1.Discovery and investigation of six polio vaccine derived viruses in Guangzhou City

Objective To understand the surveillance situation of poliovirus in Guangzhou from 2011 to 2024, and to further strengthen polio surveillance and ensure the continued maintenance of a polio-free status. Methods An analysis was conducted on the discovery and investigation results of six cases of vaccine-derived poliovirus (VDPV) detected in Guangzhou. Results A total of 6 VDPV incidents were reported in Guangzhou from 2011 to June 2024, among which 5 incidents were from sewage sample testing in the Liede Sewage Treatment Plant in Guangzhou, all of which were confirmed as VDPV, with 1 for type I, 1 for type II, and 3 for type III. In addition, one confirmed HFMD case was identified as a type VDPV II carrier. No presence of any wild poliovirus (WPV), VDPV cases, or circulating VDPV (cVDPV) was reported. Conclusion Guangzhou City has maintained a high level of vigilance and effectiveness in the monitoring and prevention of polio. Continuously strengthening the construction of the polio monitoring network, optimizing vaccination strategies, and comprehensively improving public health awareness are still the focus of the prevention and control work in the future.
2.Factors related to type 2 diabetes mellitus with frailty in the elderly

Objective To investigate and analyze clinical characteristics and related factors of elderly patients with type 2 diabetes mellitus (T2DM) and frailty. Methods A total of 310 elderly patients with T2DM admitted to the hospital from January 2023 to June 2024 were selected as the research subjects. Their general information and disease-related information was collected through questionnaires. The Fried Frailty Scale was used to evaluate frailty status, and the patients were divided into frailty group and non-frailty group based on the Fried Frailty Scale score. Factors related to T2DM with frailty in the elderly were analyzed. Results The incidence of frailty in this study was 31.61% (98/310), and those without frailty accounted for 68.39% (212/310). There were statistically significant differences between the two groups in terms of age, body mass index (BMI), Self-rating Depression Scale (SDS) score, number of chronic complications, glycosylated hemoglobin (HbA1c) level, hemoglobin level, Mini-Nutritional Assessment-Short Form (MNA-SF) score, and Charlson Comorbidity Index (CCI) score (P<0.05). Multivariate logistic regression analysis showed that age, HbA1c level, SDS score, MNA-SF score, and CCI score were risk factors for frailty in elderly patients with T2DM (P<0.05). Conclusion The incidence of frailty is relatively high in elderly patients with T2DM. It is influenced by factors such as age , SDS score , HbA1c level , MNA-SF score and CCI score, and deserves clinical attention.
3.Argatroban in the treatment of acute ischemic stroke:a rapid health technology assessment

Objective Rapid health technology assessment(HTA)was used to evaluate the efficacy,safety and economy of argatroban in the treatment of acute ischemic stroke,so as to provide evidence-based reference for clinical treatment decision making.Methods PubMed,Embase,Web of Science,Cochrane Library,CNKI,WanFang Data,VIP,SinoMed databases and HTA website were electronically searched to collect the HTA report,systematic review/Meta-analysis and pharmacoeconomic research of argatroban in the treatment of acute ischemic cerebral infarction from inception to February 6,2024.Two reviewers independently identified studies,extracted data,assessed the quality of included studies,and descriptively analyzed and summarised the results.Results A total of 21 articles were included,including 15 systematic reviews/Meta-analysis and 6 pharmacoeconomic studies.The analysis results of effectiveness showed that argatroban could improve the clinical effective rate,neurological deficit score compared with placebo,routine treatment,or other drugs(such as alteplase),but a few studies were inconsistent with the current results.The analysis results of safety showed that argatroban alone or in combination with other drugs did not increase the risk of systemic hemorrhage,intracranial hemorrhage,mortality and other adverse reactions compared with the control group,and had a favorable safety profile.The analysis results of pharmacoeconomic studies that argatroban combined with routine treatment or argatroban combined with urokinase had economic advantages.Conclusion Argatroban is generally effective and safe in the treatment of acute ischemic stroke,and it has economic advantages for patients to choose argatroban combined with routine treatment or urokinase treatment.
4.Impact of early nasogastric tube removal on functional recovery after total gastrectomy

Objective:To investigate the impact of early removal of nasogastric tubes on functional recovery after total gastrectomy for gastric cancer patients, to provide scientific evidence for enhanced recovery after surgery strategies in gastric cancer.Methods:A retrospective cohort study was conducted on 102 gastric cancer patients who underwent total gastrectomy at Beijing Friendship Hospital affiliated with the Capital Medical University from March 2018 to July 2022. Patients were divided into two groups based on whether the gastric tube was removed within 24 hours post-operation: the early removal group (within 24 hours, 55 patients) and the non-early removal group (beyond 24 hours, 47 patients). The recovery outcomes, including time to first flatus, time to fluid intake, length of hospital stay, and the incidence of postoperative complications, were compared between the two groups. Non-normally distributed data were expressed as M( Q1, Q3) and compared using the Wilcoxon rank-sum test. Categorical data were expressed as frequencies or percentages and compared using the chi-square test or Fisher′s exact test. To minimize the impact of potential confounders, multivariable linear regression and logistic regression analyses adjusted for propensity scores were further employed to compare quantitative and qualitative data between the groups. Statistical analyses were performed using R software. Results:The exhaust time, water intake time, and hospital stay in the early removal group were 3.0(2.0, 3.5) days, 4.0(3.0, 5.0) days, and 7.0(7.0, 8.0) days, respectively, while those in the non-early removal group were 4.0(3.0, 5.0) days, 6.0(5.0, 7.0) days, and 8.0(7.5, 11.0) days, respectively. Statistically significant differences were observed between the two groups ( P<0.05). However, there was no significant difference in the incidence of postoperative complications between the two groups (5.45% vs 17.02%, P=0.060). Propensity score-adjusted regression analysis suggested that early tube removal might reduce the risk of postoperative complications ( P=0.042). Conclusion:Early removal of nasogastric tubes can significantly accelerate functional recovery after total gastrectomy for gastric cancer patients and may reduce the risk of postoperative complications, providing important clinical guidance for enhanced recovery after surgery management in gastric cancer.
5.Changes of immunoglobulins and Th9 subset in patients with Parkinson's disease and their correlation with IGF-1 and S-100B protein

Objective:To study the changes of immunoglobulins(IgG,IgA,IgM)and helper T cell Th9 subset,and their cor-relation with insulin-like growth factor-1(IGF-1)and S-100B protein in patients with Parkinson's disease(PD).Methods:A total of 108 patients with PD confirmed in Hebei Hospital of Traditional Chinese Medicine were enrolled as study group between December 2020 and December 2022.According to disease severity,they were divided into mild group(35 cases),moderate group(44 cases)and severe group(29 cases);a total of 108 healthy adults were enrolled as control group.Levels of immunoglobulins and Th9 subset were compared between study group and control group,and levels of immunoglobulins,Th9 subset,IGF-1 and S-100B protein were compared among mild group,moderate group and severe group.Correlation between immunoglobulins,Th9 subset and IGF-1,S-100B proteins were analyzed by Pearson correlation analysis.Correlation between disease severity and all differential indexes in PD patients were analyzed by Spearman correlation analysis.Results:IgM level in study group was lower than that in control group,and which was the lowest in severe group,followed by moderate group and mild group(P<0.05).Levels of IgG,IgA,IL-9 and Th9 subset in study group were higher than those in control group,and which were the highest in severe group,followed by moderate group and mild group(P<0.05).IGF-1 level was the lowest in severe group,followed by moderate group and mild group,while level of S-100B protein was on the contrary(P<0.05).Results of Pearson correlation analysis showed that IgM level was positively correlated with IGF-1 level,while negatively correlated with level of S-100B protein.Levels of IgG,IgA,IL-9 and Th9 subset were negatively correlated with IGF-1 level,while positively correlated with level of S-100B protein(P<0.05).Results of Spearman correlation analysis showed that the disease severity was negatively correlated with IgM and IGF-1 levels,while positively correlated with levels of S-100B protein,IgG,IgA,IL-9 and Th9 subset(P<0.05).Conclusion:IgM level is decreased,while levels of IgG,IgA and Th9 subset are increased in PD patients.Changes of the above indexes are significantly correlated with IGF-1 and S-100B,which can be applied to evaluate dis-ease severity.
6.Posttraumatic growth development trajectory of first stroke patients based on latent class growth model

Objective:To explore the posttraumatic growth of first stroke patients and analyze its dynamic development trajectory.Methods:From January to December 2022, 180 stroke patients admitted to Xinxiang Central Hospital were selected as research subjects by convenience sampling. The Posttraumatic Growth Inventory (PTGI) score was dynamically evaluated at the end of the first month (M 1), second month (M 2), third month (M 3), fourth month (M 4), fifth month (M 5), and sixth month (M 6) of onset. The latent class growth model (LCGM) was used to analyze the developmental trajectory categories, and multiple Logistic regression was used to analyze the influencing factors of posttraumatic growth trajectory categories. Results:A total of 180 questionnaires were distributed, and 177 valid questionnaires were collected, with a valid response rate of 98.33% (177/180). Within six months, the patient's PTGI score gradually increased, with the lowest PTGI score (29.11±5.65) at M 1 and the highest proportion of low-level growth, accounting for 85.31% (151/177). At M 6, the PTGI score was (42.14±13.04), and the proportion of low-level growth was 40.68% (72/177). Two latent categories of posttraumatic growth, namely sustained low growth type and growth improvement type, were identified, with 72 cases of sustained low growth type and 105 cases of growth improvement type. Gender ( OR=2.648, P=0.008), age ( OR=1.055, P=0.002), education level ( OR=6.403, P<0.001), and stroke type ( OR=2.863, P=0.014) were the influencing factors for posttraumatic growth trajectories among stroke patients. Conclusions:Post traumatic growth in stroke patients is a dynamic state of change, with low overall levels and a common type of sustained low growth. There may be differences in the sociodemographic and disease characteristics of patients with different types of posttraumatic growth trajectories, providing reference for the development of clinical nursing strategies, but further research is still needed.

Multiple sclerosis(MS)is a chronic autoimmune disease of the central nervous system characterized by neuroinflammation and neurodegeneration.The pathogenesis of the disease is complex,and involves various central nervous system and peripheral cells.Single-cell sequencing has recently been increasingly applied in the study of neurological disorders,leading to important advances in our understanding of intercellular heterogeneity,disease development mechanisms,and treatment strategies.This review summarizes research progress in single-cell sequencing and its applications in MS.
8.Construction and external validation of a non-invasive pre-hospital screening model for stroke patients: a study based on artificial intelligence DeepFM algorithm

Objective:To construct a non-invasive pre-hospital screening model and early based on artificial intelligence algorithms to provide the severity of stroke in patients, provide screening, guidance and early warning for stroke patients and their families, and provide data support for clinical decision-making.Methods:A retrospective study was conducted. The clinical information of stroke patients ( n = 53?793) were extracted from the Yidu cloud big data server system of the Second Affiliated Hospital of Dalian Medical University from January 1, 2001 to July 31, 2023. Combined with the results of single factor screening and the opinions of experts with senior professional titles in neurology, the input variable was determined, and the output variable was the National Institutes of Health Stroke Scale (NIHSS) representing the severity of the disease at admission. Python 3.7 was used to build DeepFM algorithm model, and five data mining models including Logistic regression, CART decision tree, C5.0 decision tree, Bayesian network and deep neural network (DNN) were built at the same time. The original data were randomly divided into 80% training set and 20% test set, which were used to train and test the models, adjust the parameters of each model, respectively calculate the accuracy, sensitivity and F-index of the six models, carry out the comprehensive comparison and evaluation of the model. The receiver operator characteristic curve (ROC curve) and calibration curve were drawn, compared the prediction performance of DeepFM model and the other five algorithms. In addition, the data of stroke patients ( n = 1?028) were extracted from Dalian Central Hospital for external verification of the model. Results:A total of 14?015 stroke patients with complete information were selected, including 11?212 in the training set and 2?803 in the testing set. After univariate screening, 14 indicators were included to construct the model, including gender, age, recurrence, physical impairment, facial problems, speech disorders, head reactions, disturbance of consciousness, visual disorders, abnormal cough and swallowing, high risk factor, family history, smoking history and drinking history. DeepFM model adopted the two-order crossover feature. The number of hidden layers in DNN layer was 3. Dropout was used to discard the neurons in the neural network. Rule was used as the activation function. Each layer used Dense full connection. The objective function was random gradient descent. The number of iterations was 15. There were 133?922 training parameters in total. Comparing the predictive value of the six models showed that the accuracy of DeepFM model was 0.951, the sensitivity was 0.992, the specificity was 0.814, the F-index was 0.950, and the area under the curve (AUC) was 0.916. The accuracy of the other five data mining models were between 0.771-0.780, the sensitivity were between 0.978-0.987, the F-index were between 0.690-0.707, and the AUC were between 0.568-0.639. The calibration curve of the DeepFM model was more aligned with the ideal curve than the other five data mining models. Suggesting that the prediction performance of DeepFM model was the best. External validation was conducted on the DeepFM model, and its accuracy was 0.891, indicating good generalization performance of the model.Conclusion:The pre-hospital non-invasive screening prediction model based on DeepFM can accurately predict the severity grading of stroke patients, and has potential application value in rapid screening and early clinical decision-making of stroke.
9.Best evidence summary for anticoagulation management in blood purification of perioperative liver transplantation patients

Objective To search and evaluate the relevant evidence of blood purification and anticoagulant management of perioperative liver transplantation patients,and to summarize the best evidence,so as to provide evidence-based evidence for clinical blood purification and anticoagulant management practice.Methods Evidence-based questions were determined.Systematic search was conducted on relevant domestic and foreign databases and websites for clinical decisions,guidelines,evidence summaries,system evaluations,and expert consensuses related to blood purification anticoagulation management.The retrieval period is from the establishment to May 2,2023.Literature screening,quality evaluation and evidence summary were conducted by 2 researchers.Results A total of 17 articles were included,including 2 clinical decisions,3 guidelines,2 evidence summaries,4 systematic evaluations and 6 expert consensuses.24 pieces of best evidence were summarized from 6 dimensions,including coagulation function evaluation,anticoagulant strategy,mode selection,vascular access,standardized operation implementation points and personnel organization management.Conclusion The best evidence was summarized in the study on perioperative blood purification and anticoagulant management of liver transplantation,which can provide evidence-based basis for clinical medical staff to implement anticoagulant management.However,it is necessary to further carry out evidence-based practice of perioperative blood purification and anticoagulant management of liver transplantation to verify its anticoagulation effect and nursing safety in clinical practice.

11β-Hydroxylase deficiency(11β-OHD) is a relatively rare type of autosomal recessive hereditary disease congenital adrenal cortical hyperplasia. This article reports the diagnosis and treatment of a case of 11β-OHD combined with subacute thyroiditis leading to severe hypokalemia treated in our hospital, along with a review of relevant literature.
