1.Trends in incidence of notifiable infectious diseases in Nanjing City from 2004 to 2022

Objective:
To investigate the incidence trend and epidemic characteristics of notifiable infectious diseases in Nanjing City from 2004 to 2022, so as to provide the basis for improving the prevention, control, and monitoring strategies of infectious diseases.
Methods:
Data pertaining to notifiable infectious diseases reported in Nanjing City from 2004 to 2022 were retrieved from the Infectious Disease Surveillance System of Chinese Disease Prevention and Control Information System. Infectious diseases were classified by law and transmission routes. Temporal distribution incidence of notifiable infectious diseases were descriptively analyzed. The trends in incidence of notifiable disease were analyzed using annual percent change (APC) and average annual percent change (AAPC).
Results:
A total of 33 types of notifiable infectious diseases with 505 275 cases were reported in Nanjing City from 2004 to 2022. The average annual reported incidence was 347.45/105, showing a decreasing trend from 2018 to 2022 (APC=-13.499%, P<0.05), and there was no significant trend overall (AAPC=-1.586%, P>0.05). A total of 203 235 cases of 25 types of class A and B notifiable infectious diseases were reported, with an average annual reported incidence of 139.75/100 000, showing an overall decreasing trend (AAPC=-4.954%, P<0.05). Eight types of class C notifiable infectious diseases with 302 042 cases were reported, with an average annual reported incidence of 207.69/100 000. The reported incidence showed an increasing trend from 2004 to 2018 (APC=10.117%, P<0.05), and a decreasing trend from 2018 to 2022 (APC=-27.467%, P<0.05). There was no trend overall (AAPC=-0.360%, P>0.05). The reported incidence of blood-borne and sexually transmitted infectious diseases was the highest in class A and B infectious diseases, with an average annual reported incidence of 69.88/100 000, which was at a high epidemic level throughout the year, except February. The reported incidence of respiratory infectious diseases was 51.30/100 000, with a high reported incidence in April and December. The reported incidence of intestinal infectious diseases was the highest (178.06/100 000) in class C infectious diseases, with a high reported incidence in June and November.
Conclusions
The reported incidence of notifiable infectious diseases in Nanjing City was generally stable from 2004 to 2022. The peak incidence of respiratory infectious diseases occurred in winter and spring, and that of intestinal infectious diseases was in summer and autumn. It is necessary to strengthen the surveillance and intervention of blood-borne and sexually transmitted infectious diseases, respiratory infectious diseases, and intestinal infectious diseases to reduce the risk of infectious diseases.
2.Analysis of the intervention effects of skill training for parents with autism child on toddlers with autism spectrum disorder

Objective:To explore the intervention effects of the skill training for parents with autism child (STPAC) on toddlers with autism spectrum disorder (ASD).Methods:A multicenter non-randomized concurrent controlled study design was conducted. Thirty children with ASD aged 15-30 months, first diagnosed at the Children′s Hospital of Fudan University, the First Hospital of Jilin University, and Chengdu Women′s and Children′s Central Hospital from 2019 to 2020, were enrolled in the STPAC group. Thirty children with ASD who visited the same hospitals during the same period but refused the STPAC intervention were selected as the control group. The STPAC group received an 8-week intervention (3 h/week) followed by quarterly follow-ups for 1 year, while the control group voluntarily chose community-based routine interventions. The Griffiths development scales-Chinese (GDS-C) was used to assess the developmental levels, and the communication and symbolic behavior scales developmental profile infant-toddler checklist (CSBS-DP-ITC) was completed by the primary caregivers to evaluate social, language and symbolic behavior. The independent samples t-tests or Mann-Whitney U tests, etc.was used for inter-group comparison. The paired t-tests or Wilcoxon signed-rank tests, etc. was used for inter-group pre-post intervention comparison. Results:The STPAC group included 30 children (22 males and 8 females, aged (23.9±2.2) months), and the control group included 30 children (20 males and 10 females, aged (24.2±2.6) months). Before the intervention, there were no statistically differences in GDS-C development quotient (DQ) and CSBS-DP-ITC scores between groups (all P>0.05). After 1-year intervention, GDS-C DQ in personal-social, hearing-language, hand-eye coordination, performance domains of STPAC group and GDS-C DQ in personal-social, hearing-language domains of control group were all increased (all P<0.01). After 1-year intervention, CSBS-DP-ITC scores of both groups were all improved in socia, language, symbolic behavior, and total scores (all P<0.001). GDS-C DQ changes before and after 1 year of intervention in hearing-language, hand-eye coordination, performance domains of the STPAC group were all higher the those of control group (34(15, 48 vs. 10(-4, 39), 11±20 vs. -1±19, 23±25 vs. 8±22, all P<0.05). CSBS-DP-ITC scores changes before and after 1 year of intervention in social and total scores of the STPAC group were both higher the those of control group (10(5, 30) vs. 3(1, 7), 26±17 vs. 11±8, both P<0.001). Conclusion:Compared with the community routine interventions, the STPAC better improves the language, hand-eye coordination, visual-spatial, social communication, and play skills in ASD toddlers.
3.Exploration on the Mechanism of Modified Erchen Decoction in Preventing and Treating Hyperlipidemia with Phlegm-Dampness Syndrome through AMPK Signaling Pathway Based on Network Pharmacology and Animal Experiment

Objective To explore the mechanism of the modified Erchen Decoction in preventing and treating hyperlipidemia with phlegm-dampness syndrome based on network pharmacology and animal experiment.Methods Active components and potential targets of modified Erchen Decoction were screened using TCMSP database.Hyperlipidemia disease targets were retrieved from GeneCards,DrugBank,DisGeNET,OMIM and TTD databases,and the intersection of drugs and disease targets was taken.A protein-protein interaction network was constructed using STRING database,and GO and KEGG pathway enrichment analysis on intersecting targets was conducted using DAVID database.Cytoscape 3.9.1 software constructed a component-target-pathway network to analyze key pathways.The molecular docking between main active components and key targets was performed.A rat model of hyperlipidemia with phlegm dampness syndrome was prepared,and was intervened with modified Erchen Decoction.Liver index was calculated,lipid levels were detected,liver tissue morphology was observed by HE staining,and related protein expressions in liver tissue were detected by Western blot.Results Network pharmacology analysis showed that there were 41 targets of modified Erchen Decoction in preventing and treating hyperlipidemia,including HMGCR,SREBF1,FASN,etc.,involved in AMPK signaling pathway,lipid and atherosclerosis,chemical carcinogenesis-receptor activation,etc.The animal experiment results showed that modified Erchen Decoction could effectively regulate the blood lipid levels of model rats,reduce liver fat accumulation,significantly increase p-AMPK protein expression in liver tissue(P<0.05),and reduce HMGCR,SREBP1c and FAS protein expressions(P<0.05).Conclusion Modified Erchen Decoction may inhibit the synthesis and absorption of cholesterol and fatty acids by activating AMPK/SREBP1c signaling pathway,so as to improve hyperlipidemia.

6.Research progress on the role of JAK2/STAT3 signaling pathway in cerebral ischemia-reperfusion injury and TCM intervention

Janus kinase 2(JAK2)/signal transducer and activator of transcription 3(STAT3)signaling pathway regulates multiple processes in neurons,including growth,development,proliferation and differentiation,and is closely associated with the onset,progression and treatment of cerebral ischemia-reperfusion injury(CIRI).This article provides an overview of the JAK2/STAT3 signaling pathway and its relationship with CIRI,followed by a comprehensive review of the current research status on the regulation of the JAK2/STAT3 signaling pathway by traditional Chinese medicine(TCM)to ameliorate CIRI.Among them,TCM monomers and compound prescriptions such as glycyrrhetinic acid,triptolide,and Taohong siwu decoction can inhibit inflammatory responses,enhance cellular autophagy,and suppress apoptosis by inhibiting the JAK2/STAT3 signaling pathway,thereby alleviating CIRI.In addition,TCM monomers and compound prescriptions like astragaloside Ⅳ,ginkgolide K,and Qingnao dripping pills can mitigate CIRI by activating the JAK2/STAT3 signaling pathway to inhibit oxidative stress,promote angiogenesis,and enhance neuronal survival rates.
7.Bakuchiol inhibits macrophage-derived foam cell formation by reducing ERK1/2 phosphorylation and upregulating ABCA1 expression

Aim To investigate the impact of bakuchiol(BAK)on lipid accumulation in macrophage-derived foam cell and explore the underlying mechanisms.Methods MTT assay was used to determine the non-toxic concen-tration of BAK on foam cell.Oil red O staining,NBD cholesterol,and Dil-ox-LDL were used to assess lipid accumulation in foam cell.RT-qPCR and Western blot were utilized to measure mRNA and protein expression,respectively.Re-sults BAK promoted cholesterol effux and reduced lipid accumulation in foam cell.BAK upregulated the mRNA and protein expression levels of ATP-binding cassette transporter A1(ABCA1)and concurrently downregulated the phosphoryl-ation levels of extracellular signal-regulated kinase 1/2(ERK1/2).Treatment with the ERK1/2 activator Ro 67-7476 re-sulted in decreased ABCA1 protein expression compared with the BAK-treated group.Conclusion BAK reduced lipid accumulation in foam cell by inhibiting ERK1/2 phosphorylation,upregulating ABCA1 expression,and promoting choles-terol efflux,thereby suppressing foam cell formation.
8.Impact of cecal ligation and puncture-induced sepsis on the proliferation and differentiation of intestinal stem cells

Objective:To analyze the impact of cecal ligation and puncture (CLP)-induced sepsis on the proliferation and differentiation of intestinal epithelial cells.Methods:① Animal experiment: sixteen male C57BL/6 mice were divided into sham operation group (Sham group) and CLP-induced sepsis model group (CLP group) by random number table method, with 8 mice in each group. After 5 days of operation, the jejunal tissues were taken for determination of leucine-rich-repeat-containing G-protein-coupled receptor 5 (LGR5) and intestinal alkaline phosphatase (IAP) by polymerase chain reaction (PCR). The translation of LGR5 was detected by Western blotting. The expression of proliferating cell nuclear antigen (Ki67) was analyzed by immunohistochemistry. IAP level was detected by modified calcium cobalt staining and colorimetry. Immunofluorescence staining was used to detect the expression of Paneth cell marker molecule lysozyme 1 (LYZ1) and goblet cell marker molecule mucin 2 (MUC2). ② Cell experiment: IEC6 cells in logarithmic growth stage were divided into blank control group and lipopolysaccharide (LPS) group (LPS 5 μg/mL). Twenty-four hours after treatment, PCR and Western blotting were used to analyze the transcription and translation of LGR5. The proliferation of IEC6 cells were detected by 5-ethynyl-2'-deoxyuridine (EdU) staining. The transcription and translation of IAP were detected by PCR and colorimetric method respectively.Results:① Animal experiment: the immunohistochemical results showed that the positive rate of Ki67 staining in the jejunal tissue of CLP group was lower than that of Sham group [(41.7±2.5)% vs. (48.7±1.4)%, P = 0.01]. PCR and Western blotting results showed that there were no statistical differences in the mRNA and protein expressions of LGR5 in the jejunal tissue between the CLP group and Sham group (Lgr5 mRNA: 0.7±0.1 vs. 1.0±0.2, P = 0.11; LGR5/β-actin: 0.83±0.17 vs. 0.68±0.19, P = 0.24). The mRNA (0.4±0.1 vs. 1.0±0.1, P < 0.01) and protein (U/g: 47.3±6.0 vs. 73.1±15.3, P < 0.01) levels of IAP in the jejunal tissue were lower in CLP group. Immunofluorescence saining analysis showed that the expressions of LYZ1 and MUC2 in the CLP group were lower than those in the Sham group. ②Cell experiment: PCR and Western blotting results showed that there was no significant difference in the expression of LGR5 between the LPS group and the blank control group (Lgr5 mRNA: 0.9±0.1 vs. 1.0±0.2, P = 0.33; LGR5/β-actin: 0.71±0.18 vs. 0.69±0.04, P = 0.81). The proliferation rate of IEC6 cells in the LPS group was lower than that in the blank control group, but there was no significant difference [positivity rate of EdU: (40.5±3.8)% vs. (46.5±3.6)%, P = 0.11]. The mRNA (0.5±0.1 vs. 1.0±0.2, P < 0.01) and protein (U/g: 15.0±4.0 vs. 41.2±10.4, P < 0.01) of IAP in the LPS group were lower than those in the blank control group. Conclusion:CLP-induced sepsis inhibits the proliferation and differentiation of intestinal epithelial cells, impairing the self-renewal ability of intestinal epithelium.
9.Sema3A secreted by sensory nerve induces bone formation under mechanical loads.

Bone formation and deposition are initiated by sensory nerve infiltration in adaptive bone remodeling. Here, we focused on the role of Semaphorin 3A (Sema3A), expressed by sensory nerves, in mechanical loads-induced bone formation and nerve withdrawal using orthodontic tooth movement (OTM) model. Firstly, bone formation was activated after the 3rd day of OTM, coinciding with a decrease in sensory nerves and an increase in pain threshold. Sema3A, rather than nerve growth factor (NGF), highly expressed in both trigeminal ganglion and the axons of periodontal ligament following the 3rd day of OTM. Moreover, in vitro mechanical loads upregulated Sema3A in neurons instead of in human periodontal ligament cells (hPDLCs) within 24 hours. Furthermore, exogenous Sema3A restored the suppressed alveolar bone formation and the osteogenic differentiation of hPDLCs induced by mechanical overload. Mechanistically, Sema3A prevented overstretching of F-actin induced by mechanical overload through ROCK2 pathway, maintaining mitochondrial dynamics as mitochondrial fusion. Therefore, Sema3A exhibits dual therapeutic effects in mechanical loads-induced bone formation, both as a pain-sensitive analgesic and a positive regulator for bone formation.
