As the core vehicle for preserving and transmitting traditional Chinese medicine(TCM) academic thought and clinical experience, the establishment of a robust quality evaluation system for TCM clinical case reports is a crucial component in the current standardization and modernization of TCM. Based on the practical experience of constructing the China Clinical Cases Library of Traditional Chinese Medicine by the China Association of Chinese Medicine, this study conducted a comprehensive analysis of critical challenges, including insufficient authenticity and unfocused evaluation criteria. It proposed a three-dimensional evaluation framework grounded in the structure-process-outcome logic, encompassing three dimensions of authenticity and standardization, characteristics and advantages, application and translational impact. This framework integrated 12 key evaluation indicators in a systematic manner. The model preserved the academic characteristics of TCM syndrome differentiation and treatment, while aligning with modern scientific research standards, achieving a balance between individualized TCM experience and standardized evaluation. Concurrently, this study provided theoretical foundations and methodological guidance for evaluating the quality of TCM clinical cases, contributing significantly to the inheritance of TCM knowledge, evidence-based practice, and the reform of talent evaluation mechanisms.
2.Differences of calorie restriction and time-restricted feeding on metabolic indices and gut microbiota of mice

BACKGROUND:Both calorie restriction and time-restricted feeding,as two common dietary patterns,have been shown to improve health by regulating metabolism.However,the difference between these dietary patterns,metabolic indices,as well as the gut microbiota still requires further attention.OBJECTIVE:To explore the differences of calorie restriction and time-restricted feeding on the metabolic indices and gut microbiota of mice.METHODS:The C57BL/6J mice were randomly divided into three groups of ad libitum,calorie restriction,and time-restricted feeding(n=6 per group)for 28 weeks of dietary intervention.Various parameters such as body weight,food intake,glucose tolerance,serum fasting insulin,Homeostasis Model Assessment of Insulin Resistance,and leptin were measured.The impact of different interventions on the gut microbiota structure in mice was explored using 16S rRNA sequence analysis.Key operational taxonomic units responsive to dietary interventions were identified through LEfSe analysis.RESULTS AND CONCLUSION:(1)Compared with the ad libitum group,the body weight,food intake,area under the glucose tolerance curve of the calorie restriction and time-restricted feeding groups were decreased(P<0.01),and the serum leptin was decreased(P<0.05).The fasting insulin level and serum leptin level of the calorie restriction group were decreased(P<0.05)and were significantly lower than those of the time-restricted feeding group(P<0.05);homeostasis model assessment of insulin resistance decreased in the calorie restriction group(P<0.01).(2)Compared with the ad libitum group,the αdiversity of gut microbiota in the calorie restriction group and the time-restricted feeding group was decreased(P<0.05),but the diversity of the time-restricted feeding group was slightly lower than that in the calorie restriction group.(3)There were 15 key operational taxonomic units related to calorie restriction and the time-restricted feeding intervention,of which 8 were positively correlated with metabolic phenotypes and their abundance decreased,and 3 were negatively correlated with metabolic phenotypes and their abundance increased(P<0.05).OTU819 Lachnospiraceae_UCG-006 was positively correlated with body weight,area under the glucose tolerance curve,homeostasis model assessment of insulin resistance,and fasting insulin,while OTU1397 Muribaculaceae was negatively correlated with these indicators.The results show that both calorie restriction and the time-restricted feeding intervention can improve the weight and glucose metabolism of mice,and both intervention modes caused the remodeling of the gut microbiota,which helped to improve the metabolic disorders.
3.The plague epidemic and environmental factors in Lincang City from 1990 to 2020: a multiple correspondence analysis

Objective:This study aims to investigate the relationship between plague epidemics and environmental factors in Lincang City.Methods:Epidemiological survey data and environmental factors (including altitude, temperature, rainfall, and season) from plague occurrence sites in Lincang City from 1990 to 2020 were collected from Lincang Center for Disease Control and Prevention and the data sharing platform of the data sharing service network of the China Meteorological Administration, and analyzed using multiple correspondence analysis (MCA).Results:From 1990 to 2005, a total of 38 outbreaks of plague among rodents had been reported, 17 of which involved human beings. Since 2006, the plague had entered a dormant period, with no further epidemics reported up to 2020. The plague epidemic in Lincang City exhibited a clear seasonal variation, with the primary epidemic season occurred from autumn to early winter, peaking in September and October. Furthermore, the plague epidemic in Lincang City exhibited a pronounced spatial aggregation, with the primary affected regions including Linxiang County, Gengma Dai and Va Autonomous County, Zhenkang County, and Yun County. MCA showed that the Cronbach' α coefficients of all variables in the first and second dimensions were 0.87 and 0.82, respectively, and the characteristic roots were 3.27 and 2.91, respectively. The cumulative contribution rate of the two dimensions was 84.60%. Plague outbreaks were more likely in winter in areas at altitudes of > 1 400 - 1 650 m with 1 000 - 1 200 mm average annual rainfall and 16.0 - 17.9 ℃ average annual temperature, with increased severity. Regions at altitudes of > 500 - 700 m and > 900 - 1 150 m, with average annual rainfall of < 1 000, 1 201 - 1 400 and > 1 600 mm and average annual temperature of 18.0 - 19.9 ℃, showed a higher susceptibility to plague outbreaks, with notably pronounced incidences.Conclusions:The prevalence of plague epidemics is closely related to the local climatic conditions of natural foci in Lincang City. Therefore, it is imperative to enhance the monitoring of these climatic conditions, particularly meteorological data, to facilitate more effective prevention and control of plague outbreaks.

6.Prevalence and risk factors of work-related musculoskeletal disorders among male bus drivers in a mega-city

Background Bus drivers are a high-risk group for work-related musculoskeletal disorders (WMSDs). There are a large number of bus drivers in mega-cities. High volumes of passenger traffic and complexity of road conditions may elevate their risk of WMSDs, but there are few studies related to this group. Objective To investigate the prevalence of WMSDs among bus drivers in a mega-city and to analyze potential influencing factors. Methods Based on cross-sectional study design and self-administered questionnaire, the prevalence of WMSDs in past 12 months were estimated by stratified cluster sampling among bus drivers in a mega-city. Pearson χ2 and logistic regression models were used to analyze the influencing factors for the body regions with a high prevalence. Objective:To explore the safety and efficacy of laparoscopic right posterior sectionectomy (LRPS) in the left side lying at 60° run-hug position.Methods:The clinical data of 12 cases underwent LRPS at the Department of Biliary and Pancreatic Surgery, Sun Yat-sen Memorial Hospital, Sun Yat-sen University from August 2022 to June 2023 were analyzed. There were 9 males and 3 females, ranging in age from 26 to 76 years old. To evaluate the safety and effectiveness of the operation, operation time, intraoperative blood loss, the number and time of intraoperative hilar occlusion, postoperative drainage time, postoperative hospitalization time and postoperative complications were summarized.Results:Twelve operations were successfully completed under laparoscopy, and no patient was converted to laparotomy due to the difficulty in controlling intraoperative bleeding. 12 patients included 1 case of intrahepatic bile duct stones, 5 cases of primary liver cancer, 2 cases of hepatic hemangioma, 2 cases of focal nodular hyperplasia of the liver, 1 case of metastatic liver cancer, and 1 case of liver abscess. Operation time was (246.2±90.4) min, and the intraoperative blood loss was 125(22, 275) ml. The number of intraoperative hilar occlusion was 2.00(0.25, 2.75), and intraoperative hilar occlusion time was (27±22) min. Postoperative drainage time was (5.0±2.6) d, and the length of postoperative hospital stay was 6.0(4.3, 8.0) d. Intraoperative blood transfusion occurred in 1 patient, and postoperative abdominal effusion occurred in 1 patient. There was no postoperative death. All patients were recovered and discharged successfully.Conclusion:LRPS is safe and effective for the lesions in the right posterior hepatic region with the left side lying at 60° run-hug position.
8.Antibiotics-mediated intestinal microbiome perturbation aggravates tacrolimus-induced glucose disorders in mice.

Both immunosuppressants and antibiotics (ABX) are indispensable for transplant patients. However, the former increases the risk of new-onset diabetes, whereas the latter impacts intestinal microbiota (IM). It is still unclear whether and how the interaction between immunosuppressants and ABX alters the IM and thus leads to glucose metabolism disorders. This study examined the alterations of glucose and lipid metabolism and IM in mice exposed to tacrolimus (TAC) with or without ABX. We found that ABX further aggravated TAC-induced glucose tolerance and increased insulin secretion. Combined treatment resulted in exacerbated lipid accumulation in the liver. TAC-altered microbial community was further amplified by ABX administration, as characterized by reductions in phylum Firmicutes, family Lachnospiraceae, and genus Coprococcus. Analyses based on the metagenomic profiles revealed that ABX augmented the effect of TAC on microbial metabolic function mostly related to lipid metabolism. The altered components of gut microbiome and predicted microbial functional profiles showed significant correlation with hepatic lipid accumulation and glucose disorders. In conclusion, ABX aggravated the effect of TAC on the microbiome and its metabolic capacities, which might contribute to hepatic lipid accumulation and glucose disorders. These findings suggest that the ABX-altered microbiome can amplify the diabetogenic effect of TAC and could be a novel therapeutic target for patients.

10.Effects of light quality and germplasm on growth and effective ingredients of Dendrobium officinale germchit.

Effects of light quality and germplasm on the growth and effective ingredients of Dendrobium officinale germchit were studied. Under 8 light qualities (red, blue, yellow, green and three different red blue mix light), test-tube plantlets of different families (9 x 66, 17 x 30, 78 x 68) were used in the experiment to measure economical character and determine the content of chlorophyll, polysaccharides and total alkaloids. The results showed that light quality, germplasm and their interaction had a significant effect on the growth of D. officinale germchit. The maximal root length and height of seedling were obtained under red light, under blue light, the stem diameter was thickest and the content of total alkaloids were highest, the content of chlorophyll and polysaccharides of test-tube plantlets under red blue mix light were higher than that of other treatments. This work has laid a foundation for the high quality, low carbon and efficient production of D. officinale, and gives useful tips on production of high quality ingredients.
