BACKGROUND:Osteoarthritis is pathologically characterized by progressive degeneration of the articular cartilage and abnormal deformation of the subchondral bone.In recent years, with the deepening of medical research,it has been found that the mitogen-activated protein kinases(MAPK)signaling pathway has a regulatory role in inflammatory cell infiltration, inflammatory factor release, and chondrocyte proliferation, which is particularly important for the treatment of osteoarthritis.OBJECTIVE:To briefly review the main research progress in the mechanism of MAPK signaling pathway regulating osteoarthritis in recent years, aiming to provide new ideas for the treatment of osteoarthritis.METHODS:CNKI,WanFang and PubMed databases were searched for relevant literature using the search terms of"mitogen-activated protein kinases, osteoarthritis, extracellular signal-regulated MAP kinases,p38 mitogen-activated protein kinases,JNK mitogen-activated protein kinase"in Chinese and English.Relevant literature published from January 2019 to November 2024 was searched, and 108 articles were finally included for summary analysis.RESULTS AND CONCLUSION:(1)Various stimuli inside and outside the cells activate the MAPK signaling pathway, regulate gene transcription and protein synthesis, and promote the release of inflammatory factors, such as tumor necrosis factor-α,interleukin-1β,and interleukin-6.The release of these inflammatory factors aggravates the progression of osteoarthritis.(2)The active ingredients of traditional Chinese medicine, mainly saponins and flavonoids,as well as Chinese herbal formulas and preparations with the main effects of activating blood circulation and removing blood stasis, tonifying the liver and kidney, can play a therapeutic role in osteoarthritis by inhibiting the MAPK signaling pathway, regulating the release of matrix metalloproteinases, balancing the homeostatic state of osteogenesis and osteoblastogenesis, attenuating the synovial inflammation, decreasing the release of inflammatory factors and inflammatory vesicles, decreasing cellular pyroptosis, promoting autophagy, and ameliorating oxidative stress.(3)Although traditional Chinese medicine has become popular in the treatment of osteoarthritis by virtue of its own advantages of multi-components,multi-targets,multi-pathways,and low side effects, the use of MAPK signaling pathway to guide the treatment of individual osteoarthritis is the difficulty of the technology, which needs to be continuously researched and explored.(4)Therefore,further development of relevant herbal inhibitors that can modulate the MAPK signaling pathway may be a potential drug strategy for the treatment of osteoarthritis in the future.
2.Influencing factors of cerebral visual field defect after ischemic stroke and its impact on long-term prognosis
International Eye Science 2026;26(5):918-922
AIM:To explore the related factors of cerebral visual field defect after ischemic stroke and its impact on its long-term prognosis. METHODS: A retrospective study was conducted on patients with ischemic stroke admitted to the hospital from January 2021 to July 2023. Visual field examination was performed 7 d after onset, and patients were divided into cerebral visual field defect group and non-cerebral visual field defect group according to whether cerebral visual field defect occurred. Demographic information and clinical indicators were collected from both groups, and multivariate Logistic regression analysis was performed to identify factors associated with cerebral visual field defect after ischemic stroke. After discharge, a 2-year follow-up was conducted to calculate the number of poor prognosis, recurrence, and death cases to evaluate long-term prognosis.RESULTS:Among 218 patients with ischemic stroke, according to the visual field examination, 54 patients were in cerebral visual field defect group(incidence rate was 24.8%, 32 males and 22 females, mean age was 63.81±5.78 y)and 164 patients were in non-cerebral visual field defect group(95 males and 59 females, mean age was 62.78±6.01 y). There were no significant differences in gender, BMI, education level, smoking history, drinking history, diabetes, hypertension, hyperlipidemia, time from onset to admission, and type of infarction between the two groups(all P>0.05). There were significant differences in age, atrial fibrillation, degree of neurological deficit, infarct location and infarct volume between the two groups(all P<0.05). Multivariate Logistic regression analysis showed that the risk factor of cerebral visual field defect after ischemic stroke was the degree of neurological deficit at admission(OR: 5.106; 95%CI: 1.034-25.205), infarct location [parietal lobe(OR: 5.760; 95%CI: 1.643-20.194); temporal lobe(OR: 6.437; 95%CI: 1.591-26.036); occipital lobe(OR: 8.998; 95%CI:1.748-46.319)],infarct volume(OR: 5.123; 95%CI: 1.318-19.906). The poor prognosis and recurrence rate in the cerebral visual field defect group were higher than those in the non-cerebral visual field defect group(both P<0.05), and there was no difference in mortality rate between the two groups(P>0.05).CONCLUSION:The occurrence of cerebral visual field defects after ischemic stroke is closely related to the severity of the condition, infarct location and infarct volume infarction, and can lead to poor long-term prognosis. Therefore, prevention and treatment should be strengthened.

Background and Aims:Microsatellite-stable(MSS)colorectal cancer(CRC)generally exhibits poor responsiveness to immune checkpoint inhibitors(ICIs),and effective immunotherapy strategies remain lacking.Anti-angiogenic agents such as bevacizumab(BEV)can improve the tumor immune microenvironment and act synergistically with ICIs.This multicenter real-world study compared the efficacy of different immunotherapy-based combination regimens in patients with MSS/MSI-L/pMMR advanced CRC,aiming to identify the optimal treatment strategy. Methods:A total of 100 patients with MSS/MSI-L/pMMR advanced CRC who received systemic treatment between November 2019 and February 2025 at four tertiary hospitals in Hunan,China, were retrospectively enrolled.Patients were classified into six treatment groups:chemotherapy alone, chemotherapy+targeted therapy, immunotherapy alone, immunotherapy+chemotherapy, immunotherapy+targeted therapy, and immunotherapy+chemotherapy+targeted therapy. The primary endpoints were overall survival(OS)and progression-free survival(PFS),while secondary endpoints were objective response rate(ORR)and disease control rate(DCR).Additionally, among patients receiving immunotherapy, subgroup analysis was performed according to BEV administration. Results:Among all 100 patients, the immunotherapy+chemotherapy+targeted therapy group achieved the highest ORR(32.0%)and DCR(76.0%)and was the only regimen yielding a complete response(CR).Compared with chemotherapy or immunotherapy alone, the triplet regimen significantly improved OS(P<0.05);although PFS improvement did not reach statistical significance,a clear late-stage separation of survival curves was observed. In the immunotherapy subgroup,BEV-containing regimens achieved markedly better outcomes than non-BEV regimens, with DCR of 75.0%vs.48.8%,median OS of 18.9 vs.11.5 months, and median PFS of 13.8 vs.7.2 months(all P<0.001).Cox regression analysis showed that compared with chemotherapy alone, the triplet regimen significantly reduced the risk of death(HR=0.11)and disease progression(HR=0.25)(both P=0.002).Vascular invasion was identified as an adverse prognostic factor for PFS(HR=3.0,P=0.007).Conclusion:This multicenter real-world study demonstrated that combining immunotherapy with chemotherapy and targeted therapy significantly improves DCR and survival outcomes in patients with MSS/MSI-L/pMMR advanced CRC,with BEV-containing triplet regimens providing the most pronounced benefit.BEV may enhance immune responsiveness by modulating the tumor microenvironment and promoting effector T-cell infiltration, offering a promising therapeutic direction for"immune-cold"CRC.Prospective randomized studies are warranted to further validate its clinical value and define appropriate patient populations.

Objective:To analyze the clinical characteristics of patients with chronic obstructive pulmonary disease (COPD) complicated by fatigue. Methods:COPD patients enrolled in the RealDTC study from June 2023 to March 2024 were included. Demographic data, history of acute exacerbations in the past year, smoking status, biofuel exposure, occupational exposure, modified Medical Research Council (mMRC) dyspnea score, COPD Assessment Test (CAT) score, forced expiratory volume in the first second predicted of percentage (FEV 1%pred), forced expiratory volume in one second (FEV 1)/forced vital capacity (FVC), and comorbidities (bronchial asthma, bronchiectasis, tuberculosis, cardiovascular disease, diabetes mellitus) were collected. Fatigue was evaluated using the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) questionnaire, with a score ≤43 defined as fatigue. Patients were divided into fatigue and non-fatigue groups, and multivariate regression analysis was used to screen factors associated with fatigue in COPD patients. Results:A total of 597 COPD patients were included, of which 280(46.9%) had fatigue symptoms. Compared with non-fatigue patients, fatigue patients had lower FEV 1%pred, FEV 1/FVC, and body mass index (BMI), higher CAT and mMRC scores, and a higher proportion of occupational exposure, bronchiectasis, and treatment with long-acting β 2-agonists (LABA)/long-acting muscarinic antagonists (LAMA)/inhaled corticosteroids (ICS) (all P<0.05). Multivariate regression analysis showed that high CAT score ( OR=2.312, 95% CI: 1.366-3.911), high mMRC score ( OR=1.484, 95% CI: 1.053-2.091), occupational exposure ( OR=1.513, 95% CI: 1.082-2.116), comorbid bronchiectasis ( OR=2.452, 95% CI: 1.102-5.457), low BMI ( OR=0.935, 95% CI: 0.891-0.981), and high CAT-energy score ( OR=1.301, 95% CI: 1.149-1.473) were risk factors for fatigue in COPD patients. The CAT-energy score was highly correlated with the FACIT-F score ( r=0.260, P<0.001), and a CAT-energy score ≥2 could preliminarily screen COPD patients with fatigue. Conclusions:COPD patients with comorbid fatigue have a heavy symptom burden, are more likely to have a history of occupational exposure and bronchiectasis, and the CAT-energy score is of great reference value for screening COPD patients with fatigue.

Objective:To analyze the clinical characteristics of patients with chronic obstructive pulmonary disease (COPD) complicated by anorexia. Methods:This cross-sectional study included patients registered in the RealDTC study from May 2023 to December 2023. Demographic data, COPD Assessment Test (CAT) score, modified Medical Research Council (mMRC) dyspnea questionnaire score, Clinical COPD Questionnaire (CCQ) score, forced expiratory volume in one second (FEV 1), forced expiratory volume in the first second predicted of percentage (FEV 1%pred), FEV 1/forced vital capacity (FVC), Global Initiative for Chronic Obstructive Lung Disease (GOLD) grade, GOLD group, number of acute exacerbations and hospitalizations in the past year, and score of the Functional Assessment of Anorexia Cachexia Therapy-Anorexia/Cachexia Subscale-12 (FAACT-A/CS-12) were collected. Patients with a FAACT-A/CS-12 score ≤30 were diagnosed as having anorexia. Multivariate logistic regression analysis was used to evaluate the influencing factors of anorexia in COPD patients. Results:A total of 617 COPD patients were included, of whom 109(17.7%) had anorexia. Compared with non-anorexia patients, COPD patients with anorexia had higher age, CAT, mMRC and CCQ scores, and more acute exacerbations and hospitalizations in the past year, while body mass index, FEV 1, FEV 1%pred and FEV 1/FVC were lower (all P<0.05). The proportions of patients with primary education or below, GOLD 3-4 grade and GOLD E group were higher in COPD patients with anorexia (all P<0.05). Logistic regression analysis showed that a CAT score of 10-<20 [odds ratio ( OR)=4.017, 95% confidence interval ( CI): 1.673-59.645], a CAT score of 20-<30 ( OR=9.686, 95% CI: 3.777-24.842), a CAT score of ≥30 ( OR=78.286, 95% CI: 7.654-800.689) and ≥1 hospitalization in the past year ( OR=2.050, 95% CI: 1.292-3.254) were independent risk factors for anorexia in COPD patients (all P<0.05). Conclusions:COPD patients with anorexia have poor lung function, high symptom burden and high risk of acute exacerbation. Clinicians should pay attention to the management of COPD patients with anorexia and take corresponding intervention measures.
