1.Clinical characteristics as well as infection prevention and control of Can-dida auris infection/colonization in a general hospital

Objective To explore the clinical characteristics as well as infection prevention and control strategies of patients with Candida auris(C.auris)infection/colonization.Methods Clinical characteristics as well as preven-tion and control measures of patients detected with C.auris in a general hospital from September 2023 to March 2024 were analyzed,prevention and control measures were evaluated.Results A total of 9 patients were detected C.auris,and the specimen sources were urine from 5 cases(55.56%),sputum from 3 cases(33.33%),and ab-dominal drainage fluid from 1 case(11.11%).There were 5 cases of community-associated infection(55.56%),3 cases of community colonization(33.33%),and 1 case of hospital colonization(11.11%).There were 5 cases of deaths(55.56%),3 cases of discharges(33.33%),and 1 case of transfer(11.11%).Through measures such as training,early warning,single-room isolation,personal protection,hand hygiene,environmental cleaning and disin-fection and strengthening supervision on sampling process,supervising sampling detection and screening,terminal disinfection,and ensuring a qualified detection result of specimens before admitting new patients,no C.auris was detected from patient's surrounding environment sampling,and no cross-transmission occurred in the hospital.Conclusion Paying attention to the implementation of prevention and control measures for C.auris infection can effectively control the occurrence and cross transmission of healthcare-associated infection.
2.Relationship of the Cyclin B2 and Cyclin E1 expression with clinicopathological characteristics and 5-year survival rate of patients with hepatocellular carcinoma

Objective To investigate the expressions of cyclin B2(CCNB2)and cyclin E1(Cyclin E1)in hepatocellular carcinoma(HCC)and their significance for the prognosis of HCC.Methods Sixty HCC patients were admitted and treated from October 2013 to October 2016.The expressions of CCNB2 and Cyclin E1 in 60 HCC tissues and adjacent normal tissues were detected by immunohistochemistry,and the relationship between the two and the clinicopathological characteristics of HCC patients was analyzed;Spearman method was performed to analyze the correlation of CCNB2 and Cyclin E1 expression;Kaplan-Meier was performed to analyze the relationship between CCNB2,Cyclin E1 expression and clinical prognosis;and univariate and multivariate Cox regression were performed to analyze the influencing factors of clinical prognosis of HCC patients.Results The positive expression rates of CCNB2 and Cyclin E1 in HCC(61.67%,56.67%)were significantly higher than those in adjacent normal tissues(20.00%,13.33%,P<0.05);the positive expressions of CCNB2 and Cyclin E1 were positively correlated with serum AFP level,liver cirrhosis status,tumor thrombus and recurrence(P<0.05).The expressions of CCNB2 and Cyclin E1 in HCC tissues were positively correlated(r=0.487,P=0.000).The positive expressions of CCNB2 and Cyclin E1 in HCC tissues predicted a lower survival time(x2=7.104,7.140,P=0.008,0.008).In addition,the positive expressions of CCNB2 and Cyclin E1 were independent factors affecting the prognosis of HCC patients(P<0.05).Conclusion CCNB2 and Cyclin E1 are highly expressed in HCC and are associated with serum AFP level,cirrhosis status,tumor thrombus and recurrence,and can be used as independent prognostic markers.

Objective:To investigate the current status of endoscopic treatment for gastroesophageal varices in portal hypertension in China, and to provide supporting data and reference for the development of endoscopic treatment.Methods:In this study, initiated by the Liver Health Consortium in China (CHESS), a questionnaire was designed and distributed online to investigate the basic condition of endoscopic treatment for gastroesophageal varices in portal hypertension in 2022 in China. Questions included annual number and indication of endoscopic procedures, adherence to guideline for preventing esophagogastric variceal bleeding (EGVB), management and timing of emergent EGVB, management of gastric and isolated varices, and improvement of endoscopic treatment. Proportions of hospitals concerning therapeutic choices to all participant hospitals were calculated. Guideline adherence between secondary and tertiary hospitals were compared by using Chi-square test.Results:A total of 836 hospitals from 31 provinces (anotomous regions and municipalities) participated in the survey. According to the survey, the control of acute EGVB (49.3%, 412/836) and the prevention of recurrent bleeding (38.3%, 320/836) were major indications of endoscopic treatment. For primary [non-selective β-blocker (NSBB) or endoscopic therapies] and secondary prophylaxis (NSBB and endoscopic therapies) of EGVB, adherence to domestic guideline was 72.5% (606/836) and 39.2% (328/836), respectively. There were significant differences in the adherence between secondary and tertiary hospitals in primary prophylaxis of EGVB [71.0% (495/697) VS 79.9% (111/139), χ2=4.11, P=0.033] and secondary prophylaxis of EGVB [41.6% (290/697) VS 27.3% (38/139), χ2=9.31, P=0.002]. A total of 78.2% (654/836) hospitals preferred endoscopic therapies treating acute EGVB, and endoscopic therapy was more likely to be the first choice for treating acute EGVB in tertiary hospitals (82.6%, 576/697) than secondary hospitals [56.1% (78/139), χ2=46.33, P<0.001]. The optimal timing was usually within 12 hours (48.5%, 317/654) and 12-24 hours (36.9%, 241/654) after the bleeding. Regarding the management of gastroesophageal varices type 2 and isolated gastric varices type 1, most hospitals used cyanoacrylate injection in combination with sclerotherapy [48.2% (403/836) and 29.9% (250/836), respectively], but substantial proportions of hospitals preferred clip-assisted therapies [12.4% (104/836) and 26.4% (221/836), respectively]. Improving the skills of endoscopic doctors (84.2%, 704/836), and enhancing the precision of pre-procedure evaluation and quality of multidisciplinary team (78.9%, 660/836) were considered urgent needs in the development of endoscopic treatment. Conclusion:A variety of endoscopic treatments for gastroesophageal varices in portal hypertension are implemented nationwide. Participant hospitals are active to perform emergent endoscopy for acute EGVB, but are inadequate in following recommendations regarding primary and secondary prophylaxis of EGVB. Moreover, the selection of endoscopic procedures for gastric varices differs greatly among hospitals.
6.Risk factors for complications of the retromandibular approach in patients with parotid gland posterior lower pole tumors

Objective:To analyze the risk factors for complications of the retromandibular approach in patients with parotid gland posterior and lower pole tumors.Methods:A retrospective analysis was conducted on the clinical data of 140 patients with parotid posterior lower pole tumors admitted to the Xingtai Third Hospital from October 2019 to October 2021. They were divided into two groups based on whether complications occurred: the occurrence group and the non occurrence group. General data of the two groups of patients were collected, including age, gender, course of disease, previous surgical history, number of tumors, tumor length, resection range, facial nerve dissociation, tumor site resection frequency, and fascia preservation; Single factor and logistic multivariate analysis were conducted to determine the risk factors for complications of the posterior retromandibular approach in patients with parotid gland posterior and lower pole tumors.Results:A total of 140 patients with parotid gland posterior lower pole tumors underwent retromandibular approach treatment, with complications occurring in 38 cases (27.14%), including 7 cases of temporary facial paralysis, 10 cases of facial depression, 11 cases of Frey syndrome, 2 cases of fistula, and 8 cases of sensory abnormalities of the greater auricular nerve. Through logistic multivariate analysis, it was found that the number of tumors ≥ 2 ( OR=2.856), the resection range (total resection) ( OR=2.477), the number of surgeries ≥3 ( OR=5.637), facial nerve dissociation ( OR=3.526), and lack of fascia preservation ( OR=2.551) were all risk factors for postoperative complications in patients with parotid posterior pole tumors (all P<0.05). Conclusions:In clinical practice, relevant prevention and treatment measures should be formulated for these high-risk factors to reduce the incidence of postoperative complications.
7.Correlated research between the expressions of C-met and COX-2 in papillary thyroid cancer

Objective To study the expressions of C -met and COX -2 in thyroid papillary cancer ( TPC) and to analyze the relationship between their expressions and clinicopathological features .Methods S-P immunocytochemical technique was used to examine the expressions of C -met and COX-2 in 96 cases of TPC and 46 cases of benign tissues ,and their relationship with clinicopathological features were analyzed .Results The positive rates of C -met and COX -2 in 96 cases of TPC were 92.71% and 71.88%,showing obvious differences between TPC and benign tissues (P<0.001).In TPC there is a statistically significant difference of the expression of C-met in older than45 years and less than 45 years of age(P<0.05).Difference of COX-2 expression in TPC was statistically significant in patients with and without lymph node metastasis ( P<0.05 ) . The positive expression of COX -2 was closely related to the expression of C -met(r=0.451,P<0.05).Con-clusion The abnormal expressions of C -met and COX-2 could be important significance in the growth and in-vasion of TPC,and COX-2 might have some relationship with lymph node metastasis .
