1.Effects of inflammation on serum hepcidin and iron metabolism related parameters in patients with type 2 diabetes mellitus:a meta-analysis

OBJECTIVE:Disorders in iron metabolism increase the risk of type 2 diabetes mellitus.Hepcidin play an important role in maintaining iron homeostasis in the body,but its level increases with increased inflammation.Changes in hepcidin and iron homeostasis and the extent of their association with inflammation in people with and without type 2 diabetes mellitus are unknown.Meta-analysis was used to evaluate the effect of inflammation on serum hepcidin and iron metabolism related parameters in patients with type 2 diabetes mellitus.METHODS:CNKI,PubMed,Web of Science and EBSCOhost databases were searched by computer to collect observational studies related to inflammatory index and hepcidin in patients with type 2 diabetes mellitus.The search time was from September 1,2000 to September 30,2024.Three researchers independently screened the literature,extracted data and evaluated the quality of the included literature.Meta-analysis was performed by Review Manager 5.3,Stata 17.0 and GraphPad Prism 8.0.2 software.RESULTS:A total of 15 articles(17 studies)involving 3 159 participants,including 1 357 patients with type 2 diabetes mellitus,were included.Meta-analysis results showed that compared with the control group,patients with type 2 diabetes mellitus had higher levels of serum hepcidin[standardized mean difference(SMD)=0.35,95%confidence interval(CI)(0.05,0.65),P<0.05],serum ferritin(SMD=0.49,95%CI(0.21,0.78),P<0.01)and serum transferrin(SMD=0.19,95%CI(0.00,0.37),P<0.05).Subgroup analysis results indicated that inflammation had a significant effect on serum hepcidin(SMD=0.76,95%CI(0.17,1.34),P<0.05)and serum ferritin(SMD=0.77,95%CI(0.06,1.47),P<0.05)in patients with type 2 diabetes mellitus.CONCLUSION:Hepcidin concentration is positively correlated with type 2 diabetes mellitus.Inflammation is one of the risk factors of type 2 diabetes mellitus.Early prevention of inflammation has certain significance in preventing iron metabolism disorder in patients with type 2 diabetes mellitus.

3.Automatic measurement of acetabular cup anteversion angle using an accurate recognition technology based on improved Otsu algorithm and feature point.

The orientation of the acetabular cup in hip joint anteroposterior radiograph is a key factor in evaluating the postoperative outcomes of total hip arthroplasty (THA). Currently, measurement of the acetabular cup anteversion angle primarily relies on manual drawing of auxiliary lines by orthopedic surgeons and calculations using scientific calculators. This study proposes an automated computer-aided measurement method for the acetabular cup anteversion angle based on hip joint anteroposterior radiograph. The proposed method segments hip prosthesis images using an improved Otsu algorithm, identifies feature points at the acetabular cup opening by combining circle-fitting theory and the cup's geometric characteristics, and fits an ellipse to the cup opening to calculate the anteversion angle. A total of 104 hip joint anteroposterior radiographs, including 71 right-sided and 81 left-sided prostheses, were analyzed. Two orthopedic surgeons independently measured the postoperative anteversion angles, and the results were compared with computer-generated measurements for correlation analysis. Spearman and Pearson correlation analyses demonstrated significant correlations between the proposed method and manual measurements for both the right group ( r = 0.795, P < 0.01) and the left group ( r = 0.859, P < 0.01). This method provides a reliable reference for orthopedic surgeons to assess postoperative prognosis.

4.Characteristics of basic research articles published in Chinese Journal of Trauma from 1985 to 2024

Objective:To analyze the characteristics of basic research articles published in Chinese Journal of Trauma (hereinafter referred to as the "Journal") from 1985 to 2024. Methods:Data of basic research articles published in the Journal from 1985 to 2024 were retrieved from the Chinese Medical Journal Full-text Database. Excel was used to analyze publication volume, number of cited articles, citation rate, citation frequency, funding support, disciplinary distribution, key words, authors and institutional affiliations within 40 yearsResults:There were 10 687 publications in the Journal within 40 years, among which 617 (5.77%) were basic research articles, comprising 0 article in 1985-2000, 72 articles in 2001-2005, 200 articles in 2006-2010, 231 articles in 2011-2015, 84 articles in 2016-2020, and 30 articles in 2021-2024. Among them, 443 articles were cited, with a citation rate of 71.80% and total citations of 1 465. The average citation count per article was 2.37, with a maximum of 22 citations for a single article. Funded articles accounted for 76.67% (473/617) and showed a marginally higher citation rate (71.88%) compared to non-funded articles (71.53%). The top 4 disciplines by publication volume included orthopedics (203 articles, 32.90%), neurosurgery (119 articles, 19.29%), trauma orthopedics (75 articles, 12.16%), and emergency surgery (24 articles, 3.89%). High-frequency key words included "brain injury","spinal cord injury", "neurons", "stem cell", and "tissue engineering", etc. The first authors mainly came from Chongqing Municipality, Shanghai Municipality, Shandong Province, Jiangsu Province, Zhejiang Province and Tianjin Municipality; 7 authors published more than 2 articles, 3 of whom were from medical institutions in Chongqing Municipality.Conclusion:Over 40-year period, the basic research articles published in the Journal have demonstrated phased fluctuations, interdisciplinary convergence and shift in hotspots (e.g., from neural injury to regenerative medicine), as well as geographical and institutional disparities in contribution.
5.Clinical analysis of donor-purified CD34 + stem cell boost in 11 patients with poor hematopoietic reconstruction after haploid hematopoietic stem cell transplantation for aplastic anemia

Objective:To evaluate the safety and efficacy of donor-purified CD34 + stem cell boosts in patients with poor hematopoietic reconstruction (PHR) after haploid hematopoietic stem cell transplantation (haplo-HSCT) for aplastic anemia (AA) . Method:A retrospective analysis was conducted on 11 patients with AA and PHR who underwent haplo-HSCT and received donor-purified CD34 + stem cell boosts at Peking University People’s Hospital. Recovery of blood cell counts, incidence of graft-versus-host disease (GVHD), and overall survival (OS) were assessed. Results:Of the 11 patients with PHR, two were diagnosed with prolonged isolated thrombocytopenia (PT), one was primary poor graft function (PGF), and eight were diagnosed with secondary PGF. The median time to PHR diagnosis was 110 days (range: 60-330 days), and the median interval from transplantation to purified CD34 + hematopoietic stem cell infusion was 194 days (range: 125-456 days). The two patients with PT achieved complete platelet recovery at 22 and 13 days after CD34 + stem cell infusion, respectively. Among the remaining nine patients with PGF, six achieved complete hematopoietic recovery, with a median absolute neutrophil count recovery time of 19 days (8-158 days), HGB recovery time of 32.5 days (range: 13-158 days), and platelet recovery time of 31.5 days (range: 7-171 days). The incidence of chronic GVHD after infusion was 18.2%, with no cases of acute GVHD observed. The OS rate was 90.9% (10/11) in the 11 patients, with a median follow-up of 614 days (range: 153-1 765 days) . Conclusion:Donor-purified CD34 + stem cell boost may be an effective therapeutic strategy for PHR in patients with AA after haplo-HSCT.
6.Diagnostic value of targeted next-generation sequencing for community-acquired respiratory virus infections in patients with hematological diseases

Objective:To evaluate the diagnostic value of targeted next-generation sequencing (tNGS) of throat swab samples for detecting community-acquired respiratory viruses (CARV) in patients with hematological diseases.Methods:Clinical and laboratory data from 64 episodes involving patients with hematological diseases and suspected infections—who underwent both pharyngeal swab tNGS and CARV polymerase chain reaction (PCR) testing concurrently—were retrospectively analyzed. The cases were drawn from the Department of Hematology, Peking University People’s Hospital, between September 2023 and April 2024. Concordance between tNGS and CARV PCR results, as well as the diagnostic performance of tNGS in detecting CARV, were evaluated.Results:Among the 64 episodes, 29 were clinically diagnosed with respiratory tract infections, including one case of cytomegalovirus pneumonia and 28 CARV-positive cases. The remaining 35 episodes involved patients with fever or respiratory symptoms attributed to other causes, including 14 with extrapulmonary infections and 21 with noninfectious etiologies. The median follow-up duration was 215.5 days (range: 7-271 days). PCR detected 24 strains of seven CARV types, whereas tNGS detected 25 strains of eight CARV types. Using PCR results as the reference standard, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of tNGS were 85.0%, 88.6%, 77.3%, 92.9%, and 87.5%, respectively. The two methods showed good concordance (Kappa=0.717, P<0.001) . Conclusion:Pharyngeal swab tNGS may serve as a viable alternative to PCR for diagnosing CARV infections in patients with hematological diseases.

Statistical analysis was performed on the publication count, total citation frequency, number of cited articles, number of highly cited articles, proportion of fund-supported articles and their citation rate, distribution of article categories, author distribution, and keyword frequency.Results:Over the 40-year period, the Journal published a total of 10 687 articles, with the total citation frequency of 45 242. Among them, 7 497 articles (70.15%) were cited and 389 (3.64%) were highly cited. A total of 2450 articles (22.93%) were supported by research funds, with a citation rate of 80.29%. The clinical researoh column showed the largest number of publications, accounting for 7 996 articles (74.82%). First authors were distributed across 31 provincial-level regions, contributing 5 878 articles and 29 583 total citations. Thirty-five corresponding authors published more than 30 articles each, 17 of whom were from medical institutions in Chongqing Municipality. A total of 20 686 keywords were identified, with the top 5 being "fracture fixation, internal" (421 times), "craniocerebral injury" (395 times), "diagnosis" (359 times), "brain injury" (354 times), "spinal cord injury" (314 times).Conclusion:Over the 40-year period, the Journal has been characterized by a high overall academic influence despite a relatively low proportion of highly impactful publications, a substantial rate of fund-supported articles, a broad geographical distribution of first authors, with the most significant contributions coming from Chongqing Municipality, and a strong academic foundation in specialized disciplines such as orthopedics and neurosurgery.
