1.Effects of safflower polysaccharide on ischemic stroke in rats by regulating the Gas6/Axl signaling pathway

OBJECTIVE To investigate the effects of safflower polysaccharide (SPS) on ischemic stroke (IS) in rats by regulating the growth arrest-specific protein 6 (Gas6)/Axl receptor tyrosine kinase (Axl) signaling pathway. METHODS The modified suture-occluded method was employed to establish a rat model of middle cerebral artery occlusion (MCAO) for IS. The rats were then randomly divided into the model (Model) group, SPS low- and high-dose (SPS-L, SPS-H, 50 and 100 mg/kg) groups, and integrated traditional Chinese and Western medicine (SPS-H+Nim, SPS 100 mg/kg+Nim 15 mg/kg) group, with 10 rats in each group. Another 10 rats were selected as the sham operation (Sham) group. Rats in each group were administered corresponding doses of medication or an equal volume of normal saline intragastrically, with continuous intervention for 14 days. The neurological function of rats in each group was evaluated 24 hours after drug administration. The morphological changes in hippocampal tissue were observed. The contents of interleukin-6 (IL-6), IL-1β, and tumor necrosis factor-α (TNF-α) in serum, the percentage of cerebral infarction area, the neuronal apoptosis rate, as well as the contents of malondialdehyde (MDA), superoxide dismutase (SOD), and brain-derived neurotrophic factor (BDNF) in hippocampal tissue, and the relative protein expression levels of Gas6, phosphorylated Axl (p-Axl), Axl, B-cell lymphoma-2 (Bcl-2), and Bcl-2 associated X protein (Bax) were measured. RESULTS Compared with Model group, the neurological function of rats in SPS-L group, SPS-H group and SPS-H+Nim group improved significantly, while serum contents of IL-6, TNF-α, and IL-1β, the percentage of cerebral infarction area, the apoptotic rate of neurons as well as MDA content and relative protein expression of p-Axl and Bax were decreased significantly ( P <0.05 or P <0.01). The contents of SOD and BDNF, as well as the relative protein expression of Gas6, Axl, and Bcl-2 in hippocampal tissue, were significantly increased ( P <0.05 or P <0.01), and the ischemic injury in hippocampal tissue imp roved to varying degrees. CONCLUSIONS SPS can reduce neuroinflammation, oxidative stress, and neuronal apoptosis in IS rats, alleviate nerve injury, and improve neurological function, which may be achieved by activating the Gas6/Axl signaling pathway.
2.Protective effects of quercetin, the key component of Zuo Gui Wan, against Alzheimer's disease via the PI3K/AKT pathway: insights from network pharmacology, molecular docking, and cell experiments.

OBJECTIVES:
To investigate the protective mechanism of quercetin, the core component of Zuo Gui Wan, against Alzheimer's disease through the PI3K/AKT signaling pathway, based on network pharmacology, molecular docking, and cell experi-ments.
METHODS:
The active components of Zuo Gui Wan were identified by searching TCMSP, PubChem, Swiss Target Prediction, and BATMAN-TCM databases, and their potential targets were predicted. The target information was standardized using Uniprot, and Alzheimer's disease-related target genes were obtained from Drugbank, GeneCards, and OMIM. The intersection of these datasets was used to identify the potential targets of Zuo Gui Wan for treating Alzheimer's disease. Gene Ontology (GO) functional enrichment and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses were performed. The protein-protein interaction (PPI) network of potential targets was visualized using Cytoscape 3.10.1 software and the STRING database. The key active compounds and core potential targets for treating Alzheimer's disease with Zuo Gui Wan were identified through calculation. Based on the enrichment analysis results and literature, quercetin and the PI3K/AKT pathway were selected for verification. Molecular docking and binding ability prediction between quercetin and the core target AKT were performed using CB-Dock2, and visualization was conducted with AutoDock and PyMOL software. Finally, Aβ1-42-induced HT-22 mouse hippocampal neuronal cells were used to construct an Alzheimer's disease cell model. Quercetin, the PI3K inhibitor LY294002, and the activator EGF were used as interventions. The groups were divided as follows: Control, Aβ1-42, Aβ1-42+Quercetin 2.5 μM, Aβ1-42+Quercetin 5 μM, Aβ1-42+Quercetin 10 μM, Aβ1-42+EGF, and the PI3K/AKT modulation group: Control, LY294002, LY294002+Quercetin 10 μM, LY294002+EGF. CCK-8 assays were performed to detect cell viability, while JC-1, Calcein AM-PI, and Hoechst staining were used to assess cell apoptosis. Western blotting was employed to detect the expression of relevant target proteins.
RESULTS:
Network pharmacology and cell experiments collectively demonstrate that the key active ingredient of Zuo Gui Wan, quercetin, targets core proteins such as AKT1 and GSK3β through a network-based approach, significantly enriching the PI3K/AKT pathway. Molecular docking results indicate that quercetin has a strong binding affinity with AKT. Experimental validation in the Aβ1-42 oligomer-induced HT-22 model reveals that quercetin significantly activates the PI3K/AKT signaling pathway, which is inhibited by Aβ1-42 oligomers, as well as Bcl-2 protein expression. It also suppresses the expression of Cleaved Caspase 3/Caspase 3, BAX, and Cytochrome C proteins. JC-1, Hoechst 33342, and Calcein AM-PI staining results further show that quercetin can significantly alleviate apoptosis induced by Aβ1-42 oligomers in HT-22 cells. Treatment with the PI3K inhibitor LY294002 in HT-22 cells leads to reduced cell viability and decreased expression of p-AKT/AKT and Bcl-2 proteins, while increasing the expression of Cleaved Caspase 3/Caspase 3, BAX, and Cytochrome C proteins. Additionally, apoptosis levels increase as observed in JC-1, Hoechst 33342, and Calcein AM-PI staining, all of which can be reversed by quercetin and the PI3K agonist EGF.
CONCLUSIONS
Quercetin, the key active ingredient of Zuo Gui Wan, exerts its protective effects against Alzheimer's disease by regulating the PI3K/AKT signaling pathway, inhibiting neuronal cell damage and apoptosis.
3.Research progress on the origin of gut microbiota in liver cancer and its application in diagnosis and treatment

In recent years, the research on the microbial community within cancer has gradually attracted attention. The microbiota within the tumor affects the immune regulation of liver cancer and may promote the progression of liver cancer through inflammatory responses, metabolic regulation, etc. This article explores the sources and distribution characteristics of microorganisms in liver cancer, their potential mechanism of action in the occurrence and development of liver cancer, and evaluates the application prospects of intratumoral microorganisms in the early diagnosis, treatment response prediction and personalized treatment of liver cancer, especially their potential value in targeted therapy, providing new ideas for microbial intervention in future precision medicine.
4.Application value of intravenous paracetamol combined with nerve block in preoperative analgesia of elderly patients with hip fracture

Objective To evaluate the safety and efficacy of intravenous paracetamol combined with nerve block in preoperative analgesia in elderly patients with hip fracture.Methods A total of 80 elderly patients with hip fracture admitted to Fourth People's Hospital of Langfang from December 2023 to August 2024 were selected.According to random number table method,the patients were divided into study group and control group,40 cases in each group.Both groups were blocked by pericapsular nerve group(PENG).Based on this,the study group was given intravenous paracetamol and the control group was given oral paracetamol.The analgesic effect,adverse reactions and perioperative period indexes were compared between two groups.Results The duration of pain relief in study group was significantly shorter than that in control group(P<0.05).The number of analgesia and the amount of morphine used within 24h in study group were significantly lower than those in control group(P<0.05).There were no significant differences in the occurrence of adverse reactions,preoperative waiting time,hospital stay and proportion of patients discharged with improvement between two groups(P>0.05).Conclusion Intravenous paracetamol combined with nerve block can be used safely and effectively for preoperative analgesia in elderly patients with hip fracture.
5.Research on the construction and application of an intelligent internet of things-enabled dental chair platform based on dental chair domain interconnection

To address the problem of data silos in dental specialties caused by equipment heterogeneity, this study developed an Intelligent Internet of Things (IoT)-enabled dental chair platform (hereinafter referred to as the intelligent platform) based on the concept of medical-engineering integration. The platform adopts a three-tier chair-domain interconnection architecture: the bottom tier integrates multi-source sensors and standardized interfaces for automated data acquisition and linkage with hospital information systems; the middle tier provides clinic-level management and remote teaching collaboration; and the top tier employs a blockchain-based secure cloud database for resource allocation and data management. Clinical validation at The Affiliated Stomatological Hospital of Nanjing Medical University demonstrated that, compared with a control group from the same period in 2023, the trial group achieved a 38.0% increase in average daily patient visits (80.6±6.8 vs. 58.4±5.2, t=15.16, P<0.001), a 24.6% reduction in average treatment time [(36.1±6.3) min vs. (47.9±8.5) min, t=7.72, P<0.001], a 39.2% reduction in waiting time [23.3 (16.5, 30.1) min vs. 38.3 (28.3, 48.3) min, U=32.00, P<0.001], a 30.4% reduction in equipment idle rate [8.7% (5.1%, 12.3%) vs. 12.5% (7.4%, 17.6%), U=251.00, P=0.003], and an increase in patient satisfaction from 88.2% (1 519/1 723) to 94.3% (2 186/2 318) ( t=7.26, P<0.001). User research confirmed that the functions most favored by clinicians and patients were "dental chair parameter updating and clinical data integration" [74.7% (80/107)] and "chairside display of diagnostic images" [76.8% (119/155)], respectively. Looking forward, the intelligent platform has the potential to integrate artificial intelligence-assisted diagnosis and 5G-enabled multicenter collaboration to further expand its clinical applications and accelerate the digital transformation of dental healthcare.
6.Relationship between serum NfL,Endocan,BAFF and epilepsy secondary to acute cerebral infarction and its predictive value

Objective To explore the relationship and predictive value between serum neurofilament light chain protein(NfL),endothelial cell specific molecule-1(Endocan),B cell activating factor(BAFF)and epi-lepsy secondary to acute cerebral infarction(ACI).Methods From June 2020 to June 2023,135 patients with secondary epilepsy due to ACI admitted to the hospital were included in the epilepsy group,and 90 ACI pa-tients without secondary epilepsy admitted during the same period were included in the non-epilepsy group.The general data of the two groups and the levels of serum NfL,Endocan and BAFF were determined and compared.Multivariate Logistic regression was used to analyze the related influencing factors of epilepsy sec-ondary to ACI.The receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum NfL,Endocan and BAFF for epilepsy secondary to ACI.Results Compared with non-epilepsy group,the age,the proportion of infarction lesions involving cortex,the proportion of National Institutes of Health Stroke Scale(NISSH)score≥15 points,the proportion of infarct volume>10 cm3,the proportion of family history of epilepsy,and serum NfL,Endocan and BAFF were significantly increased in epileptic group(P<0.05).Among patients with infarction lesions involving cortex and NISSH score ≥15 points,the expression levels of serum NfL,Endocan,and BAFF were significantly higher in those with epilepsy than in those without epilepsy(P<0.05).Multivariate Logistic regression analysis showed that infarction involving the cortex,NISSH score ≥15 points,elevated serum NfL,Endocan and BAFF were risk factors for secondary epilepsy in ACI(P<0.05).The results of ROC curve analysis showed that the area under the curve and specificity of the combined prediction of serum NfL,Endocan,and BAFF for secondary epilepsy in ACI were superior to those of individual detection or the combined detection of the two(P<0.05).Conclusion The high expression of serum NfL,Endocan and BAFF are risk factors for secondary epilepsy in ACI.The combined detection of the three has a high predictive value for secondary epilepsy in ACI.

Objective To assess the clinical value of a novel surgical technique—Tubeless subxiphoid uniportal video-assisted thoracoscopic surgery with percutaneous suspension technique via balance-shaped sternal elevation device in the resection of anterior mediastinal masses. Methods Patients who underwent tubeless subxiphoid uniportal video-assisted thoracoscopic surgery via balance-shaped sternal elevation device in anterior mediastinal masses process at the Department of Thoracic Surgery, West China Hospital, Sichuan University from March to April 2025 were included, and their clinical data were analyzed. Results A total of 4 patients were included, with 2 males and 2 females, aged 58-75 years. The diameter of the tumor was 2.5-3.0 cm. The operation time was 60.0-150.0 min, intraoperative blood loss was 5-10 mL, pain score on the 3rd day after surgery was 0 points, and postoperative hospital stay was 2-3 days. All patients achieved complete resection of the masses and thymus without perioperative complications. Conclusion The tubeless subxiphoid uniportal video-assisted thoracoscopic surgery with percutaneous suspension technique via balance-shaped sternal elevation device technique optimizes surgical visualization and instrument maneuverability while avoiding complications related to conventional anesthesia and tubing, thereby markedly enhancing the minimally invasive profile of anterior mediastinal masses resections. In addition to maintaining procedural safety, this approach effectively reduces postoperative pain and accelerates patient recovery, highlighting its potential for widespread clinical adoption.
8.Gut microbiome combined with clinical features for preoperative microvascular invasion prediction in hepatocellular carcinoma

Objective:To explore the value of combining gut microbiota and clinical features for preoperative microvascular invasion (MVI) prediction in hepatocellular carcinoma (HCC).Methods:Clinical data and fecal samples were collected from 71 HCC patients who underwent curative resection at Ningbo Second Hospital between Jan 2023 and Aug 2024. Among them, 41 patients were assigned to the training set and 30 to the validation set. Gut microbiota composition was analyzed using 16S rRNA sequencing. Redundancy analysis (RDA) was used to evaluate the influence of clinical features on the microbiota. Differences in alpha and beta diversity between the MVI-negative and MVI-positive groups were assessed. Differential genera were identified using the Wilcoxon test and LEfSe analysis. A random forest model and Logistic regression were employed to screen key differential genera, followed by ROC analysis. Genera with high ROC values were further validated in the validation set.Results:RDA indicated that MVI was a key factor influencing gut microbiota composition. The random forest model (AUC=0.925), combined with Logistic regression analysis, identified four genera: Acidovorax ( OR=0.618), Tissierella ( OR=1.293), Chitinophaga ( OR=4.596), and Virgisporangium ( OR=0.960), as well as two clinical features: tumor diameter ( OR=0.668) and liver cirrhosis ( OR=14.011), as independent risk factors. ROC analysis showed that in the training set, the combination of Chitinophaga (AUC=0.71) and tumor diameter (AUC=0.75) had the best diagnostic performance (AUC=0.87). In the validation set, the combination of Virgisporangium (AUC=0.80) and tumor diameter (AUC=0.79) yielded the highest diagnostic performance (AUC=0.87). Conclusions:A genomics-based model combining gut microbiota and clinical features shows promising predictive value for noninvasive preoperative assessment of MVI status in HCC patients.
9.Fusobacterium nucleatum upregulates ABCG2 by activating the E-cadherin/β-catenin signaling pathway to promote oxaliplatin resistance in colorectal cancer

Objective:To investigate whether F.nucleatum affects the chemotherapy resistance of colorectal cancer by regulating ABC subfamily G subtype 2 (ABCG2), in view of the fact that the correlation between the two has not been reported. Methods:Oxaliplatin was used to interfere with colorectal cancer cells and co-cultured with F.nucleatum to establish a chemotherapy-induced model of microbial infection. Calcein AM/PI cell staining, trypan blue staining, and cell counting kit 8 (CCK-8) method were used to detect cell activity. Real-time fluorescence quantitative polymerase chain reaction and western blot were used to detect ABCG2 mRNA and protein expression levels in colorectal cancer cells. The target gene was knocked down by constructing shRNA plasmids. The HCT-116 cell F.nucleatum infection model was constructed and transcriptome sequencing and Kyoto Encyclopedia of Genes and Genomes (KEGG) and gene ontology (GO) analyses were performed to determine the differential gene expression enrichment pathways. Genistein (GST) was used as an E-cadherin blocker, and triclabendazole sulfoxide (TCBZSO) was used as an ABCG2 blocker. Immunofluorescence was used to detect E-cadherin and β-catenin protein expressions and intracellular localization levels. The subcutaneous xenograft model of nude mice was constructed in vivo, and the expression level of ABCG2 protein in tissues was detected by immunohistochemical staining. Results:CCK-8 results showed that F. nucleatum+oxaliplatin group [HT-29, (92.26±1.66)%; HCT-116, (82.13±1.84)%] cell relative survival rate was higher than that of oxaliplatin group [HT-29, (79.64±3.72)%; HCT-116, (67.56±2.96)%; P<0.001]. The relative survival rate of oxaliplatin and F. nucleatum co-culture group with ABCG2-knockdown HCT-116 cells [(61.44±1.48)%] was lower than that of F. nucleatum and oxaliplatin co-cultured with HCT-116 cells [(69.29±4.45)%, P=0.015]. GO enrichment analysis showed that HCT-116 cells co-cultured with F.nucleatum were significantly enriched in "β-catenin binding", "cadherin binding" and "regulation of Wnt signaling pathway". RT-qPCR results showed that the relative expression of ABCG2 mRNA in F.nucleatum + genistein group was significantly lower than that in F.nucleatum group ( P<0.001). The results in vivo showed that the tumor weight in the F.n+L-OHP+TCBZSO group [(0.12±0.02)g] was lower than that in the F.n+L-OHP group [(0.33±0.05)g, P<0.001]. Immunohistochemistry suggested that the promotion of ABCG2 protein expression by F. nucleatum was blocked after TCBZSO intervention. Conclusion:F. nucleatum up-regulates ABCG2 expression through E-cadherin/β-catenin signaling pathway to promote colorectal cancer resistance to oxaliplatin.
