1.Protection of exosomes derived from bone marrow mesenchymal stem cells of different mouse ages on radiation-induced lung injury

BACKGROUND:Mesenchymal stem cells show extremely therapeutic potential for radiation-induced lung injury through delivering exosomes.Age is a primary factor affecting the function and biological efficacy of mesenchymal stem cells.OBJECTIVE:To investigate the protective effects of exosomes derived from bone marrow mesenchymal stem cells of different mouse ages on radiation-induced lung injury in mice.METHODS:Bone marrow mesenchymal stem cells of young mice and old mice were obtained by whole bone marrow adherent culture.The exosomes were isolated from the supernatant of passage 3 bone marrow mesenchymal stem cells.Ten 2-month-old C57BL/6J mice were randomly selected as the control group after anesthesia and not irradiated.The remaining 30 2-month-old C57BL/6J mice were used to establish a mouse radiation-induced lung injury model and were randomly divided into three groups.Exosomes derived from bone marrow mesenchymal stem cells of young mice,exosomes derived from bone marrow mesenchymal stem cells of old mice,and PBS were injected through the tail vein,respectively.The survival rate of mice was monitored.The lung function,lung inflammation and fibrosis were assessed at 1 and 12 weeks after irradiation.RESULTS AND CONCLUSION:(1)The concentrations of particles and proteins in exosomes derived from bone marrow mesenchymal stem cells of young mice were higher than those in exosomes derived from bone marrow mesenchymal stem cells of old mice.(2)Compared with the control group,the survival rate of mice in the PBS group was low,and lung inflammation was obvious at week 1 after irradiation,and the levels and mRNA expressions of interleukin-1β,interleukin-6,and tumor necrosis factor-α were increased.Collagen deposition in lung tissues was observed at week 12 after irradiation,and the mRNA level of E-cadherin was decreased,while the mRNA levels of α-smooth muscle actin,transforming growth factor-β1,and β-catenin were increased.(3)Compared with the PBS group,the survival rate of mice in the exosome group was significantly improved,and the level of proinflammatory factors and their mRNA expression were reduced at week 1 after irradiation,the mRNA level of E-cadherin was increased,and the mRNA levels of α-smooth muscle actin,transforming growth factor β1 and β-catenin were reduced at week 12 after irradiation.(4)Among all the above indicators,the therapeutic effect of exosomes derived from bone marrow mesenchymal stem cells of young mice was better than that of exosomes derived from bone marrow mesenchymal stem cells of old mice.(5)The results showed that exosomes derived from bone marrow mesenchymal stem cells of young mice contained more particles and proteins,and the effect of alleviating early inflammation and late fibrosis of radiation-induced lung injury in mice was better than that of exosomes derived from bone marrow mesenchymal stem cells of old mice.

3.Experimental Research and Clinical Application of Shenling Baizhusan in Gastric Ulcer Treatment: A Review

Gastric ulcer (GU) is a high-incidence digestive system disease characterized pathologically by disruption of gastric mucosal integrity, with clinical features including a prolonged course and periodic recurrence. Modern medicine attributes its pathogenesis to the dynamic imbalance between aggressive and defensive factors,while traditional Chinese medicine (TCM) posits its development as closely linked to spleen deficiency. Current therapies combining acid suppressants and antibiotics face challenges such as high recurrence rates,poor mucosal healing,and adverse drug reactions. Long-term use may induce metabolic disturbances like hypergastrinemia and reduced intestinal microbiota diversity. Therefore,exploring safer and longer-lasting therapeutic strategies has become a critical focus. TCM has extensive clinical experience and unique advantages in GU prevention and treatment. Studies demonstrate that the classic formula Shenling Baizhu San exhibits therapeutic properties of "invigorating spleen and tonifying Qi to restore physiological balance and eliminating dampness and regulating middle energizer to unblock Qi movement", enabling a holistic approach targeting both symptoms and root causes in GU with spleen deficiency as the core pathology by suppressing aggressive factors and strengthening defensive factors. Experimental research reveals its mechanisms involve enhancing the physicochemical barrier of the mucus layer,repairing epithelial barriers and microcirculation,modulating gastric acid secretion and gastrointestinal motility,and regulating microecological barriers and mucosal immunity. Clinical evidence confirms its synergistic effects in promoting ulcer healing,improving Helicobacter pylori eradication rates,and reducing recurrence risks. This review examined the etiology and pathogenesis of GU and systematically evaluated Shenling Baizhu San from three perspectives-clinical application,pharmacological effects, and experimental research-to provide insights for optimizing integrated traditional Chinese and Western medicine protocols and expanding its clinical applications.
4.Vector analysis for evaluating the effect of rotation on myopic astigmatism correction after V4c toric collamer lens implantation

Objective:To evaluate the effect of toric collamer lens (TICL) rotation on myopia combined with astigmatism correction after V4c TICL implantation using standardized vector analysis.Methods:An observational case series study was performed.A total of 152 patients (268 eyes) who underwent V4c TICL implantation for the correction of myopia and myopic astigmatism were enrolled at Henan Eye Hospital from January to December 2021.The preoperative spherical diopter, cylindrical diopter, and spherical equivalent (SE) were -15.00 to -3.25 D, -5.00 to -0.50 D, -16.50 to -3.88 D, respectively.Postoperative TICL vault and axis were measured by anterior segment optical coherence tomography, and postoperative TICL rotation was calculated.Uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), spherical diopter, cylindrical diopter, SE, and target astigmatism vector were recorded before and 1 month after surgery.The effectiveness index, safety index, surgically induced astigmatism vector (SIA), margin of error (ME), absolute value of the difference vector |DV|, correction index (CI), and achievement index (IS) were calculated based on the subjective refraction results 1 month after surgery and the cylindrical axis of the V4c TICL.Vector analysis parameters was compared among patients with different preoperative astigmatism, intraoperative TICL fixation, postoperative TICL rotation, and postoperative vault.The correlation between postoperative TICL rotation and TICL fixed angle deviation, postoperative vault, visual acuity and refraction, and vector analysis parameters were evaluated.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Henan Eye Hospital (No.HNEECKY-2021[13]).Written informed consent was obtained from each subject.Results:At 1 month postoperatively, the mean safety and efficacy indices were 1.27 and 1.29, respectively.There was no statistically significant difference between the predicted SE of (-0.11±0.13)D and the postoperative SE of (-0.07±0.52)D ( t=-0.994, P=0.321).UCVA, BCVA and cylinder diopter in the operative eyes were all improved after surgery compared to before surgery and the differences were statistically significant ( t=17.854, 5.446, -25.634; all P<0.001).The rotation of V4c TICL was 0° to 25°, with a mean of (5.41±4.35)°, which was positively correlated with the V4c TICL fixed angle deviation ( rs=0.461, P<0.001), negatively correlated with postoperative cylindrical diopter and positively correlated with postoperative UCVA, |DV| and SIA ( r=-0.360, 0.191, 0.205, 0.142; all P<0.05).Vector analysis between groups showed that vault, SIA and |DV| were higher and CI and IS were lower in low astigmatism group than in high astigmatism group ( Z=-3.017, -13.569, -2.793, -2.761, -4.779; all P<0.05).V4c TICL rotation and |DV| were lower in low fixation angle difference group than in high fixation angle difference group ( Z=-7.865, -2.080; both P<0.05).Preoperative cylindrical diopter, intraoperative V4c TICL fixed angle deviation, SIA and |DV| were lower in low rotation group than in high rotation group ( Z=-2.104, -4.578, -2.456, -2.090; all P<0.05).There were no statistically significant differences in the TICL rotation or in each vector between patients with different vault after the surgery (all P>0.05). Conclusions:V4c TICL implantation can achieve good results for the correction of myopia and myopic astigmatism.There is a slight difference between the fixation axis and the target axis of V4c TICL, which affects the rotation of V4c TICL and |DV|.Postoperative V4c TICL rotation is related to TICL fixed angle deviation, postoperative UCVA, SIA, |DV| and postoperative cylindrical diopter.Postoperative vault has no significant effect on TICL rotation and differences in each vector.
5.Best evidence summary for postoperative pulmonary infections management in patients with aneurysmal subarachnoid haemorrhage

Objective To integrate relevant evidences on postoperative pulmonary infections in patients with aneurysmal subarachnoid haemorrhage so as to provide references for clinical practice.Methods Domestic and international databases were searched,including BMJ Best Practice,UpToDate,Cochrane Library,the U.S.National Guidelines Library,JBI,NICE,Medlive,Embase,CINAHL,Web of Science,PubMed,CNKI,VIP,Wanfang Data,SinoMed and Yiigle for clinical decisions,expert consensus,clinical guidelines,best evidence summaries,systematic evaluations,Meta-analyses,and randomized controlled trials on pulmonary infections in patients with aneurysmal subarachnoid haemorrhage.Literature was screened according to the inclusion criteria.Qualitative assessment was performed,and evidences were extracted.Results A total of 16 articles were included,comprising 2 clinical decisions,7 expert consensus,3 guidelines,3 systematic reviews and 1 RCT.Finally,30 pieces of evidence were obtained in 7 domains:team management,risk assessment,mis-inhalation management,airway management,nutritional support,oral care and rehabilitation.Conclusion The best evidence for the management of postoperative pulmonary infection in patients with aneurysmal subarachnoid haemorrhage summarised in this study can provide references for clinical interventions.Clinical staff should reasonably apply the evidence according to the specific situations.

7.Exploration of the lymph node dissection range and strategies for adenocarcinoma of esophagogastric junction from the perspective of membrane anatomy

The incidence of adenocarcinoma of the esophagogastric junction (AEG) continues to rise globally, with surgical resection representing the primary curative approach. Due to the complex anatomy and heterogeneous metastatic pathways of AEG, lymphadenectomy has become a critical focus in modern surgical oncology. However, traditional TNM staging and Siewert classification exhibit limitations in precisely guiding the optimal extent of lymphadenectomy. Consequently, this review integrated emerging evidence from membrane anatomy theory - a field experiencing breakthrough advances - to systematically explore lymphadenectomy strategies for AEG. Specifically, we analyzed the application of membrane anatomy principles to delineate lymphadenectomy strategies, dissection scope, and key surgical techniques tailored to each Siewert subtype (I, II, III) and varying degrees of esophageal involvement. Based on membrane anatomy, the review proposed specific, anatomically guided lymph node dissection strategies. Furthermore, we evaluated the feasibility, standardization, and clinical significance of membrane anatomy-guided dissection strategies considering recent advances, while also addressing current challenges and future directions for development and validation. Membrane anatomy is anticipated to serve as a novel and essential anatomical foundation for optimizing surgical approaches. Its application thus may hold significant promise for optimizing surgical pathways, enhancing the quality and precision of lymph node dissection, and ultimately improving oncological outcomes.
8.Feasibility study of HRCM preoperative assessment combined with NOSES Jinling surgery in the treatment of slow transit constipation

Objective:To investigate the feasibility of employing high-resolution colon manometry (HRCM) in conjunction with natural orifice specimen extraction surgery (NOSES) during laparoscopic Jinling surgery for the treatment of colonic slow transit constipation (STC).Methods:This study retrospectively analyzed data from nine patients with STC who underwent HRCM preoperative evaluation and subsequently received NOSES Jinling surgery at Tianjin Union Medical Center between October 2016 and March 2023. The general intraoperative conditions and postoperative evaluation indicators were recorded.Results:All 9 patients underwent HRCM examination before surgery. High amplitude propagation contraction (HAPC) was observed in the ascending colon segment of all patients. The occurrence frequency of HAPC was 4.22±1.86 (times/day). The operation time of 9 patients was 200.0 (167.5, 250.0) minutes, the intraoperative blood loss was 100 (50, 150) ml, the postoperative hospitalization time was 13.0 (12.5, 24.0) days, and the first postoperative exhaust time was 3.0 (2.0, 3.5) days, the first postoperative defecation time was 5.0 (4.5, 5.5) days, the first postoperative eating time was 5.0 (5.0, 5.5) days, and the first postoperative time out of bed was 5.0 (5.0, 8.0) days. Notably, none of the 9 patients experienced postoperative complications. The Wexner scores at 6 months [5 (4, 7)], and 12 months [4 (3, 5)] post-surgery were significantly lower than the preoperative score [20 (16, 21)] ( P<0.05). Additionally, the patient assessment of constipation quality of life (PAC-QOL) scores at 6 months [22 (20, 25)], and 12 months [18 (12, 45)] after surgery showed a significant decrease compared to preoperative scores [79 (72, 82)] ( P<0.05). Conclusions:The combination of HRCM preoperative assessment and NOSES Jinling surgery is both safe and feasible for the treatment of slow transit constipation, warranting further development and promotion.
9.Research on medication rules of Chinese medicine in treating chronic kidney disease with spleen and kidney deficiency based on the mutual admiration of spleen and kidney

Objective Based on the theory of mutual admiration of spleen and kidney,this study intends to explore the medication patterns of traditional Chinese medicine for the treatment of spleen-kidney deficiency-type chronic kidney disease(CKD)by using data mining methods and to provide reference for the clinical treatment of spleen-kidney deficiency-type CKD.Methods The literatures included in China National Knowledge Infrastructure,Wanfang Data Knowledge Service Platform,and VIP databases were used as data sources.The literature related to traditional Chinese medicine treatment of spleen-kidney deficiency-type CKD was analyzed by Excel 2021,IBM SPSS Modeler 18.0,IBM SPSS Statistics 27,and systematic clustering analysis and finally visualized by Cytoscape 3.7.2,RStudio.Results A total of 90 prescriptions were included,involving 146 flavors of drugs.The top 5 high-frequency drugs were Huangqi,Fuling,Baizhu,Dahuang,and Danshen.The medicinal properties are mainly mild and warm;The medicinal taste is characterized by sweetness,bitterness,and pungent;The main meridians of drugs are spleen,liver,and kidney meridians.Association rule analysis demonstrated that the commonly used couplet drugs were Huangqi-Fuling and Huangqi-Baizhu;Commonly used corner drugs included Huangqi-Baizhu-Fuling and Huangqi-Dahuang-Fuling.Cluster analysis found that the clustering effect of spleen-kidney deficiency-type CKD treatment drugs in five categories was better.Conclusion The medication rules of traditional Chinese medicine in the treatment of spleen-kidney deficiency-type CKD are preliminarily clarified,which provides a basis for clinical medication and new prescription development.

Objective To explore the mechanism of Bufei Jiedu Granules in the treatment of methicillin-resistant Staphylococcus aureus(MRSA)chronic infection using network pharmacology;To verify it through animal experiments.Methods Active components and potential targets in Bufei Jiedu Granules were screened through the TCMSP database,and genes related to MRSA infection were retrieved through GeneCards,OMIM,DisGeNET,TTD,DrugBank and PharmGKB databases.The STRING database was employed to construct a protein-protein interaction network on common targets,and GO and KEGG pathway enrichment analysis were conducted to identify key signaling pathways for Bufei Jiedu Granules treatment of MRSA infection.The effects of Bufei Jiedu Granules on bacterial load and pathological changes in the lung,liver and kidney of MRSA chronic infection mice models were evaluated through WT and T/B immune cell deficient Rag2-/-mouse animal experiments.The mRNA expressions of inflammatory factors(IFN-γ,IL-10)and immune checkpoints(PD1,TIM3)were detected.Results Totally 54 active components related to Bufei Jiedu Granules were selected,and 50 potential targets related to MRSA infection were identified.118 signaling pathways significantly associated with MRSA infection were identified through GO and KEGG pathway enrichment analysis,in which the JAK-STAT signaling pathway,Th17 cell differentiation,and PD-L1 expression and PD-1 checkpoint pathway were closely related to cell activation and T cell differentiation.Animal experimental results indicated that Bufei Jiedu Granules could effectively reduce the bacterial load in organs and ameliorate the pathological damage in the chronic MRSA infection mouse model,increase the mRNA expression of IFN-γ in the lung tissue,and decrease the mRNA expressions of IL-10,PD1 and TIM3.Conclusion Bufei Jiedu Granules has the characteristics of multi-component and multi-target in the treatment of MRSA infection,and may be involved in adaptive immune activation to effectively treat chronic MRSA infection.
