Objective To compare the efficacy and adverse reactions of doxycycline and levofloxacin in children with macrolide non-responsive mycoplasma pneumonia(MUMPP).Methods Clinical data of 240 MUMPP children admitted to our hospital from November 2023 to July 2024 were retrospectively analyzed, and they were divided into doxycycline group and levofloxacin group according to drug use, with 120 cases in each group. The efficacy of the two groups were compared, the time when body temperature dropped to nor-mal after taking antipyretics, the time when cough was reduced, the time when lung riles were resolved, the length of hospital stay, the peripheral blood indicators[WBC,neutrophilic granulocyte percentage,C reactive protein(CRP),lactate dehydrogenase(LDH),D-dimer],the lung imaging recovery and the occurrence of ad-verse reactions. Results There was no significant difference in total effective rate between doxycycline group and levofloxacin group(93.33%vs.90.00%,P<0.05).Compared with levofloxacin group, the duration of hospital stay in doxycycline group was shorter[(6.22±2.24)d vs.(7.39±2.47)d],and the duration of lung crackles disappeared longer[(4.47±2.06)d vs.(3.50±1.79)d],with statistical significance(P<0.05).Af-ter treatment, the neutrophil percentage,CRP and LDH levels in the two groups were lower than before treat-ment,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between doxycycline group and levofloxacin group(15.83%vs.15.00%,P>0.05).Conclusion Both doxycycline and levofloxacin have good efficacy in MUMPP children, with low inci-dence of adverse reactions and no serious adverse reactions.

Objective To establish educational milestones for nursing students at different educational stages and compare their distinctions and interrelationships. Methods Methodologies including literature review, role analysis, brainstorming, and the Delphi expert consultation method were used to determine the competency level classification framework and the target competency levels for different categories of students, thereby to formulate the educational milestones. Results A competency classification framework comprising 7 levels was developed. This culminated in an educational milestone system for nursing students, which includes 7 first-level competency indicators,17 second-level competency indicators,61 third-level indicators, and their correspondig target proficiency levels. Conclusion The milestone system can delineate the training objectives for various core competency dimensions among different student categories and illustrate their distinctions and connections. This system is conducive to enhancing the specificity, differentiation, and systematicity of nursing education.

Objective The visualization analysis system was used to explore the development context, research hotspots, and trends of diabetes-related cognitive dysfunction, providing a reference for subsequent studies and new ideas. Methods The literature on cognitive impairment in diabetes mellitus was retrieved from the Web of Science Core collection database from October 2022,and visualized by Citespace 6.1.R3.Results A total of 6448 English literature were included, and the annual number of publications showed an overall increasing trend.America posted the most, and the top 10 are dominated by authors from these America and China.Research focuses on the pathogenesis and risk factors of cognitive impairment in diabetes mellitus. Conclusion Understanding the risk factors of cognitive impairment in diabetic patients, early intervention is helpful to reduce the incidence of diabetic cognitive impairment,so as to prevent, delay and slow down the occurrence of cognitive impairment more effectively.

Ulcerative colitis(UC)is a common disease of the digestive system.Phosphatidylinositol-3-kinase(PI3K)/synuclein/threonine kinase(AKT)is closely related to cell survival, apoptosis, inflammation and other biological processes, and the expression levels of PI3K and AKT significantly increase during the course of UC,with accelerated apoptosis, improved inflammation, and damaged intestinal mucosal barrier function. In recent years,a large number of basic and clinical trials have been conducted on PI3K/AKT signaling pathway in TCM,and the results indicate that PI3K/AKT signaling pathway is expected to be an important potential target for UC treatment. This article analyzed the mechanism of the regulation of PI3K/AKT signaling pathway in TCM from monomer, extract, compound and acupuncture, and suggested that the regulation of this signaling pathway is of great significance for the prevention and treatment of UC,and provide reference for drug development.
5.Progress on peripheral blood biomarkers in children with bronchial asthma
International Journal of Pediatrics 2024;51(6):369-372
Bronchial asthma is one of the common chronic respiratory diseases in children, mainly manifested as airway hyperresponsiveness and chronic airway inflammation. The disease varies in phenotype, clinical presentation, course and response to treatment. Therefore, more precise and accessible biomarkers are needed for diagnosis and evaluation.Clinical assay of peripheral blood biomarkers only requires blood samples to be collected, can be repeatedly extracted and easily quantified, and are expected to improve the efficiency of asthma diagnosis and treatment, becoming a choice for clinical diagnosis and treatment. This article reviews the research progress of peripheral blood biomarkers in identifying the phenotype of childhood asthma, evaluating the severity, and predicting the therapeutic effect in recent years, with the aim to provide personalized medical strategies for children with asthma.

Airway mucus hypersecretion is one of the important pathophysiological and clinical manifestations of chronic obstructive pulmonary disease. It has been reported in the literature that COPD patients with chronic airway mucus hypersecretion have more frequent acute exacerbations, more severe lung function decline, and higher hospitalizations and mortality. Therefore, it is particularly critical to understand the pathogenesis of hypersecretion of mucus in chronic obstructive pulmonary disease and find out effective treatment. This article focuses on the structure, significance of airway mucus and the mechanism of hypersecretion of mucus in chronic obstructive pulmonary disease (COPD). In addition, we also summarized drug and non-drug therapy for chronic airway mucus hypersecretion in this article. Drug therapy includes traditional drug therapy, some new targeted drug therapy for pathogenesis and traditional Chinese medicine therapy, and non-drug therapy includes smoking cessation, physical therapy and bronchos-copy therapy. We hope that it will provide new ideas and directions for the treatment of mucus hypersecretion in COPD patients.
