1.Interpretation of critical update: AASLD practice guidance on prevention, diagnosis, and treatment of hepatocellular carcinoma

In February 2025, the American Association for the Study of Liver Diseases (AASLD) published online “Critical Update: AASLD Practice Guidance on Prevention, Diagnosis, and Treatment of Hepatocellular Carcinoma”. This update mainly focuses on the latest analysis results of the IMbrave050 study and performs corresponding updates and adjustments to recommendations based on the issues in clinical practice. As for the postoperative adjuvant therapeutic strategies for hepatocellular carcinoma (HCC) based on immune checkpoint inhibitors, the AASLD has re-evaluated and updated the practice guidance. The update revises related texts and recommendations for adjuvant therapy and the management algorithms for HCC recurrence during or after adjuvant therapy. Furthermore, the AASLD emphasizes that even for HCC patients at a high risk of recurrence after resection or local ablation, close monitoring for recurrence remains the current standard treatment regimen. Our team makes an excerpt of the update, systematically introduces the background and specific contents of the update, and discuss the adjuvant therapy for HCC, in order to provide a reference for readers.
2.Characteristics of immune response induced by mucosal immunization with recombinant adenovirus of Mycobacterium tuberculosis phosphodiesterase.

Objective The prevalence of drug-resistant Mycobacterium tuberculosis (Mtb) strains is exacerbating the global burden of tuberculosis (TB), highlighting the urgent need for new treatment strategies for TB. Methods The recombinant adenovirus vaccine expressing cyclic di-adenosine monophosphate (c-di-AMP) phosphodiesterase B (CnpB) (rAd-CnpB), was administered to normal mice via mucosal immunization, either alone or in combination with drug therapy, to treat Mtb respiratory infections in mice.Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of antibodies in serum and bronchoalveolar lavage fluid (BALF). Real-time quantitative PCR was performed to assess the transcription levels of cytokines interferon γ(IFN-γ) and interleukin 10(IL-10) in mouse lungs. Flow cytometry was used to determine the proportions of CD4+ and CD8+ T cell subsets in the lungs and spleens. ELISA was employed to measure the levels of cytokines IFN-γ, IL-2, IL-10, inflammatory factors IL-6, and tumor necrosis factor α (TNF-α) secreted by spleen cells following antigen stimulation. The bacteria loads in the lungs and spleens of Mtb-infected mice were enumerated by plate counting methods. Resluts Intranasal immunization with rAd-CnpB induced high titers of IgG in mouse serum and the production of IgG and IgA in BALF, along with alterations in T lymphocyte subsets in the lungs and spleens. Administration of rAd-CnpB, either alone or in combination with drugs, to Mtb-infected mice significantly increased serum IgG levels as well as IgA and IgG levels in BALF. rAd-CnpB immunization promoted the secretion of CnpB-specific cytokines and inflammatory factors by splenocytes in Mtb-infected mice. However, rAd-CnpB immunotherapy, either alone or combined with drugs, did not significantly affect the bacterial loads in the lungs and spleens of mice with Mtb respiratory infections. Conclusion Mucosal immunization with rAd-CnpB induced significant mucosal, humoral and cellular immune responses in mice, and significantly enhanced CnpB-specific cellular immune responses in Mtb-infected mice.

3.Correlation between pen-holding posture, grip strength and myopia in school-aged children

Objective:To investigate the correlation between pen-holding posture, grip strength and myopia.Methods:A cross-sectional study was performed.A total of 496 school-aged children in grades 1-6, who underwent eye examinations at Shenzhen Eye Hospital from November 2021 to May 2022 were consecutively enrolled as subjects.The subjects' age, grade, sex, refractive error and age of school enrollment were recorded.Subjects' grip strength and pen-holding posture were obtained with an electronic grip strength meter and a tool pen, and subjects were divided into correct pen-holding posture group and incorrect pen-holding posture group according to their pen-holding posture.Propensity scores for sex, grade, age at school entry, and grip strength in the two groups were matched using the nearest neighbor matching method.Multifactorial binary logistic regression analysis was performed with binocular myopia, monocular myopia, and emmetropia as dependent variables and grip strength, grade, refractive error, and pen-holding posture as independent variables.The correlation between grip strength and spherical equivalent was examined by Spearman rank correlation analysis and partial correlation analysis.This study protocol adhered to the Declaration of Helsinki and was approved by the Ethics Committee of Shenzhen Eye Hospital (No.2022KYPJ032).Results:After matching propensity scores, there were 101 cases and 197 cases in the correct pen-holding posture group and incorrect pen-holding posture group, respectively.There was no statistical significance in sex, grade, age at enrollment, and grip strength between the two groups (all at P>0.05).Multinomial logistic regression results showed that using emmetropia as a reference, higher grade (4 versus 1∶7.601[1.307-44.206]; 5 versus 1∶4.392[1.039-18.562]), presence of anisometropia (21.366[5.750-79.397]) were relative risk factors for monocular myopia (all at P<0.05), and higher grade (3 versus 1∶4.389[1.783-10.800]); 4 versus 1∶15.398[3.267-72.574]; 5 versus 1∶7.447[2.232-24.851]; 6 versus 1∶6.462[2.116-19.734]) were relative risk factors for binocular myopia (all at P<0.05).Taking monocular myopia as a reference, higher grade (6 versus 1∶4.582[1.193-17.589]), presence of anisometropia (0.141[0.069-0.289]), and irregular pen-holding posture (2.608[1.340-5.075]) were associated factors for binocular myopia (all at P<0.05).According to partial correlation analysis, the spherical equivalent of the right eye of children with incorrect pen-holding posture was negatively correlated with the grip strength ( rs=-0.141, P=0.047). Conclusions:There is an association between pen-holding posture and myopia in school-aged children.Children with greater grip strength and incorrect pen-holding posture may have higher degrees of right eye refractive error.

5.Treatment of Endometriosis from the Perspective of "Retention due to Deficiency Qi"

It is believed that retention due to deficient qi is an important pathogenesis of endometriosis (EMs). Deficient qi is the root of the disease, mainly manifested as spleen deficiency, while retention is the branch pathogenesis of the disease, mainly with blood stasis, complicated with constraint, phlegm, heat, toxin and other pathological factors. Therefore, it is proposed to follow the treatment principle of supplementing deficiency and unblocking stagnation, and take the methods of replenishing qi and fortifying the spleen, removing stasis and eliminating concretions. Self-made Fuzheng Huayu Formula (扶正化瘀方) is taken as the basic formula, and can be modified with the symptoms in menstrual and non-menstrual periods. Additionally, the methods of moving qi, dispelling phlegm, clearing heat, relieving toxin and others can be combined, and it is recommended to treat the root and the branch simultaneously.
6.Peripheral blood biomarkers in children and adolescents with depressive disorders

Depressive disorder is a highly prevalent mood disorder among children and adolescents, which can exert profoundly negative impacts on psychosocial development. However, in the current medical system there still lacks objective indicators for the risk assessment, diagnosis and treatment selection of depression in this population. Biomarkers, which serve as objective indicators of physiological states, can offer crucial insights into pathological mechanisms of depressive disorders. This paper provides a comprehensive review of existing research on peripheral blood biomarkers associated with depression in children and adolescents, highlighting the feasibility of their clinical application. Studies indicate that substances in immune, neuroendocrine, neurotrophic, and multi-level omics system in peripheral blood can be significantly linked to the diagnosis and severity of depression in this population. Nevertheless, the predictive role of specific biomarkers necessitates integration with other multimodal data, and replication through studies with large sample size is needed.
7.The strategy of extrahepatic organs management in early period after adult liver transplantation

With the improvements in surgical technique,the indications for liver transplantation(LT)are being expanded to more complex and severe patients,who not only have liver function in the terminal stage,but also extrahepatic organs involve of varying degrees dysfunction.It was demonstrated that the strategy of extrahepatic organs management in early period after LT was associated with improving recovery.This paper summarized the strategy of extrahepatic organs management in early period after adult LT.Continuing to monitor system hemodynamics is helpful for stable graft perfusion.It is necessary to manage derangements of preload,myocardial contractility,and afterload.Early postoperative extubation is recommended by current guidelines.Routine screening pulmonary complications may help weaning from the ventilator.The diagnosis of acute kidney injury(AKI)after LT has been reported to using Kidney Disease:Improving Global Outcomes(KDIGO)criteria,a delayed calcineurin inhibitor initiation strategy may be beneficial for renal protection.Currently,there is no consensus on timing of renal replacement therapy initiation in patients with AKI.Patients with cirrhosis and a history of hepatic encephalopathy(HE)displays impaired neurological recovery,the diagnosis of HE is crucial and challenging.

9.Evaluation of Muscle Fatigue in Spinal Surgery Instruments Based on sEMG-JASA.

Based on the joint analysis of EMG spectrum and amplitude method (JASA), a study on muscle fatigue assessment of spinal surgical instruments based on surface EMG signals was carried out, and a comparative evaluation of the operating comfort before and after the optimization of spinal surgical instruments was completed. A total of 17 subjects were recruited to collect the surface EMG signals of their brachioradialis and biceps. Five surgical instruments before and after optimization were selected for data comparison, and the operating fatigue time proportion of each group of instruments under the same task was calculated based on the RMS and MF eigenvalues. The results showed that when completing the same operation task, the operation fatigue time of the surgical instruments before optimization was significantly higher than that after optimization (P<0.05); there was no significant difference in the fatigue status of brachioradialis and biceps when operating the same instrument (P>0.05). These results provide objective data and reference for the ergonomic design of surgical instruments and fatigue damage protection.

10.Progress in Diagnosis and Endoscopic Treatment of Esophageal Leiomyoma

Esophageal leiomyoma is the most common benign tumor of the esophagus,usually asymptomatic.With the development and widespread application of endoscopic ultrasonography technology,its detection rate has been increasing year by year.Its diagnostic methods have evolved from initial esophagography and chest electronic computed tomography,to endoscopic ultrasonography,endoscopic ultrasonography-guided fine-needle aspiration,and endoscopic ultrasonography-guided fine-needle biopsy.The technology is constantly updated,and the diagnostic accuracy is constantly improving.The treatment methods have also shifted from previous open chest surgery to thoracoscopic surgery,and in recent years,there has been a shift towards ultra minimally invasive techniques such as endoscopic mucosal resection,endoscopic submucosal dissection,endoscopic submucosal excavation,endoscopic full-thickness resection,and submucosal tunnel endoscopic resection.This article provides a review of the diagnosis and endoscopic treatment progress of esophageal leiomyoma.
