1.Isolation,identification and biological characteristics of Aeromonas hydrophila from South China tiger

The aim of this study is to identify the cause of death of a South China tiger cub at the Meihuashan breeding institute of Fujian Province.Pathogenic bacteria were isolated and cultured from liver,spleen,lung and other tissue samples of the dead South China tiger aseptically.The iso-lated bacteria were identified through morphological observation,biochemical characterization,sequence analysis of housekeeping gene gyrB,virulence gene detection,animal pathogenicity test and drug sensitivity test.A pathogenic Aeromonas hydrophila strain,designated FJ/Tiger-201809 was successfully isolated from the trachea of dead South China tiger.The nucleotide sequence ho-mology between the isolate and 11 strains of Aeromonas gyrB ranged from 91.2%to 99.1%,with the highest homology of 99.1%observed with Aeromonas hydrophila(AF208251.1).Genetic evo-lution analysis showed that the isolated strain FJ/Tiger-201809 was in the same evolutionary branch as other reference strains of Aeromonas hydrophila and was closely related.The pathoge-nicity test in mice showed artificial infection of mice with the strain resulted in varying degrees of lesions in several organs of the mice,and the median lethal dose(LD50)was 1 × 107.8 CFU/mL.Virulence gene test results showed that the isolate FJ/Tiger-201809 carried two virulence genes,aer and act.The results of drug sensitivity test showed that FJ/Tiger-201809 was highly sensitive to enrofloxacin and ampicillin among 18 commonly used antibiotics,relatively sensitive to penicil-lin G and doxycycline,and resistant to the other 14 antibiotics.In conclusion,this study isolated and identified a strain of Aeromonas hydrophila from a dead South China tiger with multiple drug resistance and strong pathogenicity,which provided an important reference for the prevention and control of bacterial diseases in South China tiger.
2.Correlation of fetal rectal ampulla abdominal diameter with gestational age and establishment of reference values in low-risk fetuses at 18~40 weeks of pregnancy

Objective To examine the correlation between fetal rectal ampulla diameter and gestational age,and to establish reference value ranges for low-risk fetuses between 18 and 40 weeks of gestation in Yibin region.Methods A total of 1,103 low-risk singleton pregnant women between 18 and 40 weeks of gestation were recruited from five hospitals in Yibin City(the Second People's Hospital,the First People's Hospital,the Fifth People's Hospital,the Maternal and Child Health Hospital of Yibin City,and the Maternal and Child Health Hospital of Cuiping District)for routine level Ⅰ,Ⅱ,and Ⅲ prenatal ultrasound screening from October 2022 to March 2024.Fetal rectal ampulla diameters,including anteroposterior diameter,transverse diameter,and area,were measured using prenatal ultrasound.The normality of these measurements was assessed using the Shapiro-Wilk test.Scatter plots depicting the relationship between fetal rectal ampulla diameter parameters and gestational age were generated using the"Overlap Scatter Plot"function in SPSS.Percentiles were calculated using the"Explore"function in SPSS,with reference value ranges described by P5,P10,P50,P90,and P95.Results The visual-ization rate of the fetal rectal ampulla diameter was 55%at 18~20 weeks of gestation,100%at 21~37 weeks,and 96%at greater than 37 weeks.The fetal rectal ampulla diameter exhibited a significant positive correlation with gestational age(r=0.925~0.949,P<0.01).Conclusions Prenatal ultrasound measurement of fetal rectal ampulla diameter demonstrates a robust correlation with gestational age.The reference intervals for the rectal ampulla diameter of low-risk fetuses between 18 and 40 weeks of gestation,established in this study,may offer valuable theoretical guidance for prenatal diagnosis of fetal rectal and anal abnormalities in Yibin region.
3.Practice and analysis of implementing drug traceability code management in outpatient pharmacy

OBJECTIVE To explore optimization pathways for the drug traceability code management model in outpatient pharmacy workflows, providing practical evidence for enhancing the efficiency of pharmaceutical service. METHODS Taking the outpatient pharmacy of the First Affiliated Hospital of Sun Yat-sen University as the research subject, a comprehensive drug traceability system was established through three key interventions: upgrading the information system architecture [including integration of the hospital information system (HIS) with the traceability platform], workflow optimization (reorganizing the inventory-dispensing-verification tripartite process), and designing a dual-mode traceability data collection mechanism (primary data capture at dispensing stations and supplementary capture at verification stations). Operational efficiency differences before and after implementation were analyzed using the medical insurance data and service timeliness metrics in September 2024. RESULTS After the implementation of drug traceability code management, in terms of data collection: Mode Ⅰ (verification-stage capture) uploaded 26 144 records, while Mode Ⅲ (inventory-as-sales capture) uploaded 443 061 records, totaling 469 205 entries; in terms of time efficiency: average drug dispensing time increased from 28.74 s to 43.37 s (enhanced by 51%). Through dynamic staffing adjustments, patient wait time only extended from 8.04 min to 8.67 min (enhanced by 8%). CONCLUSIONS Drug traceability code management can be effectively implemented via a “system reconstruction-process reengineering-human-machine collaboration” trinity strategy, leveraging informatization (e.g., dual-mode data capture) to offset manual operation delays, which validates the feasibility of balancing national traceability demands with service efficiency in outpatient pharmacies.

6.Relationship Between Hearing Loss and Cognitive Function in Elderly Chinese People:A Study Based on Propensity Score Matching

Objective To explore the relationship between hearing loss and cognitive function in the elderly population through propensity score matching method.Methods We analyzed the data of 7605 participants aged 60 and above who were included in the 2018 China Health and Retirement Longitudinal Study(CHARLS).The non-substitutable 1∶1 nearest neighbor matching method without caliper value was used for propensity score matching and G-computation was used to estimate the average treatment effect(ATE)of hearing loss on all dimensions of cognitive function.Results Before matching,there were 3626(47.68%)women,with 1409(18.53%)of whom suffering from hearing loss and 3031(39.86%)of whom suffering from cognitive impairment.After matching,1409 subjects were included in the hearing loss group and 1409,in the normal hearing group,with both groups sharing similar distribution of basic demographic characteristics.The results for the average treatment effect of the population indicated that the cognitive function scores of the hearing loss group were lower than those of the normal hearing group,with the overall cognitive function being 0.593 points lower(95%confidence intervel[CI]:-0.916--0.257,P<0.001),orientation being 0.183 points lower(95%CI:-0.302--0.055,P=0.004),immediate memory being 0.150 points lower(95%CI:-0.218--0.085,P<0.001),and language skills being 0.178 points lower(95%CI:-0.303--0.058,P=0.006).The prevalence of cognitive impairment of the hearing loss group was 4.2%higher than that of the normal hearing group(95%CI:0.007-0.077,P=0.020).Conclusion Hearing loss adversely affects the orientation,memory,and language skills of the elderly population and forms a potential risk factor for cognitive impairment in the elderly population.
7.Research progress of traditional external preparations and novel nanoformulations of Arnebiae Radix

Arnebiae Radix is used widely in TCM external treatment. It has obvious curative effect on skin diseases, wound infection and local inflammation, and is used to treat water and fire burns, skin ulcers, eczema, psoriasis, vitiligo and atopic dermatitis, etc. The clinical and commercial preparations mainly include ointment, liniment and suppository. Modern research has proved that microcapsules, nano-micelles, nanofiber membranes, nanogels and other novel nanoformulations can significantly improve the stability of drug-effective substances, improve local drug concentration and targeting, and perform sound drug release properties in vitro. This paper reviews the variety and application of Arnebiae Radix traditional preparations for external use and the research progress of novel nanoformulations of Arnebiae Radix, from which we prospect to provide some valuable references for the future application and development of Arnebiae Radix external preparations.
8.Phosphate and tension homology-induced kinase 1/Parkin signaling mediates cognitive dysfunction in sepsis-associated encephalopathy through activation of hippocampal mitochondrial autophagy.

OBJECTIVE:
To investigate the effects of gene of phosphate and tension homology (PTEN)-induced putative kinase 1 (PINK1)/Parkin pathway on hippocampal mitophagy and cognitive function in mice with sepsis-associated encephalopathy (SAE) and its possible mechanism.
METHODS:
A total of 80 male C57BL/6J mice were randomly divided into Sham group, cecal ligation puncture (CLP) group, PINK1 plasmid transfection pretreatment groups (p-PINK1+Sham group, p-PINK1+CLP group), empty vector plasmid transfection control group (p-vector+CLP group), with 16 mice in each group. The mice in CLP groups were treated with CLP to reproduce SAE models. The mice in the Sham groups were performed laparotomy only. Animals in the p-PINK1+Sham and p-PINK1+CLP groups were transfected with PINK1 plasmid through the lateral ventricle at 24 hours before surgery, while mice in the p-vector+CLP group were transfected with the empty plasmid. Morris water maze experiment was performed 7 days after CLP. The hippocampal tissues were collected, the pathological changes were observed under a light microscope after hematoxylin-eosin (HE) staining, and the mitochondrial autophagy was observed under a transmission electron microscopy after uranyl acetate and lead citrate staining. The expressions of PINK1, Parkin, Beclin1, interleukins (IL-6, IL-1β) and microtubule-associated protein 1 light chain 3 (LC3) were detected by Western blotting.
RESULTS:
Compared with the Sham group, CLP group mice in Morris water maze experiment had longer escape latency, shorter target quadrant residence time, and fewer times of crossing the platform at 1-4 days. Under the light microscope, the hippocampal structure of the mouse was injured, the neuronal cells were arranged in disorder, and the nuclei were pyknotic. Under the electron microscope, the mitochondria appeared swollen, round, and wrapped by bilayer or multilayer membrane structures. Compared with the Sham group, CLP group had higher expressions of PINK1, Parkin, Beclin1, LC3II/LC3I ratio, IL-6 and IL-1β in hippocampus, indicating that sepsis induced by CLP could activated inflammatory response and caused PINK1/Parkin-mediated mitophagy. Compared with the CLP group, p-PINK1+CLP group had shorter escape latencies, spent more time in the target quadrant and had more number of crossings in the target quadrant at 1-4 days. Under the light microscope, the hippocampal structures of mice was destroyed, the neurons were arranged disorderly, and the nuclei were pyknotic. Under transmission electron microscope, swollen and rounded mitochondria and mitochondrial structure wrapped by double membrane or multilayer membrane structure were observed. Compared with the CLP group, the levels of PINK1, Parkin, Beclin1 and LC3II/LC3 ratio in the p-PINK1+CLP group were significantly increased [PINK1 protein (PINK1/β-actin): 1.95±0.17 vs. 1.74±0.15, Parkin protein (Parkin/β-actin): 2.06±0.11 vs. 1.78±0.12, Beclin1 protein (Beclin1/β-actin): 2.11±0.12 vs. 1.67±0.10, LC3II/LC3I ratio: 3.63±0.12 vs. 2.27±0.10, all P < 0.05], while the levels of IL-6 and IL-1β were significantly decreased [IL-6 protein (IL-6/β-actin): 1.69±0.09 vs. 2.00±0.11, IL-1β protein (IL-1β/β-actin): 1.11±0.12 vs. 1.65±0.12, both P < 0.05], suggesting that overexpression of PINK1 protein could further activate mitophagy and reduce the inflammatory response caused by sepsis. There was no statistically significant difference in the above pathological changes and related indicators between Sham group and p-PINK1+Sham group, CLP group and p-vector+CLP group.
CONCLUSIONS
PINK1 overexpression can further activate CLP-induced mitophagy by upregulating Parkin, thereby inhibiting inflammation response and alleviate cognitive function impairment in SAE mice.

9.Effect of Mirabegron combined with Silodosin on detrusor hyperactivity with impaired contractile

Objective To observe the clinical effect of Mirabegron combined with Silodosin on detrusor hyperactivity with impaired contractile (DHIC).Methods From September, 2019 to December, 2021, 40 patients with DHIC in the Department of Urology of Beijing Bo'ai Hospital were selected and randomly divided into control group and experimental group, with 20 cases in each group. The control group took Silodosin only, and the experimental group took Mirabegron in addition, for four weeks. The urinary diary, residual urine volume, Overactive Bladder Symptom Score (OABSS) and quality of life (QOL) score were compared before and after treatment.Results A total of 18 patients in the control group and 19 in the experimental group finished the trial. After treatment, the number of night urination per day, the residual urine volume and QOL score improved in the control group (P<0.01); the number of urination per 24 hours, the number of night urination per day, the volume per urination, the residual urine volume, the daily urgency score, the OABSS score and QOL score improved in the experimental group (P<0.01). The number decrease of urination per 24 hours, the volume decrease of per urination, the daily urgency score decrease, the OABSS score decrease and QOL score decrease were more in the experimental group than in the control group (P<0.01). The adverse reactions included palpitations, increased heart rate, dyspareunia, increased blood pressure, gastric discomfort, postural hypotension, and retrograde ejaculation, and there was no significant difference between two groups (P > 0.05).Conclusion The efficacy of Mirabellone combined with Silodosin on DHIC is better than Silodosin only, and there was no significant increase in drug-related adverse effects.
10.Imaging evaluation of carotid vulnerable plaques

Carotid atherosclerotic plaque rupture and thrombosis are the independent risk factors of acute ischemic cerebrovascular disease. Early identification of vulnerable plaques provides the evidence for early intervention in patients with asymptomatic carotid artery stenosis, and also helps to reduce the recurrence risk in patients with symptomatic carotid artery stenosis. This article focuses on comparing the advantages and limitations of the available clinical imaging methods in identifying vulnerable carotid artery plaque, and provides relevant prognostic indicators for patient risk stratification in clinical work.
