1.Qualitative and Quantitative Analysis of Chemical Constituents in Gualou Niubangtang by UPLC-Q-TOF-MS/MS and HPLC

ObjectiveThis paper aims to clarify the material basis of Gualou Niubangtang and establish a quantitative analysis method for its main constituents, providing a reference for the overall quality control of this preparation. MethodsThe constituents in the formula were systematically characterized based on ultra-performance liquid chromatography-quadrupole time-of-flight tandem mass spectrometry (UPLC-Q-TOF-MS/MS). Identification was performed by matching with the UNIFI 9.6 software and utilizing database platforms such as PubChem, ChemicalBook, and ChemSpider, combined with relevant literature reports. A quantitative analysis method for the seven main constituents in Gualou Niubangtang was established by using high performance liquid chromatography (HPLC). ResultsUPLC-Q-TOF-MS/MS analysis identified 155 constituents, including 69 flavonoids, 36 terpenoids, 23 phenylpropanoids, 8 phenylethanoid glycosides, and 19 other types of constituents. In the established quantitative analysis method, the seven main constituents showed good linearity within their respective linear ranges. The precision, repeatability, stability, and spike recovery all met the required standards. The results showed that the content ranges of geniposide, liquiritin, hesperidin, arctiin, baicalin, oroxylin A-7-O-β-D-glucuronide, and wogonoside in 15 batches of Gualou Niubangtang were 13.67-21.25, 1.20-7.64, 5.45-7.45, 22.97-33.51, 29.95-39.07, 2.58-4.80, and 6.56-9.31 mg·g-1, respectively. ConclusionThis study successfully characterizes and attributes multi-category constituents in Gualou Niubangtang, clarifying that its material basis is primarily composed of flavonoids, terpenoids, phenylethanoid glycosides, and phenylpropanoids. Furthermore, it enables the quantification of seven constituents within the formula. This work lays a foundation for research on the quality control, action mechanism, and clinical application of this formula.
2.Construction of Surgical Pharmaceutical Risk Management Index System by Delphi Method and Analytic Hierarchy Process

Objective To construct a surgical pharmaceutical risk management index system and then to enhance surgi-cal pharmaceutical service quality.Methods Literature research and consulting pharmaceutical experts were used to collect relevant data and construct initial scale items.The Delphi expert consultation method was used to revise and improve indicators with 20 experts,after 2 rounds of inquiry improvement.The analytic hierarchy process(AHP)was used to calculate the weight of each indicator,finally forming the surgical pharmaceutical risk management index system.Results The final construction of the scale entries includes four dimensions,14 second-grade indexes,and 71 third-grade indexes.The questionnaire response rates were 100.00%,while the authority coefficient of experts was 0.832.In the consultation,the harmony coefficients of the first,second,and third indicators were 0.743,0.491,and 0.277,respectively.The AHP was used to determine the weights of indicators,and the re-sult passed the consistency test.The professional factors and pharmacist factors of the first indicators are large in weight.Con-clusion The constructed surgical pharmaceutical risk management index system is scientific and practical,which can provide a reference for the clinical work of surgeons and maximize the risk of avoidance.
3.Interventional treatment of lumbar disc herniation based on degeneration and fibrous ring rupture degree

Objective To observe the value of interventional treatment of lumbar disc herniation(LDH)based on degree of degeneration and fibrous ring rupture.Methods Totally 198 patients with LDH were retrospectively enrolled.Based on findings in digital subtraction angiography(DSA),the degree of degeneration and fibrous ring rupture were evaluated and different treatments were performed,and the efficacies were observed.Results Intradiscal electrothermal annuloplasty(IDET)were performed in 48 cases with fibrous ring degeneration and rupture of intervertebral disc but without protrusion,percutaneous lumbar diskectomy(PLD)/percutaneous laser disk decompression(PLDD)+ozone ablation were performed in 25 cases with grade 1 degeneration and grade 2 rupture with mild protrusion,targeted radiofrequency+ozone ablation were done in 23 cases with grade 2 degeneration and grade 2 or above rupture with mild protrusion,collagenase dissolution+PLD were done in 100 cases with grade 2 or above degeneration and grade 2 or above rupture with moderate or above protrusion and contrast agent entered into protruded intervertebral disc,while percutaneous cement discoplasty(PCD)were performed in 2 cases with grade 3 degeneration and grade 3 rupture with intervertebral stenosis.Twelve months after treatment,among 198 cases,excellent,good,acceptable and poor therapeutic effect was observed in 75,101,16 and 6 cases,respectively,with the effective rate of 96.97%(192/198).Conclusion Interventional treatments of LDH based on the degree of degeneration and fibrous ring rupture were valuable.
4.Exploring effect of grifola frondosa extract on intestinal inflammatory response in rats with ulcerative colitis based on SPHK1/S1P signaling pathway

Objective:To investigate the immunological mechanism by which grifola frondosa extract improves colonic tissue inflammation in rats with ulcerative colitis(UC)through the sphingosine kinase 1(SPHK1)/sphingosine-1-phosphate(S1P)signaling pathway.Methods:Forty male SD rats were randomly divided into five groups:blank group,model group,sulfasalazine treatment group(SASP group),grifola frondosa extract treatment group(GF group),and sulfasalazine combined with grifola frondosa extract treatment group(SASP+GF group).UC model was established using a 3%dextran sulfate sodium(DSS)free drinking method.After one week,each treatment group received sulfasalazine 0.3 g/(kg·d),grifola frondosa extract 10 mg/(kg·d),and combination of both drugs by gavage.During the experiment,the general condition of the rats was observed,the disease activity index(DAI)score was re-corded and the protein content and positive expression levels of SPHK1,S1P,tumor necrosis factor receptor-associated factor 2(TRAF2)and TNF-α in rat colon tissue were detected by immunohistochemistry.mRNA and protein expression levels of SPHK1,S1P,TRAF2 and TNF-α in rat colon tissue were measured by RT-qPCR and Western blot.Results:Compared with the blank group,the general condition of the model group rats were poor,the DAI score was significantly increased,and the protein positive expres-sion,mRNA and protein expression levels of SPHK1,S1P,TRAF2 and TNF-α in colon tissue were significantly increased(P<0.01).Compared with the model group,the general condition of the rats in each treatment group improved significantly,the DAI score was decreased(P<0.01),and the positive expression of each target protein was significantly reduced(P<0.01),especially in the GF group and SASP+GF group;the mRNA and protein expression levels of SPHK1 and TRAF2 were reduced to varying degrees(P<0.01 or P<0.05),while the mRNA and protein expression levels of S1P and TNF-α only decreased significantly in the GF group and SASP+GF group(P<0.01).Compared with the SASP group,the GF group only showed a decrease in SPHK1 protein expression,TNF-α mRNA,and protein expression levels,while the SASP+GF group showed significant reductions in all targets(P<0.01 or P<0.05).Compared with the GF group,the SASP+GF group showed significant reductions in SPHK1 protein positive expression and content,S1P mRNA expression levels,and TNF-α protein content(P<0.05 or P<0.01).Conclusion:Grifola frondosa extract may alleviate co-lonic tissue inflammation in rats with UC by inhibiting the activation of the SPHK1/S1P pathway,restoring intestinal mucosal barrier function,and improving symptoms of UC.
5.Application of wearable devices in weight management for adults: a scoping review

Objective:To implement a scoping review of the application of wearable devices in adult weight management, identifying the types and characteristics, working principles and functions, impact on healthy lifestyles, effectiveness and user adherence of wearable devices.Methods:According to the scoping review framework, the literature on the application of wearable devices in weight management for adults was systematically searched in PubMed, Embase, Web of Science, Cochrane Library, CINAHL, PsycINFO, China National Knowledge Infrastructure, WanFang Data, VIP, and China Biology Medicine disc. The search period was from database establishment to June 20, 2024. Two researchers performed data extraction and summary analysis of the included literature.Results:A total of 23 papers were included, including 21 randomized controlled trials and two quasi-experimental studies, published from 2011 to 2024. Wearable devices combined with lifestyle interventions achieved weight management goals by reducing adult weight, waist circumference and body fat, and increased physical activity, reduced sedentary behavior and improved eating habits.Conclusions:Wearable devices demonstrate some advantages in weight management, but their use still needs to be supported by further empirical studies to ensure their effectiveness in different populations and settings to inform the development of weight management intervention programs.

10.Investigation on the current status of medication education in medical institutions in China

OBJECTIVE To investigate the development and current status of medication education in domestic medical institutions, aiming to provide reference and suggestions for better development of medication education. METHODS The online questionnaire survey was conducted in March 2023 to investigate the implementation of medication education in medical institutions at all levels nationwide; descriptive analysis and influencing factors analysis were conducted. RESULTS A total of 1 368 questionnaires were recycled, 1 304 of which were effective questionnaires, with the effective rate of 95.32%. The average rate of providing medication education was 73.62% in medical institutions nationwide, 76.05%, 67.68% and 73.76% respectively in Eastern, Central and Western regions, 87.11%, 60.57% and 46.32% respectively in tertiary, secondary and primary medical institutions. The commonest place and way of carrying out medication education were dispensing window and oral instructions, and both were more diverse in tertiary medical institutions, compared with second and primary medical institutions. The median annual service volume of medication education in medical institutions PUMCH-A- at all levels was 500 people; the higher the level of medical institutions, the larger the annual service volume of medication education (P=0.023). More than half of medical institutions didn’t have any form of compensation for medication education, which was mostly reflected in the workload of medical institutions with compensation. Grade of medical institutions, degree of information and automation were the major influencing factors of carrying out medication education. CONCLUSIONS The ratio of carrying out medication education is improved in the central region in China in recent years, compared with 2019; while that of primary institutions in all regions is at a low level and should be improved. The place of carrying out medication education should be set according to the characteristics of medical institutions, and medication education forms should be enriched as much as possible. The quantity and quality of medication education talents still need to be improved. Compensation for pharmaceutical care should be continually explored and implemented to prompt high-quality and sustainable development of medication education.
