1.Mental health literacy and its relationship with symptoms of depression,anxiety,and insomnia in young adults

Objective:To explore the level of mental health literacy and its relationship with symptoms of de-pression,anxiety,and insomnia in young adults.Methods:A total of 10 273 young adults aged 18-23 in Guang-zhou were selected and assessed with the National Mental Health Literacy Questionnaire,Patient Health Question-naire-9(PHQ-9),General Anxiety Disorder 7-Item Scale(GAD-7)and Insomnia Severity Index(ISI).Results:Totally 1 137(11.1%)participants met the criteria for adequate mental health literacy,2 758(26.8%)participants were with symptoms of depression,1 355(13.2%)with symptoms of anxiety and 4 936(48.0%)experiencing symptoms of insomnia.Logistic regression analyses showed that after adjusting for other factors,substandard mental health literacy was an independent risk factor for symptoms of depression,anxiety and insomnia(OR=2.08,1.93,1.49;95%CI:1.74-2.48,1.52-2.46,1.31-1.70).Conclusion:The level of mental health literacy in young a-dults is insufficient,and the lack of mental health literacy is significantly associated with the symptoms of depres-sion,anxiety and insomnia.
2.Effects of exercise load on cardiac function in different bodily positions evaluated with three-dimensional speckle tracking technology

Objective:To investigate the effects of 10° head up tilt bed rest (HUT) on human cardiac function via 3D speckle tracking echocardiography (3D-STE), and to study the difference in cardiac function under the submaximal exercise load between the horizontal position and 10° HUBR.Methods:Thirty young healthy volunteers were recruited as the subjects, who were randomly divided into an 10° HUT exercise group and horizontal exercise group with 15 subjects in each. Subjects in both groups were asked to ride the bicycle ergometer in the 10° HUBR position and supine position respectively. The load started with 50 W and was increased by 25 W every 3 min until it reached the maximum of 125 W. Before the exercise (resting state), 1 min after the load was increased each time, and 3 min after exercise (recovery period), the following indices were collected: ①basic cardiac function indices: heart rate (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP), ②conventional cardiac ultrasound indices: left ventricular ejection fraction (LVEF), stroke volume (SV) and cardiac output (CO), ③left ventricular strain indices: global longitudinal strain (GLS), global circumferential strain (GCS), and global area strain (GAS) measured by 3D-SET. The changes of these indices in the 2 groups of subjects under different exercise loads were observed.Results:The differences in the major effect of the basic heart indices (HR, SBP and DBP), conventional cardiac ultrasound indices (LVEF, SV and CO) and left ventricular strain indices (GLS, GCS and GAS) in response to the exercise load were statistically significant ( F=194.90, 113.66, 17.19, P=0.017, 0.018, 0.001). With the increase of the exercise load, the basic heart indices and conventional cardiac ultrasound indices kept rising, the left ventricular strain indices reached the minimum under a moderate exercise load (75 W), HR, SBP and CO were higher than those of the resting state ( P<0.05 or 0.01). Both LVEF under exercise loads of 75, 100, 125 W and during recovery, and SV under exercise loads of 100, 125 W and during recovery were significantly higher than those of the resting state ( P<0.05 or 0.01), while GLS and GCS under exercise loads of 50, 75, 125 W ( P<0.05 or 0.01), and GAS under exercise loads of 50, 75 W ( P<0.01) were significantly lower. There were statistically significant differences not only in GCS across the groups ( F=4.60, P=0.026) but also in DBP due to the interactions between the grouping and exercise loads ( F=3.13, P=0.031). DBP was higher than that of the resting state when the exercise load was 125 W in both groups. Conclusions:During submaximal exercise, myocardial contractility shows sustained enhancement with the increase of the exercise load. The results of GLS, GCS and GAS indicate that myocardial strain reaches its lowest value under a moderate exercise load, suggesting that moderate exercise can be used to evaluate cardiac function via 3D-SET. Under a simulated lunar gravity of 10° HUT, there is less deformation in the short axis direction of the myocardium, indicating that GCS can be used as a sensitive indicator to detect changes in cardiac function under different gravities.

Objective:To evaluate the efficacy and safety of nebulized inhalation of recombinant human interferon (IFN) α1b injection in the treatment of respiratory syncytial virus (RSV) associated lower respiratory tract infections (pneumonia and bronchiolitis) in children.Methods:A randomized, double-blind, parallel, placebo-controlled add-on design was used.Children with pneumonia or bronchiolitis aged 2 months to 5 years who tested positive for RSV antigen within 72 hours of onset from 30 clinical trial sites including Beijing Children′s Hospital, Capital Medical University between February 2021 and December 2022 were included in this study and randomly divided into 2 groups at a ratio of 1∶1 based on a stratified-block method.Both groups received basic treatments such as cough control, asthma relieving, expectorant treatment, fever reduction, oxygen therapy, etc.The experimental group received additional nebulized inhalation of IFN α1b injection at a dose of 2.0 μg/(kg·time), twice a day.The control group received nebulized inhalation of placebo twice a day.Clinical efficacy was evaluated based on indicators such as the duration of clinical symptoms and signs, and the Kaplan-Meier method was used to calculate the median and 95% CI of the duration of clinical symptoms and signs.The Log-rank test was used to compared data between groups.Safety was assessed through the incidence of adverse reactions and laboratory tests, and the Chi-square test was used to analyze the difference between groups. Results:There were 123 children in the experimental group and 122 children in the control group.The median durations of all the 5 clinical symptoms and signs [including shortness of breath, wheezing, dyspnea (visible retractions), decreased transcutaneous oxygen saturation, and abnormal mental state] in the experimental group after treatment were slightly shortened than those in the control group [2.7 d(95% CI: 1.9-3.0 d)] vs.[2.9 d(95% CI: 2.6-3.6 d), P=0.027].The improvement in dyspnea (retractions) was especially pronounced in the experimental group, with a relief rate of 50.0% (0, 100%) on the first day of administration[compared with 0 (0, 50.0%) in the control group ( Z=2.002, P=0.025)].The median duration of dyspnea in the experimental group was nearly 1 day shorter than that in the control group [1.0 d(95% CI: 0.7-1.7 d) vs.1.8 d(95% CI: 1.0-2.5 d), P=0.046].There were no significant difference in hospital stay [6.0(5.0, 8.0) d vs.6.5(5.0, 8.0) d, Z=0.675, P=0.500], oxygen therapy duration [32.0(14.0, 96.3) h vs.39.0 (24.0, 83.2) h, Z=0.094, P=0.925], the recovery rate from clinical symptoms during treatment [(105/106, 99.1%) vs.(96/101, 95.0%)], and recurrence rate [(0/106, 0) vs.(2/101, 2.0%)] between the 2 groups (all P>0.05).However, the above-mentioned four indicators in the experimental group showed a trend of clinical benefits.The quantitative virus detection results showed that the RSV viral load in both groups decreased after treatment compared to before treatment.After 2 days of treatment, the decline rate of RSV viral load from the baseline was 0.90 lg copies/(mL·d) in the experimental group and 0.25 lg copies/(mL·d)in the control group, with a statistically significant difference ( P<0.05).Furthermore, there was no statistically significant difference in the incidence of adverse reactions between the 2 groups ( P>0.05).Importantly, no drug-related serious adverse reactions occurred in both groups. Conclusions:The nebulized inhalation therapy of IFN α1b demonstrates efficacy and safety in treating pediatric RSV associated lower respiratory tract infections.It particularly offers outstanding clinical therapeutic value for severe children.

Cancer stem cells (CSCs) are proposed to account for the progression, metastasis, and recurrence of diverse malignancies. However, the disorganized vasculars in tumors hinder the accumulation and penetration of nanomedicines, posing a challenge in eliminating CSCs located distantly from blood vessels. Herein, a pair of twin-like small-sized nanoparticles, sunitinib (St)-loaded ROS responsive micelles (RM@St) and salinomycin (SAL)-loaded GSH responsive micelles (GM@SAL), are developed to normalize disordered tumor vessels and eradicate CSCs. RM@St releases sunitinib in response to the abundant ROS in the tumor extracellular microenvironment for tumor vessel normalization, which improved intratumor accumulation and homogeneous distribution of small-sized GM@SAL. Sequentially, GM@SAL effectively accesses CSCs and achieves reduction-responsive drug release at high GSH concentrations within CSCs. More importantly, RM@St significantly extends the window of vessel normalization and enhances vessel integrity compared to free sunitinib, thus further amplifying the anti-tumor effect of GM@SAL. The combination therapy of RM@St plus GM@SAL produces considerable depression of tumor growth, drastically reducing CSCs fractions to 5.6% and resulting in 78.4% inhibition of lung metastasis. This study offers novel insights into rational nanomedicines designed for superior therapeutic effects by vascular normalization and anti-CSCs therapy.

8.Predictive value of lung immune prognostic index combined with prognostic nutritional index for the response to immunotherapy in patients with non-small cell lung cancer

Objective:To evaluate the prognostic value of the lung immune prognostic index (LIPI) combined with the prognostic nutritional index (PNI) for predicting immunotherapy response in patients with non-small cell lung cancer (NSCLC). Methods:A total of 106 patients with NSCLC who received immunotherapy in Zhumadian Central Hospital from January 2022 to December 2023 were selected and assigned into a response group (n=72) and non-response group (n=34) according to their response after immunotherapy. Clinical data,LIPI,and PNI of the two groups were compared to analyze the relationship between LIPI and PNI with clinicopathological features and treatment response and to analyze the predictive value of LIPI and PNI in the immunotherapy response of patients with NSCLC. The precision-recall (PR) curve was used to evaluate combined predictive values. Results:The TNM stage,lymph node metastasis,and distant metastasis were significantly different between the two groups (P<0.05). The proportion of patients with good LIPI in the non-response group was lower than that in the response group,the proportion of patients with poor LIPI was higher than that in the response group (P<0.05),and the PNI in the non-re-sponse group was lower than that in the response group (P<0.05). LIPI positively correlated with TNM stage,lymph node metastasis,distant metastasis,and treatment response,whereas PNI negatively correlated with TNM stage,lymph node metastasis,distant metastasis,and treatment response (P<0.05). Objective:To explore the clinical effects of Wenjing Decoction in treating endometriosis syndrome with cold coagulation and blood stasis.Methods:A randomized controlled trial was performed. A total of 90 cases were collected and with distributed random number method of drugs were divided into control group and the treatment group with 45 cases each. The treatment group was given Wenjing Decoction granules, while control group was given placebo (10% Wenjing Decoction). Both groups were taken for 14 consecutive days from the first day of menstruation as a course of 1, a total of 3 courses. Before and after treatment, main observation indexes including dysmenorrhea, Traditional Chinese Medicine (TCM) syndromes and the secondary outcome including scores of menstruation, short form McGill pain questionnaire (SF-MPQ) were detected by subjective evaluation, interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α), and prostaglandin F2α (PGF2α) level in human serum tissue were evaluated by enzyme linked immunosorbent assay and the clinical and TCM syndromes specific effects were determined.Results:Totally 7 patients were lost, including 1 case in the treatment group and 6 cases in control group. The number of actual observation cases was 83 (44 cases in the treatment group and 39 cases in control group). The clinical effective rate in the treatment group [75.00% (33/44)] was statistically higher than that in control group [23.08% (9/39), P<0.001]. As for therapeutic effect of TCM syndromes, the effective rate in the treatment group [70.45% (31/44)] was obviously higher than that in control group [20.51% (8/39), P<0.001]. After treatment, compared with the results of control group, menstruation [2.00 (1.00,4.00)], dysmenorrhea [7.25 (5.00,9.88)], SF-MPQ [10.00 (4.50,18.00)], TCM syndromes [13.50 (9.25, 20.00)] and the data of IL-8 [9.80 (5.73,27.53) ng/L], TNF-α [5.05 (4.10,6.20) ng/L] and PGF2α [(147.16±22.24) ng/L] in the treatment group were decreased obviously [4.00 (2.00,6.00), P=0.022; 8.00 (6.50,12.00), P=0.030; 13.00 (4.00,24.00), P<0.001; 17.00 (9.00, 26.00), P<0.001; 18.20 (10.00,30.90) ng/L, P=0.013; 5.80 (4.50,10.70) ng/L, P=0.035;(165.17±30.33) ng/L, P=0.001]. Conclusion:Wenjing Decoction can treat TCM syndromes of endometriosis patients with cold coagulation and blood stasis, and the clinical curative effect was significant. The mechanism may involve enhancing immunity and relieving uterine smooth muscle contraction.
