1.Patent analysis of key technologies for mRNA vaccines
Chinese Journal of Biologicals 2025;38(07):882-889
As a major breakthrough in biomedical field, mRNA vaccine technology has broad application prospects in the prevention and treatment of infectious diseases and tumor treatment. However, mRNA vaccines are a patent-intensive industry, and related core technology patents are mainly controlled by foreign companies. By analyzing the global patent application situation of key technology branches of mRNA vaccines and the differences between domestic and foreign enterprises' products and technology platforms, this paper constructs a patent barrier index model covering patent layout density and claim intensity, and systematically combs the technology competition pattern combined with patent dispute cases of leading enterprises, in order to provide ideas for domestic enterprises to break through patent barriers, optimize research and development paths and risk prevention and control strategies.

BACKGROUND:
To evaluate the efficacy and safety of the first cohort of people in China treated with a responsive neurostimulation system (Epilcure TM , GenLight MedTech, Hangzhou, China) for focal drug-resistant epilepsy in this study.
METHODS:
This multicenter, before-and-after self-controlled study was conducted across 8 centers from March 2022 to June 2023, involving patients with drug-resistant epilepsy who were undergoing responsive neurostimulation (RNS). The study was based on an ongoing multi-center, single-blind, randomized controlled study. Efficacy was assessed through metrics including median seizure count, seizure frequency reduction (SFR), and response rate. Multivariable linear regression analysis was conducted to explore the relationships of basic clinical factors and intracranial electrophysiological characteristics with SFR. The postoperative quality of life, cognitive function, depression, and anxiety were evaluated as well.
RESULTS:
The follow-up period for the 19 participants was 10.7 ± 3.4 months. Seizure counts decreased significantly 6 months after device activation, with median SFR of 48% at the 6th month (M6) and 58% at M12 ( P <0.05). The average response rate after 13 months of treatment was 42%, with 21% ( n = 4) of the participants achieving seizure freedom. Patients who have previously undergone resective surgery appear to achieve better therapeutic outcomes at M11, M12 and M13 ( β <0, P <0.05). No statistically significant differences were observed in patients' scores of quality of life, cognition, depression and anxiety following stimulation when compared to baseline measurements. No serious adverse events related to the devices were observed.
CONCLUSIONS:
The preliminary findings suggest that Epilcure TM exhibits promising therapeutic potential in reducing the frequency of epileptic seizures. However, to further validate its efficacy, larger-scale randomized controlled trials are required.

Journal of Interventional Radiology 2025;34(10):1043-1047
With the development of medical technology and interventional equipment, neurointervention has become an important treatment method for cerebrovascular diseases.Intravascular stenting plays a crucial role in the treatment of cerebral artery stenosis and intracranial aneurysms, and its clinical efficacy and safety are constantly improving. However,as the neurointervention enters the mature stage, the long-term effect of the interventional materials placed within the brain and the problem of the non-degradable nature of the stent material have got obvious concern in clinical practice. The application of biodegradable materials holds promise for solving this dilemma,as their degradation products can be absorbed or excreted by the body. The development history of the biodegradable stents shows that biodegradable stents began to be studied as early as in the 1980s.Due to the features of intracranial arteries, the stents used for intracranial stenosis have to meet higher clinical requirements in terms of flexibility, precision and the influence of degradation products, which are still studied by animal experiment and preliminary clinical research.Flow-diverter devises have already achieved definite clinical effect.Due to the disadvantages of conventional metal stents, such as artifacts, which have a long-term influence on the imaging quality, the degradable flow-diverter devices have become a current research hot spot, the materials include metal,metal-polymer mixture, polymer, etc.Researchers are still exploring the material selection, control of degradation time and optimization of pore density. In conclusion, biodegradable stents used for intracranial stenosis and biodegradable flow-diverter devices have broad application prospects, although more studies are need to be conducted before biodegradable stents can be widely used in clinical practice.

Objective:To investigate the application effect of nylon rope loop closure in endoscopic full-thickness resection (EFTR) for patients with gastric fundus stromal tumor. Methods:A retrospective cohort study was conducted. A total of 98 patients with gastric fundus stromal tumor admitted to the Second People's Hospital of Kunshan from January 2019 to June 2023 were selected, of which 47 were treated with EFTR (the control group) and 51 were treated with nylon rope closure combined with EFTR (the observation group). The operation related index, nutritional status index, immune index, complications and the recurrence rate of 1-year follow-up were compared between the 2 groups. Results:There were no statistically significant differences in gender, age, tumor diameter, the disease duration and body mass index between the observation group and the control group (all P > 0.05). The operation time [(50±5) min vs. (70±6) min], hospital stay [(6.8±1.0) d vs. (8.6±1.7) d] and anal exhaust time [(1.05±0.21) d vs. (1.39±0.23) d] in the observation group were all shorter than those in the control group; the amount of blood loss in the observation group was lower than that of the control group [(45±5) ml vs. (80±6) ml], and the differences were statistically significant ( t values were 18.28, 6.31, 7.65, 29.28, respectively, all P < 0.05). The levels of nutritional indexes prealbumin, transferrin and albumin in the observation group were higher than those in the control group 3 months after surgery (all P < 0.05); immune indexes CD4 + level and CD4 +/CD8 + in the observation group were lower than those in the control group 3 months after surgery, and the level of CD8 + in the observation group was higher than that in the control group, and the differences were statistically significant (all P < 0.05). In the observation group, 1 case had fever and 1 case had peritonitis, while in the control group, 4 cases had fever, 2 cases had delayed hemorrhage, 2 cases had peritonitis and 1 case had delayed perforation. The incidence of complications in the observation group was lower than that in the control group [3.92% (2/51) vs. 19.15% (9/47)], and the difference was statistically significant ( χ2 = 5.69, P = 0.017). After 1-year follow-up, the recurrence rate in the observation group was lower than that in the control group [1.96% (1/51) vs.12.77% (6/47)], and the difference was statistically significant ( χ2 = 4.31, P = 0.038). Conclusions:Nylon rope loop closure can accelerate the recovery, improve the immune and nutritional status, and reduce the incidence of complications and recurrence of patients with gastric fundus stromal tumor in EFTR.

Aim Systematic evolution of ligands by exponential enrichment(SELEX)techniquewas employed to screen and identify nucleic acid aptamers that specifically bind to mouse atherosclerotic pathological tissues, aiming to pro-vide a research foundation for the development of molecular targets and diagnostic reagents for early atherosclerosis. Methods A single-stranded DNA(ssDNA)library with a capacity of 1015~1016 was constructed, which was then subjec-ted to binding-elution(negative selection)with normal mouse vascular tissue slices. The eluted library was subsequently bound to atherosclerotic tissue slices for binding-elution(positive selection).PCR was used to amplify the positive and negative screening products, and agarose gel electrophoresis was used to verify the amplified products. The ssDNA library after multiple rounds of selection was sequenced using T-A cloning and sequencing to obtain the primary structure of the nu-cleic acid aptamers, and the secondary structure was predicted using the Mfold online software. The selected nucleic acid aptamers were labeled with a FAM fluorescent group at the 5'-end and were bound to both positive and negative selection tissue slices, with fluorescence intensity observed under a fluorescence microscope.Image Pro Plus 6.0 was used to cal-culate the relative average fluorescence intensity to evaluate the binding specificity of nucleic acid aptamers. Results After 8 rounds of selection, agarose gel electrophoresis imaging showed PCR amplification products in the positive selection lanes, while no PCR amplification products were observed in the negative selection lanes, indicating the successful acquisi-tion of a nucleic acid aptamer library that specifically binds to atherosclerotic tissues.Five nucleic acid aptamers were i-dentified by T-A cloning and sequencing, and their predicted secondary structures all had stem-loop structures.Immuno-fluorescence staining verified that five nucleic acid aptamers had different degrees of binding with As blood vessels, and the quantitative results of the relative average fluorescence intensity showed that nucleic acid aptamer No.11 had the highest relative average fluorescence intensity value, which can be used as a candidate nucleic acid aptamer for subsequent re-search.Conclusion Specific nucleic acid aptamers that bind to atherosclerotic vesselswere successfully obtained, providing a research foundation for further screening of early molecular targets of Asand developing in vivo early diagnostic reagents.
