1.Ferroptosis in bone diseases:therapeutic targets of osteoporosis

BACKGROUND:With the aging of the global population,the incidence rate of osteoporosis is also increasing.It is very important to further understand its pathogenesis and propose new therapeutic targets.Recent studies have shown that ferroptosis is closely related to the pathogenesis of some bone diseases,such as inflammatory arthritis,osteoporosis and osteoarthritis. OBJECTIVE:To summarize the previous studies on the mechanism of ferroptosis in osteoporosis,so as to provide new therapeutic ideas and potential therapeutic targets for osteoporosis. METHODS:The first author used the computer to search the documents published from 2000 to 2022 in CNKI,WanFang,VIP,PubMed and Web of Science with the key words of"ferroptosis,osteoporosis,osteoblasts,osteoclasts,iron chelators,reactive oxygen species,nuclear factor erythroid 2-related factor 2,heme oxygenase-1,glutathione peroxidase 4,review"in Chinese and English.A total of 70 articles were finally included according to the inclusion criteria. RESULTS AND CONCLUSION:Ferroptosis is significantly different from necrosis,apoptosis and autophagy.In terms of cell morphology and function,it does not have the morphological characteristics of typical necrosis,nor does it have the characteristics of traditional apoptosis,such as cell contraction,chromatin condensation,the formation of apoptotic bodies and the disintegration of cytoskeleton.Contrary to autophagy,ferroptosis does not form a classical closed bilayer membrane structure(autophagic vacuole).Morphologically,ferroptosis is mainly manifested by obvious contraction of mitochondria,increased membrane density,and reduction or disappearance of mitochondrial cristae,which are different from other cell death modes.Iron overload can destroy bone homeostasis by significantly inhibiting osteogenic differentiation and stimulating osteoclast formation,leading to osteoporosis.Iron overload interferes with the differentiation of stem cells to osteoblasts,leading to a weakened osteoblast function and further imbalance of bone metabolism in the body,which eventually leads to osteoporosis.Stimulated by iron overload,osteoclast bone resorption is enhanced and bone loss exceeds new bone formation.Iron chelators have been proved to have osteoprotective effects by inhibiting osteoclast activity and stimulating osteogenic differentiation of osteoblasts.Its potential mechanism is related to inhibiting osteoclast differentiation and promoting osteoblast differentiation.Antioxidants can prevent reactive oxygen species production and inhibit bone absorption,thus improving bone metabolism and effectively preventing osteoporosis.
2.Analysis of the Relationship Between the Quality of Small Biopsy Specimens of Non-small Cell Lung Cancer and the Mutation Rate of EGFR Gene.

BACKGROUND:
Epidermal growth factor receptor (EGFR) is the gene with the highest mutation rate in non-small cell lung cancer (NSCLC) patients, and the accurate evaluation of its mutational status can facilitate patients receiving targeted drug therapy and thereby prolong patients' survival. The gene testing platform has adequacy requirements for the specimen quality in order to obtain accurate examination results. It has been reported that the number and proportion of tumor cells in samples will affect the detection rate of EGFR gene mutation. The present study aims to analyze the relationship between the quality of small biopsy specimens of NSCLC and the mutation rate of EGFR gene with amplification refractory mutation system (ARSM) test.
METHODS:
After collecting the clinical characteristics of 299 cases small biopsy of lung adenocarcinoma, DNA concentration of the specimens and the mutational status of EGFR gene, the number and proportion of tumor cells in HE stained sections evaluated using light microscopy, the relationship between specimen quality and the mutation rate of EGFR gene were analyzed.
RESULTS:
The mutation rates of EGFR for the groups with tumor cell number ≤500 and >500 were 40.7% (11/27) and 43.8% (119/272) respectively, without significant difference (P=0.764). The mutation rates for the groups with DNA concentration ≤20.4 ng/μL and >20.4 ng/μL were 42.7% (64/150) and 44.3% (66/149) respectively, without significant difference (P=0.776). The mutation rates for the groups with tumor cells proportion ≤30% and >30% were 29.4% (20/68) and 47.6% (110/231) respectively, demonstrating significant difference (P=0.008). Multivariate Logistic analysis showed that male, thyroid transcription factor-1 (TTF-1) negative, smoking history and tumor cell proportion less than 30% were main factors that contributes to the low detection rate of EGFR gene mutation.
CONCLUSIONS
After meeting the minimum requirements for detection, the EGFR mutation rate is affected by the proportion of tumor cells in the sample. Therefore, it is necessary to re-evaluate the tumor cell proportion in the last section after the genetic test section. For samples with lower tumor cell proportion, enriching tumor cells through microdissection and other methods is recommended for a more accurate detection result. For specimens that cannot be enriched with tumor cells, circulating tumor DNA (ctDNA) test can be performed as a supplement. If the result is still negative, another biopsy should be considered to obtain enough tumor specimens for molecular testing.

Objective:To select the relevant evidence of percutaneous pericardial drainage tube nursing and summarize the best evidence.Methods:Evidence-based questions were established based on PIPOST model. BMJ Best Clinical Practice, China Biology Medicine disc (CBM) , UpTodate, Cochrane Library, Joanna Briggs Institute Evidence-based Health Care Database, China Guide Network, British Guide Network, National Guide Line Clearing House (NGC) , PubMed, EMbase, Evidence-based Medicine (EBM) , Registered Nurses' Association of Ontario (RNAO) , The European Society of Cardiology (ESC) , American Heart Association (AHA) , Chinese Journal Full-text Database and Wanfang Database were conducted computer retrieval. The search time was from the establishment of the database to December 31, 2020. Two researchers respectively evaluated the quality of the included literature and extracted data and summarized and summarized the evidence that met the standards.Results:Finally, 12 articles were included, including 1 evidence summary, 2 systematic reviews, 1 systematic assesment, 2 guidelines, 1 expert consensus and 5 case series studies. Finally, 11 pieces of evidence were formed, including 6 themes such as drainage tube selection, puncture wound nursing, drainage flow control, flushing and sealing of the tube, observation and recording points, extubation indications and care.Conclusions:This study summarizes the best evidence for percutaneous pericardial drainage tube nursing, which provides evidence-based basis for improving the quality of percutaneous pericardial drainage tube care.
4.Advices on the prevention and control of nosocomial infection of COVID-19 within children′s hospitals

The epidemic of COVID-19 has become the topmost public health threat worldwide. The authors suggested that in addition to strengthening the organization and leadership of the abovementioned work, greater attention be paid to establishing and improving the prevention and control mechanism.Furthermore, special efforts should be given to the safety of the medical workers, by strengthening their infection monitoring and outbreak management. Medical workers in different work areas and positions should be placed under careful protection with due cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. Such special issues in the management of pediatric patients, as allocation of nurses, the screening and management of caregivers, the problem of breastfeeding, and the disinfection of children′s toys and diapers were discussed. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments, which aim to guide the prevention and control of COVID-19 nosocomial infection in the pediatric outpatient and ward.

The pneumonia caused by the novel coronavirus (2019-nCoV), which began in December 2019, has become the most serious public health problem, threatening people's health and life. This threat is posing a severe challenge on the diagnosis and treatment of 2019-nCoV infection, the prevention and control of hospital cross infection of medical staff. It is suggested that in addition to strengthening the organization and leadership of the abovementioned work, establishing and improving the prevention and control mechanism deserve greater attention. Furthermore, special attention should be given to the safety of the medical staff, strengthening their infection monitoring and outbreak management. Medical staff in different work areas and positions should be placed under careful protection, cleaning and disinfection measures. 6.Reconstruction of anterior chest wall with titanium plate and mesh: a report of 13 cases with manubrium tumor resection

Objective:To investigate the effect and quality of life after anterior chest wall reconstruction with titanium plate and mesh due to manubrium tumor resection.Methods:A retrospective analysis was performed from January 2012 to December 2016, a total of 13 patients with primary(11cases) or secondary(2 cases) manubrium tumor, aged 41-72 years(medium aged as 56), underwent oncological resection, following by immediate chest wall reconstruction with titanium plate and titanium mesh. The quality of life was evaluated by EuroQol five dimensions questionnaire (EQ-5D) and EQ-5D visual analogue scale (ED-VAS) before and 3 months after operation. Computed tomographic scan was taken 3 months after operation.Results:All the patients received successful operation. During the perioperative period, 12 cases recovered well without any complications, 1 patient experienced mild paradoxical movement within 9 days after operation and then recovered. The median follow-up period after surgery was 25 months (range from 7 to 41). Computed tomographic scans for the 13 patients showed neither dislocation nor deformation of the titanium plate and mesh 3 months after operation. One patient with chondrosarcoma died as a result of relapse 11 months after surgery. In pain/discomfort dimension, the pre-operation score was 1.85±0.80 and the post-operation score was 1.15±0.38, showing a significant difference( P=0.013). There was no significant difference in scores of other four dimensions between pre- and post-operation( P>0.05). The total score of EQ-5D decreased significantly after the operation (7.08±2.02 vs 5.45±0.52, P=0.010). The pre- and post-operation ED-VAS score was 85.69±7.58 and 92.54±2.53, with a significant difference( P=0.008). Conclusions:The immediate reconstruction of chest wall with titanium plate and mesh after oncological manubrium resection is effective and safe with improvement of the quality of life.

8.Effect of peer support education on the survival quality of urbanized type 2 diabetes patients in rural areas

Objective To explore the effect of peer support education on self-management ability and survival quality of urbanized type 2 diabetes patients in rural areas. Methods From January to September 2015, a total of 117 patients with type 2 diabetes were selected from three large-scale living communities of Fengxian District.Then,they were randomly divided into observation group(58 cases)and control group (59 cases) using random number method, respectively. And the same health education contents were conducted for the patients in both two groups.The patients in control group were educated by the diabetic nurse in classroom. Additionally, the peer support education was conducted for these patients in observation group for 6 months on this basis.The diabetes patient's self-management behavior scale and 36-item Short-Form Health Survey Questionnaire(SF-36)were used to assess the patients before intervention and 6 months later,respectively.Besides,the level of glycosylated hemoglobin was measured by the automatic biochemical analyzer. The self- management ability and survival quality and glycometabolism level were assessed as well. Results 6 months later after intervention, the survival quality score in observation group was as following:physical function(PF) 73.6±11.5,role physical(RP) 86.7 ± 14.5, body pain (BP) 76.3 ± 12.3, general health (GH) 64.5 ± 13.6, vitality (VT) 67.5 ± 12.3, social function (SF) 87.1 ± 16.2, role emotional (RE) 81.1 ± 14.2, mental health (MH) 68.9 ± 12.6, while that in control group was as following:physical function(PF)51.3±10.6,role physical(RP)62.1±12.1,body pain (BP)51.3±10.8,general health(GH)51.1±11.6,vitality(VT)56.4±12.1,social function(SF)70.1±14.1, role emotional (RE) 71.2 ± 13.9, mental health (MH) 56.1 ± 10.8 (t=2.316-6.365, P<0.05). The self-management score in observation group was following:diet management 5.79±0.98,exercise management 5.41 ± 0.57, blood glucose monitoring 4.01 ± 0.90, foot care 5.26 ± 0.89, drug administration 4.47 ± 0.90, smoking 0.86 ± 0.34, while that in control group was following: diet management 3.62 ± 0.99, exercise management 3.66 ± 0.94, blood glucose monitoring 1.90 ± 0.84, foot care 2.54 ± 0.62, drug administration 3.73 ± 0.80, smoking 0.76 ± 0.42 (t=4.659-19.137, P of rest was less than 0.01 except that of smoking equaled to 0.172 and t=1.375). Conclusions Peer support education can improve the quality of life, enhance self-management ability, control the level of glucose metabolism and has a strong operability. Therefore,it is worthy of promotion.
9.Effect of APP based continuous nursing on the quality of life of children with cerebral palsy

Objective To discuss the use of the APP to continue nursing effect on the quality of life of children with cerebral palsy. Methods A total of 100 cases of cerebral palsy hospitalized in July to December of 2016 were selected as the observation group, and the continuation nursing based on"Xinjiang cerebral palsy mobile APP"was carried out. A total of 102 cases of cerebral palsy were used as the control group in the hospital in January to June of 2016, and the traditional continuation nursing was carried out, and the self-care ability and quality of life of the children were evaluated by Activity of Daily Living Scale (ADL), Pediatric Quality of Life Inventory TM3.0-Rheumatology (PedsQLTM3.0) on admission hospital, discharge and 6 months after discharge. Results The time, time and group interaction of 2 groups of children with cerebral palsy in ADL score were statistically significant (F=231.819, 33.382, P<0.01). The ADL score of 6 months after discharge in the observation group was (66.100 ± 7.368) points, the control group was (59.230 ± 10.534) points, and there was statistically significant (t=3.802, P<0.01). The time, group and time group interaction of 2 groups of children with cerebral palsy in PedsQLTM3.0 score were statistically significant (F=23.398, 234.374, 149.209, P<0.01). There was a statistically significant difference in the observation group between admission hospital and discharge, 6 months after discharge and discharge (t=-3.384,-4.198, P<0.05 or 0.01). The difference was statistically significant in control group between discharge and admission hospital(t =-17.809, P <0.01), but there was no significant difference between 6 months after discharge and discharge (P> 0.05). Conclusions Based on the continuation of"Xinjiang cerebral palsy mobile APP", nursing can improve the ability to care for children with cerebral palsy life, help children improve quality of life. Worthy of further promotion.
10.Research of newly diagnosing the insulin Injections of aged patients with type 2 diabetes through interactive education norms

Objective To explore the application effect of interactive health education in insulin injections of aged patients with type 2 diabetes in the incipient stage of diagnosis and treatment. Methods 100 cases of aged patients with type 2 diabetes were hospitalized into endocrinology department from January 2014 to December 2014, the pen insulin injections were conducted for them at the beginning. They were equally divided into experimental group and control group through random number method. And, the general education guidance was conducted in control group. Meanwhile, the diabetes health education team was established in observation group on the basis of control group. Then, it was educated through the interactive lectures, interviews, teaching, repeated teaching, psychological intervention and individual assessment. Before and after education, the KAP survey of standard insulin injections were used for the patients in two groups to evaluate their degree of proficiency in the knowledge and skills about insulin injections, and the incidence of glucose metabolism level and low blood sugar. Results After intervention, the insulin injection skill in observation group was better than that of in the control group, (64.6%,31/48 vs. 24.5%, 12/149,χ2=16.441, P<0.01), while the level of glycosylated hemoglobin was reduced from (7.95±0.65)%to (6.58±0.62)%(t=7.85, P<0.01). Conclusions In the incipient stage of diagnosis and treatment, the ability of aged patients with type 2 diabetes in learning new knowledge and technology was improved through interactive health education mode. Besides, the insulin injections were standardized. Also, the glucose metabolism was improved, and the incidence of hypoglycemia was significantly reduced to ensure the patients′safety when the insulin was injected outside hospital, which was worth being clinically applied and promoted.
