1.Surveillance of schistosomiasis and snail status in Jiaxing City from 2001 to 2024

Objective:
To analyze the surveillance data of schistosomiasis and snail status in Jiaxing City, Zhejiang Province from 2001 to 2024, so as to provide the reference for prevention and control of schistosomiasis in jiaxing City.
Methods:
Data on schistosomiasis and snail surveillance in Jiaxing City from 2001 to 2024 were collected through schistosomiasis control work reports and the Zhejiang Provincial Schistosomiasis (Parasitic Diseases) Control Information Management System. These data included serological testing results, stool etiological examination (stool examination) results, area surveyed for snails, snail-infested areas, number of snail-positive frames, and number of live snails. Indicators, including the positive rate of serological testing, the positive rate of stool examinations, the rate of snail-positive frames, and the density of live snails were analyzed. The Prophet time series model was employed to forecast the schistosomiasis epidemic in Jiaxing City from 2025 to 2029.
Results:
A total of 636 493 serological testing were conducted in Jiaxing City from 2001 to 2024, with a positive rate of 1.532%, showing a decreasing trend (P<0.05). Among 7 582 stool examinations, positive cases were all imported, resulting in a positivity rate of 0.066%. During the same period, snail surveys covered a cumulative area of 30 545.999 hm2, with snail-infested areas totaling 69.355 hm2; no significant trend was observed (P>0.05). All snail habitats were identified as recurrent foci within hydrographic network regions, primarily distributed across Xiuzhou District, Nanhu District, Pinghu City, Jiashan County, and Tongxiang City, with snail-infested areas of 39.588, 12.538, 10.728, 4.315, and 2.186 hm2, respectively. From 2009 to 2024, a total of 35 692 134 frames of snails were surveyed, of which 16 543 were snail-positive, yielding a snail-positive frame rate of 0.046%. A total of 33 175 live snails were collected, with a mean density of 0.000 98 snails per frame. No infected Oncomelania snails were detected. The projection results indicated that from 2025 to 2029, the positive rate of serological testing rate in Jiaxing City would range between 0.253% to 0.389%, the snail-infested areas would range from 0.025 to 1.818 hm2, and the density of live snails would vary from 0.001 56 to 0.001 66 snails per frame. None of these indicators showed a significant trend (all P>0.05).
Conclusions
From 2001 to 2024, the positive rate of serological testing rate of schistosomiasis in Jiaxing City showed a declining trend, with no new autochthonous cases or infected Oncomelania snails detected. However, imported cases were still reported. All identified snail habitats were recurrent foci within hydrographic network regions. It is recommended to enhance schistosomiasis and snail status surveillance in high-risk areas.
2.The efficacy of vonoprazan fumarate in the treatment of laryngopharyngeal reflux disease evaluated by the Chinese version of the RSS-12 scale.

Objective: To explore the efficacy of vonoprazan fumarate in the treatment of laryngopharyngeal reflux disease(LPRD) evaluated by the Chinese version of the RSS-12 scale. Methods:A total of 100 LPRD patients treated in the otolaryngology-head and neck surgery outpatient clinic of our hospital were randomly divided into two groups(50 cases each). The observation group was treated with vonoprazan fumarate(20 mg, once daily), and the control group was treated with esomeprazole enteric-coated capsules(20 mg, twice daily) for 12 weeks. The selected observation indicators in this study included RSI, the Chinese version of RSS-12, and RFS scores before and after treatment. Results:Prior to treatment, there was no statistically significant difference in the Chinese version of RSS-12, RSI, and RFS between the two groups(P>0.05). After 8 and 12 weeks of antacid treatment, both the Chinese version of RSS-12 and RSI significantly decreased in both group (P<0.05). The changes in symptoms were particularly noticeable between 0-8 weeks of treatment according to the Chinese version of RSS-12 and RSI. After 12 weeks of treatment, the RFS scores significantly decreased compared to pretreatment levels (P<0.05). After 12 weeks of antacid treatment, according to the Chinese version of RSS-12, 80% of patients in the observation group showed a good therapeutic response, compared to 64% in the control group; according to RSI, 90% of patients in the observation group showed a good therapeutic response, compared to 84% in the control group. There was no statistically significant difference in the treatment effect between the two groups after 12 weeks of treatment(P>0.05). Conclusion:Vonoprazan fumarate can significantly improve the symptoms and signs of laryngopharyngeal reflux, and their treatment effect is not inferior to proton pump inhibitors. Compared with RSI, the Chinese version of RSS-12 can serve as a new screening tool for clinical diagnosis of LPRD in China.

Alzheimer's disease(AD), a neurodegenerative disorder associated with aging, is the most prevalent form of dementia.As the aging population continues to expand, AD presents significant health and caregiving challenges for families and society, making it a pressing international public health concern.In recent years, numerous countries have implemented dementia prevention and treatment strategies that emphasize community-based comprehensive approaches.Currently, the community-based AD prevention and treatment model in China is still in the exploratory phase, with community efforts lacking organization.In alignment with China's action plan for advancing dementia prevention and treatment, and to achieve the strategic objective of "healthy aging, " this consensus is based on the principle of three-level prevention and is tailored to the characteristics of AD disease progression.It aims to develop a comprehensive prevention and treatment strategy for AD that is suitable for communities in China, providing technical guidance and support to establish a scientific basis for formulating community AD prevention and treatment models.
4.Clinical effect of minimally invasive rotational curettage combined with water-assisted liposuction in treatment of gynecomastia

Objective To observe the clinical effect of minimally invasive rotational curettage combined with water-assisted liposuction(WAL)in the treatment of gynecomastia(GYN)patients.Methods The clinical data of 80 GYN patients were retrospectively analyzed.According to different surgical methods,the patients were divided into minimally invasive surgery group(41 cases)and study group(39 cases).The minimally invasive surgery group underwent minimally invasive rotation-al curettage,while the study group underwent minimally invasive rotational curettage combined with WAL.The surgical indicators,pain intensity 24 hours postoperatively,postoperative complications,and subjective satisfaction were observed and compared between the two groups.Results The unilat-eral surgical duration in the study group was shorter than that in the minimally invasive surgery group,with a statistically significant difference(P<0.05).No statistically significant differences were found in unilateral incision length,intraoperative blood loss,and postoperative hospital stay between the two groups(P>0.05).No statistically significant difference was found in pain intensity 24 hours postoperatively between the two groups(P>0.05).The incidence of postoperative complicationsin the study group was 5.13%,which was lower than 21.95%in the minimally invasive surgery group(P<0.05).The study group had higher satisfaction scores for chest flatness without depression and breast symmetry than the minimally invasive surgery group(P<0.05).Conclusion Compared with minimally invasive rotational curettage alone,the combination of minimally invasive rotational curet-tage and WAL for GYN patients results in shorter surgical duration,a lower incidence of postoperative complications,and higher patient satisfaction with breast appearance.
5.Development and validation of a risk prediction model for severe acute pancreatitis induced by hypertriglyceridemia

Objective:To investigate the risk factors for patients with hypertriglyceridemia-related acute pancreatitis (HTG-AP) developing into severe acute pancreatitis or experiencing organ failure.Methods:This retrospective cohort study collected clinical data from 2 429 patients diagnosed with acute pancreatitis from five hospitals in China between January 2019 and December 2023 using a pre-designed data collection form. The cohort included 1 516 males and 913 females,with an age of (50.2±16.5)years(range: 11 to 99 years). Among them,353 patients (16.1%) had HTG-AP,while 1 846 (83.9%) had non-HTG-AP. HTG-AP was defined as serum triglyceride levels>500 mg/dl with other etiologies excluded. Intergroup comparisons were performed using t-tests,Mann-Whitney U test or χ2 tests,respectively. Univariate and multivariate logistic regression analyses were conducted to assess risk factors for severe acute pancreatitis after adjusting for potential confounders,and a predictive model was developed and validated. Results:Compared with other etiologies,HTG-AP patients had a higher risk of progressing to SAP ( OR=1.415,95% CI: 0.866 to 2.312, P=0.017) and organ failure ( OR=1.256,95% CI: 1.015 to 1.554, P=0.036). Among HTG-AP patients,risk factors for SAP included body mass index ( OR=1.856,95% CI: 1.742 to 1.987, P=0.033),fasting blood glucose ( OR=1.128,95% CI: 1.036 to 1.229, P=0.006),white blood cell count( OR=1.162,95% CI: 1.055 to 1.281, P=0.002),and the presence of pleural effusion ( OR=13.151,95% CI: 4.330 to 19.946, P<0.01). A nomogram prediction model for SAP in HTG-AP was constructed based on these risk factors,demonstrating good discriminative ability with area under the curve values of 0.877 in the training set and 0.894 in the validation set,along with satisfactory calibration. Conclusions:HTG-AP patients are at higher risk of developing SAP and organ failure. The risk prediction model incorporating body mass index,fasting blood glucose,white blood cell count,and pleural effusion shows good predictive value for SAP.

Objective:To examine the funding landscape of the National Natural Science Foundation of China(NSFC)concerning research focused on the elderly as a specific demographic group.It seeks to analyze prevailing research hotspots and to offer insightful research ideas for scholars in this field.Methods:The study employed a retrospective research design.It involved retrieving data on approved projects related to the elderly from the NSFC's official database, covering the period from 2000 to 2020.The data set included variables such as funding year, project title, project category, supporting institution, and funding amount(application codes: H1901, H1902, H1903).Statistical analysis was conducted using Excel software, while visual analysis was performed using CiteSpace software.Results:Between 2000 and 2020, a total of 710 projects focused on geriatrics received funding, amounting to RMB 361, 032, 300, 000.This period exhibited a fluctuating upward trend in both the number of projects and the total funding amount.The primary funding categories were face-to-face projects and youth science fund projects, which together accounted for 44.4%(315/710), 34.4%(244/710)respectively.The distribution of project-relying units demonstrated significant regional imbalance, with Beijing, Zhejiang, Jiangsu, and Shanghai being the predominant areas.Notably, Nanjing Medical University, Peking University, and Shanghai Jiao Tong University ranked as the top three institutions, with funding percentages of 5.9%(42/710), 5.4%(38/710), and 4.5%(32/710), respectively.The research themes encompassed nucleoplasmic interactions, brain networks, and bioinformatics, among others.Conclusions:The NSFC has consistently provided funding for in-depth research in the critical field of geriatrics.The primary research directions focus on aspects such as the mechanisms of aging and the pathogenesis of neurological diseases associated with aging.With the application of innovative technologies and methodologies, significant breakthroughs and achievements are anticipated in geriatrics, contributing to the improvement of health outcomes and the enhancement of the quality of life for the elderly.

10.Population genetics analysis of Oncomelania hupensis in Jiaxing, Zhejiang Province, 2022

ObjectiveTo genotype Oncomelania hupensis, based on microsatellites, in different snail-bearing environments in Jiaxing City, Zhejiang Province, for population genetics analysis in order to explore the reasons and influencing factors for the existence or proliferation of snails and to provide scientific basis for effective monitoring and control of snails. MethodsA total of 90 snail samples from three populations were collected in Yaobang Village (YB) and Sanxing Village (SX) in Pinghu City, and Yunhe Farm (YH) in Xiuzhou District, all were selected for snail checking in key snail habitats of Jiaxing City in 2022. DNA of the snails was genotyped and analyzed for population genetics using nine microsatellite loci. ResultsA total of 84 alleles were observed, and the mean number of alleles (Na) was 7.889, 5.667, and 3.778 for YB, SX, and YH respectively; the number of effective alleles (NeA) was 4.807, 3.329, and 2.294, respectively; and the coefficients of inbreeding (FIS) were 0.400, 0.377, and 0.493, respectively. Under the Infinite Allele Model (IAM), the SX and YH might have a recent bottleneck. The NEstimator and LDNe software calculated effective population sizes (Ne) were above 31.9. AMOVA analysis showed that the variation of snails in the three populations mainly existed among individuals, accounting for 41.4% of the total variation. The value of the index of genetic differentiation between populations (FST) was 0.286, indicating a high degree of genetic differentiation. The results of the principal component analysis and phylogenetic tree were consistent, and the three populations were divided into two lineages, YB and SX were one lineage, and YH belonged to another independent lineage. Population history and dynamics analysis showed that the gene flow of the three populations was insufficient, population divergence history indicated that YH might have diverged from SX first, and YB was produced by the contact fusion of SX and YH. ConclusionThe genetic diversity of snail populations in Jiaxing City is generally low, and the snail populations are unstable, with a great degree of genetic differentiation and insufficient gene flow among populations. This study can provide a basis for evaluating the effectiveness of the control of the snail as well as monitoring the trend of the spread of the snail.
