1.Clinical manifestations and ultrasound features of primary high-grade serous carcinoma of the fallopian tube

Objective To explore the clinical manifestations and ultrasound features of primary high-grade serous carcinoma of the fallopian tube (PHGSC-FT) under the new pathological criteria. Methods A retrospective study was conducted on 45 PHGSC-FT patients diagnosed by postoperative pathology at Obstetrics & Gynecology Hospital of Fudan University from April to June 2023. Clinical data, ultrasound characteristics, and prognosis were analyzed. Results The age of patients was (56.2±10.1) years, with postmenopausal women accounting for 68.9%. The main clinical symptoms were abdominal pain and (or) distension (53.3%), pelvic mass (31.1%), and abnormal vaginal discharge (11.1%). Preoperative ultrasound revealed that 40 cases presented mainly as solid pelvic or abdominal masses (82.5%, 33/40), while 5 cases involved widely disseminated small lesions not describable by ultrasound; 28 lesions had irregular shapes 70.0%. Intraoperative findings showed that the major lesions were located in the ovary in 27 cases (60.0%), fallopian tube in 7 cases (15.6%), extra-adnexal pelvic in 6 cases (13.3%), and widespread disseminated lesions in 5 cases (11.1%). The accuracy rate of ultrasound localization was 82.2%, and the accuracy rate for benign and malignant differential diagnosis was 75.6%. Serum carbohydrate antigen 125 (CA125) levels were significantly lower in patients whose primary lesion was located in the fallopian tube than in those with lesions outside the fallopian tube (P=0.001). FIGO stage Ⅰ: 4 cases (8.9%), stage Ⅱ: 10 cases (22.2%), stage Ⅲ: 23 cases (51.1%), and stage Ⅳ: 8 cases (17.8%), with 3-year survival rates of 75.0%, 80.0%, 56.5%, and 50.0%, respectively. Conclusions PHGSC-FT predominantly occurs in postmenopausal women, and its biological behavior is characterized by “small primary lesion–large metastatic lesion”. Prognosis of these patients is related to FIGO stage; Ultrasound demonstrates high accuracy in localization of large lesions and in determining benign versus malignant nature.

Congenital orofacial cleft, the most common birth defect in the maxillofacial region, exhibits a wide range of prognosis depending on the severity of deformity and underlying etiology. Non-syndromic congenital orofacial clefts typically present with milder deformities and more favorable treatment outcomes, whereas syndromic congenital orofacial clefts often manifest with concomitant organ abnormalities, which pose greater challenges for treatment and result in poorer prognosis. This consensus provides an elaborate classification system for varying degrees of orofacial clefts along with corresponding diagnostic and therapeutic guidelines. Results serve as a crucial resource for families to navigate prenatal screening results or make informed decisions regarding treatment options while also contributing significantly to preventing serious birth defects within the development of population.

3.Misdiagnosis analysis of conjoined twins:a case report

This paper collected a case of conjoined twins admitted to the Obstetrics and Gynecology Hospital of Fudan University in October 2024.The patient was a 28-year-old female.Multiple structural abnormalities of the fetus were detected during the early pregnancy malformation screening.After induced labor,the diagnosis was conjoined twins.This paper elaborated on the characteristics of this case of conjoined twins from two aspects:prenatal ultrasound examination and characteristics of the postpartum specimens,and analyzed the reasons for misdiagnosis.The aim was to improve the comprehensive understanding of conjoined twins and provide reference information for accurate prenatal diagnosis.
4.Prenatal ultrasound diagnosis of fetal uterine vaginal effusion:a case report

Fetal hydrometrocolpos is caused by the accumulation of secretions due to lower genital tract obstruction. It often occurs in the late pregnancy,with an incidence of 1/16 000. In this case,the pregnant woman was 30 years old,primigravida,at 38 +3 weeks of pregnancy. Prenatal ultrasound in the Obstetrics and Gynecology Hospital Affiliated to Fudan University indicated a cystic structure in the center of the fetal pelvic and abdominal cavity. The lower edge reached the bottom of the pelvic cavity,and a uterine-like echo was seen at the upper edge-suggesting possible hydrometrocolpos. After birth,the newborn underwent longitudinal incision and drainage of the hymen. This article focuses on the prenatal ultrasound manifestations and differential diagnosis of fetal hydrometrocolpos to improve the understanding of this disease.

7.A novel PAX9 variant in a Chinese family with non-syndromic oligodontia and genotype-phenotype analysis of PAX9 variants

Objective This study aimed to identify PAX9 variants in non-syndromic tooth agenesis families of Chi-na,as well as to analyze the genotype-phenotype of non-syndromic tooth agenesis caused by PAX9 variants,which can provide a basis for the genetic diagnosis of tooth agenesis.Methods We collected the data of 44 patients with non-syn-dromic oligodontia who underwent treatment at Stomatological Hospital of Hebei Medical University between 2018 and 2023.Whole-exome sequencing was performed on the peripheral blood of the proband and its core family members,and the variants were verified by Sanger sequencing.Pathogenicity analysis and function prediction of the variants were per-formed using bioinformatics tools.The correlation between the genotype of PAX9 variant and its corresponding pheno-type was examined by reviewing 55 publications retrieved from PubMed.The studies involved 232 tooth agenesis pa-tients with PAX9 variants.Results A novel PAX9 c.447delG(p.Pro150Argfs*62)and a reported PAX9 c.406C>T(p.Gln136*)were identified in two Chinese families.Through bioinformatics analysis and three-dimensional structural mod-eling,we postulated that the frameshift variant was pathogenic.The outcome was the premature cessation of PAX9 pro-tein,which caused severe structural and functional deficiencies.Summarizing the PAX9 genotype-phenotype relationship revealed that patients carrying the PAX9 variant commonly led to loss of the second molars.Conclusion We identified the novel PAX9 c.447delG(p.Pro150Argfs*62)in a Chinese family of non-syndromic oligodontia,expanding the known variant spectrum of PAX9.The most susceptible tooth position for PAX9 variants of tooth agenesis was the second mo-lars and the deciduous molars during the deciduous dentition.
8.Value of transvaginal ultrasonography combined neutrophil-to-lymphocyte ratio in diagnosis of endometrial cancer in postmenopausal women

Objective:To explore the value of transvaginal ultrasound (TVS) combined with neutrophil to lymphocyte ratio (NLR) in postmenopausal women with endometrial cancer(EC).Methods:A retrospective analysis was conducted on 348 postmenopausal patients who underwent hysteroscopy or surgery in the Obstetrics and Gynecology Hospital of Fudan University from January 2019 to December 2021 and had pathological results of EC, and they were included in the malignant group. One hundred and six benign patients with pathological results in the same period were randomly selected for inclusion in the benign group. The differences in age, menopausal time, gravidity, body mass index (BMI), post-menopausal bleeding, TVS and NLR were compared between the two groups. Binary Logistic regression analysis was performed to determine the independent predictive factors for endometrial cancer in postmenopausal women. ROC curves were plotted to evaluate the diagnostic efficacy of predicting EC in postmenopausal women.Results:Parity (≥2 times), post-menopausal bleeding, intrauterine fluid, the diameter of the mass, blood flow and NLR showed statistically significant differences between the malignant group and the benign group (all P<0.05). Logistic regression analysis showed that post-menopausal bleeding, intrauterine fluid and long diameter of the mass were independent predictors for EC in postmenopausal women( OR=0.433, P=0.006; OR=14.933, P<0.001; OR=1.062, P=0.001). The area under the curve (AUC) of EC in postmenopausal women was 0.901(95% CI=0.867-0.934) by the combination of the above independent factors and the parameters of P<0.10 (blood flow and NLR), the sensitivity was 78.4% and the specificity was 90.6%. Conclusions:The combination of post-menopausal bleeding, intrauterine fluid, the diameter of the mass, blood flow and NLR have certain clinical application value in predicting EC in postmenopausal women.

10.Interpretation of Teacher Training Syllabus for Clinical Pharmacist Training Program (2023 edition)

OBJECTIVE To interpret Teacher Training Syllabus for Clinical Pharmacist Training Program (2023 edition) (hereinafter referred as to the “new syllabus”), and to provide reference and guidance for promoting the implementation of the new syllabus and realizing the quality-improving goal of the reform of the clinical pharmacist teacher training program initiated by China Hospital Association. METHODS From the perspective of the management and based on the position of the designer, the new syllabus was interpreted from four aspects: the background of its compilation and release, the process of its compilation and its characteristics, the key improvements of the program and the points for attention about its subsequent implementation. RESULTS & CONCLUSIONS The development and release of the new syllabus provide a “construction blueprint” for the reform of the clinical pharmacist teacher training program of the China Hospital Association. The whole process of compiling the new syllabus is characterized by four basic features: theory-led, goal-oriented, research-based, and synergistic. Compared with the previous syllabus, in addition to the adjustment of the text structure,the new syllabus presents more complete and clearer competence requirements for clinical teaching competence in terms of training objectives; in terms of training content, it further structures the group of task items, pays attention to the 育。E-mail:zhenjiancun@163.com sequential planning and time arrangement of items, and puts forward both quantitative and qualitative refinement requirements for each specific training task;in terms of training methods, it emphasizes the interaction of lecturing, demonstrating and guiding, and the progression of observation, operation and reflection, with the intention of guiding teacher trainees to “learn how to teach by teaching”. In the subsequent implementation of the new syllabus, it is necessary for the teacher training bases to attach great importance to the guarantee of training conditions and process quality management, and to organize the teacher training team to do a good job in the two training programs of “clinical pharmacist training” and “clinical pharmacist teacher training”. Based on further improving the connection between the two training programs, the teacher training team should continue to explore the scientific model of clinical pharmacist teacher training oriented by clinical teaching competence.
