1.Evaluation of effect of hydrogen peroxide disinfectant on terminal disinfection in wards

OBJECTIVE To explore the effect of hydrogen peroxide disinfectant on terminal disinfection in wards of medical institutions.METHODS The surfaces of highly frequent contact objects of the wards of the First Affilia-ted Hospital of Nanchang University from which the public health center patients were discharged between Apr.2024 and Jun.2024 were respectively disinfected with 0.5%(low)and 5%(high)concentrations of hydrogen peroxide disinfecting wipes,totally 180 samples were randomly collected before and after the disinfection,and the pathogens were detected.The air of the wards from which the public health center patients were discharged be-tween Jul.2024 and Aug.2024 were disinfected with hydrogen peroxide dry mist disinfection device,and 90 sam-ples were respectively collected before and after the disinfection.Geobacillus stearothermophilus was used as the biological indicator and placed at various points within the air-disinfected wards to evaluate the disinfection level.The environmental sampling results and distribution of bacteria were observed and compared.RESULTS The qualified rates of disinfection of the object surfaces were 95.56%(86/90)and 98.89%(89/90)respectively for the low and high concentratioins of hydrogen peroxide disinfecting wipes,and there was no significant difference in the disinfection effect.Totally 120 strains of pathogens were isolated from unqualified samples before the disinfection,among which gram-negative bacteria(69.17%)were dominant,and the isolation rate of multidrug-resistant or-ganisms was 22.50%(27/120);only 1 strain of carbapenem-resistant Acinetobacter baumannii was isolated after the disinfection.The qualified rate of disinfection of air in the wards was 96.00%by 7.5%hydrogen peroxide dry mist disinfection device,the average bacterial colony counts in the air were 2 CFU/(5 min·vsl)after the dis-infection,and the killing rate of Geobacillus stearothermophilus was 100.00%by the air disinfection.CONCLUSION The hydrogen peroxide disinfectant can meet the requirement for terminal disinfection of the wards of the medical institutions,and it is portable and highly efficient.
2.Comparative study on the efficacy of Zero-PVA versus titanium plate in single-level ACDF treatment and analysis of radiographic influencing factors

Objective:To investigate the clinical and radiological differences in treating single-level cervical spondylosis using the Zero-profile interbody fusion cage (Zero-PVA) versus the titanium plate interbody fusion cage in anterior cervical discectomy and fusion (ACDF) and to analyze the correlation between clinical outcomes and radiological parameters.Methods:this retrospective study collected data from patients with cervical spondylosis who underwent single-level ACDF surgery at the First Affiliated Hospital of Kunming Medical University between January 2019 and June 2022. A total of 70 patients (32 males and 38 females) with an average age of 51.34 ± 8.62 years (range: 29 to 77 years) were included. Based on the type of interbody fusion device used, patients were divided into two groups: the cage-plate construct (CP) group (35 patients; average age: 51.51 ± 10.02 years, range: 29 to 77 years) and the zero-profile interbody fusion device with variable angle (ZP) group (35 patients; average age: 51.17±7.11 years, range: 38 to 68 years). Preoperative, one-week postoperative, and final follow-up (≥6 months postoperative) data were collected for visual analogue scale (VAS) scores, neck disability index (NDI) scores, and Japanese Orthopaedic Association (JOA) scores. Radiological parameters assessed included surgical segment intervertebral disc height, cervical lordosis angle, surgical segment Cobb angle, T 1 slope (T 1S), and C 2-C 7 sagittal vertical axis (C 2-C 7 SVA). Additional evaluations included Bazaz dysphagia ratings at one week and final follow-up, Odom surgical outcome ratings at final follow-up, fusion grades, adjacent vertebral ectopic ossification grades, and adjacent segment degeneration. Patients were further classified into a symptom relief group and an incomplete symptom relief group based on Odom ratings. Logistic regression analysis was performed to examine the correlation between radiological parameters and postoperative outcomes. Statistical methods such as ROC curves, Youden index, product index, and Euclidean index were used to determine optimal cut-off values for radiological parameters affecting postoperative outcomes. Results:There were no statistically significant differences between the two groups in terms of gender, age, follow-up duration, surgical segment, or classification ( P>0.05), confirming comparability. At both one-week post-operation and final follow-up, VAS, NDI, and JOA scores showed no significant differences between the groups ( P>0.05). However, the incidence of dysphagia in the CP group was 49% at one week post-operation and 20% at final follow-up, significantly higher than that in the ZP group (14% at one week and 0 at final follow-up; P<0.05). The intervertebral space height in the CP group was 9.16±0.84 mm at one week post-operation and 8.19±1.11 mm at final follow-up, which was significantly higher than in the ZP group (8.22±1.23 mm at one week and 7.57±1.14 mm at final follow-up; P<0.05). The surgical segment Cobb angle in the CP group at final follow-up (7.34°±4.21°) was significantly greater than in the ZP group (4.63°±4.32°; t=2.661, P=0.010). Regarding heterotopic ossification (HO) of the superior vertebral body, in the CP group, 27 cases were grade 0, 7 cases were grade 1, and 1 case was grade 2. In the ZP group, 34 cases were grade 0 and 1 case was grade 1, with a statistically significant difference between the groups (χ 2=6.128, P=0.028). Binary logistic regression analysis revealed that a reduction in intervertebral space height of ≥0.63 mm at one week post-operation and final follow-up ( OR=2.913, P=0.021) and a heterotopic ossification grade of ≥1 ( OR=5.178, P=0.042) were correlated with incomplete symptom relief after single-level ACDF surgery. Conclusions:Both Zero-PVA and titanium plate interbody fusion devices demonstrated favorable clinical outcomes in single-level ACDF surgery. However, a reduction in intervertebral space height and heterotopic ossification of the superior vertebral body may be risk factors for incomplete symptom relief following single-level ACDF.
3.An analysis of metabolic changes and potential biomarkers in ischemic stroke based on untargeted metabolomics

Objective To investigate serum metabolites and metabolic pathways alterations in patients with ischemic stroke(IS)through metabolomic analysis,and to identify reliable serum metabolic biomarkers for IS diagnosis.Methods This prospective study enrolled patients with IS admitted to the Department of Neurology at Xiangcheng People's Hospital of Suzhou from December 1,2022 to December 31,2023.Age-and sex-matched healthy individuals were recruited as controls during the same period.Baseline characteristics were collected,including age,sex,height,body mass index,and blood pressure.Venous blood samples were obtained after an 8 h fast for biochemical analysis of blood glucose,total bilirubin,serum creatinine,urea nitrogen,total cholesterol,triglycerides,high-density lipoprotein cholesterol,and low-density lipoprotein cholesterol.Serum metabolites of both groups were extracted and analyzed using ultra-high-performance liquid chromatography coupled with tandem mass spectrometry.Metabolomic data were processed using Simca-p software for unsupervised principal component analysis(PCA)and orthogonal partial least squares discriminant analysis(OPLS-DA)to evaluate group separation and experimental stability.Differential metabolites were defined by variable importance in projection(VIP)≥1.0,fold change(FC)≥2.0 or ≤0.5,and P<0.05.Drug-derived exogenous metabolites were excluded by cross-referencing the Human Metabolome Database(HMDB,https://hmdb.ca/)and PubChem(https://pubchem.ncbi.nlm.nih.gov/).MetaboAnalyst 6.0(http://www.metaboanalyst.ca),a comprehensive web-based tool for metabolomic data analysis,was employed to map differential metabolites to the Kyoto encyclopedia of genes and genomes(KEGG)databased and to perform pathway enrichment analysis.Machine learning models were developed using Python.Least absolute shrinkage and selection operator(LASSO)regression and random forest(RF)algorithms were employed to identify diagnostic biomarkers capable of effectively distinguishing IS patients from controls.Metabolites identified by both methods were integrated into an extreme gradient boosting(XGBoost)model.Model performance was evaluated using receiver operating characteristic(ROC)curves with 5-fold cross-validation and internal validation(70%training,30%validation set).Results A total of 51 IS patients and 51 matched controls were included.(1)A total of 1 255 serum metabolites were identified(964 in positive ion mode,291 in negative ion mode).PC A and OPLS-DA demonstrated distinct metabolic separation between IS patients and controls.In IS group,260 metabolites were upregulated and 337 downregulated in positive ion mode;99 were upregulated and 34downregulated in negative ion mode.(2)Among the 1 255 metabolites,259 were identified as differential metabolites based on the criteria of VIP ≥ 1.0,FC≥2.0 or≤0.5 and P<0.05.After excluding drug-derived metabolite through referencing HMDB and PubChem databases,a total of 220 endogenous differential metabolites were found to coexist in both positive and negative ion modes.Among them,119 metabolites were up-regulated and 101 were down-regulated in the IS group.The expression of these 220 metabolites showed significant differences between the IS and control groups.(3)KEGG pathway analysis highlighted five dysregulated pathways:upregulation of denovo triacylglycerol biosynthesis,glycerophosphate shuttle,and cardiolipin biosynthesis;downregulation of bile acid biosynthesis and methylhistidine metabolism.(4)LASSO and RF algorithms identified 24 and 30 candidate biomarkers,respectively.Four overlapping metabolites were selected:2-((3R)-3-((3R,5S,7S,9S,10S,13R,14S,17R)-3,7-dihydroxy-10,13-dimethylhexadecahydro-1H-cyclopenta[a]phenanthren-17-yl)butanamido)ethane-1-sulfonic acid(m/z 971.571 29),arginine-conjugated cholic acid(m/z 587.379 21),laccaic acid A(m/z 576.010 93)and NCGC00380235-01_C32H48O9_beta-D-xylopyranoside,3,17-dihydroxyspirosta-5,25(27)-dien-1-yl(m/z 559.326 48).The expression levels of 2-((3R)-3-((3R,5S,7S,9S,10S,13R,14S,17R)-3,7-dihydroxy-10,13-dimethylhexadecahydro-1H-cyclopenta[a]phenanthren-17-yl)butanamido)ethane-1-sulfonic acid(m/z 971.571 29),arginine-conjugated cholic acid(m/z 587.379 21),and laccaic acid A(m/z 576.010 93)were upregulated,while the expression level of NCGC00380235-01_C32H48O9_beta-D-xylopyranoside,3,17-dihydroxyspirosta-5,25(27)-dien-1-yl(m/z 559.326 48)was downregulated.An IS diagnostic model was established based on four metabolic biomarkers using the XGBoost algorithm.The area under the ROC curve was 1.000(95%CI 1.000-1.000)in the training set and 0.988 in the validation set(95%CI 0.963-1.000).Conclusions Patients with IS exhibit significant metabolic disturbance.The four identified biomarkers may serve as potential biomarkers 5.Current status of diet self-efficacy in young and middle-aged hypertension patients and its influencing factors

Objective:To explore the current status of diet self-efficacy in young and middle-aged hypertension patients and analyze its influencing factors.Methods:From April to September 2022, 85 young and middle-aged patients with primary hypertension attending the Department of Cardiology of Beijing Chaoyang Hospital affiliated with Capital Medical University were selected for the study using convenience sampling method. The Cardiac Diet Self-Efficacy Scale (CDSE) and Food Frequency Questionnaire (FFQ) were used to investigate the patients, and the Dietary Approaches to Stop Hypertension (DASH) score was calculated from the food frequencies obtained from the FFQ.Results:A total of 85 questionnaires were distributed and 78 valid questionnaires were recovered, with a valid recovery rate of 91.76%. Among 78 young and middle-aged hypertension patients, the total CDSE and DASH scores were (41.01±9.78) and (19.01±4.83), respectively. Multiple linear regression analysis showed that the frequency of blood pressure measurement, body mass index, saltiness of taste, and DASH were the influencing factors of diet self-efficacy in young and middle-aged hypertension patients, and the differences were statistically significant ( P<0.05) . Conclusions:Diet self-efficacy in young and middle-aged hypertension patients is influenced by body mass index, frequency of blood pressure measurement, saltiness of taste, and DASH. Interventions should be based on the influencing factors so as to improve diet self-efficacy in young and middle-aged hypertension patients.

OBJECTIVE To explore the effect of hydrogen peroxide disinfectant on terminal disinfection in wards of medical institutions.METHODS The surfaces of highly frequent contact objects of the wards of the First Affilia-ted Hospital of Nanchang University from which the public health center patients were discharged between Apr.2024 and Jun.2024 were respectively disinfected with 0.5%(low)and 5%(high)concentrations of hydrogen peroxide disinfecting wipes,totally 180 samples were randomly collected before and after the disinfection,and the pathogens were detected.The air of the wards from which the public health center patients were discharged be-tween Jul.2024 and Aug.2024 were disinfected with hydrogen peroxide dry mist disinfection device,and 90 sam-ples were respectively collected before and after the disinfection.Geobacillus stearothermophilus was used as the biological indicator and placed at various points within the air-disinfected wards to evaluate the disinfection level.The environmental sampling results and distribution of bacteria were observed and compared.RESULTS The qualified rates of disinfection of the object surfaces were 95.56%(86/90)and 98.89%(89/90)respectively for the low and high concentratioins of hydrogen peroxide disinfecting wipes,and there was no significant difference in the disinfection effect.Totally 120 strains of pathogens were isolated from unqualified samples before the disinfection,among which gram-negative bacteria(69.17%)were dominant,and the isolation rate of multidrug-resistant or-ganisms was 22.50%(27/120);only 1 strain of carbapenem-resistant Acinetobacter baumannii was isolated after the disinfection.The qualified rate of disinfection of air in the wards was 96.00%by 7.5%hydrogen peroxide dry mist disinfection device,the average bacterial colony counts in the air were 2 CFU/(5 min·vsl)after the dis-infection,and the killing rate of Geobacillus stearothermophilus was 100.00%by the air disinfection.CONCLUSION The hydrogen peroxide disinfectant can meet the requirement for terminal disinfection of the wards of the medical institutions,and it is portable and highly efficient.
9.Etiological diagnostic value of metagenomic Next Generation Sequencing for bloodstream infection in patients with sepsis

Objective:To evaluate the value of metagenomic Next Generation Sequencing (mNGS) for the pathogenetic diagnosis of bloodstream infections in patients with sepsis.Methods:A retrospective analysis was performed on 105 sepsis cases who received blood mNGS and blood culture tests during their hospitalization in the Intensive Care Department of the Affiliated Hospital of North Sichuan Medical College from September 2021 to August 2023, and the results were compared and analyzed. According to mNGS and blood culture results, the cases were divided into the positive group and negative group.The distribution of pathogens in the enrolled patients were analyzed, and the diagnostic performance and consistency of blood culture and mNGS were compared. The differences in clinical characteristics of patients in each group were analyzed, and the pathogenic diagnostic value of mNGS for bloodstream infections in patients with sepsis was evaluated.Results:①Among 105 blood samples, 61 cases (58.10%) had positive results of mNGS, and 32 cases (30.48%) had positive results of blood culture. The positive rate of mNGS was higher than that of blood culture, and the difference was statistically significant ( P < 0.05).The accuracy of mNGS was lower than that of blood culture②The mean values of ESR, PCT and CRP in positive group were higher than those in negative group, and the proportion of septic shock was higher than that in negative group, and the differences were statistically significant ( P < 0.05). There was no significant difference in 28-day mortality rate between the two groups ( P > 0.05). Conclusions:mNGS is beneficial to the etiological diagnosis and treatment of bloodstream infection in patients with sepsis.
10.Effect of Qiwei Baizhusan on Cognitive Dysfunction in Rats with Diabetic Encephalopathy Based on PI3K/Akt/GSK-3β Signaling Pathway

ObjectiveTo observe the therapeutic effect of Qiwei Baizhusan(QWBZS) on diabetic encephalopathy(DE) rat model, and to explore the possible mechanism of QWBZS in the treatment of DE based on phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt)/glycogen synthase kinase-3β(GSK-3β) signaling pathway. MethodForty-eight SPF male Wistar rats were randomly divided into blank group(8 rats) and high-fat diet group(40 rats). After 12 weeks of feeding, rats in the high-fat diet group were intraperitoneally injected with 35 mg·kg-1 of 1% streptozotocin(STZ) for 2 consecutive days to construct a DE model, and rats in the blank group were injected with the same amount of sodium citrate buffer. After successful modeling, according to blood glucose and body weight, model rats were randomly divided into model group, low, medium and high dose groups of QWBZS(3.15, 6.3, 12.6 g·kg-1), combined western medicine group(metformin+rosiglitazone, 0.21 g·kg-1), with 6 rats in each group. The administration group was given the corresponding dose of drug by gavage, and the blank group and the model group were given an equal volume of 0.9% sodium chloride solution by gavage, 1 time/day for 6 weeks. Morris water maze was used to detect the spatial memory ability of DE rats. Fasting insulin (FINS) level was detected by enzyme-linked immunosorbent assay(ELISA) and insulin resistance index(HOMA-IR) was calculated. Hematoxylin-eosin(HE) staining was used to observe the morphological changes of hippocampus in rats, ELISA was used to detect the indexes of oxidative stress in hippocampal tissues, real-time fluorescence quantitative polymerase chain reaction(Real-time PCR) was used to detect mRNA expression levels of PI3K, Akt, nuclear transcription factor-κB(NF-κB), tumor necrosis factor-α(TNF-α) and interleukin-1β(IL-1β) in hippocampus, and Western blot was used to detect the protein expression of PI3K, Akt, phosphorylated(p)-Akt, GSK-3β and p-GSK-3β in hippocampus of rats. ResultCompared with the blank group, FINS and HOMA-IR values of the model group were significantly increased(P<0.01), the path of finding the original position of the platform was significantly increased, and the escape latency was significantly prolonged(P<0.01), the morphology of neuronal cells in hippocampal tissues was disrupted, the levels of reactive oxygen species(ROS) and malondialdehyde(MDA) in hippocampus of rats were increased, and the activity of superoxide dismutase(SOD) was decreased(P<0.05, P<0.01), mRNA expression levels of PI3K and Akt were decreased(P<0.01), mRNA expression levels of NF-κB, TNF-α and IL-1β were increased(P<0.05, P<0.01), the protein expression levels of PI3K, p-Akt and p-GSK-3β were significantly decreased, and the protein expression of GSK-3β was significantly increased(P<0.01). Compared with the model group, the FINS and HOMA-IR values of the medium dose group of QWBZS and the combined western medicine group were significantly decreased(P<0.01), the path of finding the original position of the platform and the escape latency were significantly shortened(P<0.01), the hippocampal tissue structure of rats was gradually recovered, and the morphological damage of nerve cells was significantly improved, the contents of ROS and MDA in hippocampus of rats decreased and the level of SOD increased(P<0.01), the mRNA expression levels of PI3K and Akt were increased(P<0.01), and the mRNA expression levels of NF-κB, TNF-α and IL-1β were decreased (P<0.05, P<0.01), the protein expression levels of PI3K, p-Akt and p-GSK-3β were significantly increased(P<0.01), and the expression of GSK-3β was significantly decreased(P<0.01). ConclusionQWBZS can alleviate insulin resistance in DE rats, it may repair hippocampal neuronal damage and improve learning and cognitive ability of DE rats by activating PI3K/Akt/GSK-3β signaling pathway.
