1.Processing technology of calcined Magnetitum based on concept of QbD and its XRD characteristic spectra.

Guided by the concept of quality by design(QbD), this study optimizes the calcination and quenching process of calcined Magnetitum and establishes the XRD characteristic spectra of calcined Magnetitum, providing a scientific basis for the formulation of quality standards. Based on the processing methods and quality requirements of Magnetitum in the Chinese Pharmacopoeia, the critical process parameters(CPPs) identified were calcination temperature, calcination time, particle size, laying thickness, and the number of vinegar quenching cycles. The critical quality attributes(CQAs) included Fe mass fraction, Fe~(2+) dissolution, and surface color. The weight coefficients were determined by combining Analytic Hierarchy Process(AHP) and the criteria importance though intercrieria correlation(CRITIC) method, and the calcination process was optimized using orthogonal experimentation. Surface color was selected as a CQA, and based on the principle of color value, the surface color of calcined Magnetitum was objectively quantified. The vinegar quenching process was then optimized to determine the best processing conditions. X-ray diffraction(XRD) was used to establish the characteristic spectra of calcined Magnetitum, and methods such as similarity evaluation, cluster analysis, and orthogonal partial least squares-discriminant analysis(OPLS-DA) were used to evaluate the quality of the spectra. The optimized calcined Magnetitum preparation process was found to be calcination at 750 ℃ for 1 h, with a laying thickness of 4 cm, a particle size of 0.4-0.8 cm, and one vinegar quenching cycle(Magnetitum-vinegar ratio 10∶3), which was stable and feasible. The XRD characteristic spectra analysis method, featuring 9 common peaks as fingerprint information, was established. The average correlation coefficient ranged from 0.839 5-0.988 1, and the average angle cosine ranged from 0.914 4 to 0.995 6, indicating good similarity. Cluster analysis results showed that Magnetitum and calcined Magnetitum could be grouped together, with similar compositions. OPLS-DA discriminant analysis identified three key characteristic peaks, with Fe_2O_3 being the distinguishing component between the two. The final optimized processing method is stable and feasible, and the XRD characteristic spectra of calcined Magnetitum was initially established, providing a reference for subsequent quality control and the formulation of quality standards for calcined Magnetitum.

2.Short-term effects of ambient ozone on pediatric pneumonia hospital admissions: a multi-city case-crossover study in China.

BACKGROUND:
Children's respiratory health demonstrates particular sensitivity to air pollution. Existing evidence investigating the association between short-term ozone (O3) exposure and childhood pneumonia remains insufficient and inconsistent, especially in low- and middle-income countries (LMICs).
METHOD:
To provide more reliable and persuasive evidence, we implemented a multi-city, time-stratified case-crossover design with a large sample size, using data from seven representative children's hospitals across major geographical regions in China. To avoid the impact of the COVID-19 pandemic, individual-level medical records of inpatient children under 6 years of age diagnosed with pneumonia during 2016-2019 were collected. Conditional logistic regression models were fitted for each city, and city-specific estimates were pooled through a meta-analysis using a random-effects model.
RESULTS:
In total, the study included 137,470 pediatric pneumonia hospital admissions. The highest pooled estimate for O3 occurred at lag0-1, with a 10 µg/m3 increase in O3 associated with a 1.57% (95% CI: 0.67%-2.48%) higher risk of pediatric pneumonia hospital admissions. Stratified analyses indicated that the effects of O3 were robust across different sexes, age groups, and admission seasons. We also observed a statistically significant increase in risk associated with O3 concentrations exceeding the World Health Organization Air Quality Guidelines (WHO-AQGs).
CONCLUSIONS
This study revealed a significant positive association between O3 and pediatric pneumonia hospital admissions. Our findings substantially strengthen the evidence base for the adverse health impacts of O3, underscoring the importance of O3 pollution control and management in reducing the public health burden of pediatric pneumonia.

3.Exploration of BOPPPS-based online and offline hybrid teaching model of evidence-based medicine course

The online and offline hybrid teaching model of evidence-based medicine (EBM) is currently in the stage of development. Previous teaching focused on the teaching process in the classroom, and did not organically combine all the course contents before, during, and after class. The BOPPPS model can be used to establish coherence and integrity in the EBM teaching process. Considering the discipline characteristics and teaching objectives of EBM, this study initially explored and designed a BOPPPS-based online and offline hybrid teaching model. Taking the "diagnostic evidence" module as an example, the teaching implementation details were introduced. A pre-designed questionnaire was used to conduct baseline survey and follow-up survey on students before and after class to evaluate the teaching model and effect. The surveys showed that half of the students (77/154) preferred the new online and offline hybrid teaching model of EBM. The students found that all aspects of BOPPPS teaching were generally acceptable and satisfactory. Compared with before teaching, the students' proficiency in EBM was significantly improved after the teaching ( P<0.001), particularly in their ability to retrieve literature and evaluate the quality of evidence, which is of great significance for expanding their knowledge and clinical thinking.

Objective: To analyze and compare therapy responses, outcomes, and incidence of severe hematologic adverse events of flumatinib and imatinib in patients newly diagnosed with chronic phase chronic myeloid leukemia (CML) . Methods: Data of patients with chronic phase CML diagnosed between January 2006 and November 2022 from 76 centers, aged ≥18 years, and received initial flumatinib or imatinib therapy within 6 months after diagnosis in China were retrospectively interrogated. Propensity score matching (PSM) analysis was performed to reduce the bias of the initial TKI selection, and the therapy responses and outcomes of patients receiving initial flumatinib or imatinib therapy were compared. Results: A total of 4 833 adult patients with CML receiving initial imatinib (n=4 380) or flumatinib (n=453) therapy were included in the study. In the imatinib cohort, the median follow-up time was 54 [interquartile range (IQR), 31-85] months, and the 7-year cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) were 95.2%, 88.4%, 78.3%, and 63.0%, respectively. The 7-year FFS, PFS, and OS rates were 71.8%, 93.0%, and 96.9%, respectively. With the median follow-up of 18 (IQR, 13-25) months in the flumatinib cohort, the 2-year cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) were 95.4%, 86.5%, 58.4%, and 46.6%, respectively. The 2-year FFS, PFS, and OS rates were 80.1%, 95.0%, and 99.5%, respectively. The PSM analysis indicated that patients receiving initial flumatinib therapy had significantly higher cumulative incidences of CCyR, MMR, MR(4), and MR(4.5) and higher probabilities of FFS than those receiving the initial imatinib therapy (all P<0.001), whereas the PFS (P=0.230) and OS (P=0.268) were comparable between the two cohorts. The incidence of severe hematologic adverse events (grade≥Ⅲ) was comparable in the two cohorts. Conclusion: Patients receiving initial flumatinib therapy had higher cumulative incidences of therapy responses and higher probability of FFS than those receiving initial imatinib therapy, whereas the incidence of severe hematologic adverse events was comparable between the two cohorts.

5.Difference in Network Mechanism of Shoutaiwan and Juyuanjian in Reversing Pathology of Decidua of Spontaneous Abortion Patients: Based on "Uterine Collaterals Connecting Kidney" and "Fetal Collaterals Connecting Spleen" Theory

ObjectiveTo explore difference in the mechanism of Shoutaiwan, a representative kidney-tonifying and abortion-preventing formula, and Juyuanjian, a typical spleen-invigorating and abortion-preventing formula in reversing the pathology of decidua of spontaneous abortion (SA) patients and to expound the connotation of "uterine collaterals connecting kidney" and "fetal collaterals connecting spleen" theory. MethodThe targets of SA were retrieved from GeneCards, followed by gene ontology-biological process (GO-BP) annotation. Based on Cytoscape and previous research, the main processes and core targets were screened out. High-performance liquid chromatography-mass spectrometry (HPLC-MS) was used to identify the potential active components of Shoutaiwan and Juyuanjian and the regulatory networks were constructed. SA was induced in rats and the model rats were treated with Shoutaiwan and Juyuanjian at the same unit. Hematoxylin and eosin (HE) staining, transmission electron microscopy (TEM), enzyme-linked immunosorbent assay (ELISA), immunohistochemistry (IHC), immunofluorescence (IF), and other methods were employed to verify the mechanisms against miscarriage. ResultThe dysregulation of cell adhesion, inflammatory response, cell death, and angiogenesis was the core pathological process of SA. A total of 13 potential specific active components of Shoutaiwan and 14 active components of Juyuanjian were screened out. The regulatory networks showed that the potential active components of the two prescriptions modulated vascular endothelial growth factor (VEGF), interleukin (IL)-2, estrogen receptor (ESR)-1, matrix metalloproteinase-9 (MMP-9), and other targets to regulate the pathological process of SA. The two can significantly improve the pregnancy rate and the integrity rate and blood supply of decidua cells, control the apoptosis morphology and the expression of estrogen (E2), progesterone (P), and its receptor, and down-regulate the expression of MMP-2, MMP-9, IL-2, and IL-6 in decidua tissue of SA rats. At the same time, they up-regulated the expression of anti-apoptotic protein B-cell lymphoma-2 (Bcl-2) and IL-4. Shoutaiwan significantly up-regulated the expression of VEGF, and Juyuanjian significantly down-regulated the expression of E-cadherin (E-Cad). ConclusionBoth Shoutaiwan and Juyuanjian regulate the core pathological process of SA to prevent miscarriage. At the same unit, Shoutaiwan is overall superior to Juyuanjian. Shoutaiwan is better than Juyuanjian in regulating angiogenesis and Juyuanjian is superior to Shoutaiwan in regulating cell adhesion. This conclusion can partly explain the biological basis of "treating the same disease with different methods", and provide objective data reference for the identification of quality marker (Q-marker) of anti-miscarriage Chinese medicine and further study of formula-syndrome metabolome.
6.Molecular Mechanism of Qinghuangsan Against Acute Myeloid Leukemia Based on Network Pharmacology and In Vitro Experiments

ObjectiveTo predict the therapeutic target genes and related signaling pathways of Qinghuangsan (QHP) in the treatment of acute myeloid leukemia (AML) by network pharmacology,molecular docking,and further clarify its mechanisms through in vitro cell experiment. MethodThe active components and targets of QHP were retrieved from traditional Chinese medicine systems pharmacology database and analysis platform (TCMSP),traditional Chinese medicine integrated database (TCMID),TargetNet and SwissTargetPrediction databases,and AML-related target genes were obtained by GeneCards and online mendelian inheritance in man (OMIM) databases. After screening the common targets of QHP and AML,the protein-protein interaction (PPI) network of the common targets was constructed with STRING,followed by gene ontology (GO) term and Kyoto encyclopedia of genes and genomes (KEGG) pathway enrichment analysis based on RStudio software and clusterProfiler,Bioconductor packages. At the same time,Cytoscape software is used to construct the network of "disease-component-target" and "compound-target-pathway". Select the active ingredients of QHP for molecular docking with the top 8 targets in the "compound-target-pathway" network. In vitro cell experiment and Western blot were used to further verify the anti-AML effect of QHP. ResultThe prediction results show that there are 11 main active components of QHP,and 22 common targets of QHP and AML are collected. KEGG pathway analysis results show that phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) and mitogen-activated protein kinase (MAPK) signaling pathways may play a key role in the treatment of AML disease by QHP. "Compound-target-pathway" network analysis showed that the top 8 targets include Akt1,phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha (PIK3CA),mitogen-activated protein kinase kinase 1 (MAP2K1),TP53,serine/threonine kinase (RAF1),B cell lymphoma(Bcl)-2,cysteine aspartic acid specific protease(Caspase)-9 and JUN. Molecular docking results showed that 3-indolyl-β-D-glucopyranoside was optimally docked with MAP2K1,isovitexin docked with PIK3CA,and indirubin docked with Bcl-2. Cell experiments show that 3-indolyl-β-D-glucopyranoside,isovitexin and indirubin can effectively inhibit the proliferation of AML cells,regulate the MAPK/PI3K signaling pathway,and inhibit the expression of Bcl-2 protein. ConclusionQHP can treat AML through "multi-component,multi-target,multi-pathway" synergistic treatment,and its mechanism of pharmacology may be related to the regulation of MAPK signaling pathway and PI3K/Akt signaling pathway.
7.Progress of three-dimensional multimodal ultrasound in the evaluation of neoadjuvant chemotherapy for breast cancer

In recent years, the incidence rate of breast cancer has increased year by year, and gradually tends to be younger. Neoadjuvant chemotherapy (NACT) has become the standard treatment for locally advanced breast cancer (LABC). Therefore, it is of great clinical significance to evaluate the efficacy of neoadjuvant chemotherapy in early and effective manner. Multimodal ultrasound plays an important role in evaluating the response of neoadjuvant chemotherapy. This article will review the recent research progress of three-dimensional multimodal ultrasound in the evaluation of neoadjuvant chemotherapy for breast cancer .
8.Effect of Shenwei Ningyu Pills on Immunoinflammatory Response Mediated by TLR4/MyD88 Signaling Pathway in Hippocampus of Rats Exposed to Chronic Restraint Stress

Objective:To explore the effects of Shenwei Ningyu pills (SNP), a new Chinese medicine for depression, on the immunoinflammatory response mediated by Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88) signaling pathway in the hippocampus of rats exposed to chronic restraint stress (CRS). Method:Forty-four male Sprague Dawley rats were randomly enrolled into a normal group, a model group, an escitalopram group, and an SNP group. Except for the rats in the normal group, all rats were exposed to CRS and isolated rearing for 21 days continuously. Rats in the escitalopram group and the SNP group were administered with escitalopram (30 mg·kg-1) and SNP (18 mg·kg-1) one hour prior to CRS, respectively. The changes in body weight, sucrose preference index, horizontal movement scores, and vertical movement scores were observed by body weight assessment, sucrose preference test, and open field test. 9.Mental health and influencing factors among residents under standardized residency training during the epidemic of COVID-19

ObjectiveTo analyze the mental health status and the influencing factors among the residents taking standardized residency training during the outbreak and stable period of COVID-19, so as to provide theoretical basis for their mental health education. MethodsOn February 8 to February 10 and April 11 to April 13, 2020, Symptom Checklist 90 (SCL-90), Beck Srivastava Stress Inventory (BSSI) and Simplified Coping Style Questionnaire (SCSQ) were distributed in online chat groups involving all grades of residents taking standardized residency training in the Second Affiliated Hospital of Kunming Medical University. Through two rounds of questionnaire survey, non-probability sampling method was used to obtain survey samples, and their mental status were analyzed. ResultsA total of 159 valid questionnaires were collected in the first round, and 99 valid questionnaires in the second round. The first survey showed that the total score of SCL-90 was (117.69±37.74) and the detection rate of positive symptoms in SCL-90 was 25.8%, and mainly dominated by obsession, fear and interpersonal sensitivity. In the second survey, the results showed that the total score of SCL-90 was (127.19±51.44), and the main positive symptoms included obsession, depression and interpersonal sensitivity, with a positive detection rate of 30.3 %. The first survey found that the mental health status had significant differences among residents of different grades (χ2=7.46, P<0.05), furthermore, the results indicated that lower grade was a risk factor while non-singleton was the protective factor of mental health status (P<0.05), and SCL-90 total score was positively correlated with score of negative coping styles (r=0.45, P<0.01). The second survey also classified lower grade as risk factor and non-singleton as protective factor (P<0.05), and SCL-90 total score was positively correlated with study stress, economic pressure, interpersonal relationship, clinical practice and negative coping styles (r=0.52, 0.46, 0.55, 0.54, P<0.05 or 0.01). ConclusionResidents under standardized residency training have obvious mental health problems during the outbreak of COVID-19, and the problems become more serious during the stable period of COVID-19.
10.The value of ABVS in evaluating the response of neoadjuvant chemotherapy for different molecular subtypes of breast cancer

Objective:To explore the value of Automated Breast Volume Scanner (ABVS) in evaluating the response of neoadjuvant chemotherapy(NACT) for different molecular subtypes of breast cancer.Methods:Based on immunohistochemical technique, breast cancer was divided into three molecular subtypes according to the negative or positive results of estrogen receptor (ER), progesterone receptor (PR) and proto oncogene human epidermal growth factor receptor 2 (HER2), namely luminal group [ER(+ ) or PR(+ ), and HER2(-/+ )], triple-negative group [ER(-), PR(-) and HER2(-)], HER2 group [ER(-), PR(-)and HER2(+ )]. ABVS was used to analyze the changes of tumor size related parameters in 78 breast cancer patients with three subtypes during NACT.Results:After NACT, the size , area and volume of primary lesion diminished obviously ( P<0.05). 19 cases achieved complete response and 40 cases achieved partial response according to response evaluation criteria in solid tumors (RECIST). After the end of the second cycle of NACT, the primary tumor shrank significantly in HER2 group, and luminal and tri-negative groups were significantly reduced after the end of the third cycle. Conclusions:ABVS has a good value in evaluating the efficacy for different molecular substypes of NACT in breast cancer.
