Objective:To compare the efficacy and safety of ticagrelor tablets produced by Zhejiang Hisun Pharmaceutical Co., Ltd. (the generic drug) and ticagrelor tablets produced by AstraZeneca Pharmaceutical Co., Ltd. (the original drug) in antiplatelet therapy.Methods:The study design was a retrospective cohort study. The subjects were patients who underwent percutaneous coronary intervention (PCI) for acute coronary syndrome (ACS) and postoperative antiplatelet therapy with ticagrelor tablets at First Affiliated Hospital of Dalian Medical University during January 2020 to July 2021. Through the hospital electronic medical record system, relevant clinical data of patients (age, gender, comorbidities, blood lipid level on admission, PCI indications, antiplatelet treatment regimen, efficacy and safety assessment endpoint events within 12 months of treatment, etc.) were collected. The patients were divided into the generic drug group and the original drug group. To exclude confounders, propensity score matching (PSM) method was used. The efficacy evaluation index was the incidence of the primary endpoint events (cardiogenic death, stroke, target revascularization, recurrent infarction) and secondary endpoint events (all-cause mortality, peripheral artery occlusion, stent thrombosis, angina attacks) within 12 months of treatment. The safety evaluation index was the incidence of bleeding event within 12 months of treatment.Results:A total of 1 486 patients were included in this study, including 734 in the generic drug group and 752 in the original drug group. The proportion of women and unstable angina, and the level of high-density lipoprotein cholesterol were higher than those in the original drug group (all P<0.05). The proportion of patients with hyperlipidemia and ST-segment elevation myocardial infarction were lower than those in the original drug group (both P<0.05). After PSM, 690 patients were enrolled in the generic drug group and 690 patients in the original drug group (all P>0.05). No differences in the comparison of clinical features between the 2 groups was significant (all P>0.05). No differences in the incidences of primary endpoints, secondary endpoints, and bleeding events between the 2 groups was significant before and after PSM [before PSM: 12.1%(89/734) vs. 10.9%(82/752), 10.8%(79/734) vs. 8.4%(63/752), 0.3%(2/734) vs. 0.5%(4/752); after PSM: 12.6%(87/690) vs. 12.3%(85/690), 11.0%(76/690) vs. 8.3%(57/690), 0.3%(2/690) vs. 0.4%(3/690); all P>0.05]. No death occurred in patients of both groups. Bleeding is predominantly characterized by epistaxis and subcutaneous petechiae, which did not lead to interruption of antiplatelet therapy. Conclusion:The efficacy and safety of ticagrelor tablets produced by Zhejiang Hisun Pharmaceutical Co., Ltd. for antiplatelet therapy in ACS patients after PCI surgery were basically the same as those of the original drug.

The development of efficient molecular imaging probes with high specificity, high sensitivity, and low toxicity is the core in promoting the development of cancer molecular imaging technology. Natural expressed purified protein-based nanocage with self-assembling ability to form intrinsically cavity structure has been widely used to prepare various molecular imaging probes of cancer for its ideal physical properties, excellent biological compatibility, exceptional biodegradability as well as diverse engineering strategies. This paper focuses on the research progress of natural protein-based nanocage from the following aspects: various types of protein-based nanocages of different origins and compositions, cargos loading mechanisms, functionalization strategies, and their recent advances in cancer molecular imaging.

Objective To analyze the 10-year survival outcome and failure patterns for patients with nasopharyngeal carcinoma (NPC) after intensity-modulated radiotherapy (IMRT),aiming to provide reference for optimized treatment for NPC.Methods Clinical data of 866 patients with NPC receiving IMRT from January 2001 to December 2008 were retrospectively analyzed.Survival analysis was performed using the Kaplan-Meier estimator.Univariate analysis was carried out by log-rank test and multivariate analysis was performed using Cox proportional hazards model.Results The median follow-up time was 132 months.The 10-year local recurrence-free survival (LRFS),distant metastasis-free survival (DMFS),progression-free survival (PFS) and disease specific survival (DSS) were 92.0％,83.4％,75.7％ and 78.6％,respectively.A total of 210 patients died including 124 patients (59.0％) from distant metastasis,which was the primary cause of death,and 47 (22.3％) from local regional recurrence.Independent negative factors of DSS included age＞50 years (P=0.00),LDH ≥ 245 IU/L (P=0.00),Hb＜ 120 g/L (P=0.01),T2-T4 staging (P=0.00),N1-N3 staging (P=0.00) and GTV-nx＞20 cm3(P=0.00).The 10-year LRFS,DMFS and DSS of stage Ⅱ NPC patients did not significantly differ after IMRT alone and chemoradiotherapy (P=0.83,0.22,0.23).For patients with stage Ⅲ NPC,the 10-year LRFS and DSS in the chemoradiotherapy arm were significantly higher than those in the IMRT alone (P=0.01,0.01),whereas no statistical significance was observed in the DMFS between two groups (P=0.14).The overall survival of stage Ⅳa+Ⅳb NPC patients is relatively poor.Conclusions IMRT can improve the long-term survival of NPC patients.Distant metastasis is the primary failure pattern.Patients with stage Ⅰ-Ⅱ NPC can obtain satisfactory survival outcomes after IMRT alone.The addition of chemotherapy can further enhance the LRFS and DSS of stage Ⅲ NPC patients.However,the optimal therapeutic strategy remains to be urgently investigated for stage a+ Ⅳb NPC patients.

Objective:To evaluate the dental biofilm penetration efficiency of a novel stabilized stannous-containing sodium fluoride dentifric(EXP) and its lipopolysaccharide (LPS) neutralization efficiency.Methods:A controlled,randomized,examiner-blind in situ clinical trial was conducted with the treatment of PBS(control),EXP and a marketed stannous-containing sodium fluoride dentifrice (MKD).Fluorescent dye,fluorescent probe and fluorescence colocalization were used for sample examination and analysis.Results:EXP offered better stannous penetration into the biofilm than MKD and PBS(P ＜0.05),as well as greater LPS neutralization efficiency(P ＜0.05).There was a 96.52％ overlap of stannous ions and bounded LPS at the same sites treated by EXP.Conclusion:EXP is more effective than MKD in the delivery of stannous into the biofilm and in the neutralization of LPS.

Objective: To establish a method to detect the concentrations of dimethyl sulfate in the air of workplace by GC-MS. Methods: DMS in the air of workplace adsorpted by Silicone tube, then desorped by acetone, add 1.0 ml of acetone, shake 1 min, placed after 30 min, the desorption solution qualitative and quantitative determination by gas chromatography-mass. Results: The calibration curves were liner in the range of 0.1-200.0 μg/ml. The within-run and between-run precisions were 2.6%-4.7% and 4.0%-9.0% respectively. The method detection limit is 0.1 μg/ml, the minimum detectable concentration is 0.02 mg/m³ (in terms of sampling 4.5 L) . Add 1 ml of desorption liquid, place 30 min, the average desorption efficiency of more than 90%. Conclusion This method has simple pretreatment, short analysis period, and optimized linear rage and limit of detection, and is suitable for the determination of DMS in workplace air.

Objective To compare the clinical effect of the two surgical methods of posterior pedicle screw fixation decompression in treatment of thoracolumbar burst fractures,and in order to provide the basis for choosing the rational treatment.Methods Retrospectively analyzed the clinical data of 98 patients with thoracolumbar burst fractures underwent surgery of posterior pedicle screw fixation decompression from January 2007 to January 2011.Fifty-six patients were given posterior fixed indirect decompression surgery (indirect decompression group) and 42 patients were given posterior fixed direct decompression (direct decompression group).The patients were followed up for (1.03 ± 0.36) years after surgery,the image,recovery of neurological function,postoperative complications and capacity for independent living of the two groups were compared and evaluated.Results After surgery,the vertebral height ratio,Cobb angle,canal compromise rate in indirect decompression group were (91.67 ± 26.19)％,(10.10 ± 2.89)°,(18.61 ±5.32)％,in direct decompression group were(86.23 ± 24.64)％,(11.98 ± 3.42)°,(22.37 ± 6.39)％.There was significant difference compared with before surgery (P ＜ 0.05) and no significant difference between two groups (P ＞0.05).After surgery,the neurological function of the two groups were improved,and the improvement in indirect decompression group was better than that in direct decompression group (P ＜ 0.05).The postoperative complications ratio in indirect decompression group was 23.2％ (13/56),significantly lower than that in direct decompression group[83.3％(35/42)](x2 =10.370,P＜ 0.01).There was 60.7％(34/56)patients with capacity for independent living in indirect decompression group,significantly higher than that in direct decompression group [40.5％ (17/42)] (x2 =4.329,P ＜ 0.05).Conclusion The posterior pedicle screw fixation indirect decompression in treatment of thoracolumbar burst fractures is a feasible operation method,and is worth to utilize in clinic.

BACKGROUND: Body mass index(BMI)is a commonly used phenotype for obesity,which is determined by multiple genetic factors.OBJECTIVE: To investigate whether osteocalcin(also known as bone Gla protein,BGP)Hind Ⅲ genetic polymorphism is associated with BMI variation.METHODS: A total of 390 premenopausal women from a local population of Nanchang City were selected.Body weight and height were measured.All participants were genotyped at the BGP Hind Ⅲ locus using polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).RESULTS AND CONCLUSION: The BGP genotype frequencies of HH,Hh and hh were 0.077,0.408 and 0.515,respectively.The distribution of BGP Hind Ⅲ genotypes was consistent with Hardy-Weinberg equilibrium(P > 0.05).The BGP Hind Ⅲ were significantly associated with BMI(P=0.002),which could explain about 5.47% of BMI variation.On average,BMI of individuals with HH genotype was the highest[(22.81±0.73)kg/m2],individuals with Hh genotype was intermediate[(21.50±0.53)kg/m2],while individuals with hh genotype was the lowest[(20.23±0.63)kg/m2].Therefore,carriers of HH and Hh genotypes had,respectively,approximately 12.75% and 6.28% higher BMI than carriers of the hh genotype.To our best knowledge,this is the first study reporting the association of BGP Hind Ⅲ genetic polymorphism and BMI in healthy premenopausal women.

Objective Assess the value of pleura sliding sign with chest ultrasonography in the prediction of pleura adhesion prior to video-assisted thoracoscopic surgery(VATS).Method 63 patients were evaluated for pleura sliding signs with chest ultrasonography at 9 points along the chest wall prior to thoracotomies and were compared with the findings of the same points during the operation.Methods Pleura sliding signs on 567 points were examined in 63 cases,and 106 points pleura adhesion were found by chest ultrasonography and 72 points were proved by operations.461 points were no pleura adhesion under chest unltrasonography and 495 points had no pleura adhesion confirmed by operations.Results The sensitivity,specificity,negative predictive value,positive predictive value and overall accuracy were 80.56%,90.03%,96.96%,54.72% and 9.07%,respectively.The Receiver Operating Characteristic(ROC)curve showed that there should be no pleura adhesion if there were more than 8 points positive pleura sliding signs.Conclusion Examination of pleura sliding sign by chest ultrasonography is helpful to predict the presence and location of pleura adhesion prior to VATS.

Objective To investigate the value of blood glucose concentration in estimating risk stratification and early prognosis in patients with massive cerebral infarction.Methods 136 patients were divided into three groups according to the level of the blood glucose concentrations:group A with irritable hyperglycemia,group B with diabetes' hyperglycemia and group C with normal blood glucose.The early prognosis and the degree of neurologic impairment were observed and compared with three groups.Results The degree of neurologic impairment in group A was significantly higher than that in group C.The patients in group A had higher adverse prognosis than those in group C (P＜0.05).No significance was occurred between group A and group B about the early prognosis and the degree of neurologic impairment(P＞0.05).Conclusion The blood glucose level for the patients suffered from acute massive cerebral infarction is helpful to the diagnosis of disease situation,evaluation for prognosis and instruction for treatment.
