1.Summary of clinical experience of 9 children with anterior cervical enterogenic cysts.

Objective:To summarized the clinical characteristics and surgical management of anterior cervical enterogenic in pediatric patients. Methods:Clinical data were retrospectively analyzed for 9 children with pathologically confirmed anterior cervical enterogenic cysts(including bronchogenic and esophagogenic subtypes) treated at the Children's Hospital of Shandong University(Jinan Children's Hospital) between January 1, 2020, and November 30, 2023. Results:Nine patients(6 males and 3 females) were involved in this study, aged 14 days to 10 years old. There were 4 cases on the left side, 4 on the right side, and 1 in the middle of the neck. All patients presented with neck masses. The patients were followed up from 3 months to 35 months after surgery and recovered well, with no recurrence or complications observed. Conclusion:①Anterior intestinal cysts in children are rare and easy to be misdiagnosed. ②Concurrent branchial cleft fistulas or associated anomalies may coexist, necessitating comprehensive evaluation. ③Preoperative diagnosis is not easy and mainly depends on pathological diagnosis. ④The treatment of anterior cervical enterogenic cysts in children is surgical resection of the lesion.

2.Video-assisted Thoracoscopic Surgery in the Treatment of 7 Children With Pulmonary Airway Malformation or Pulmonary Isolation Complicated With Ipsilateral Mediastinal Bronchogenic Cysts

Objective To explore the feasibility of video-assisted thoracoscopic surgery(VATS)in the treatment of congenital pulmonary airway malformation(CPAM)or pulmonary isolation complicated with ipsilateral mediastinal bronchogenic cyst in children.Methods From July 2019 to July 2024,VATS was carried out to treat CPAM or pulmonary isolation with ipsilateral mediastinal bronchogenic cyst in 7 children.A three-hole thoracoscopic surgery via lateral thoracic approach was performed.The patients were placed in a healthy lateral position.The observation hole was located at the intersection of the subscapular line and the 5th intercostal space,and the operating hole was established according to the surgical requirements in combination with the lumboscopic diamond rule.A 5 mm trocar was used for all three holes.The pressure of CO2 pneumothorax was 4 mm Hg and the flow rate was 2 L/min,which was adjusted at any time according to the intraoperative conditions of the children.The operation was mainly designed for lung operation.The pulmonary operation was conducted firstly,and then the bronchogenic cyst was treated.If necessary,block resection was applied to avoid serious complications of trachea.Results All the operations were performed under thoracoscopy without conversion to open surgery.The operation time was 37-191 min(median,101 min).The intraoperative bleeding volume was 1-15 ml(median,5 ml).One case was not given a closed chest drainage tube placed,and the other 6 cases were placed a closed chest drainage for 1-5 d(median,3d).Postoperative pathology showed 5 cases of CPAM combined with bronchogenic cysts,including 4 cases of type 2(bronchiole type)and 1 case of type 3(bronchiole/alveolar type),and 2 cases of extralobular pulmonary isolation combined with bronchogenic cysts.All the 7 cases were followed up for 6-57 months(median,27 months),and chest CT showed no recurrence of lesions.Conclusions CPAM or pulmonary isolation may be accompanied by bronchogenic cysts.Preoperative imaging examination should correspond to surgical observation,and careful exploration should be conducted to avoid missed diagnosis.VATS is safe and feasible for treating CPAM or pulmonary isolation with ipsilateral mediastinal bronchogenic cysts.
3.Recombinant expression and immunogenicity evaluation of seasonal influenza virus hemagglutinin

Objective:To construct four recombinant hemagglutinin (HA) antigens from seasonal influenza viruses and evaluate their immunogenicity in mouse models.Methods:HA coding sequences from four seasonal influenza virus strains Wisconsin (H1N1), Darwin (H3N2), Austria (B/Victoria lineage, BV) and Phuket (B/Yamagata lineage, BY) were optimized and synthesized, and then used to construct four recombinant plasmids. Recombinant baculoviruses were obtained through transformation and transfection. The expression of recombinant HA antigens was identified by SDS-PAGE and Western blot. The recombinant HA antigens were purified by nickel column affinity chromatography and intramuscularly administered to BALB/c mice after formulation with Al(OH) 3 or AddaVax adjuvant. Humoral immune responses were assessed by indirect ELISA and hemagglutination inhibition test, while cellular immune responses were evaluated by ELISPOT. Microneutralization test was used to detect the titers of serum antibodies in mice. Statistical analysis was performed using t test or non-parametric rank sum test. Results:PCR amplification and agarose gel electrophoresis confirmed the correct construction of the recombinant bacmids. Western blot showed verified the successful expression of the four recombinant antigens (H1-HA, H3N2-HA, BV-HA, and BY-HA). SDS-PAGE results showed that the purity of all four recombinant HA antigens exceeded 95%. After three-dose immunization, the total IgG levels in mice immunized with the recombinant H1N1-HA, H3N2-HA, or BV-HA formulated with AddaVax adjuvant were higher than those in the corresponding groups immunized with the same recombinant antigen alone (all P<0.05). The secretion levels of IFN-γ, IL-2, and IL-4 in the group receiving the mixture of all four recombinant HA antigens formulated with AddaVax adjuvant were higher than those in the group immunized with a commercial quadrivalent split influenza vaccine (all P<0.01). Results of the microneutralization test showed that the antibody titer in the quadrivalent split influenza vaccine group was 1∶225, whereas the titer in the group immunized with the mixture of four recombinant HA antigens formulated with AddaVax adjuvant could reach up to 1∶1 200. Conclusions:In this study, four recombinant seasonal influenza virus HA antigens are successfully expressed and demonstrated good immunogenicity in mice when formulated AddaVax adjuvant.
4.Single center experience in surgical resection of pediatric pancreatic tumors

Objective:To summarize the surgical management strategy for pediatric pancreatic tumors.Methods:Clinical data of 17 children with pancreatic tumors who underwent surgical treatment in Children's Hospital Affiliated to Shandong University from January 2018 to April 2024 were retrospectively analyzed, including 6 males and 11 females, ranging in age from 1 month to 13 years, with a median age of 10 years and 2 months. The data of tumor length and diameter, tumor location, surgical method and postope-rative pathology were analyzed. The prognostic data of all children were followed up by regular telephone and outpatient follow-up.Results:In 17 cases, the tumor diameter was (7.3±2.2) cm, 10 cases were located in the head of the pancreas, 7 cases were located in the tail of the pancreatic body, 1 case was accompanied by liver metastasis, and 1 case was accompanied by left adrenal invasion. Among these cases, 10 cases had pancreatic head tumor, 6 cases had simple pancreatic tumor resection, 1 case had laparoscopic pancreatic tumor resection, and 3 cases had pancreatic head resection plus pancreato-jejunal Roux-en-Y anastomosis. Among the 7 cases of pancreatic body and tail tumors, 3 cases underwent pancreatic body and tail resection, 1 case underwent laparoscopic pancreatic body and tail resection, 1 case underwent tumor resection, 1 case underwent pancreatic body and tail resection plus splenectomy plus hepatic metastasis resection, 1 case underwent pancreatic body and tail resection plus left adrenal resection plus splenectomy. Postoperative pathology showed that there were 13 cases of solid pseudopapilloma, 3 cases of pancreatic blastoma, and 1 case of pancreatic cyst. The median follow-up time was 36 months (2-71 months). All patients survived well.Conclusion:Solid pseudopapillary of the pancreas is the most common type of pancreatic tumor in children, with surgical resection being the first choice of of treatment, offering a favorable prognosis postope-ratively.
5.Spatio-temporal clustering analysis of influenza in Jiaxing City

Objective:
To investigate the epidemiological and spatio-temporal characteristics of influenza in Jiaxing City, Zhejiang Province, so as to provide insights into perfecting the prevention and control strategies of influenza.
Methods:
Data of influenza in Jiaxing City from 2019 to 2023 were collected from the Chinese Disease Prevention and Control Information System. Population data of the same period were collected from the Zhejiang Health Information Network Reporting System. The epidemiological characteristics of influenza were analyzed using descriptive analysis. Vector map information was collected from the Open Street Map, and the spatio-temporal clustering characteristics of influenza were analyzed using spatial autocorrelation and spatio-temporal scanning.
Results:
A total of 181 501 cases of influenza were reported in Jiaxing City from 2019 to 2023, with an average annual reported incidence of 653.93/105. The majority of cases were aged 5 to <15 years (59 785 cases, 32.94%). The majority of the occupations were students (78 239 cases, 43.11%) and pre-school children (33 715 cases, 18.58%). The county (city, district) with the highest reported incidence was Haining City (1 451.70/105), and the town (street) with the highest reported incidence was Chang'an Town (1 932.78/105). Spatial autocorrelation analysis showed that the incidence of influenza in Jiaxing City from 2019 to 2023 had positive spatial correlations (all Moran's I>0, all P<0.05), with a high-high clustering in the southern region. Spatio-temporal scanning analysis showed that there was a spatio-temporal clustering of influenza in Jiaxing City from 2019 to 2023, with the southern region being the primary-type clustering area and the period between November and January of the following year being the clustering time.
Conclusion
There was a significant spatio-temporal clustering of influenza in Jiaxing City from 2019 to 2023, with winter being the peak season and the southern region being the primary area.
6.The prognostic value of serum homocysteine, neutrophil to lymphocyte ratio and total imaging load score on cognition disorders in small cerebral vascular disease patients

Objective:To investigate the predictive value of serum homocysteine (Hcy), neutrophil to lymphocyte ratio (NLR) and total imaging load score on cognition disorders in patients with small cerebral vascular disease (CSVD).Methods:A total of 106 CSVD patients treated in Shanxi Hongdong County People's Hospital from June 2021 to June 2024 were retrospectively selected as the study objects. The Montreal Cognitive Assessment Scale (MoCA) was used to assess cognitive function. Among them, 43 patients with cognition disorders and 63 patients without cognition disorders were evaluated. Another 30 healthy subjects over 60 years old were selected as control group. General data and serum Hcy, NLR and other indicators of the three groups were compared, MRI examination was performed, and the total imaging load score was calculated. Logistic regression was used to analyze the risk factors of cognition disorders in CSVD patients, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of Hcy, NLR and radiographic total load score for cognition disorders in CSVD patients.Results:The levels of Hcy, NLR and total imaging load scores in the CSVD group were higher than those in the CSVD group and the control group : (20.32 ± 3.24) μmol/L vs. (17.75 ± 2.81), (14.96 ± 3.84) μmol/L; 2.77 ± 0.75 vs. 2.27 ± 0.68, 1.93 ± 0.47; (1.28 ± 0.73) scores vs. (0.92 ± 0.55), (0.36 ± 0.21) scores, there were statistical differences ( P<0.05). The results of Logistic regression showed that Hcy, NLR and total load score were risk factors for cognition disorders ( P<0.05). The results of ROC curve showed that the area under the curve (AUC) of Hcy, NLR, total imaging load score combined prediction of cognition disorders was 0.798. Conclusions:Hcy, NLR and total imaging load score are risk factors for cognition disorders in CSVD, and have certain predictive value for cognition disorders.
7.The application prospects of mesenchymal stem cell-derived exosomes in the treatment of temporoman-dibular joint osteoarthritis

Temporomandibular joint osteoarthritis(TMJOA)is a chronic degenerative disorder.Although current clinical interven-tions can effectively mitigate disease progression,they remain limited in fully reversing formed pathological structural alterations.Mesenchymal stem cell-derived exosomes(MSC-Exos),which mimic the primary therapeutic functions of mesenchymal stem cells,exhibit advantages such as excellent biocompatibility,low immunogenicity,and absence of cytotoxicity.In TMJOA,MSC-Exos exert therapeutic effects by modulating chondrocytes,immune cells,and the cartilage matrix.However,the clinical application of MSC-Exos still faces certain limitations.Enhancing their therapeutic efficacy has become a central research focus in this field.This review summarizes existing therapeutic strategies for TMJOA and explores the potential applications of MSC-Exos in TMJOA management,aiming to provide insights for future research.
8.Spatial distribution of iodine content in residential drinking water in Shaanxi Province in 2022

Objective:To study the spatial distribution and characteristics of iodine content in residential drinking water in Shaanxi Province, and provide a basis for scientific prevention and control of iodine deficiency disorders.Methods:From March to October 2022, an investigation was conducted in all counties (districts, referred to as counties) of Shaanxi Province, with administrative villages as survey units. For centralized water supply systems, two terminal water samples were collected, and iodine content was measured separately, with the average value representing the drinking water iodine content at that location. For decentralized water supply systems, a 10% sampling method was used to divide each administrative village into five areas (east, south, west, north, and center), with 10% of water wells sampled from each area (if fewer than 10 water wells, the well with the largest drinking population was selected; if fewer than 5 wells, all wells were sampled). One water sample was collected from each water well, and iodine content was determined using the arsenic-cerium catalytic spectrophotometry method. Spatial autocorrelation and spatial scan analysis were used to analyze the spatial distribution and characteristics of drinking water iodine content.Results:A total of 53 990 drinking water samples were collected, with a median water iodine content of 6.66 μg/L, ranging from 0.10 to 779.40 μg/L. Drinking water iodine content was detected in 112 counties, showing a significant spatial positive correlation (global autocorrelation, Moran's I = 0.74, Z = 43.83, P < 0.001). Local autocorrelation analysis showed that the difference in the distribution of iodine content in drinking water among 36 counties was statistically significant ( P < 0.05), with 22 counties showing low-low clustering and 12 counties showing high-high clustering. Spatial scanning identified three water iodine clustering areas, including two high-water iodine cluster areas and one low-water iodine cluster area. Conclusions:The distribution of iodine content in residential drinking water in Shaanxi Province shows significant spatial clustering and heterogeneity, requiring targeted interventions to achieve precise prevention and control of iodine deficiency disorders.
9.Surveillance results of iodine content in drinking water in Shaanxi Province

Objective To investigate iodine content in drinking water and clarify the distribution characteristics of iodine in water in Shaanxi Province.Methods A cross-sectional survey method was used to collect water samples from administrative villages in the province.Two tap water samples were taken from the centralized water supply villages,and 10%samples were taken from the decentralized water supply villages.Water iodine was detected by arsenic cerium catalytic spectrophotometry.Water iodine areas were divided according to national standards:<40 μg/L was iodine deficiency area,40-100 μg/L was iodine adequate area,>100 μg/L was high iodine area.According to the ecological regionification scheme of the Chinese Academy of Sciences,Shaanxi Province was divided into three types:the central and eastern Inner Mongolia Platea-Loess Plateau ecological region,the Fenwei Basin ecological region,and the Qinba Mountains ecological region.SPSS 25.0 was used for statistical analysis.Results The median of water iodine in Shaanxi Province was 6.66 μg/L.The survey of water iodine content was conducted in 22 848 administrative villages,1 309 townships,112 counties and 14 municipalities in the whole province.The median water iodine was less than 40 μg/L in 91.75%(20 963/22 848)of the administrative villages,between 40 and 100 μg/L in 7.40%(1 691/22 848)of the administrative villages,and more than 100 μg/L in 0.85%(194/22 848)of the administrative villages.The median of water iodine in the central and eastern Inner Mongolia Platea-Lose Plateau ecological zone,the Fenwei Basin ecological zone,and Qinba Mountains ecological zone was 12.35,8.88,and 2.00 μg/L,respectively,and the differences among different ecological zones were statistically significant(H=6 616.23,P<0.001).The median of water iodine of centralized and decentralized water supply was 6.72 and 6.21 μg/L,respectively,and differences between different water supply methods were statistically significant(Z=5.638,P<0.01).Conclusion The overall external environment of Shaanxi Province is iodine deficient,and most of the administrative villages are iodine deficient areas.There are a certain proportion of high iodine water source areas and suitable iodine areas.
10.Compound sabal berry tablets for the treatment of overactive bladder symptoms after laser enucleation of the prostate in patients with benign prostatic hyperplasia

Objective:To investigate the clinical efficacy of compound sabal berry tablets on overactive bladder symptoms in patients with benign prostatic hyperplasia after transurethral laser enucleation of the prostate.Methods:This study was a prospective study. Eighty patients with benign prostatic hyperplasia who underwent laser enucleation at Tongliao People's Hospital from January 2024 to December 2024 were included in this study. The patients were randomly divided into a study group and a control group using the random number table method, with 40 patients per group. The control group received 0.2 mg of tolterodine tartrate tablets twice a day after surgery. The study group was given compound sabal berry tablets (0.5 g orally three times a day) in addition to the treatment provided to the control group. Both groups of patients were treated for 4 weeks after surgery. The clinical efficacy of the two groups was compared, including the International Prostate Symptom Score (IPSS), Overactive Bladder Symptom Score (OABSS), Maximum Postoperative Urinary Flow Rate (Qmax), Post-Void Residual (PVR), and the incidence of postoperative bladder irritative symptoms.Results:The differences in the preoperative indicators, including IPSS, OABSS, Qmax, and PVR, between the two groups were not statistically significant (all P > 0.05). Preoperatively, in the control group, Qmax was (8.64 ± 2.83) mL/s, IPSS was (25.10 ± 4.37), OABSS was (10.52 ± 1.87), and PVR was (80.70 ± 6.34) mL; in the study group, the respective values were (9.12 ± 2.95) mL/s, (24.60 ± 4.53), (10.83 ± 1.73), and (80.10 ± 5.61) mL. Postoperatively, in the control group, Qmax was (20.30 ± 3.65) mL/s, IPSS was (8.50 ± 1.58), OABSS was (4.09 ± 0.52), and PVR was (9.70 ± 2.48) mL, while in the study group, the respective values were (21.40 ± 4.38) mL/s, (7.40 ± 1.76), (1.71 ± 0.36), and (9.00 ± 1.75) mL. Postoperatively, both groups showed a significant increase in Qmax, while IPSS, OABSS, and PVR all significantly decreased (all P < 0.05). Postoperatively, the IPSS and OABSS in the study group were significantly lower than those in the control group ( t = -3.28, -25.89, both P < 0.05). However, there were no statistically significant differences in Qmax and PVR between the two groups (both P > 0.05). The incidence of bladder irritative symptoms in the study group [12.50% (5/40)] was significantly lower than that in the control group [35.00% (14/40), χ2 = 8.64, P < 0.05]. Conclusions:Compound sabal berry tablets can reduce postoperative prostate symptoms and overactive bladder symptoms in patients undergoing transurethral laser enucleation of the prostate for benign prostatic hyperplasia, demonstrating a certain clinical efficacy.
