1.Study on the evaluation value the effected ejection fraction in assessment of cardiac function in the elderly patients with heart failure

Objective To Study the value of effected ejection fraction(eEF)in assessment of cardiac function in elderly patients with heart failure.Methods A total of 1 134 elderly inpatients in this hospital from January 2019 to December 2023 were selected as the research subjects.After admission,relevant examinations and tests were completed,and the cardiac function classification of the New York Heart Association(NYHA)was carried out.Sequential electrocardiogram(ECG)and echocardiogram examinations were completed before treatment.Age,under-lying diseases,left ventricular ejection fraction(LVEF),N-terminal pro-brain natriuretic peptide(NT-proB-NP),and QT interval(QTc)of the ECG were included for classification and statistics,and eEF was obtained,and the evaluation value of eEF on cardiac function was analyzed.Results The numbers of patients with NY-HA cardiac function grades Ⅰ—Ⅳ were 86,434,454 and 160,respectively,and LVEF decreased with the in-crease of the NHYA cardiac function grade.There was statistically significant differences in LVEF among dif-ferent NHYA cardiac function classifications(P<0.05).The QTc of the patients was(434.71±40.96)ms,the eEF was(8.73±2.03)ms,and the ln(NT-proBN)was 7.05±1.70.QTc,eEF,and ln(NT-proBN)all in-creased with the increase of the NHYA cardiac function classification,and there were statistically significant differences in indicators above among different NHYA cardiac function classfications(P<0.05).There was a low correlation between QTc of the ECG,age and cardiac insufficiency(r<0.5,P<0.05);there was a high correlation between eEF,LVEF,and ln(NT-proBNP)and cardiac insufficiency(r≥0.5,P<0.05).Conclusion eEF still has discriminatory value for patients with heart failure with preserved ejection fraction,and it is a good indicator for evaluating the cardiac function of elderly patients with heart failure.
2.A review of the relation between peripheral anatomical structures and external apical root resorption during orthodontic treatment

External apical root resorption(EARR)is one of the most common side effects of orthodontic treatment.Contact with sur-rounding anatomical structures during tooth movement is a significant cause for EARR.However,a comprehensive review of factors leading to EARR due to direct contact of surrounding anatomical structures with the root apex during orthodontic treatment is still lac-king.This review summarizes the anatomical structures related to EARR,including alveolar bone,incisive canal,maxillary sinus,ad-jacent teeth,and bone islands.Alveolar bone,incisive canal,and adjacent teeth can directly cause EARR during orthodontic treat-ment,while the impact of the maxillary sinus and bone islands on EARR has not been discovered so far.Analyzing the anatomical structures around the tooth roots can help develop more effective methods to prevent or reduce the occurrence of EARR during orthodon-tic treatment.
3.Establishment of a prognostic model for HER2 low expression breast cancer with lung metastasis

Purpose This study aimed to evaluate the consistency of human epidermal growth factor receptor 2(HER2)status between primary breast cancer lesions and lung metastatic lesions and to establish a prognostic model for predicting the survival rate of HER2 low expression(HER2-low)breast cancer patients with lung metastasis.Methods Clinicopathological data from a cohort of 252 patients with breast cancer and lung metastasis were retrospec-tively analyzed.Results 50.00%of the patients had HER2-low expression in metastatic lesions,and HER2-low ex-pression was the most prevalent subgroup in both primary and metastatic lesions.A discordance in HER2 status be-tween primary and metastatic sites was observed in 28.07%of cases.The most frequent shift was from HER2-zero in the primary tumor to HER2-low expression in the metastasis(12.28%of all cases).Estrogen receptor(ER)status,menopausal status,and histological type were identified as independent prognostic factors for overall survival(OS)by univariate and multivariate Cox regression analyses.A prognostic model incorporating these factors was constructed to predict 3-year and 5-year survival.The model demonstrated area under the curve(AUC)values of 0.765 and 0.780 for 3-year and 5-year OS in the training cohort,and 0.667 and 0.706 in the validation cohort,respectively.Conclu-sion HER2-low expression is the most common subtype among breast cancer patients with lung metastasis.The ob-served shift from HER2-zero in primary lesions to HER2-low in metastases underscores the clinical necessity of re-biop-sy at metastatic sites.The developed prognostic model effectively predicts OS in this patient population.
4.Analysis of DRG Cost Structure Variations from the Perspective of Resource Allocation

Objective:To deconstruct the overall medical expense and analyze systemic characteristics of cost structure dynamics in China from the perspective of optimizing healthcare resource allocation.Methods:Utilizing hospitalization records in a sample city from 2022-2023 as the research literature,the linear regression models for different cost components was established over the two-year period within each DRG-based case group to identify statistically significant temporal trends and directional changes in cost structures.Results:Most DRG groups exhibited significant trends in at least one cost component.Notable concentration patterns emerged in medication,comprehensive care and therapeutic service costs.When evaluating structure improvement,most achieved optimized allocation in at least one major cost component.Conclusion:The sample city demonstrates continuous optimization in healthcare cost structures,yet specific cost categories and DRG groups deviated from resource allocation optimization goals.Policy interventions should prioritize granular,DRG-specific adjustments to address heterogeneous structural dynamics.
5.Exercise prevention and treatment of insulin resistance:role and molecular mechanism of Keap1/nuclear factor erythroid2-related factor 2 signaling pathway

BACKGROUND:Exercise interventions play a key role in disease prevention and treatment,which can effectively activate the nuclear factor erythroid2-related factor 2(Nrf2)signaling pathway to prevent the occurrence and development of insulin resistance.However,the potential mechanism of Nrf2-targeting exercise therapy strategies in alleviating insulin resistance remains unclear.OBJECTIVE:Based on the relationship between Nrf2,insulin resistance and exercise,to analyze the mechanism and influence of different exercise modes on the activation of Nrf2,thereby elucidating the potential mechanism of Nrf2-targeting exercise therapy strategies in the process of alleviating insulin resistance.METHODS:"Diabetes mellitus,insulin,insulin resistance,nuclear factor E2 related factor 2,oxidative stress,ferroptosis,autophagy,inflammatory response,exercise"were used as search terms in Chinese and English.WanFang Database,CNKI,Google Scholar and PubMed database were searched for relevant literature from inception to October 2024,and a total of 127 core related articles were obtained according to the inclusion and exclusion criteria.RESULTS AND CONCLUSION:(1)Keap1/Nrf2 signaling pathway,as an important endogenous antioxidant stress pathway,plays an important role in insulin resistance.Activation of Keap1/Nrf2 signaling pathway can enhance the antioxidant capacity of cells,reduce oxidative stress and inflammation,improve insulin signaling,and thus protect insulin resistance.(2)A variety of exercise methods(including aerobic exercise,resistance exercise and high-intensity intermittent exercise)can effectively activate Nrf2 signal,improve the activity of antioxidant kinase and enhance the antioxidant capacity of cells,so as to alleviate oxidative stress damage to a certain extent.(3)By activating Nrf2 signaling pathway,exercise can up-regulate the activity of antioxidant enzymes such as Heme oxygenase 1,superoxide dismutase,catalase,glutathione peroxidase,glutathione,and enhance the antioxidant capacity of cells.In addition,it can also regulate the key enzymes and proteins in the ferroptosis pathway,such as glutathione peroxidase 4,membrane iron transport protein 1,and ferritin heavy chain 1,inhibit the ferroptosis pathway and promote the balance of iron metabolism.In autophagy,the expression of autophagy related genes p62 and ATG5/7 can be enhanced,and the level of microtubule-associated protein 1 light chain 3 is increased,thus regulating autophagy process.At the same time,the activity of tumor necrosis factor-α,nuclear factor-κB,interleukin-6,interleukin-1β and other pro-inflammatory cytokines is decreased,and the inflammatory response can be effectively inhibited.These combined effects can reduce oxidative stress damage,increase insulin sensitivity and improve insulin signaling,and thus have a positive effect on improving insulin resistance.(4)Given that the Keap1/Nrf2 signaling pathway plays an important role in the pathogenesis and treatment of insulin resistance,exercise therapy strategies targeting the Keap1/Nrf2 signaling pathway will help promote the development of"exercise+drug"precision medicine for insulin resistance.

10.Analysis of the therapeutic effect of ultrasound-guided microwave ablation in the treatment of liver metastasis from breast cancer and the selection of beneficiaries

Objective:To evaluate the therapeutic effect of ultrasound-guided microwave ablation (MWA) in the treatment of breast cancer liver metastases (BCLM), and to select the beneficiaries.Methods:A retrospective study was performed in 63 patients with BCLM who underwent ultrasound-guided microwave ablation in Department of Interventional Ultrasound, the Fifth Medical Center of Chinese PLA General Hospital, and had detailed follow-up data from February 2010 to October 2022. General patient data and laboratory results were collected. Univariate and multivariate Cox regression analyses were performed to determine the independent influencing factors for prognosis in patients with BCLM. A nomogram model was established to predict the survival rates of patients after operation.Calibration curve and decision curve were plotted to evaluate the calibration degree and the clinical benefit of the model, respectively. Time-dependent ROC curve was plotted to evaluate the predicting ability of the model. All patients were divided into high-risk group and low-risk group according to prognostic index. Survival curves were plotted to compare differences in survival between the two groups.Results:Multivariate Cox regression analysis showed that tumor burden score(TBS) and the presence of extrahepatic metastasis before MWA were independent influencing factors for prognosis of BCLM(all P<0.001). The combined prediction model was established based on TBS, extrahepatic metastasis and axillary nodal metastases. The areas under the 1-year, 2-year and 3-year ROC curves were 0.849, 0.855 and 0.878, respectively, suggesting that the model had good predicting ability. The calibration curve showed that the model had good calibration degree, while the decision curve showed that the model had good clinical practicability. The median survival time between the two groups was statistically significant[13.5 (9, 20) months vs 63.0 (39, 140) months, P<0.001]. Conclusions:Low-risk patients with BCLM gain significant clinical benefits after microwave ablation treatment, while high-risk patients do not exhibit a noticeable survival advantage.
