1.Potential target genes for spondylolisthesis:drugable genome analysis based on the European population-based biodatabase

BACKGROUND:Spondylolisthesis is a common disease,and there is a lack of effective drugs to treat it.There is still a need to further define the pathogenesis and screen out more suitable therapeutic targets for spondylolisthesis.Mendelian randomization analysis can be used to explore the drugable genes associated with spondylolisthesis and provide valuable guidance for the development of more effective and targeted therapeutic drugs.OBJECTIVE:To explore potential therapeutic targets and effective drugs for spondylolisthesis by means of pharmaceutically available genome-wide Mendelian randomization analysis.METHODS:Using the Finnish database,eQTLGen consortium,drug signature database,drug-gene interaction database,protein-protein interaction database,organic small molecule biological activity database and protein structure database,which contains genome and health information of half a million Finns,data on druggable genes were subjected to two-sample Mendelian randomization analysis and co-localization analysis with data from genome-wide association studies of spondylolisthesis to identify genes highly associated with spondylolisthesis.In addition,GO and KEGG enrichment analysis,protein network construction,drug prediction and molecular docking were performed to provide valuable guidance for the development of more effective and targeted therapeutic agents.RESULTS AND CONCLUSION:In this study,we identified 34 potential drug target genes that were significantly associated with spondylolisthesis,particularly the gene APOBEC3G.This gene showed a significant association with spondylolisthesis outcomes through Mendelian analysis and co-localization analysis,suggesting that APOBEC3G may be a priority therapeutic target.As for other potential mechanisms and drugs,we still need to conduct more in-depth research to determine their roles.This study used a database from a European population,which can be used as a reference for the study of population genetics in China.

3.Analyses of risk factors for mortality in patients with Klebsiella pneumoniae bloodstream infection at a tertiary hospital in Hangzhou from 2018 to 2024

ObjectiveTo explore the 28-day mortality risk of Klebsiella pneumoniae (KP) bloodstream infections (BSI) and its related influencing factors, thereby providing a scientific basis for the effective control of KP-BSI and improvement of patient prognosis. MethodsFrom January 2018 to December 2024, a retrospective review was conducted on hospitalized patients aged >18 years old treated for KP-BSI at a tertiary hospital in Hangzhou. Logistic and Cox regression analyses were performed to identify the epidemiological characteristics and the risk factors for the 28-day mortality associated with KP-BSI. ResultsA total of 123 patients with KP-BSI were included in this study, comprising 64 cases infected with carbapenem-resistant Klebsiella pneumoniae (CRKP) and 59 cases infected with carbapenem-susceptible Klebsiella pneumoniae (CSKP). Compared with CSKP-BSI, patients with CRKP-BSI more frequently presented with chronic pulmonary disease (χ²=4.29, P=0.038), concomitant infections at other sites (χ²=10.90, P=0.001), and a higher frequency of invasive procedures prior to infection (central venous catheterization, mechanical ventilation, and indwelling urinary catheter), as well as glucocorticoid use, hemodialysis, and blood transfusion (all P<0.05). The 28-day mortality was significantly higher in BSI cases caused by CRKP compared to that caused by CSKP (37.50% vs 5.08%, P<0.001). Cox regression analyses revealed that carbapenem resistance (HR=6.67, 95%CI: 1.48‒30.08, P=0.014) and blood transfusion (HR=3.58, 95%CI: 1.15‒11.19, P=0.028) were risk factors for the 28-day mortality in KP-BSI, while removal of central venous catheters after infection (HR=0.24, 95%CI: 0.08‒0.67, P=0.006) was associated with a reduced risk for the 28-day mortality. ConclusionCarbapenem resistance is associated with mortality outcomes in patients with KP-BSI. Strengthening infection control measures targeting the identified risk factors for CRKP-BSI may improve patient prognosis.
4.Hydraulic sinus floor elevation for clinical efficacy: a systematic review and Meta-analysis

Objective The present study aimed to evaluate the clinical efficacy of hydraulic sinus floor elevation for the treatment of insufficient bone height in the posterior maxilla through a systematic review and Meta-analysis, so as to provide evidence for clinical practice. Methods Databases including PubMed, Embase, Cochrane Library, and Clinical Trials.gov were searched up to May 2025. Randomized controlled trials, non-randomized controlled trials, cohort studies, cross-sectional studies, and case-control studies that involved patients receiving hydraulic sinus floor elevation with simultaneous or delayed implant placement were included. Exclusion criteria comprised in vitro studies, animal experiments, narrative reviews, unpublished data, commentaries or expert opinions, studies with unclear sample sizes, and publications without full-text access or unavailable outcome data. Three investigators independently performed literature screening and quality assessment. A single-arm Meta-analysis was conducted using Stata 18.0 for studies meeting the inclusion criteria. The outcome measures were the incidence of sinus membrane perforation, implant survival rate, and endo-sinus bone gain. The Cochrane Risk of Bias 2 (RoB2) tool and the Joanna Briggs Institute (JBI) critical appraisal checklist were used to assess the methodological quality of randomized controlled trials and non-randomized controlled trials, respectively. The overall certainty of evidence was assessed using the GRADE(Grading of Recommendations Assessment, Development and Evaluation)system. Results Nineteen studies with 982 patients, 1 151 elevated sites, and 1 263 implants were finally included. The Meta-analysis showed that the intraoperative sinus membrane perforation rate of hydraulic sinus floor elevation was 2.0% [ES = 0.02, 95% CI (0.00, 0.04)] (18 studies, 1 061 sites); the implant survival rate was 99.0% [ES = 0.99, 95% CI (0.97, 1.00)] (17 studies, 1 253 implants); and the mean vertical bone gain was 7.80 mm. For this outcome, heterogeneity among studies was extremely high (I2 = 98.6%), thus only descriptive analysis was performed. Egger's test revealed no significant publication bias for the three outcomes. GRADE grading indicated very low certainty of evidence for mucosal perforation, implant survival rate, and endo sinus bone gain. Conclusion Current preliminary evidence suggests that hydraulic sinus lift may be a safe minimally invasive technique, but the evidence is very low, and more high quality, long term follow up randomized controlled clinical trials are still needed for further verification.

Operation management is an important tool to promote the high-quality development of public hospitals in China.Since the founding of New China,based on China's economic and social development and medical and health system reform,the evolution of operation management of public hospitals in China can be divided into four stages,the system building stage(1949-1978),the liberalization and revitalization stage(1979-1996),the operation mechanism reform stage(1997-2020),and the new stage of high-quality development(2021 present).The development trend of public hospital operation management in future should deepen the public welfare-oriented public hospital operation management,explore the value-oriented medical operation management model based on high-quality development and the refined operation and management model of public hospitals,so as to promote the high-quality development of public hospitals in China.
6.Association of preoperative plasma fibrinogen levels with adverse outcomes 1 year after endovascular revascularization in diabetes complicated with lower extremity arteriosclerosis obliterans

Objective To explore the impact of preoperative fibrinogen levels on the 1-year adverse outcomes after endovascular revascularization in patients with diabetes complicated with lower extremity arteriosclerosis obliterans(LEASO).Methods We collected the baseline clinical data of 289 patients with diabetes complicated with LEASO,who were admitted to The First Affiliated Hospital of Xi'an Jiaotong University from May 2020 to December 2022 for endovascular revascularization.All patients were followed up for 13 to 24 months after interventional therapy,with the follow-up information including major adverse cardiovascular events(MACEs)such as all-cause death,acute myocardial infarction and acute stroke,as well as major adverse lower extremity events(MALEs)such as rest pain in the lower extremities,ulcers or skin defects,gangrene,reocclusion and amputation.A multivariable Cox regression model was used to analyze the related risk factors for adverse outcomes 1 year after endovascular revascularization in patients with diabetes complicated with LEASO,and receiver operating characteristic(ROC)curves were constructed to evaluate the predictive efficacy and optimal cutoff value of fibrinogen levels for endpoint events,and Kaplan-Meier survival curves were drawn.Sensitivity analysis was made to assess the differences in the impact of fibrinogen on endpoint events across various subgroups.Results We recruited a total of 289 patients(55 patients in MACEs and 234 in non-MACEs;68 patients in MALEs and 221 in non-MALEs),with a mean age of 67.6±9.3 years,including 215 males.Multivariate Cox regression analysis showed that elevated plasma fibrinogen was an independent risk factor for MACEs(HR=1.250,95%CI:1.053-1.484,P=0.011)and all-cause death(HR=1.297,95%CI:1.030-1.633,P=0.027)in the cohort followed up 1 year after interventional therapy,but had no significant impact on the occurrence of MALEs(P=0.625).Baseline plasma fibrinogen level 4.32 g/L was the optimal cutoff value for predicting MACEs(sensitivity=0.673,95%CI:0.582-0.767;specificity=0.688,95%CI:0.562-0.775)and all-cause death(sensitivity=0.679,95%CI:0.483-0.880;specificity=0.651,95%CI:0.465-0.755).The AUC for predicting MACEs and all-cause death after interventional therapy was 0.652(95%CI:0.564 2-0.739 1)and 0.619(95%CI:0.507-0.733),respectively.After a median follow-up of 14.03 months,patients with preoperative fibrinogen level ≥ 4.32 g/L had a significantly higher risk of MACEs and all-cause death compared to patients with preoperative fibrinogen<4.32 g/L(P<0.001),and there were no significant differences in different subgroups,including gender(male/female,interaction P=0.836),age(<65 years/≥65 years,interaction P=0.211),smoking status(never smoked/current or former smoker,interaction P=0.779),chronic kidney disease(yes/no,interaction P=0.360),and heart failure(yes/no,interaction P=0.114).Conclusion Preoperative plasma fibrinogen≥4.32 g/L is an effective indicator for predicting MACEs and all-cause mortality following endovascular revascularization in patients with diabetes and LEASO.

8.Donor-driven advancements in reconstructive surgery: 20 years of facial transplantation reconstruction

'Injury but not disability, disability but not useless’ is the guiding principle behind the establishment of plastic and reconstructive surgery, making it a unique specialty in modern medicine. Its development relies not only on the optimized use of donor resources but also on technological advancements and innovations in concepts. However, in the face of severe tissue damage caused by burns, trauma, and congenital deformities, traditional repair method often fall short, necessitating more effective solutions. Against this backdrop, progress in donor sourcing and interdisciplinary innovation will inject new vitality into reconstructive surgery. In this context, we take facial transplantation reconstruction as a starting point, reviewing and organizing the technological development, issues, and challenges of autologous and allogeneic facial transplantation reconstruction from the perspective of donors. This exploration aims to shed light on the future direction of this field and gain a deeper understanding of the present and future of donor-driven reconstructive surgery.
9.Analysis of clinical and endoscopic features of elderly patients with early gastric neuroendocrine tumors

Objective:To analyze the clinical and endoscopic features of elderly patients with early gastric neuroendocrine neoplasms(g-NEN)to provide insights for clinical diagnosis and treatment.Methods:Seven cases of early-stage g-NEN in elderly patients admitted to Beijing Hospital from May 2020 to October 2024 were reported.Information including patient demographics, medical history, laboratory findings, endoscopic features, and pathological results was summarized an analyzed.Meanwhile, a literature review was conducted on g-NEN cases reported domestically and internationally from the establishment of databases to April 2025, with a comparative analysis of baseline characteristics, background mucosal conditions, and pathological diagnoses.Results:A total of 7 elderly patients with early g-NEN were included, including 3 males and 4 females, with an average age of 67 years at diagnosis.Endoscopic and pathological features: endoscopic lesions were mainly located on the greater curvature of the stomach, mostly slightly elevated, with 4/7(57.1%)being single lesions and the others accompanied by multiple polyps.The size of gastric neuroendocrine tumor(g-NET) lesions ranged from 1 mm to 19 mm, with a median Kyoto gastritis score of 3.In terms of pathological grading, there were 6 cases of G1 and 1 case of gastric neuroendocrine carcinoma(g-NEC). All 7 patients underwent endoscopic submucosal dissection(ESD). The follow-up period after surgery ranged from 3 months to 4 years, and no tumor was found at the vertical and horizontal margins of all lesions on pathology.A literature review of 32 reported cases showed that 40.6%(13/32)of cases exhibited background mucosal atrophy or intestinal metaplasia, with positive rates of chromogranin A(CgA) and synapsin(Syn)reaching 95.8%(23/24) and 100.0%(25/25), respectively.Notably, elderly patients with g-NEC demonstrated aggressive invasiveness, with a metastasis rate of 62.5%(5/8).Conclusions:Elderly patients with g-NEN exhibit high heterogeneity and significant differences in invasiveness, necessitating heightened clinical vigilance.Precise management should integrate background mucosal status, Kyoto gastritis score, pathological biomarkers, and imaging techniques.Blue laser imaging with magnifying endoscopy(BLI-ME)and endoscopic ultrasonography(EUS)can further analyze the characteristics of these tumors, and endoscopic submucosal dissection(ESD)remains the primary treatment modality for early-stage g-NEN.
10.Development and validation of predictive efficacy of enhancement factors for the China-PAR assessment model based on a university employee cohort

Objective:To explore the enhancement factors for the China-PAR assessment model based on a university employee cohort and validate its predictive efficacy.Methods:In this retrospective cohort study, 423 employees from a university in Beijing who underwent health checks at Peking University Third Hospital from 2019 to 2022 were enrolled. General demographics, physical examination data, biochemical indicators, and carotid artery ultrasound results were collected. The China-PAR model was used to predict initial atherosclerosis risk in those subjects. Univariate and multivariate analyses of carotid atherosclerosis progression were used to identify enhancement factors for atherosclerosis. Using carotid atherosclerosis progression during follow-up as the endpoint, the ROC curve was employed to compare the predictive efficacy of the China-PAR model for the risk of atherosclerosis before and after the incorporation of the enhancement factors.Results:Among the 423 employees included in the analysis, there were 224 males and 199 females, with a baseline age of (56.2±11.8) years. The proportions of carotid atherosclerosis progression were significantly higher in populations with hyperuricemia, hyperhomocysteinemia, body mass index≥28 kg/m2, waist-to-height ratio>0.5, and low-density lipoprotein cholesterol (LDL-C)≥3.4 mmol/L than those in patients without these health issues (66.0% vs 55.3%, 68.0% vs 56.2%, 72.1% vs 56.6%, 64.5% vs 54.2%, 80.5% vs 56.8%, respectively) (all P<0.05). Hyperhomocysteinemia ( OR=1.833, 95% CI: 1.076-3.123) and LDL-C≥3.4 mmol/L ( OR=3.891, 95% CI: 1.600-9.116) were positively correlated with carotid atherosclerosis progression (all P<0.05) and could be considered as enhancement factors for atherosclerosis. Using carotid atherosclerosis progression during follow-up as the endpoint event, the AUC of the China-PAR model after the incorporation of the enhancement factors was higher than that before incorporation (0.743 vs 0.684) ( P<0.05). The C-statistics of the China-PAR model before and after the incorporation of the enhancement factors was 0.702 and 0.757, respectively, with a net reclassification index of 0.090. Conclusion:Using carotid atherosclerosis progression as the follow-up endpoint, the China-PAR model incorporating hyperhomocysteinemia and LDL-C≥3.4 mmol/L as enhancement factors can better predict the risk of atherosclerosis among university employees.
