OBJECTIVE: To observe the clinical efficacy of acupoint thread-embedding for overweight and obese young adults with phlegm-dampness constitution. METHODS: A total of 66 young adults with overweight and obesity of phlegm-dampness constitution were randomly divided into an observation group (33 cases, 2 cases dropped out, 1 case was discontinued) and a control group (33 cases, 3 cases dropped out). Health education guidance was adopted in the control group. On the basis of the intervention in the control group, acupoint thread-embedding was applied in the observation group, two groups of acupoints were used alternately, i.e. bilateral Tianshu (ST25), Daheng (SP15), Daimai (GB26), Shuidao (ST28), Huaroumen (ST24), Fenglong (ST40) and bilateral Feishu (BL13), Geshu (BL17), Weiwanxiashu (EX-B3), Pishu (BL20), Shenshu (BL23), Dachangshu (BL25), once a week. Treatment of 8 weeks was required in the two groups. Before treatment, after 4 weeks and 8 weeks of treatment, the body weight, body fat rate, waist circumference and hip circumference were measured, and the body mass index (BMI) was calculated in the two groups; before and after treatment, the phlegm-dampness constitution score and the heart rate variability (HRV) related indexes (total power [TP], low frequency [LF], high frequency [HF], LF/HF, standard deviation of the normal to normal inter beat intervals [SDNN], root mean square of the successive differences [RMSSD], percentage of normal RR intervals with duration [PNN50]) were observed, and the clinical efficacy and constitution improvement rate were compared in the two groups. Six months after the treatment completion, the body weight of patients with effective clinical efficacy was observed, and the body weight rebound rate was compared in the two groups. RESULTS: After 4 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment in the two groups (P<0.05); after 8 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment and after 4 weeks of treatment in the observation group (P<0.05), and the body weight was decreased compared with that before treatment in the control group (P<0.05). After 8 weeks of treatment, the body weight, BMI, waist circumference, hip circumference and phlegm-dampness constitution score in the observation group were lower than those in the control group (P<0.05). The differences in the body weight, BMI, body fat rate, waist circumference and hip circumference between before treatment and after 4 weeks of treatment, as well as after 4 weeks of treatment and after 8 weeks of treatment in the observation group were higher than those in the control group (P<0.05). After treatment, the TP, LF, HF, SDNN and RMSSD in the observation group were higher than those in the control group (P<0.05). After treatment, the total effective rate was 90.0% (27/30) in the observation group, which was higher than 46.7% (14/30) in the control group (P<0.05); the improvement rate of phlegm-dampness constitution was 90.0% (27/30) in the observation group, which was higher than 50.0% (15/30) in the control group (P<0.05). Six months after treatment completion, the weight rebound rate was 37.0% (10/27) in the observation group, which was lower than 71.4% (10/14) in the control group (P<0.05). CONCLUSION Acupoint thread-embedding can effectively reduce the body weight, BMI, waist circumference, hip circumference and improve the phlegm-dampness constitution in overweight and obese young adults with phlegm-dampness constitution, regulate autonomic nerve disorders and improve vagus nerve activity, and has a certain long-term effect.

The consensus focuses on patients with herpes zoster, and forms recommendations for 9 key clinical issues, covering simple fire needling and TCM comprehensive therapy based on fire needling, including fire needling combined with cupping, fire needling combined with Chinese herb, fire needling combined with cupping and Chinese herb, fire needling combined with filiform needling, fire needling combined with moxibustion, and provides specific recommendations and operational guidelines for various therapies.

Cleft lip and palate is a common congenital developmental defect,which is mainly manifested as congenital oral and maxil-lofacial malformations.In patients with cleft lip and palate,jaw deformity often involves three-dimensional orientation,among which the correction of sagittal dysmorphism is the key point and difficulty in its diagnosis and treatment.In this article,we will elaborate on the indications and corrective methods for the treatment of maxillary retraction of cleft lip and palate,and introduce our experience in the sequential treatment of cleft lip and palate maxillary retraction in order to promote the standardization of its diagnosis and treatment.

Objective To investigate the clinical manifestations,endoscopic,histopathological and im-munofluorescence IgA detection characteristics in children patients with Henoch-Schonlein purpura (HSP) with gastrointestinal tract symptoms as initial symptoms.Methods The clinical data of 29 children admitted to this hospital from April 2018 to December 2019 with gastrointestinal symptoms as the first manifestation and initially diagnosed with HSP were retrospectively analyzed.The clinical manifestations,laboratory and imageological examination results,endoscopic and histopatological examination results,immunofluoresence IgA detection results,treatment regimen and outcome were analyzed.Results The clinical manifestations were mainly abdominal pain,vomiting,hematochezia,rash,joint swelling and pain,testicular swelling pain,etc.Hemoglobin was decreased (<110 g/L) in 3 cases (10.34%),the eosinophilic cells count was increased (>0.6×109/L) in 1 case and serum total IgE was increased in 8 cases (27.58%).The food specificity IgE was positive in 4 cases (13.79%),including crab,nut,soybean and mugwort 1 case each (3.45%).Twenty-two cases conducted the abdominal imageological examination,in which there were 5 cases of abdominal cavity hydrops,4 cases (13.79%) of pelvic cavity hydrops and 15 cases of intestinal wall edema and thickness.The endoscopic manifestations were mucosal hyperemia and edema in 29 cases (100.00%),erosion in 24 cases (82.76%),mucosal roughness in 10 cases (34.48%),ulceration in 8 cases (27.59%),and punctated bleeding in 6 cases.The main manifestations of mucosal pathology were mucosal inflammation with IgA deposition in 10 cases (34.48%).Immunofluorescence IgA detection was positive in 10 cases (34.48%).Conclusion When abdominal HSP is highly suspected,the endoscopic examination could be performed as soon as possible to evaluate the degree of mucosal lesions and perfect the immunofluorescence IgA detection to further identify the disease cause

Objective To investigate the effects of harmine(HM)on the expression level of mitochondrion fusion related proteins and mitochondrial function injury in PC 12 cells.Methods PC 12 cells were divided into cell control group,HM group,mitochondrion mitosis inhibitor Mdivi-1 group,HM+Mdivi-1 group,mitochondrion fission agonist WY14643 group,HM+WY14643 group,with drug concentrations of 1,10,25,50,100 μmol·L-1.After 24 h treatment,the MTT method was used to detect the cell survival rate,and a microscope was used to observe the cell morphology,MitoTracker Red probe staining was used to observe the mitochondrial morphology and the length ratio of vertical and horizontal axes,JC-1 staining was used to detect the mitochondrial membrane potential,and a kit was used to detect ATP level and lactate dehydrogenase(LDH)activity.Immunofluorescence staining and Western blotting were used to assess the expression levels of caspase-3,apoptosis-promoting protein(Bax)cytochrome C(cyt-c),mitochondrial fusion protein(Mfn2)and mitochondrial mitotic protein(Drp-1).The interference sequence of Drp1 was transfected by electroporation,and the siRNA sequence with good transfection effect was screened.The related indicators were detected by fluorescence method,MTT method,and immunoblotting method in cooperation with drug intervention.Results MTT results showed that compared with the cell control group,the survival rate of HM group,Mdivi-1 group,HM+Mdivi-1 group,WY14643 group and HM+WY14643 group decreased significantly(P＜0.01),and the EC50 were(11.48±2.32),(12.35±1.67),(14.88±2.07),(39.14±3.25),(20.09±1.97),respectively.According to this,subsequent experiments selected 20 μmol·L-1for HM,WY 14643 and HM+WY14643 as working concentrations to construct PC 12 cell model.Microscopic observation and MitoTracker Red probe staining showed that the cell density in the drug group decreased in varying degrees,and a transition from branched to round morphology in the drug-treated groups was observed.The morphology of mitochondria tended to be round,and the ratio of the length of the longitudinal axis to transverse axis was(3.33±0.72)in the cell control group,(2.19±0.58)in the HM group,(2.45±0.44)in Mdivi-1 group,and(1.43±0.62)in HM+Mdivi-1 group,respectively.The results of JC-1 staining showed that compared with the cell control group,the mitochondrial mode potential of the HM group significantly decreased(P＜0.01).ROS significantly increased(P＜0.01)and ATP levels decreased(P＜0.01),and LDH enzyme activity increased(P＜0.01).Immunofluorescence staining and Western blotting results showed that compared with the cell control group,the expression levels of proapoptotic proteins Bax,cytochrome C,and caspase-3 in the HM group were significantly increased(all P＜0.01).Compared with the cell control group,the expression level of mitochondrial fission related protein Drp1 in HM group was significantly higher(P＜0.01).The expression level of mitochondrial fusion related protein Mfn2 significantly decreased(P＜0.01).After specific interference with Drp1 and synergistic intervention with HM,the survival rate of PC 12 cells in each interference group decreased compared to each drug intervention group.The expression of Drp1 and Mfn2 was downregulated,and the differences were statistically significant(P＜0.05 or P＜0.01).Conclusion HM can reduce the mitochoudrial membrane potential and ATP levels by accumulating ROS,there by activating the caspase-3 apoptosis pathway and promoting cell apoptosis.Mitochondrial fusion division may be involved in the damage of PC12 cells caused by HM,initiating apoptosis through the mitochondrial pathway.

Alzheimer's disease(AD), the most common neurodegenerative disease, has shown an increasing incidence among younger people. With the onset of disease, most patients' cognitive function will show a progressive decline, bringing a heavy burden to the society and the family. Studies have shown that fibroblast growth factor (FGF) may be involved in the pathogenesis of AD through multiple mechanisms. This article reviews the mechanism of FGF in AD, with the hope of providing new ideas for elucidating the pathogenesis and early diagnosis of AD.

Objective To compare the correlation and consistency of liquid chromatography-tandem mass spectrometry(LC-MS/MS)and electrochemiluminescence immunoassay(ECLIA)in the determination of se-rum 25-hydroxyvitamin D[25(OH)D],in order to guide the clinical selection of appropriate detection meth-ods.Methods A total of clinical serum samples were collected from the laboratory,and 25(OH)D levels were detected by LC-MS/MS and ECLIA,respectively.Passing-Boblok regression was used to analyze the correla-tion between the two methods,and Bland-Altman and Mountain plot were used to evaluate the agreement be-tween the two methods.Serum 25(OH)D<20.0 ng/mL was defined as vitamin D deficiency,and serum 25(OH)D as 20.0-<30.0 ng/mL was defined as vitamin D insufficiency.Kappa analysis was used to determine the coincidence rate of the two methods in the diagnosis of vitamin D nutritional status.Results The 25(OH)D levels detected by LC-MS/MS and ECLIA were(26.67±4.79)ng/mL and(39.33±4.09)ng/mL,respec-tively.The regression equation of the two methods was YECLIA=-4.558 1+1.719 8XLC-MS/MS,the slope was 1.719 8(95%CI 1.586 3-1.828 4),excluded 1,and the intercept was 4.558 1(95%CI-7.692 2--2.122 1),excluded 0,prompt system difference or ratio differences of the two methods.There were system-atic or proportional differences between the two methods.The Bland-Altman figure showed two methods aver-ages was 12.7,and the difference of out points(ratio)was 3.19%.The peak value of the mountain plot was-9.17 ng/mL,with more deviations from 0,indicating poor agreement between the results measured by the two methods.The Kappa coefficient of the two methods for judging vitamin D deficiency was 0.875,and the diagnostic coincidence rate was 94.68%.The Kappa coefficient of the two methods for judging vitamin D in-sufficiency was 0.538,and the diagnostic coincidence rate was 75.53%.Conclusion The agreement between ECLIA and LC-MS/MS is poor,but the agreement between ECLIA and LC-MS/MS in the diagnosis of vita-min D deficiency and insufficiency based on nutritional status is high.

In recent years, the relationship between non-alcoholic fatty liver disease (NAFLD) and vascular cognitive impairment (VCI) has received increasing attention. The mechanism underlying the relationship between the two diseases remains unclear, which may be associated with neuroinflamation, oxidative stress, cerebrovascular disease, gut microbiota imbalance, and insulin resistance. This article reviews the correlation and potential mechanisms between NAFLD and VCI, with the aim of providing reference for further exploration of their correlation.

Objective To evaluate the dynamic efficacy of Invigorating Qi and Dissipating Depression Herbs in the treatment of depressive disorders and its effect on recurrence rate and suicidal thoughts.Methods The prospective cohort study was used to divide the patients into three cohorts according to the different exposure factors(treatment regimens):Western medicine group,integrated traditional Chinese and Western medicine group,and Invigorating Qi and Dissipating Depression Herbs group,and followed up for 2 years,and the treatment effects of each cohort at half a year,one year,one and a half years,and two years were observed respectively,and the evaluation indicators were Montgomery Depression Scale(MADRS),Hamilton Anxiety Scale(HAMA)and Social Deficit Screening Scale(SDSS)reduction rates,and endpoint events(relapse,suicidal ideation)incidence;By comparing the efficacy and endpoint event rate of the three cohorts,the effect of Invigorating Qi and Dissipating Depression Herbs on improving depressive disorders was evaluated.Results A total of 409 patients(67 in the Western medicine group,131 in the integrated traditional Chinese and Western medicine group,and 211 cases in the Invigorating Qi and Dissipating Depression Herbs group)were enrolled,and the rate of score reduction after treatment with MADRS,HAMA and SDSS increased compared with the pre-treatment treatment with the extension of the drug taking time.The difference between the MADRS and SDSS reduction rates in the Invigorating Qi and Dissipating Depression Herbs group on taking the drug for half a year and the SDSS reduction rate on one year of taking the drug were statistically significant compared with the Western medicine group,and there was no significant difference compared with the integrated Chinese and Western medicine group.On one and a half years and two years of medication,the integrated Chinese and Western medicine group had more advantages in MADRS reduction rate than the Invigorating Qi and Dissipating Depression Herbs group.The recurrence rates of depression in the three groups within 2 years were 20.9％,30.5％and 36.0％,and the difference was not statistically significant,and the length of taking medication was used as a protective factor by logistic regression analysis.The incidence rates of suicidal thoughts were 44.8％,19.1％and 17.5％,and the differences were statistically significant,and the exposure factors and income level were the protective factors by logistic regression analysis.Conclusion Invigorating Qi and Dissipating Depression Herbs can significantly improve the depressive state of patients,promote the recovery of social function,reduce the occurrence of long-term suicidal ideation,and appropriately extend the time of taking medicine can increase the long-term efficacy and reduce recurrence.

Objective:To evaluate the role of α7 nicotinic acetylcholine receptor (α7nAChR) in penehyclidine hydrochloride-induced reduction of endotoxin-induced acute lung injury (ALI) in mice.Methods:Forty SPF healthy male C57BL/6 mice, aged 6-8 weeks, weighing 18-25 g, were divided into 4 groups ( n=10 each) using a random number table method: control group (group C), ALI group, penehyclidine hydrochloride group (PHC group), and α7nAChR inhibitor MLA group (MLA group). ALI was induced by intraperitoneal injection of lipopolysaccharide 15 mg/kg in anesthetized animals, while normal saline was given instead in group C. In PHC group, penehyclidine hydrochloride 2 mg/kg was intraperitoneally injected at 30 min before developing the model. MLA 10 mg/kg was intraperitoneally injected at 10 min before administration of penehyclidine hydrochloride in MLA group. Mice were sacrificed at 6 h after lipopolysaccharide administration, and lung tissues were collected for microscopic examination of the pathological changes (by HE staining) and for determination of the wet/dry weight ratio (W/D ratio), content of tumor necrosis factor-alpha (TNF-α), interleukin-1beta (IL-1β) and IL-10 (by enzyme-linked immunosorbent assay) and expression of α7nAChR (by Western blot). Results:Compared with C group, the W/D ratio and contents of TNF-α and IL-1β were significantly increased, the content of IL-10 was decreased, and the expression of α7nAChR was up-regulated in ALI, PHC and MLA groups ( P<0.05). Compared with ALI group, the W/D ratio and contents of TNF-α and IL-1β were significantly decreased, the content of IL-10 was increased, and the expression of α7nAChR was up-regulated in PHC group ( P<0.05). Compared with PHC group, the W/D ratio and contents of TNF-α and IL-1β were significantly increased, the content of IL-10 was decreased, and the expression of α7nAChR was down-regulated in MLA group ( P<0.05). Compared with ALI group, the pathological changes of lung tissues were significantly mitigated in PHC group, while this effect of PHC was partially reversed by α7nAChR inhibitor MLA. Conclusions:α7nAChR is involved in penehyclidine hydrochloride-induced reduction of endotoxin-induced ALI in mice.
