1.Analysis of postoperative lipid control status and influencing factors in patients undergoing coronary artery bypass grafting surgery

Objective To understand the current status of low-density lipoprotein cholesterol (LDL-C) control in patients after coronary artery bypass grafting (CABG). Methods Clinical data of patients who underwent isolated CABG in Beijing Anzhen Hospital in 2023 were collected. All patients returned to our hospital approximately one year after surgery (10-13 months) for a lipid level recheck. We analyzed their LDL-C attainment status and influencing factors. Patients were categorized into two groups based on whether their LDL-C met the target: a LDL-C attainment group and a LDL-C non-attainment group. Results This study included 1456 patients who underwent CABG, including 320 females and 1136 males, with an average age of (61.41±9.12) years. One year post-surgery, 234 patients achieved the LDL-C target, with an attainment rate of 16.07%. The proportion of patients in the LDL-C attainment group who were ultra-high risk (77.35% vs. 92.06%, P<0.001), female (16.24% vs. 23.08%, P=0.021), and those with comorbid hypertension (55.98% vs. 63.18%, P=0.038) was significantly lower than those in the LDL-C non-attainment group. Additionally, the baseline body mass index (BMI) [(25.37±3.24) kg/m2 vs. (26.03±3.56) kg/m2, P=0.017], total cholesterol levels [(3.30±0.84) mmol/L vs. (4.01±1.03) mmol/L, P<0.001], LDL-C [(1.62±0.63) mmol/L vs. (2.25±0.85) mmol/L, P<0.001], and high-density lipoprotein cholesterol [(0.98±0.26) mmol/L vs. (1.02±0.24) mmol/L, P=0.049] upon admission in the attainment group were all lower than those in the non-attainment group. Moreover, the lipid-lowering drug usage rate in the attainment group (100.00% vs. 96.24%, P=0.003) and the proportion using two types of drugs together (25.21% vs. 10.72%, P<0.001) were both higher than those in the non-attainment group, while the statin monotherapy rate was lower than that in the non-attainment group (74.79% vs. 85.19%, P<0.001). Logistic regression analysis showed that baseline BMI (OR=0.928, P=0.012) and baseline LDL-C levels (OR=0.207, P<0.001), patient cardiovascular risk stratification (OR=0.155, P<0.001) and lipid-lowering drug treatment regimen (OR=3.758, P<0.001) are significant factors affecting the LDL-C control status. Conclusion The LDL-C compliance rate of patients undergoing CABG is at a relatively low level 1 year after surgery. Patients with very high risk of atherosclerotic cardiovascular disease, high baseline LDL-C levels, and overweight or obesity should be strengthened lipid management. For these patients, the intensity of lipid-lowering drug use or combination medication should be increased upon discharge.
2.Ethical challenges and governance prospects in rehabilitation engineering

ObjectiveTo systematically review and analyze the core ethical challenges accompanying technological convergence in the field of rehabilitation engineering, and to propose a forward-looking and actionable ethical governance framework. MethodsTaking the basic principles of policies and regulations, medical ethics, and scientific and technological ethics as the theoretical foundation, and combining with the application characteristics of rehabilitation engineering technology, this paper systematically sorted out the core issues existing in the ethical review of rehabilitation engineering, and conducted targeted analyses focusing on the key points of ethical governance. ResultsThis paper systematically reviewed the core ethical challenges arising from technological convergence in rehabilitation engineering, encompassing dilemmas of identity and autonomy, inequitable resource allocation and biases, data privacy and security risks, and accountability attribution difficulties in multi-stakeholder collaboration. It further proposed targeted governance recommendations along three dimensions: deepening ethical principles, improving review mechanisms and building capacity among diverse stakeholders. ConclusionRehabilitation engineering, leveraging artificial intelligence and robotics offers novel rehabilitation pathways for individuals with functional impairments, and has substantially improved therapeutic outcomes. Nevertheless, its widespread application is accompanied by ethical challenges, including privacy breaches, difficulties in liability attribution, inequitable service access, and emotional dependency. Governance pathways should be established around the refinement of ethical principles, the formulation of specific regulatory frameworks, and the cultivation of ethical competence among diverse stakeholders. Ethical considerations must be embedded throughout the entire lifecycle of technology research and development, clinical translation and deployment. Through the coordinated advancement of policy, technology and humanistic values, a balance can be struck between technological innovation and the protection of human rights, thereby fostering the beneficent and sustainable development of rehabilitation engineering.
3.Expression of IP3R2 and RYR2 mediated Ca2+signals in a mouse model of delayed encephalopathy after acute carbon monoxide poisoning

BACKGROUND:Ca2+expression in astrocytes has been found to be closely related to cognitive function,and the Ca2+signaling pathway regulated by inositol 1,4,5-trisphosphate receptors(IP3R2)and ryanodine receptor(RYR)2 receptors has become a hot spot in the study of cognitive disorder-related diseases. OBJECTIVE:To investigate the expression of Ca2+signals mediated by IP3R2 and RYR2 in hippocampal astrocytes in animal models of delayed encephalopathy after acute carbon monoxide poisoning,and to explore the possible pathogenesis of delayed encephalopathy after acute carbon monoxide poisoning. METHODS:C57BL mice with qualified cognitive function were selected by Morris water maze experiment and randomly divided into control group and experimental group.An animal model of delayed encephalopathy after acute carbon monoxide poisoning was established by static carbon monoxide inhalation in the experimental group,and the same amount of air was inhaled in the control group.Behavioral and neuronal changes,astrocyte specific marker glial fibrillary acidic protein,IP3R2,RYR2 receptor and Ca2+concentration in astrocytes of the two groups were detected using Morris water maze,hematoxylin-eosin staining,western blot,immunofluorescence double labeling and Ca2+fluorescence probe at 21 days after modeling. RESULTS AND CONCLUSION:In the Morris water maze,the escape latency of the experimental group was significantly longer than that of the control group(P<0.05).Hematoxylin-eosin staining results showed that in the experimental group,the number of hippocampal pyramidal cells decreased,the cell structure was disordered,and the nucleus was broken and dissolved.Immunofluorescence results showed that IP3R2 and RYR2 were co-expressed with glial fibrillary acidic protein in the hippocampus,and the expressions of IP3R2,RYR2 and glial fibrillary acidic protein were up-regulated in the hippocampus of the experimental group(P<0.05).Western blot analysis showed that the expressions of IP3R2,RYR2,and glial fibrillary acidic protein in the hippocampus of the experimental group were increased(P<0.05).Ca2+concentration in hippocampal astrocytes increased significantly in the experimental group(P<0.05).To conclude,astrocytes may affect Ca2+signals by mediating IP3R2 and RYR2 receptors,then impair the cognitive function of mice with carbon monoxide poisoning,and eventually lead to delayed encephalopathy after acute carbon monoxide poisoning.
4.Association between mental health and muscle strength among Chinese adolescents aged 13-18

Objective:
To explore the association between mental health and muscle strength among Chinese adolescents aged 13- 18, providing a theoretical foundation and intervention strategies for mental health promotion.
Methods:
Data were obtained from the 2019 Chinese National Survey on Students Constitution and Health, including 98 631 Chinese adolescents aged 13- 18. Psychological distress was assessed by using the Kessler Psychological Distress Scale (K10), and mental well being was measured with the Warwick-Edinburgh Mental Well being Scale (WEMWBS). Based on the gender and age specific Z scores of various test items [grip strength, standing long jump, pull ups (for males), and sit ups (for females)], muscle strength index (MSI) was constructed to evaluate the comprehensive level of muscle strength in adolescents. According to the Dual factor Model (DFM) of mental health, participants were categorized into four groups:troubled, symptomatic but content, vulnerable, and complete mental health. Gender differences were analyzed by using Chi-square tests, trends were tested with Cochran-Armitage tests, and multinomial Logistic regression models were applied to assess associations between muscle strength and mental health among adolescents.
Results:
In 2019, 37.4% of Chinese adolescents aged 13-18 were reported of high mental distress, and 59.9% were reported of low mental well being. Boys had significantly lower rates of high mental distress (35.3%) and low mental well being (55.6%) compared to girls (39.4%, 64.3%), and the differences were of statistical significance ( χ 2=176.13, 780.42, both P <0.05). In 2019, the rate of complete mental health among adolescents showed a downward trend with increasing age ( χ 2 trend = 258.47) and a gradual upward trend with increasing muscle strength levels ( χ 2 trend =123.14),and both boys and girls exhibited similar trends ( χ 2 trend =103.83, 168.46; 57.00 , 67.34) (all P <0.05). The results of the unordered multiclass Logistic regression model showed that after controlling for confounding factors such as age and gender, when the completely pathological group as a reference, for every 1 unit increase in MSI in adolescents, the likelihood of being in a completely mental health state increased by 29% ( OR = 1.29); for every unit increase in the Z-score for pull ups, the likelihood of being in a completely mental health state increased by 6% ( OR =1.06) among boys; for every 1 unit increase in sit up Z score, the likelihood of being in a completely mental health state increased by 19% ( OR =1.19) among girls (all P <0.05).
Conclusions
The mental health status of Chinese adolescents is not good enough. Muscle strength is positively associated with mental health.
5.Evolution and development of mental health policies for children and adolescents in China

Objective:
To systematically review the development and changes in mental health policies within the National Outline for Children s Development in China from 1992 to 2030, providing a reference basis for future formulation of mental health policies among children and adolescent in China.
Methods:
Based on the four editions of the National Outline for Children s Development in China across different periods from 1992 to 2030, word frequency analysis was used to reveal shifts in policy priorities, and an internationally recognized framework for adolescent health policy analysis was applied to conduct a textual review.
Results:
Word frequency analysis revealed that the term "psychological" appeared 6 times in the National Outline for Children s Development in China (2001-2010) but increased to 20 times in the National Outline for Children s Development in China (2021-2030) (abbreviated as the National Outline of 2021), while the term "health" rose from 4 times in the National Outline for Children s Development Plan in China in the 1990s to 68 times in the National Outline of 2021. The scope of mental health policy interventions expanded to encompass five key areas:health, safety, education, welfare and legal protection. Textual analysis highlighted that the policies of the National Outline for Children s Development in China were demand driven, prioritized vulnerable groups and continuously broadened their coverage, emphasizing sustainability and appropriateness, and monitoring/evaluation mechanisms. By 2023, 42.3% of primary schools and 64.8% of secondary schools employed full time mental health education teachers. However, the National Outline for Children s Development in China lacked direct evidence of children and adolescents participation in policy formulation, and publicly available mental health data disaggregated by age and gender remained limited.
Conclusion
Mental health policies of children and adolescents in China have evolved from nonexistence to gradual refinement, yet institutionalized channels for youth involvement in policy development and evaluation remain insufficient, and transparency in age and gender specific mental health data needs improvement.
6.Mechanism of action of the nucleotide-binding oligomerization domain-like receptor protein 3 inflammasome and its regulation in liver injury.

Nucleotide-binding oligomerization domain (NOD)-like receptor protein 3 (NLRP3) is a cytosolic pattern recognition receptor that recognizes multiple pathogen-associated molecular patterns and damage-associated molecular patterns. It is a cytoplasmic immune factor that responds to cellular stress signals, and it is usually activated after infection or inflammation, forming an NLRP3 inflammasome to protect the body. Aberrant NLRP3 inflammasome activation is reportedly associated with some inflammatory diseases and metabolic diseases. Recently, there have been mounting indications that NLRP3 inflammasomes play an important role in liver injuries caused by a variety of diseases, specifically hepatic ischemia/reperfusion injury, hepatitis, and liver failure. Herein, we summarize new research pertaining to NLRP3 inflammasomes in hepatic injury, hepatitis, and liver failure. The review addresses the potential mechanisms of action of the NLRP3 inflammasome, and its regulation in these liver diseases.

7.Right ventricular-pulmonary artery connection for palliative treatment of pulmonary atresia with ventricular septal defect in children: A single-center retrospective study

Objective To compare the benefits and drawbacks of primary patch expansion versus pericardial tube right ventricular-pulmonary artery connection in patients diagnosed with pulmonary atresia with ventricular septal defect (PA/VSD). Methods A retrospective study was conducted on patients diagnosed with PA/VSD who underwent primary right ventricular-pulmonary artery connection surgery at our center between 2010 and 2020. Patients were categorized into two groups based on the type of right ventricular-pulmonary artery connection: a pericardial tube group and a patch expansion group. Clinical data and imaging findings were compared between the two groups. Results A total of 51 patients were included in the study, comprising 31 males and 20 females, with a median age of 12.57 (4.57, 49.67) months. The pericardial tube group included 19 patients with a median age of 17.17 (7.33, 49.67) months, while the patch expansion group consisted of 32 patients with a median age of 8.58 (3.57, 52.72) months. In both groups, the diameter of pulmonary artery, McGoon index, and Nakata index significantly increased after treatment (P<0.001). However, the pericardial tube group exhibited a longer extracorporeal circulation time (P<0.001). The reoperation rate was notably high, with 74.51% of patients requiring further surgical intervention, including 26 (81.25%) patients in the patch expansion group and 12 (63.16%) patients in the pericardial tube group. No statistical differences were observed in long-term cure rates or mortality between the two groups (P>0.005). Conclusion In patients with PA/VSD, both patch expansion and pericardial tube right ventricular-pulmonary artery connection serve as effective initial palliative treatment strategies that promote pulmonary vessel development and provide a favorable foundation for subsequent radical operations. However, compared to the pericardial tube approach, the patch expansion technique is simpler to perform and preserves some intrinsic potential for pulmonary artery development, making it the preferred procedure.
8.Study on Medicinal Properties of New Foreign Introducing Chinese Materia Medica Drimia maritima(L.)Stearn Based on Literature Research and Intelligent Sense

Objective To summarize the medicinal properties of new foreign introducing Chinese materia medica Drimia maritima(L.)Stearn based on literature research;To further verify its five flavors by applying intelligent sense;To provide new ideas for the medicinal properties research of new foreign introducing Chinese materia medica.Methods Literature about Drimia maritima(L.)Stearn was retrieved from CNKI,VIP,Wanfang Data,PubMed,Web of Science,and combined with the TCM theories medicinal properties of Drimia maritima(L.)Stearn was summarized.PEN3 electronic nose and SA402B electronic tongue were used to obtain intelligent sensory information of Drimia maritima(L.)Stearn,and the principal component analysis(PCA)was used to identify its five flavors.Results Literature research summarize Drimia maritima(L.)Stearn with antioxidant,anti-tumor,strengthening cardiac functions and diuresis,reducing cough and asthma and other pharmacological effects.Combined with the theories,it explored that its property was mainly slightly warm,the tastes were mainly bitter,pungent,sweet,and slightly toxic,belonging to the lung,spleen,stomach and heart meridians.Based on intelligent sense,identification results of five tastes of Drimia maritima(L.)Stearn were bitter,slightly pungent,slightly sweet,salty,non-acid,basically consistent with the literature research.Conclusion Literature research combined with intelligent sense can better summarize and recognize the medicinal properties of Drimia maritima(L.)Stearn.This study can provide a certain reference for a more objective and adequate medicinal properties analysis of new foreign introducing Chinese materia medica.
9.Directional atherectomy combined with drug-coated balloon versus bare-mental stent for elderly femoropopliteal artery disease

Objective To compare the safety and efficacy of directional atherectomy with anti-restenosis therapy(DAART,drug-coated balloon)versus conventional balloon angioplasty(bare-metal stent,BMS)in elderly patients with femoropopliteal artery disease.Methods A retrospec-tive cohort study was conducted on 116 elderly patients undergoing endovascular intervention due to femoropopliteal artery disease in our hospital between May 2016 and September 2019,divided into DAART group(57 cases)and BMS group(59 cases).Results No statistical differences were observed between the DAART and BMS groups in terms of age,risk factors,distribution of Ruth-erford classification,lesion length,lesion type,lesion location,Global Limb Anatomic Staging Sys-tem grade,infra-popliteal runoff status,or preoperative ankle-brachial index(P>0.05).However,the BMS group had significantly larger proportion of chronic limb-threatening ischemia than the other group(P<0.05).Both groups achieved a 100%success rate of surgery.The DAART group obtained obviously higher primary patency rates at 1 and 2 years than the BMS group(90.4%vs 75.0%,76.3%vs 57.3%;P<0.045).There were no significant differences in the rate of freedom from target lesion revascularization at 1 and 2 years between the two group(96.1%vs 88.8%,91.6%vs 77.7%;P>0.05).In 2 years of follow-up,the incidence of major adverse events was 10.0%(5 cases)in the DAART group and 20.8%(12 cases)in the BMS group,but no obvious difference(P>0.05).Conclusion DAART demonstrates superior mid-term efficacy than BMS in treating femoropopliteal artery disease in elderly patients.However,comprehensive preoperative assessment is essential to optimize individualized treatment strategies for this population.
10.Exploration on the Mechanism of Chaipo Decoction in Intervening Acute Lung Injury Based on Network Pharmacology and Experimental Verification

Objective To explore the mechanism of Chaipo Decoction in intervening lipopolysaccharide(LPS)-induced acute lung injury(ALI)based on network pharmacology and experimental verification.Methods TCMSP database was used to screen the active components and targets of Chaipo Decoction.ALI disease targets were retrieved through GeneCards and OMIM databases,the intersection genes of drug and disease were obtained.They were imported into STRING platform and Cytoscape 3.9.1 software to construct protein interaction network and drug-active components-target network,core components and targets were screened,and GO and KEGG pathway enrichment analysis was performed on them.AutoDock 1.5.6 and PyMOL software were used to verify the molecular docking between the main active components and core targets.The ALI model of mice was induced by LPS,and was intervened by Chaipo Decoction.The pathological changes of lung tissue in mice were observed by HE staining.The contents of IL-6,IL-10 and TNF-α in serum of mice were detected by ELISA.Western blot was used to verify the expressions of target proteins in lung tissue.Results Totally 214 active components of Chaipo Decoction,such as norwogonin,baicalein,quercetin and ursolic acid were screened,with 1 101 targets and 271 potential targets for intervention in ALI,mainly SRC,STAT3,AKT1,EGFR,GRB2,PIK3CA,etc.It mainly affected PI3K/Akt signaling pathway,MAPK signaling pathway,apoptosis,Ras signaling pathway,etc.Molecular docking showed that the main active components of Chaipo Decoction had strong binding ability with core targets SRC,STAT3,AKT1,EGFR and GRB2.The results of animal experiments showed that Chaipo Decoction could alleviate the lung lesions in ALI mice,decrease the contents of IL-6 and TNF-α in serum,increase the content of IL-10,and significantly decreased the expressions of EGFR,PI3K,AKT and NF-κB p65 protein in lung tissue.Conclusion Chaipo Decoction can exert anti-ALI effect through multiple pathways and multiple targets,and its mechanism may be related to the inhibition of EGFR/PI3K/Akt signaling pathway.
