1.Efficacy and safety of repetitive transcranial magnetic stimulation in the treatment of postpartum depression: a Meta-analysis

BackgroundPostpartum depression (PPD) is a prevalent postpartum complications that significantly compromises women's psychological and physical well-being. Repetitive transcranial magnetic stimulation (rTMS), a conventional neuromodulation technique, has been increasingly used in the treatment of PPD. However, high-quality evidence regarding its efficacy and safety remains limited. ObjectiveTo evaluate the efficacy and safety of rTMS in the treatment of PPD, thereby providing references for clinical treatment. MethodsDatabases including Cochrane Library, PubMed, Embase, CNKI, Wanfang, VIP and China Biology Medicine disc (CBM) were electronically searched for randomized controlled trials (RCTs) on rTMS for PPD, with the search spanning from database inception to February 8, 2025. Study quality was assessed using the Cochrane Handbook for Systematic Reviews of Interventions 5.0.1, and the certainty of evidence was graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Meta-analysis was conducted using RevMan 5.3 and Stata 12.0. The outcomes of the Meta-analysis included the total effective rate, Edinburgh Postnatal Depression Scale (EPDS) score, Hamilton Depression Rating Scale (HAMD) score, and adverse reactions (dizziness, headache, nausea, diarrhea, and the overall incidence of adverse reactions). ResultsA total of 11 studies involving 729 patients with PPD were included. Meta-analysis results showed that the total effective rate in the study group was significantly higher than that in the control group (OR=5.54, 95% CI: 3.07–10.01, P<0.01). Both EPDS score (SMD=-2.38, 95% CI: -3.39–-1.37, P<0.01) and HAMD score (SMD=2.53, 95% CI: 1.21–3.85, P<0.01) in the study group were significantly lower than those in the control group, with statistically significant differences. Comparisons between the study group and control group reveal no significant differences in the incidence of dizziness and headache (RR=1.47, 95% CI: 0.63–3.43, P>0.05), nausea (RR=1.46, 95% CI: 0.55–3.86, P>0.05), diarrhea (RR=0.71, 95% CI: 0.23–2.20, P>0.05), and overall adverse reactions (RR=1.30, 95% CI: 0.79–2.15, P>0.05). GRADE assessment rated the four indicators of dizziness and headache, diarrhea, overall incidence of adverse reactions, and EPDS score as "moderate-certainty evidence", and rated the total effective rate, nausea, and the HAMD score as "low-certainty evidence". ConclusionrTMS demonstrates certain therapeutic efficacy for PPD, with a safety profile comparable to conventional treatment. Biological clock is a pattern of rhythms in which organisms exhibit cycles approaching 24 h in order to adapt to periodic changes in the external environment.Gut microbiota is the sum of extremely large number of microorganisms parasitized in humans and animals.Disorders of the biological clock increase the risk of metabolic diseases,and the occurrence of these disorders is closely related to the interaction with the dysbiosis of the gut microbiota.With the deepening of the understanding of the brain-gut axis,targeting gut microbiota to ameliorate biological clock disorders and associated diseases has achieved major theoretical breakthroughs and was partially applied in clinical practice,including dietary interventions such as time restricted feeding,development of microbial-related products(probiotics and postbiotics),and fecal microbiota transplantation.This article reviews the research progress of intestinal flora and rhythmicity,as well as targeting intestinal flora for the treatment of biological clock disorders,so as to provide new ideas for the prevention and treatment of rhythm disorders and concurrent diseases.
3.Construction and evaluation of hepatocellular carcinoma models in mice with different immune microenvironments

Objective To construct mice hepatocellular carcinoma models with different tumor immune microenvironments(TIME)and explore the differences.Methods H22 and hepa1-6 were used to construct subcutaneous transplantation tumor model of C57 mice as homologous hepatocellular carcinoma cell lines(denoted as H22 group and hepal-6 group,each n=8),and the differences of TIME were evaluated.Immunohistochemistry was used to detect and quantify the infiltration of T cells,CD4+T cells,CD8+T cells,regulatory T cells and B cells in TIME.Flow cytometry was performed to detect the differences of composition of immune cell subpopulations in peripheral blood and tumor parenchyma.Gene expression profile characteristics of tumor tissue were analyzed based on high-throughput transcriptome sequencing technology,and enrichment analyses of immune-related signaling pathways were evaluated combined with gene ontology(GO)and Kyoto encyclopedia of genes and genomes(KEGG).Results H22 group showed cold and hepa1-6 group showed hot TIME characteristics.The number of T cells,CD4+T cells and CD8+T cells in tumor tissue of H22 group were all lower,while the proportion of T cells,CD4+T cells and CD8+T cells in peripheral blood were all higher than those of hepa1-6 group(all P<0.05).Compared with H22 group,up-regulated genes of tumor tissue in hepa1-6 group expressed significantly enriched in tumor immune activation-related signaling pathways.Conclusion H22 and hepa1-6 hepatocellular carcinoma models showed distinct TIME characteristics of cold and hot tumors,respectively,and the amount of immune cells in tumor tissue of the former were significantly lower than those in the latter.

5.The prevalence, awareness, treatment and control rates of diabetes among residents in Fangshan District

Objective:
To investigate the prevalence, awareness, treatment and control rates of diabetes among residents in Fangshan District, Bejing Municipality, so as to provide reference for strengthening the prevention and control of diabetes.
Methods:
The residents aged 18 to 79 years were selected in Fangshan District using multistage stratified cluster sampling method from March to June 2023. Demographic information, lifestyle, body mass index, fasting blood glucose, and blood lipid were collected through questionnaire surveys, physical examinations and laboratory tests. The prevalence, awareness, treatment and control rates of diabetes among residents were analyzed and their influencing factors were identified using a multivariable logistic regression model.
Results:
A total of 4 438 residents were enrolled, including 2 367 males (53.33%) and 2 071 females (46.67%), and had a mean age of (44.99±14.90) years. The prevalence, awareness, treatment and control rates of diabetes were 15.75%, 57.37%, 56.22% and 22.90%, respectively. Multivariable logistic regression analysis showed that age (aged 30 to 39 years, OR=1.624, 95%CI: 1.011-2.609; aged 40 to 49 years, OR=3.469, 95%CI: 2.163-5.561; aged 50 to 59 years, OR=4.202, 95%CI: 2.595-6.803; aged 60 to 69 years, OR=7.229, 95%CI: 4.406-11.862; aged 70 to 79 years, OR=7.929, 95%CI: 4.665-13.479), obesity (OR=1.368, 95%CI: 1.071-1.748), hypertension (OR=1.943, 95%CI: 1.601-2.359) and dyslipidemia (OR=2.754, 95%CI: 2.287-3.317) were the factors affecting the prevalence of diabetes. Sex (women, OR=1.529, 95%CI: 1.019-2.293), age (aged 40 to 49 years, OR=4.971, 95%CI: 1.681-14.698; aged 50 to 59 years, OR=6.070, 95%CI: 2.045-18.020; aged 60 to 69 years, OR=6.902, 95%CI: 2.287-20.832; aged 70 to 79 years, OR=6.954, 95%CI: 2.191-22.070) and hypertension (OR=1.906, 95%CI: 1.337-2.718) were the factors affecting the awareness of diabetes. Age (aged 40 to 49 years, OR=4.699, 95%CI: 1.594-13.855; aged 50 to 59 years, OR=5.990, 95%CI: 2.023-17.732; aged 60 to 69 years, OR=7.134, 95%CI: 2.370-21.470; aged 70 to 79 years, OR=6.639, 95%CI: 2.102-20.964) and hypertension (OR=1.857, 95%CI: 1.304-2.645) were the factors affecting the treatment of diabetes. Age (aged 40 to 49 years, OR=2.948, 95%CI: 1.349-6.438; aged 60 to 69 years, OR=2.344, 95%CI: 1.049-5.234) was the factor affecting the control of diabetes.
Conclusions
The prevalence rate of diabetes among residents in Fangshan District was high, and the improvements of awareness rate, treatment rate and control rate were not obvious. Health education and management of diabetes should be strengthened for elderly, hypertensive and dyslipidemic residents.
6.Clinical Characteristics and Potential Risk Factors Analysis of Liver Injury Related to Epimedii Folium Preparation

ObjectiveThis paper aims to analyze the clinical characteristics and medication rationality of liver injury related to Epimedii Folium preparation (EP) and explore the possible risk factors of liver injury, so as to provide a reference for the safe clinical application of Epimedii Folium (EF). MethodA retrospective analysis was conducted on liver injury cases related to EP from 2012 to 2016. ResultThe number of reported liver injury cases and the proportion of severe cases related to the use of EP show an increasing trend, indicating the objective existence of liver injury caused by EP. There are more cases of liver injury related to EP in women than in men, with an onset age range of 15-91 years old and a median onset age of 60 years old (median onset ages for men and women are 59 and 60 years old, respectively). The time span from taking EP alone to the occurrence of liver injury is 1-386 days, with a median of 38 days. The time span from taking both EP and Western medicine to the occurrence of liver injury is 1-794 days, with a median of 34 days. EF-related liver injury preparations are mostly composed of traditional Chinese medicines that promote immunity and tonify the liver and kidney, indicating that immune stress in the body may be the mechanism of liver injury caused by the use of EP alone or in combination. There is no increasing trend of toxicity with time or dose in the liver injury caused by EP. By further exploring its risk factors, it is found that patients have unreasonable medication methods such as excessive dosage, repeated use, and multi-drug combination, which may also be one of the important risk factors for EF-related liver injury. ConclusionEP has a certain risk of liver injury and should be emphasized in clinical diagnosis and treatment. Immune stress may be the mechanism of liver injury caused by EP, and in clinical use, it is necessary to be vigilant about the risk of liver injury caused by unreasonable use and combined use with Western medicine.
7.Clinical progress of inflatable video-assisted mediastinoscopic transhiatal esophagectomy

Surgery is an important method for the treatment of resectable esophageal cancer. With the continuous development of minimally invasive esophageal technology, video-assisted thoracoscopic esophagectomy (VATE) has demonstrated obvious advantages over conventional open surgery and has been widely accepted. However, there are still some esophageal cancer patients who cannot benefit from VATE. Inflatable video-assisted mediastinoscopic transhiatal esophagectomy(IVMTE) does not require one-lung ventilation, reduces postoperative complications, expands surgical indications, and brings surgical opportunities for patients with impaired lung function and thoracic lesions, which has become a new choice for the treatment of esophageal cancer. However, the limited field of surgical view and the tunneling surgical approach undoubtedly increase the difficulty of surgery, and how to clearly expose the anatomical structures and thoroughly dissect the lymph nodes has always been the key points and difficulties of surgery. The authors review relevant literatures to discuss the clinical progress and limitations of IVMTE.
8.Optimization of Culture Medium and Fermentation Condition of Pseudomonas ZL8

ObjectiveIn the previous stage,our research group isolated Pseudomonas ZL8 from Polyporus umbellatus, which has a broad-spectrum inhibitory effect on plant pathogenic fungi such as Fusarium oxysporum of Salvia miltiorrhiza and promotes plant growth. On this basis, this study optimized the fermentation medium and conditions of Pseudomonas ZL8,to improve the number of bacteria in the fermentation broth per unit volume, and provide a basis for the development and application of this strain. MethodThe growth curve of strain ZL8 in LB culture medium was determined and its growth law in liquid medium was clarified. With the number of live bacteria in the fermentation broth of strain ZL8, the dry weight of the bacteria and the A600 value of the broth as detection indicators, single factor investigation was carried out on different nitrogen sources,carbon sources and inorganic salts to determine the optimal medium components. Additionally, orthogonal test was conducted to optimize the component ratio to screen the optimal fermentation medium. Similarly, single factor investigation and orthogonal test were also carried out in four aspects: inoculation amount,culture temperature,time and rotating speed to obtain the optimal culture conditions. ResultThe optimal medium for liquid fermentation of strain ZL8 was peptone 15 g·L-1, glucose 7.5 g·L-1, and potassium chloride 10 g·L-1. After 48 h of fermentation,the number of viable bacteria in the fermentation liquid could reach 8.93×109 cfu·mL-1,3.93 times that of LB liquid medium (P<0.01). The optimal fermentation conditions were as follows: inoculation amount 3%, temperature 28 ℃, time 72 h,and rotating speed 220 r·min-1, and the number of viable bacteria in the fermentation broth could reach 6.37×1010 cfu·mL-1. ConclusionCompared with LB culture medium and the initial fermentation conditions,the optimized culture medium and conditions could effectively increase the bacterial volume of strain ZL8 per unit volume and prolong the stable growth of the strain, with low medium cost, which provided a theoretical basis for the efficient fermentation and application of Pseudomonas ZL8.
9.Progress in Multidisciplinary Diagnosis and Treatment of Familial Brain Tumors

The tumors of central nervous system refer to a group of benign and malignant diseases originating from tissues or structures within the central nervous system. Common tumors of central nervous system are sporadic, but a few have familial onset. Compared with sporadic brain tumors, the clinical symptoms, diagnostic ideas and follow-up review plans of familial brain tumors are more complicated. The multidisciplinary diagnosis and treatment (MDT) mode usually refers to a treatment mode in which a case involving multiple organs and systems is discussed, and the best treatment plan is formulated for the patient based on the comprehensive opinions of various disciplines. Because familial brain tumors often involve multiple organs, multiple disciplines and multiple systems, and their low incidence leads to less clinical experience for neurosurgeons, the MDT model is more conducive to efficient diagnosis, treatment and management of familial brain tumors. This review elaborates on the neurosurgeon-led MDT model, and introduces the latest research on the epidemiology, genetic characteristics, clinical manifestations, diagnostic ideas and multidisciplinary management of familial brain tumors.
10.DTI quantitative evaluation of functional changes after acute traumatic spinal cord injury in rats

Objective To explore the value of DTI quantitative parameters in evaluating neurological function changes of acute traumatic spinal cord injury (TSCI)in rat models.Methods The modified Allen's dropping weight technique was used to establish TSCI rat models.Then the rats were divided into mild injury group,moderate injury group and severe injury group (each n=10).DTI examination and Basso-Beattie-Bresnahan (BBB) score were performed pre-TSCI and 0 h,6 h,24 h,3 day,7 day and 14 day post-TSCI,respectively.The BBB scores and DTI parameters,including FA,mean apparent diffusivity (MD),radial diffusivity (RD) and axial diffusivity (AD) were measured and compared among groups.The correlation between BBB scores and the parameters was evaluated.Results The differences of FA,MD and RD value were statistically significant among varying injury degree groups and different time points after TSCI (all P<0.05).AD value had statistical difference among different time points (F=12.720,P<0.001),whereas no difference was found among varying injury degree groups (F=0.469,P=0.630).FA and MD values decreased while RD increased 0 h post-TSCI.Then RD and MD increased continuously,whereas FA decreased continuously until 24 h post TSCI (all P<0.05),and the parameters kept stable after 24 h post-TSCI (all P> 0.05).The BBB scores were lowest on 0 h post-TSCI,then maintained increasing (all P<0.05).In addition,the BBB scores and MD values had good correlation (r=0.958,P< 0.01).Conclusion DTI can quantitatively evaluate function changes of TSCI in rat models.Moreover,treatment within 24 h post-TSCI might be recommended for TSCI therapy.
