination system from January 2012 to June 2024. All patients were untreated. There were 128 male cases (31.90%) and 273 female cases (68.10%). The average age at initial diagnosis was (44.04 ± 10.24) years (range 22-78 years). The median longest diameter of the tumor at initial diagnosis was 9.0 (7.0, 11.5) mm. There were 359 cases (89.50%) with single tumors and 42 cases (10.50%) with multiple tumors. The patients were divided into the progression group(≥1 mm/year) and the non-progression group (<1 mm/year)based on the average growth rate of tumor. The differences in gender, age at initial diagnosis, initial tumor size, number of lesions and lesion site between the two groups were compared. Univariate logistic regression analysis was used to explore the relationship between the above factors and the progression of renal angiomyolipoma. Multivariate logistic regression analysis was conducted to identify the risk factors for progression.Results:A total of 401 cases were followed up for an average of (88.15 ± 21.09) months (range 48-140 months). The median maximum diameter of the tumors at the initial diagnosis was 9.0 (7.0, 11.5) mm, and at the end of the follow-up, it was 11 (8, 14) mm. The average growth rate was 0.38 mm/year, and the median growth rate was 0.25 (0, 0.60) mm/year. Among them, 341 cases (85.04%) were in the non-progression group with an average growth rate of 0.14 mm/year, and 60 cases (14.96%) were in the progression group with an average growth rate of 1.74 mm/year. The age of the progression group was lower than that of the non-progression group [(41.43 ± 9.64) years vs. (44.50±10.29) years], the initial maximum diameter of the tumors in the progression group was larger than that in the non-progression group [11.0 (8.0, 16.0) mm vs. 9.0 (7.0, 11.0) mm], and the proportion of multiple tumors in the progression group was higher than that in the non-progression group [14 cases (23.30%) vs. 28 cases (8.20%)], and the differences were all statistically significant ( P<0.05). Age at initial diagnosis( OR=0.96, 95% CI 0.93-0.99), initial tumor size ( OR=1.08, 95% CI 1.04-1.12) and number of lesions ( OR=2.96, 95% CI 1.38-6.34) were the risk factors for the growth of renal angiomyolipoma ( P<0.05), according to the results of multivariate logistic regression analysis. Conclusions:The natural history of most renal angiomyolipoma shows slow growth or relative quiescence, with a small number showing a significant increasing trend. Age at initial diagnosis, initial tumor size and number of lesions were independent risk factors for the growth of renal angiomyolipoma.

Objective:To compare the efficacy of body posture combined with pharmacotherapy and pharmacotherapy alone in the treatment of chronic subdural hematoma(CSDH).Methods:Firstly, retrospective case series study was conducted. Thirty cases of CSDH that had received body posture combined with pharmacotherapy at Department of Neurosurgery, Huashan Hospital Affiliated to Fudan University from December 2016 to October 2020 were studied retrospectively. Twenty-seven patients were male, and 3 patients were female. The age of patients ( M(IQR)) was 66(16) years (range:28 to 84). Nineteen patients had unilateral hematoma, and 11 patients had bilateral hematoma. All patients received pharmacotherapy and body posture therapy that was to raise their lower limbs 20 to 30 cm with leg lift pad and get abdominal compressed with customized abdominal belt in supine position. Patients were required to maintain the body posture as much as possible, with the maximum to 16 to 18 hours per day. Patients with unilateral hematoma should tilt the head to the affected side and avoid tilting it to the opposite side. For patients with bilateral hematoma, there was no need for head lateralization. Patient were treated with oral dexamethasone and atorvastatin simultaneously. The preliminary efficacy of body posture combined with pharmacotherapy was determined by hematoma improvement rate which was analyzed by Clopper-Pearson method. Then, the multi-center, prospective, randomized controlled trial had carried out in 9 medical centers from August 2020 to November 2021. The stratified block randomization method was adopted. Patients were randomized in a ratio of 1∶1 to either receive pharmacotherapy alone(the control group) or body posture combined with pharmacotherapy(the experiment group) for 3 months and followed up for 6 months. Effective treatment was defined as complete absorption of hematoma, or the hematoma volume decreased by more than 10 ml and Markwalder grading scale score had improved by more than 1 point compared to the baseline. The efficacy rate and surgery conversion rate at 3 months and recurrence at 6 months were observed. Comparison between groups was performed with paired sample t test, Mann-Whitney U test, χ2 test, corrected χ2 test, or Fisher exact probability method. Logistic regression was used to compare the effective rate and operation rate between the two groups. Results:In the respective study, 30 patients completed follow-up 13 to 353 days after treatment. At the last follow-up, the incidence of almost complete absorption or significantly absorption of hematoma (hematoma volume was significantly reduced accompanied by symptom improvement) was 93.3%. The 95% CI for the incidence that analyzed by the Clopper-Pearson method was 77.9% to 99.2%. One hundred and six patients were enrolled in the multicenter study. Fifty-five patients underwent body posture combined with pharmacotherapy. The age was 74(17) years (range:26 to 92). Thirty-nine patients were males and 16 were females. Fifty-one patients underwent pharmacotherapy alone. The age was 69(12) years (range:48 to 84). Thirty-seven patients were males and 14 were females. The length of body posture recorded in diary card was (15.7±2.3) hours(range:7.6 to 19.3 hours). The efficacy rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 83.6% (46/55) and 56.9% (29/51), respectively at 3 months. The result of the logistic regression analysis showed that the efficacy of body posture combined with pharmacotherapy group was better than that of pharmacotherapy alone group ( OR=3.88,95% CI:1.57 to 9.58, P=0.003). Surgery rate in the body posture combined with pharmacotherapy group and pharmacotherapy alone group were 5.5% (3/55) and 21.6% (11/51) respectively. The result of Logistic regression showed that the pharmacotherapy alone group was more likely to be converted to surgery ( OR=0.21,95% CI:0.05 to 0.80, P=0.023). At the 6 months, no recurrence of cases was found in the body posture combined with pharmacotherapy group. However, the recurrence rate of pharmacotherapy alone group was 6.3% (3/48), there was no significant difference between the two groups ( P>0.05). Conclusion:The effect of body posture combined with pharmacotherapy for chronic subdural hematoma is better than that of pharmacotherapy alone.
7.Effect of cannabinoid receptor 1 knockout on autism spectrum disorder-like behavior and synaptic plasticity in mice

Objective To investigate the regulation of synaptic plasticity by cannabinoid receptor 1(CB1R)and its effects on autism spectrum disorder(ASD)-like behavior.Methods CB1R-knockout(KO)mice and valproic acid(VPA)-induced ASD model mice(VPA mice)were used as study subjects.Behavioral experiments were used to assess the effects of CB1R on ASD-like behavior in mice,neuronal structural integrity and dendritic density were detected by microtubule-associated protein 2(MAP2)staining experiments,and the expression of synapse-associated proteins was detected by Western blot,to assess the effects of CB1R on synaptic plasticity.Results Behavioral result showed that VPA mice demonstrated significant ASD-like behavior,while CB1R-/-mice spent a significantly smaller proportion of residence time in the central region of the open field(P<0.0001),showed significant increases in the number of marbles buried and self-grooming time(P<0.01),significantly less time spent socializing with unfamiliar mice 2 and exploring unfamiliar objects(P<0.001),and significantly more time exploring old objects(P<0.05).The relative dwelling time was significantly reduced in CB1R+/-mice(P<0.001),and the number of marbles buried and self-grooming time were significantly increased(P<0.05).Synaptic plasticity assays revealed significant synaptic plasticity impairment in VPA mice.Hippocampal MAP2-positive neuron densities were significantly reduced in CB1R-/-and CB1R+/-mice,and expression levels of synapsin-1 were significantly increased(P<0.05).Conclusions CB1R KO leads to ASD-like behavior such as anxiety and repetitive stereotyped behavior,social and cognitive impairments,as well as neuronal damage,dendritic dysplasia and disrupted synaptic protein expression in mice,suggesting that CB1R is involved in regulating synaptic plasticity as a pathological mechanism for the development of ASD-like behavior.
8.Clinical characteristics of 123 patients with Brucellar spondylitis in high-incidence areas

OBJECTIVE To analyze the clinical characteristics of Brucellar spondylitis(BS)and provide references for clinical diagnosis and treatment.METHODS A retrospective analysis was conducted on the data of 123 patients with BS admitted to the Second Affiliated Hospital of Inner Mongolia Medical University from Jan.1,2015 to Jun.30,2024.The data included demographic characteristics,epidemiological history,clinical manifestations,la-boratory indicators,imaging features and treatment regimens.The clinical characteristics of BS were summarized.RESULTS Among the 123 patients with BS,there were 96 males and 27 females,with an average age of(59.19±11.71)years.The patients were predominantly farmers(74.80%),and 65.04%reported a history of livestock contact.All patients presented with local vertebral pain,mainly involving the lumbar region(56.10%),accompanied by systemic symptoms such as fever(34.96%)and fatigue(95.12%).Laboratory tests showed an abnormal plateletocrit rate of 93.50%,an abnormal C-reactive protein rate of 87.80%and an abnormal erythro-cyte sedimentation rate of 82.11%.Etiological examinations revealed a positive bacterial culture rate of 73.17%,with positive rates of 92.06%and 95.24%for the Rose Bengal plate agglutination test and tube agglutination test,respectively,and a total positive rate of 95.45%for enzyme-linked immunosorbent assay.Imaging examina-tions found a total of 308 involved vertebrae,with an average of(2.50±1.06)vertebrae involved per patient.Lumbar vertebra involvement was the highest(82.11%)and 42.28%of patients had paravertebral abscesses.In terms of treatment,45.53%of patients received comprehensive treatment with medication and surgery,and 91.06%achieved good outcomes.CONCLUSIONS Patients in high-incidence areas of brucellosis may not have a clear history of livestock contact,and atypical cases should be kept vigilant.Blood routine tests,CRP and ESR of-ten show abnormal manifestations in patients with BS.Lymphocyte count,platelet-to-lymphocyte ratio and mono-cyte-to-lymphocyte ratio have certain reference values in assessing the extent of spinal involvement in patients.
