Objective This study aimed to investigate the antimicrobial resistance profiles of bacterial strains isolated from pediatric intensive care units(PICU)in China for better antimicrobial therapy.Methods Clinical isolates were collected from 17 institutions,including tertiary care children's hospitals and pediatric department of tertiary general hospitals in China from January 1,2020 to December 31,2022.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were interpreted according to the breakpoints released by the Clinical and Laboratory Standards Institute(CLSI)in 2020.Results A total of 10 688 isolates were collected,including gram-positive organisms(39.2%)and gram-negative organisms(60.8%).The top three organisms were S.aureus(13.6%,1 453/10 688),A.baumannii(10.0%,1 067/10 688),and coagulase-negative Staphylococcus(9.9%,1 058/10 688).Multi-drug resistant organisms(MDROs)were very common in children.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant E.coli,carbapenem-resistant K.pneumoniae(CRKP),carbapenem-resistant A.baumannii(CRAB),and carbapenem-resistant P.aeruginosa(CRPA)was 41.1%,19.4%,8.8%,30.9%,67.4%,and 28.8%,respectively.Overall,more than 50%of Enterobacteriales isolates were resistant to cephalosporins,while nearly 25%of Enterobacteriales isolates were resistant to carbapenems.MDROs were highly resistant to commonly used antibiotics.More than 80%of CRE and CRAB strains were resistant to all beta-lactam antibiotics.CRE and CRAB showed low resistance rates to tigecycline and polymyxin.CRPA showed lower resistance rates to piperacillin,beta-lactamase inhibitor combinations than the resistance rates to third and fourth generation cephalosporins.All of the Staphylococcus and Enterococcus isolates were susceptible to vancomycin and tigecycline.None of PRSP strains isolated from meningitis and nonmeningitis samples were resistant to rifampicin,vancomycin,or linezolid.The prevalence of β-lactamase-negative ampicillin-resistant(BLNAR)strains was 43.3%in Haemophilus influenzae.Conclusions MDROs were prevalent in PICU.It is necessary to establish an effective multidisciplinary team(MDT)to control the antimicrobial resistance.
2.Expert consensus on nasogastric tube and intestinal tube placement in children

Objective To form an expert consensus on nasogastric tube and intestinal tube placement in children(hereinafter referred to as the"consensus"),and provide a reference for pediatric medical workers regarding the operation of gastrointestinal tube placement in children.Methods A"consensus"formulation group was established.The"5.0"EBHC Pyramid Model was employed to systematically search,evaluate,summarize and extract relevant evidence to form the initial draft of the"consensus".The RAND-UCLA expert consensus method was utilized.Through a round of expert inquiries by correspondence and a round of face-to-face expert discussions,the data were collated,analyzed,refined,and modified to form the final version of the"consensus".Results The effective recovery rate of the expert inquiry questionnaire was 100%.The coefficient of expert authority(Cr)was 0.891.The Kendall's concordance coefficient of the inquiries was 0.692(P<0.01),which was statistically significant.The median of the"RAND-UCLA"suitability score were 7-9 points,and the disagreement index(DI)value was<1.00.The final"consensus"encompasses 9 aspects,including qualification requirements for the tube placement personnel,indications and contraindications,preoperative assessment,preoperative preparation,measurement of tube length,key points of tube placement,methods for confirming the position,tube flushing,fixation,and recording,with 47 recommendations.Conclusion This"consensus"is scientific,rigorous,and practical,covering all links of the gastrointestinal tube placement process in children,providing reference and guidance for the safe and standardized implementation of gastrointestinal tube placement in children.
3.Clinical observation on acupoint thread-embedding for overweight and obesity in young adults with phlegm-dampness constitution.

OBJECTIVE:
To observe the clinical efficacy of acupoint thread-embedding for overweight and obese young adults with phlegm-dampness constitution.
METHODS:
A total of 66 young adults with overweight and obesity of phlegm-dampness constitution were randomly divided into an observation group (33 cases, 2 cases dropped out, 1 case was discontinued) and a control group (33 cases, 3 cases dropped out). Health education guidance was adopted in the control group. On the basis of the intervention in the control group, acupoint thread-embedding was applied in the observation group, two groups of acupoints were used alternately, i.e. bilateral Tianshu (ST25), Daheng (SP15), Daimai (GB26), Shuidao (ST28), Huaroumen (ST24), Fenglong (ST40) and bilateral Feishu (BL13), Geshu (BL17), Weiwanxiashu (EX-B3), Pishu (BL20), Shenshu (BL23), Dachangshu (BL25), once a week. Treatment of 8 weeks was required in the two groups. Before treatment, after 4 weeks and 8 weeks of treatment, the body weight, body fat rate, waist circumference and hip circumference were measured, and the body mass index (BMI) was calculated in the two groups; before and after treatment, the phlegm-dampness constitution score and the heart rate variability (HRV) related indexes (total power [TP], low frequency [LF], high frequency [HF], LF/HF, standard deviation of the normal to normal inter beat intervals [SDNN], root mean square of the successive differences [RMSSD], percentage of normal RR intervals with duration [PNN50]) were observed, and the clinical efficacy and constitution improvement rate were compared in the two groups. Six months after the treatment completion, the body weight of patients with effective clinical efficacy was observed, and the body weight rebound rate was compared in the two groups.
RESULTS:
After 4 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment in the two groups (P<0.05); after 8 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment and after 4 weeks of treatment in the observation group (P<0.05), and the body weight was decreased compared with that before treatment in the control group (P<0.05). After 8 weeks of treatment, the body weight, BMI, waist circumference, hip circumference and phlegm-dampness constitution score in the observation group were lower than those in the control group (P<0.05). The differences in the body weight, BMI, body fat rate, waist circumference and hip circumference between before treatment and after 4 weeks of treatment, as well as after 4 weeks of treatment and after 8 weeks of treatment in the observation group were higher than those in the control group (P<0.05). After treatment, the TP, LF, HF, SDNN and RMSSD in the observation group were higher than those in the control group (P<0.05). After treatment, the total effective rate was 90.0% (27/30) in the observation group, which was higher than 46.7% (14/30) in the control group (P<0.05); the improvement rate of phlegm-dampness constitution was 90.0% (27/30) in the observation group, which was higher than 50.0% (15/30) in the control group (P<0.05). Six months after treatment completion, the weight rebound rate was 37.0% (10/27) in the observation group, which was lower than 71.4% (10/14) in the control group (P<0.05).
CONCLUSION
Acupoint thread-embedding can effectively reduce the body weight, BMI, waist circumference, hip circumference and improve the phlegm-dampness constitution in overweight and obese young adults with phlegm-dampness constitution, regulate autonomic nerve disorders and improve vagus nerve activity, and has a certain long-term effect.

The expert consensus on the clinical treatment of herpes zoster with fire needling was developed, and the commonly used fire needling treatment scheme verified by clinical research was selected to form a standardized diagnosis and treatment scheme for acute herpes zoster and postherpetic neuralgia (PHN), so as to answer the core problems in clinical application. The consensus focuses on patients with herpes zoster, and forms recommendations for 9 key clinical issues, covering simple fire needling and TCM comprehensive therapy based on fire needling, including fire needling combined with cupping, fire needling combined with Chinese herb, fire needling combined with cupping and Chinese herb, fire needling combined with filiform needling, fire needling combined with moxibustion, and provides specific recommendations and operational guidelines for various therapies.

5.Effect of Shenshu Fujian Decoction on PDGF/NKD2/Wnt Signaling Pathway in Rats with Chronic Renal Failure

ObjectiveTo observe the effect of Shenshu Fujian decoction on platelet-derived growth factor (PDGF)/naked cuticle homolog 2 (NKD2) /Wnt signaling pathway in rats with chronic renal failure (CRF). MethodsSixty male SD rats were randomly divided into normal group, model group, Niaoduqing group (5 g·kg-1), low-dose Shenshu Fujian decoction group (5.5 g·kg-1), medium-dose Shenshu Fujian decoction group (11 g·kg-1), and high-dose Shenshu Fujian decoction group (22 g·kg-1), with 10 rats in each group. A CRF rat model was established by feeding a 0.5% adenine diet for 21 days. After successful modeling, intragastric administration was given once daily for 28 consecutive days. After treatment, the renal morphology of rats was observed. Serum creatinine (SCr) and blood urea nitrogen (BUN) levels were detected. Hematoxylin-eosin (HE) staining and Masson staining were used to detect renal histopathological changes, and collagen volume fraction (CVF) was calculated. Serum levels of inflammatory markers interleukin (IL)-1β and IL-6 were measured using enzyme-linked immunosorbent assay (ELISA). The expressions of fibronectin 1 (FN1), type Ⅰ collagen (ColⅠ), α-smooth muscle actin (α-SMA), platelet-derived growth factor receptor-β (PDGFR-β), NKD2, dishevelled protein 2 (DVL2) and β-catenin in renal tissue were detected by immunohistochemistry and Western blot. ResultsCompared with the normal group, the model group showed significant renal pathological changes, a markedly increased kidney weight/body weight ratio (P<0.01), significantly elevated CVF (P<0.01), and notably increased serum levels of SCr, BUN, IL-1β, and IL-6 (P<0.01). Expression levels of FN1, ColⅠ, α-SMA, PDGFR-β, NKD2, DVL2, and β-catenin in renal tissue were also significantly increased (P<0.01). Compared with the model group, all treatment groups showed significantly decreased kidney weight/body weight ratios and CVF (P<0.01), as well as markedly decreased serum SCr, BUN, IL-1β, and IL-6 levels. Protein expression levels of FN1, ColⅠ, α-SMA, PDGFR-β, NKD2, DVL2, and β-catenin in renal tissue were decreased, with more pronounced effects observed in the Niaoduqing, medium-dose, and high-dose Shenshu Fujian decoction groups (P<0.05, P<0.01). ConclusionShenshu Fujian decoction improves renal function, reduces inflammation, and reverses renal fibrosis in CRF rats, possibly by downregulating the expression of PDGF/NKD2/Wnt signaling pathway-related proteins.
6.Nursing care of a patient with focal segmental glomerulosclerosis for kidney re-transplantation

To summarise the nursing experience of caring for a patient with focal segmental glomerulosclerosis for kidney re-transplantation.Nursing key points:early recognition of recurrence of the original disease and rapid activation of emergency response;stabilization of cyclosporine concentration by precision management;optimizing in vitro anticoagulation regimens and vigilance for bleeding and clotting;regulate adrenocorticotropic hormone use and being alert for adverse effects;implementing the whole process of infection prevention and control to prevent infection;implementing the whole process of psychological counseling to alleviate negative emotions.After active treatment and careful care,the patient's transplanted kidney function recovered and was successfully discharged.After a follow-up for 2 months,the patient recovered well.
7.Survival analysis of newly diagnosed non-transplant multiple myeloma patients

Objective:To analyze overall survival (OS), influential factors, and causes of death in patients with newly diagnosed non-transplant multiple myeloma (NDMM) at grassroots hospitals.Methods:The clinical data of 56 patients with NDMM admitted to the Department of Hematology, Wuwei People's Hospital from January 2018 to October 2022 were retrospectively analyzed. Using the Kaplan-Meier and Cox regression survival analysis models, univariate and multivariate analyses were performed to evaluate the factors affecting the OS of patients with NDMM.Results:The median OS of patients with NDMM was 27 months. Univariate analysis indicated that whether or not to receive chemotherapy was a risk factor affecting OS. Multivariate analysis revealed that age ( HR: 2.215, 95% CI 1.056-4.648, P < 0.05), renal dysfunction ( HR: 3.482, 95% CI 1.359-8.923, P < 0.05), and whether or not to receive chemotherapy ( HR: 0.83, 95% CI 0.021-0.331, P < 0.001) were all significant risk factors affecting OS (all P < 0.05). Conclusions:The median survival time of patients with NDMM is short. Age, renal dysfunction, and not receiving chemotherapy are unfavorable factors affecting OS. Pulmonary infection, renal failure, and disease progression are the main causes of death in patients with NDMM.
8.Research progress on nutritional risk identification and screening in patients with head and neck cancer

Patients with head and neck cancer (HNC) are prone to nutritional risk, making timely screening and nutritional intervention crucial. Currently, there is no nutritional risk screening tool specifically tailored for HNC patients in clinical practice. Commonly used tools include the Nutritional Risk Screening 2002 (NRS 2002) , the Patient-Generated Subjective Global Assessment (PG-SGA) , and so on. Each tool has its strengths and limitations. This review summarizes the current application of nutritional risk screening tools in HNC patients, aiming to provide a reference for the identification and prevention of nutritional risk in clinical settings.
