1.The status and influencing factors of type 2 diabetes mellitus patients' fear of complications

Objective To investigate the status and influencing factors of type 2 diabetes mellitus(T2DM)patients' fear of complications,and to provide a reference for formulating targeted intervention measures.Methods From April to November 2024,370 patients with T2DM in 2 tertiary general hospitals in Daqing City were selected by convenience sampling method.General data questionnaire,Fear of Complications Questionnaire,Self-Perceived Burden Scale,Psychological Capital Questionnaire,Mishel Uncertainty in Illness Scale and Family Apgar Index Questionnaire were used for investigation.Univariate analysis and binary Logistic regression were performed to analyze the influencing factors.Results A total of 364 valid questionnaires were collected,with an effective recovery rate of 98.38%.The score of Fear of Complications Questionnaire was(23.47±7.47),and the incidence of fear of complications was 22.25%.Logistic regression analysis showed that medical payment methods,the number of complications,positive psychological capital and family care were the influencing factors of FoC in T2DM patients.Conclusion The fear of complications in T2DM patients is at a moderate level.Nursing staff should pay attention to the early assessment of patients' fear of complications,promptly identify and take effective measures to reduce the level of patients' fear of complications,improve their quality of life.
2.Effects of extended prone positioning ventilation on ARDS patients with VV - ECMO support

Objectives:To evaluate the effect of extended single prone positioning ventilation on survival and weaning rate of acute respiratory distress syndrome (ARDS) patients supported by VV-ECMO.Methods:ARDS patients supported by VV-ECMO admitted to Jinhua Central Hospital, the Fourth Affiliated Hospital of Zhejiang University School of Medicine and the First Hospital of Jiaxing from September 2014 to May 2025 were retrospectively enrolled into the study. The clinical data, ECMO and ventilator related parameters and outcomes of the patients were collected. The patients were divided into the extended prone positioning group and prone positioning group according to whether the duration of prone position ventilation was greater than 24 h. The clinical data of the two groups were compared to explore the effects on 30-day survival in-hospital survival and ECMO withdraw rate of these patients. Multivariate logistic regression analysis was used to explore the relationship between the duration of single prone position ventilation and the success of ECMO weaning, 30-day survival and hospital survival.Results:Total of 163 ARDS patients supported by VV-ECMO receiving prone positioning ventilation were included in study, 64 in extended prone positioning group and 72 in prone positioning group. The 30-day survival (54.7% vs. 52.8%) in-hospital survival (51.6% vs. 48.6%) and ECMO withdraw rate (57.8% vs. 61.1%) between the two groups were not statistically different ( P>0.05) as well as the duration of ECMO support [12(10,15)d vs. 11(10,13)d] the duration of ventilation [16(13,18)d vs. 16(12,18)d] the duration of ICU stay [26(15,32)d vs. 26(19,29)d] and the duration of hospital stay [32(15,42)d vs. 34(28,35)d]. Logistic regression analysis revealed that the duration of each prone position ventilation was not associated with successful weaning ( OR=0.979, 95% CI:0.952-1.006), 30-day survival ( OR=1.015, 95% CI: 0.975-1.056) and hospital survival ( OR=1.014, 95% CI: 0.974-1.055) even after adjusting for the severity of illness, age, and type of pneumonia. Conclusions:For ARDS patients supported by VV-ECMO, extended single prone positioning ventilation for more than 24 hours neither increase 30-day survival in-hospital survival and successful ECMO weaning rate, nor shorten ECMO support duration.
3.Efficacy of occipitocervical fusion with the occipital screw-rod-multiaxial screw internal fixation system in the treatment of"sandwich"deformity with atlantoaxial dislocation

Objectives:To investigate the clinical efficacy of occipitocervical fusion with the occipital screw-rod-multiaxial screw internal fixation system in treating"sandwich"deformity with atlantoaxial disloca tion.Methods:A retrospective analysis was conducted on 62 patients who underwent posterior reduction,bone graft fusion,and occipitocervical internal fixation for"sandwich"deformity with atlantoaxial dislocation from January 2017 to January 2023.There were 19 males and 43 females,aged 51.1±23.5 years(29-72 years).The patients were divided into two groups based on the different fixation methods of the occipital screws used in the two types of occipitocervical internal fixation systems:group A of 38 patients utilized the occipital screw-rod-multiaxial screw system,consisting of 12 males and 26 females,aged 50.9±24.8 years(29-72 years),and the imaging findings revealed brainstem or cervical spinal cord compression in 30 cases,syringomyelia in 6 cases,and Chiari malformation in 2 cases;And group B of 24 cases utilized the occipital plate-rod-multiaxial screw system,consisting of 7 males and 17 females,aged 51.4±22.4 years(32-69 years),and the imaging findings revealed brainstem or cervical spinal cord compression in 16 cases,syringomyelia in 6 cases,and Chiari malformation in 2 cases.Group A was followed up for 36.5±11.8 months and Group B was followed up for 36.1±9.8 months.No statistically significant differences were observed between the two groups in terms of age,sex,imaging findings,or surgery style(P>0.05).A comparison was performed between the groups regarding intraoperative blood loss,operative time,follow-up period,and complication rates.Clini-cal outcomes were evaluated preoperatively and at the final follow-up using the neck pain visual analog scale(VAS)score,neck disability index(NDI),and Japanese Orthopaedic Association(JOA)score.The cross-sectional and sagittal bone fusion area of the graft site,atlantodental interval(ADI),space available for cord(SAC),clivus canal angle(CCA),and cervico-medullary angle(CMA)were measured preoperatively,1 week postoperatively,and at the final follow-up in both groups.Results:Group A and group B weren't significantly different in intraoperative blood loss(362.4±89.1mL vs 345.1±93.8mL,P>0.05)and operative time(144.8±35.6min vs 143.7±36.2min,P>0.05).In group B,rod fractures occurred in 2 cases and loss of reduction was observed in 3 cases,whereas the remaining patients maintained stable screw-rod fixation.Neither group experienced severe complications such as spinal cord injury or cerebrospinal fluid leakage.Both groups demonstrated statistically significant differences between preoperative NDI,VAS,JOA scores and those measured at the final follow-up(P<0.05),while no statistically significant differences existed between the two groups at the same time points(P>0.05).Statistically significant differences were observed between preoperative and postoperative(1-week and final follow-up)measurements for ADI,SAC,CCA and CMA in both groups(P<0.05),while no significant in-tergroup differences were found at corresponding time points(P>0.05).In group A,the cross-sectional area and the sagittal area of the graft site were 385.44±84.51mm2 and 290.28±1 10.41mm2 in postoperative 1 week,and 267.06±66.13mm2 and 204.54±83.85mm2 at the final follow-up;which in group B were 442.61±76.85mm2 and 267.21±104.63mm2 in postoperative 1 week,and 172.43±37.37mm2 and 124.87±48.10mm2 at final follow-up.Comparisons between groups revealed no statistically significant differences in either cross-sectional or sagittal graft areas at 1 week postoperatively(P>0.05),however,significant differences were observed at the final fol-low-up(P<0.05)for both parameters between the two groups.Conclusions:The occipital screw-rod-multiaxial screw internal fixation system presents comparable restoration effects to the occipital plate-rod-multiaxial screw system in treating"sandwich"deformity with atlantoaxial dislocation.It exhibits satisfactory implant fusion postoperatively and achieves reliable occipitocervical fixation.
4.The relationship between the systemic immune-inflammation index and the clinical pathological characteristics of patients with IgA nephropathy

Objective To explore the correlation between the systemic immune-inflammation index(SII)and the clinical and pathological characteristics of patients with IgA nephropathy(IgAN).Methods A total of 350 patients who underwent renal biopsy and were initially diagnosed with primary IgAN were selected.The clinical and pathological data of the patients were collected,and SII was calculated.According to the median SII level of 554.78 in peripheral blood,the IgAN patients were divided into the low SII group(SII≤554.78,175 cases)and the high SII group(SII>554.78,175 cases).Based on the presence or absence of endocapillary hyperplastic(E)lesion,350 patients were also divided into the E0 group(279 cases,79.7%)and the E1 group(71 cases,20.3%).Multivariate Logistic regression analysis was conducted to determine the influencing factors of E1 in IgAN patients.A predictive model was established,and the predictive value of the model was evaluated using the receiver operating characteristic(ROC)curve.Results There were higher systolic blood pressure(SBP),platelet count(PLT),neutrophil count(NEU),neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),total cholesterol(TC),serum creatinine(Scr),serum C3 and 24-hour urine protein levels in the high SII group than those of the low SII group,while the lymphocyte count(LYM)was lower(P<0.05).In terms of pathological manifestations,the proportion of E1 was higher in the high SII group than that of the low SII group(P<0.05).There were lower PLT,NEU,NLR,PLR,Scr and 24-hour urine protein in patients of the E0 group than those of the E1 group,while higher Hb,LYM and ALB levels in the E0 group than those of the E1 group(P<0.05).Multivariate Logistic regression analysis showed that elevated SII,Scr and 24-hour urine protein levels were independent risk factors for E1 lesion in IgAN patients(P<0.05).The area under the curve(AUC)of the predictive model for E1 lesion in IgAN patients was 0.781(95%CI:0.722-0.840).Conclusion SII can reflect the clinical and pathological severity in IgAN patients,providing new insights for clinical evaluation of the disease progression in IgAN patients.

Meat food is an important part of people's dietary supplements,but in recent years,meat adulteration cases are hidden and frequent,which seriously affects people's health and destroys the fairness of the market.Based on this,this paper analyses the development trend of meat adulteration and the current meat adulteration detection technology is summarized,combed in detail the use of the most widely used real-time fluorescence quantitative PCR technology application of the status quo and challenges to help real-time fluorescence quantitative PCR technology testing in meat adulteration evidence testing to the direction of more high detection efficiency and higher accuracy development.

10.Preliminary study on the biological characteristics of the isolate of vaccinia virus Tiantan strain VG9

Objective:To further study on the biological characteristics of vaccinia virus VG9 strain obtained by passaging from vaccinia virus Tiantan strain(VTT).Methods:The freeze-dried vaccinia virus VG9 strain was reconstituted and inoculated into Vero cells for recovery and sequential passaging. The strain was identified by indirect immunofluorescence and PCR amplification. The whole genome of VG9 was sequenced using next-generation sequencing technology. The sequencing results were compared with the reference sequence of VTT and the genome sequences of other 33 orthopoxviral strains, and a phylogenetic tree was drawn. A purified clone of VG9, namely VG9-V3-3, was obtained by terminal dilution method. Preliminary studies were conducted to characterize the biological properties of this clone, such as virus titer and intradermal virulence in rabbit.Results:VG9 could specifically bind to the rabbit antibody of VTT by indirect immunofluorescence identification, and the PCR amplification results proved that this strain contained the characteristic TK gene fragment of vaccinia virus. Whole-genome sequencing showed that the genome length of VG9 was 183 596 bp, including 165 150 bp in coding region. The sequencing results were compared with the NCBI core nucleotide database, and showed that the sequences of VG9 and multiple existing Tiantan isolates were highly homologous (consistency>99.5%). Sequence comparison revealed a total of 749 nucleotide site differences between the gene sequences of VG9 and TP5 isolate of VTT. The viral titer of the VG9-V3-3 isolate was 6.3 lg (PFU/ml), and the rabbit intradermal virulence assay showed that its virulence was significantly weakened compared with that of VTT.Conclusions:The biological characteristics of vaccinia virus VG9 strain are preliminarily studied and a purified clone strain named VG9-V3-3 is obtained. The viral titer of the isolate is basically stable, and the virulence is significantly weakened compared with that of VTT. In-depth study on the immunogenicity of VG9-V3-3 will be carried out in the future to explore its feasibility as a vaccine production strain.
