1.lncRNA DLEU2 regulates IKKα-mediated 131I resistance in thyroid carcinoma TPC-1 cells via the EZH2/H3K27me3 axis

[摘 要] 目的:探讨lncRNA DLEU2通过EZH2/H3K27me3途径调控IKKα介导甲状腺癌(TC)放射性碘抵抗的作用机制。方法:利用TCGA数据库分析TC中DLEU2的表达及其与EZH2的相关性。构建放射性碘抵抗的TPC-1细胞(RR-TPC-1细胞)模型及裸鼠移植瘤模型,通过敲低或过表达DLEU2(si-DLEU2/OE-DLEU2)、抑制EZH2(UNC1999)、过表达IKKα(OE-IKKα)进行干预,采用qPCR、WB、RIP、ChIP、CCK-8、流式细胞术、TUNEL染色及体内成瘤实验检测基因与蛋白表达、表观修饰、细胞增殖、凋亡及肿瘤生长。结果:TCGA分析显示,DLEU2在TC组织中显著上调(P < 0.001),与患者不良预后相关(P = 0.008 4),且与EZH2表达呈正相关(r = 0.390, P < 0.001);RIP证实EZH2与DLEU2存在相互作用/结合(P < 0.05)。体外实验表明,敲低DLEU2可显著下调RR-TPC-1细胞中EZH2、IKKα表达及H3K27me3修饰水平,抑制NF-κB通路活化(P < 0.05或P < 0.01),抑制细胞增殖、促进凋亡(均P < 0.05)。联合敲低DLEU2与抑制EZH2进一步增强上述效应,而过表达IKKα则可部分逆转上述效应(P < 0.05或P < 0.01)。体内实验进一步证实,敲低DLEU2联合抑制EZH2可显著抑制移植瘤生长,增加肿瘤细胞凋亡(均P < 0.01);IKKα过表达则部分逆转上述抗肿瘤效应(P < 0.05或P < 0.01)。结论:lncRNA DLEU2通过招募EZH2催化H3K27me3修饰,间接激活IKKα/NF-κB信号并形成正反馈环路,介导TPC-1细胞131I抵抗。
2.Color-component correlation and mechanism of component transformation of processed Citri Reticulatae Semen.

High-performance liquid chromatography(HPLC) was used to determine the content of three major components in Citri Reticulatae Semen(CRS), including limonin, nomilin, and obacunone. The chromaticity of the CRS sample during salt processing and stir-frying was measured using a color difference meter. Next, the relationship between the color and content of the salt-processed CRS sample was investigated through correlation analysis. By integrating the oil bath technique for processing simulation with HPLC, the changes in the relative content of nomilin and its transformation products were analyzed, with its structural transformation pattern during processing identified. Additionally, RAW264.7 cells were induced with lipopolysaccharides(LPSs) to establish an inflammatory model, and the anti-inflammatory activity of nomilin and its transformation product, namely obacunone was evaluated. The results indicated that as processing progressed, E~*ab and L~* values showed a downward trend; a~* values exhibited a slow increase over a certain period, followed by no significant changes, and b~* values remained stable with no significant changes over a certain period and then started to decrease. The limonin content remained barely unchanged; the nomilin content decreased, and the obacunone increased significantly. The changing trends in content and color parameters during salt-processing and stir-frying were basically consistent. The content of nomilin and obacunone was significantly correlated with the colorimetric values(L~*, a~*, b~*, and E~*ab), while limonin content showed no significant correlation with these values. By analyzing HPLC patterns of nomylin at different heating temperatures and time, it was found that under conditions of 200-250 ℃ for heating of 5-60 min, the content of nomilin significantly decreased, while the obacunone content increased pronouncedly. The in vitro anti-inflammatory activity results indicated that compared to the model group, the group with a high concentration of nomilin and the groups with varying concentrations of obacunone showed significantly reduced release of nitric oxide(NO)(P<0.01). When both were at the same concentration, obacunone showed better performance in inhibiting NO release. In this study, the obvious correlation between the color and content of major components during the processing of CRS samples was identified, and the dynamic patterns of quality change in CRS samples during processing were revealed. Additionally, the study revealed and confirmed the transformation of nomilin into obacunone during processing, with the in vitro anti-inflammatory activity of obacunone significantly greater than that of nomilin. These findings provided a scientific basis for CRS processing optimization, tablet quality control, and its clinical application.

3.Prognostic value of quantitative flow ratio measured immediately after percutaneous coronary intervention for chronic total occlusion.

BACKGROUND:
The clinical impact of post-percutaneous coronary intervention (PCI) quantitative flow ratio (QFR) in patients treated with PCI for chronic total occlusion (CTO) was still undetermined.
METHODS:
All CTO vessels treated with successful anatomical PCI in patients from PANDA III trial were retrospectively measured for post-PCI QFR. The primary outcome was 2-year vessel-oriented composite endpoints (VOCEs, composite of target vessel-related cardiac death, target vessel-related myocardial infarction, and ischemia-driven target vessel revascularization). Receiver operator characteristic curve analysis was conducted to identify optimal cutoff value of post-PCI QFR for predicting the 2-year VOCEs, and all vessels were stratified by this optimal cutoff value. Cox proportional hazards models were employed to calculate the hazard ratio (HR) with 95% CI.
RESULTS:
Among 428 CTO vessels treated with PCI, 353 vessels (82.5%) were analyzable for post-PCI QFR. 31 VOCEs (8.7%) occurred at 2 years. Mean value of post-PCI QFR was 0.92 ± 0.13. Receiver operator characteristic curve analysis shown the optimal cutoff value of post-PCI QFR for predicting 2-year VOCEs was 0.91. The incidence of 2-year VOCEs in the vessel with post-PCI QFR < 0.91 (n = 91) was significantly higher compared with the vessels with post-PCI QFR ≥ 0.91 (n = 262) (22.0% vs. 4.2%, HR = 4.98, 95% CI: 2.32-10.70).
CONCLUSIONS
Higher post-PCI QFR values were associated with improved prognosis in the PCI practice for coronary CTO. Achieving functionally optimal PCI results (post-PCI QFR value ≥ 0.91) tends to get better prognosis for patients with CTO lesions.

For middle-aged and elderly patients with conditions such as spinal fractures and degenerative spinal diseases, spinal internal fixation is a core surgical procedure for reconstructing spinal stability, heavily relying on the biomechanical stability provided by pedicle screw systems. Whereas, these patients are often complicated by osteoporosis that can significantly compromise the stability of the bone-pedicle screw interface, leading to a marked increase in pedicle screw loosening and surgical failure rates. The bone cement-augmented pedicle screw technique, which involves injecting bone cement into the vertebral body or screw trajectory to optimize the mechanical properties of the bone-pedicle screw composite, has been proven to significantly enhance fixation strength and effectively prevent screw-related failures, thereby reducing the incidence of internal fixation failure in high-risk populations undergoing spinal fusion. However, the widespread clinical application of this technique has faced challenges such as inaccurate clinical decision-making (indication and contraindication selection), non-standardized operative practices, and insufficient awareness of complication prevention, resulting in considerable variability in clinical outcomes and even severe complications. To address this, Prof. Luo Fei from First Affiliated Hospital of Army Medical University initiated the project and the Chinese Association Orthopaedic Surgeons organized relevant experts to develop the Evidence-based clinical practice guideline for bone cement-augmented pedicle screw technique ( version 2025), based on current evidence. The guidelines put forward 8 recommendations regarding the clinical value, scope of application, and operational standards of the technique, aiming to provide evidence-based medical support and technical standardization for clinical decision-making.
5.Diagnostic value of B-type natriuretic peptide combined with the evaluation of guidelines in syncope study score for cardiogenic syncope

Objective:To evaluate the diagnostic efficacy of B-type natriuretic peptide(BNP)combined with the Evaluation of Guidelines in Syncope Study(EGSYS)score for cardiogenic syncope(CS), and to provide evidence for rapid clinical identification of high-risk patients.Methods:We retrospectively analyzed 366 patients with syncope hospitalized in the department of cardiovascular medicine of Beijing Hospital from January 1, 2016, to December 31, 2022.Based on the international guideline diagnostic criteria, the patients were categorized into four groups: neutrally mediated reflex syncope(NMS)group, orthostatic hypotension(OH)group, cardiogenic syncope(CS)group, and syncope of unknown origin(US)group.BNP levels were measured at admission and EGSYS scores were calculated.Receiver operating characteristic(ROC)curve analysis was performed to assess the diagnostic efficacy of individual and combined indices for CS.Results:A total of 366 syncope patients were included, among which 70 patients(19.1%)were diagnosed with NMS, 25 patients(6.8%)with OH, 44 patients(12.0%)with CS, and 227 patients(62.0%)with US.Patients in the CS group had significantly higher BNP levels and EGSYS scores compared to those in the NMS, OH, and US groups(all P<0.001). The AUC of EGSYS score for diagnosing CS was 0.783(95% CI: 0.711-0.855), while the AUC of BNP level for diagnosing CS was 0.805(95% CI: 0.727-0.884). When BNP level was combined with EGSYS score, diagnostic performance was significantly improved, with the AUC increasing to 0.855(95% CI: 0.792-0.918). Conclusions:The combination of BNP and EGSYS score significantly can improve the diagnostic accuracy of cardiogenic syncope, providing a practical diagnostic strategy for the early identification of high-risk syncope patients in clinical practice.

This consensus was developed by the Orthodontic Society of the Chinese Stomatological Association to provide a systematic, scientific, and practical guideline for informed consent in orthodontic care. Orthodontic treatment is typically lengthy, highly individualized, and involves multiple factors such as growth and development, occlusal function, and facial esthetics. Rapid technological advances and diverse risk profiles make the traditional reliance on orthodontist experience or institutional templates insufficient to ensure patients′ full understanding and autonomous decision-making. To address this, the expert panel conducted extensive reviews of domestic and international guidelines, analyzed representative dispute cases, and performed multicenter patient-clinician surveys. Using a multi-round Delphi method, the group established a standardized informed consent framework covering the initial consultation, treatment, and retention phases. The consensus emphasizes that informed consent is not only a fundamental legal and ethical requirement but also a key step in building trust, improving patient compliance, and enhancing treatment satisfaction. Orthodontists should clearly and comprehensively explain treatment plans, potential risks, uncertainties, and associated costs, while respecting the autonomy of patients or guardians, and maintain continuous communication and dynamic evaluation throughout the treatment process. The release of this consensus provides unified and authoritative guidance for clinical orthodontics, helping to standardize informed consent, enhance its transparency, safeguard patient rights, reduce medical risks, and promote high-quality, sustainable development of orthodontic practice.
7.Changes of biochemical and biophysical factors in the periodontal ligament microenvironment at different stages of periodontitis development

Objective To investigate the patterns of changes in biochemical factors(IL-1β and TGF-β1)and biophysical factors within the periodontal ligament microenvironment at different stages of periodontitis development.Methods Periodontitis models at different stages of development(early and late stages)were constructed in rats.The changes in the periodontal ligament microenvironment were determined by detecting biochemical factors((IL-1β and TGF-β1)and changes in periodontal ligament stiffness.Results Regarding biochemical factors,the growth factor TGF-β1 was at a high level in the early stage of periodontitis,while the inflammatory factor(IL-1β was slightly increased compared to the normal state,but expressed at a low level.As the inflammation progressed,the expression of TGF-β1 was suppressed and significantly decreased in the late stage of periodontitis(late stage:11.34±2.91 vs.early stage:20.41±2.64,P<0.01).In contrast,the expression of(IL-1βsignificantly increased(late stage:18.77±3.36 vs.early stage:8.24±1.47,P<0.01).Regarding biophysical factors,the stiffness of the periodontal ligament and the expression of the key protein integrin αv β3 in the mechanical signaling pathway gradually decreased with the development of periodontitis.Additionally,the osteogenic differentiation capacity of stem cells significantly decreased in the late stage of periodontitis(late stage:12.47±2.83 vs.early stage:21.15±4.38,P<0.05).Conclusion TGF-β1 may play a role in promoting periodontal tissue repair and maintaining physiological stability in normal and early stages of periodontitis,while the increased expression of(IL-1β in the late stage of periodontitis may inhibit the osteogenic differentiation of stem cells,leading to bone loss.Biophysical factors,including matrix stiffness and integrin αvβ3 expression,gradually decrease with the progression of periodontitis,potentially affecting bone tissue repair.

In proton therapy,prompt gamma-ray imaging is considered as one of the most promising methods for assessing proton beam range.Prompt gamma-ray imaging detector evaluates the proton beam range based on the prompt gamma-ray distribution obtained by the prompt gamma-ray imaging system,which enables high-precision measurement of the proton beam range.Herein a proton beam range verification algorithm is designed for the newly developed prototype of the range verification detector(pixelated prompt gamma-ray imaging detector),which verifies the range estimation accuracy of the prototype for different phantoms and different energies of homogeneous media through Monte Carlo simulation.The results show that the accuracy of the proton beam range verification algorithm is within 0.5 mm of the safety margin error of the Bragg peak,and the measurement accuracy is significantly improved with the increase of the number of protons,indicating that the prototype algorithm is feasible for proton beam range verification.
9.Effect of donor blood lipid levels and pancreatic surface fat on islet isolation outcomes

Objective:To investigate the effect of donor blood lipid levels and the degree of fat deposition on the pancreatic surface on the outcome of islet isolation.Method:A retrospective analysis was conducted on 171 cases of islet isolation data from organ donors between May 2015 and December 2024. According to the percentage of fat deposition area on the surface of the pancreatic capsule after trimming, the samples were divided into three groups: mild surface fat group (<30%, 60 cases) , moderate surface fat group (30%–70%, 55 cases) , and severe surface fat group (>70%, 56 cases). The modified Ricordi method was used to digest pancreatic tissue, and islets were purified by continuous density gradient centrifugation. The digestion efficiency, digestion time, islet yield (islet equivalent/quantity) , purity, score, and size were analyzed and compared among groups. One-way ANOVA was used for inter-group comparisons, and Pearson correlation analysis and multiple linear regression analysis were used to explore the relationship between blood lipid levels and islet isolation parameters.Result:The severe surface fat group had significantly higher pre-purification and post-purification islet equivalents, islet number, amount of digested pancreatic tissue, donor triglyceride levels, and low density lipoprotein (LDL) levels than the other groups (all P<0. 05) . Correlation analysis showed that LDL level was positively correlated with pre-purification islet equivalents (62 cases, r=0. 298, P=0. 019) and islet number (58 cases, r=0. 285, P=0. 030) . Donor high density lipoprotein (HDL) level was negatively correlated with post-purification islet equivalents (54 cases, r= – 0. 282, P=0. 039) ; donor triglyceride level was positively correlated with the amount of digested tissue (56 cases, r=0. 268, P=0. 046) and negatively correlated with purity (51 cases, r= - 0. 297, P=0. 035) ; donor very low density lipoprotein (VLDL) level was positively correlated with the amount of digested tissue (67 cases, r=0. 337, P=0. 005) and negatively correlated with purity (61 cases, r=- 0. 348 , P=0. 006) ; donor total cholesterol level was negatively correlated with pancreatic digestion efficiency (34 cases, r= - 0. 370, P=0. 032) , and the above differences were all statistically significant. Conclusion:Pancreata with heavier surface fat deposition can yield a higher number of islets. Meanwhile, donor blood lipid levels are correlated with islet isolation outcomes and can serve as important indicators for donor pancreas selection.
10.The developmental trajectory and influencing factors of kinesiophobia among post-percutaneous coronary intervention patients

Objective:To explore the developmental trajectory and influencing factors of kinesiophobia in patients undergoing percutaneous coronary intervention (PCI) .Methods:By convenient sampling, totally 217 patients undergoing PCI who enrolled from two tertiary hospitals in Guangdong Province from March 2022 to July 2023. The demographic data of the patients was collected , and kinesiophobia was measured using the Tampa scale for kinesiophobia heart(TSK-SV-Heart) at one day pre-discharge, 2 weeks, 1 month, and 4 months post-discharge. Data analysis was conducted using SPSS 25.0 and Mplus 8.7 softwares. Growth mixture modelling, chi-square test, and polynomial Logistic regression were used for data processing and analysis.Results:Three different kinesiophobia trajectory classes were identified in patients within 4 months after PCI: sustained high level of kinesiophobia group (C1 group, 22.6%(49/217)), moderate level of kinesiophobia with a rapid decrease group (C2 group, 47.4%(103/217)), and rapid decrease of kinesiophobia followed an increase group (C3 group, 30.0%(65/217)). Polynomial Logistic regression results showed that, females ( B=1.136, OR=3.113, 95% CI=1.155-8.389) , patients with NYHA Ⅱ/Killip class Ⅱ or above ( B=1.135, OR=3.112, 95% CI=1.380-7.017) were more likely to develop into the C1 compared with the C2 group. Compared with the C3 group, patients with NYHA Ⅱ/Killip class Ⅱ or above ( B=3.322, OR=27.712, 95% CI=5.251-146.244), and patients with coronary heart disease(CHD) more than two years ( B=3.855, OR=47.250, 95% CI=2.146-1 040.535)were more likely to develop into the C1 group. Compared with the C3 group, patients with NYHA Ⅱ/Killip class Ⅱ or above ( B=-2.187, OR=0.112, 95% CI=0.022-0.565), patients with three or more comorbidities ( B=-2.711, OR=0.066, 95% CI=0.008-0.528), and patients with CHD more than two years ( B=-2.376, OR=0.093, 95% CI=0.011-0.783) were more likely to develop into the C2 group. Conclusion:Kinesiophobia level in patients undergoing PCI presents a curvilinear decrease within 4 months after PCI.Different kinesiophobia trajectory classes can be observed. Sex, NYHA/Killip class, course of CHD, and comorbidity affect the development trajectory of different subgroups.
