1.Analysis of related indicators and interventions in apheresis platelet donors with low ferritin

Objective: To analyze characteristics of iron-deficient blood donors, implement targeted interventions, and evaluate their effectiveness, thereby providing a reference for formulating blood donor recruitment and care strategies. Methods: Based on serum ferritin (SF) test results, the apheresis platelet donors were divided into the low SF group (n=90; 45 males and 45 females) and the normal SF group (n=651; 510 males and 141 females). The results of related indicators of the two groups were compared and analyzed. Interventions for the low SF group included extending the blood donation interval to at least 45 days (group A) and oral iron supplementation combined with the extended donation interval implemented in group A (group B). Pre-intervention and post-intervention SF results were compared. Results: For both male and female donors, serum iron levels were significantly lower in the low SF group than those of the normal SF group, while the levels of transferrin, unsaturated iron binding capacity (UIBC) and total iron binding capacity (TIBC) were higher in the low SF group compared to the normal SF group. Some indicators related to red blood cells showed changes, with more evident alterations in females than in males. Twenty-eight donors in group A and 39 donors in group B completed the study after intervention. SF value in group A was (18.32±8.09) μg/L at baseline and (26.21±17.30) μg/L after intervention. Similarly, SF value in group B was (15.87±7.69) μg/L at baseline and (26.24±15.55) μg/L after intervention. In both groups, SF values after intervention were significantly higher than baseline values. However, the magnitude of change did not significantly differ between groups A and B. Conclusion: Other related indicators in blood donors with low ferritin have also experienced some changes, suggesting that some blood donors may have entered the stage of iron-deficient erythropoiesis. Extending blood donation interval facilitates the recovery of iron storage in low-ferritin apheresis platelet donors. Blood stations should develop care strategies for apheresis platelet donors, including, at a minimum, the prolonged blood donation interval for donors with low ferritin.
2.Empirical Study on the Impact of Infusion Media on the Flow Rate of Infusion Pumps

Objective To investigate the impact of different infusion media on the flow rate of infusion pumps.Methods Ten infusion pumps were randomly selected and tested for infusion rates using deionized water,saline solution,glucose solution,and parenteral nutrition solution.The infusion flow rate was set at 30 mL/h,and the testing methodology conformed to the standards for the calibration and quality control of syringe pumps and infusion pumps.Results For infusion set A,the infusion rate was unaffected by the infusion media,remaining within the acceptable quality control standards.For infusion set B,when infused with deionized water,saline solution,and glucose solution,the infusion errors were within the quality control standards.However,when using parenteral nutrition solution as the infusion medium,the infusion error exceeded the acceptable quality control standards.Conclusion Both the infusion set and the infusion medium can affect the flow rate of infusion pumps.It is crucial to calibrate the flow rate according to the specific infusion medium to reduce medical risks associated with infusion pumps during clinical use.
3.Study on the differences of clinical application in pediatric patients for different brands infrared ear thermometers

Objective:To explore the differences in the clinical application of different brands of infrared ear thermometers in pediatric patients,and to guide the rational use of infrared ear thermometers.Methods:A total of 164 inpatients and outpatients who received treatment in the Children's Hospital of Zhejiang University School of Medicine from May 2023 to July 2023 were randomly selected,and three different brands of ear thermometers were used to measure the ear temperature of the children in the same measurement method.The differences in temperature and time measured by different brands of ear thermometers in children of different ages were compared.Results:The temperature and time measurements of the three ear thermometers were compared,the difference was statistically significant(F=5.007,264.988,P<0.05).The temperature of children aged≤3 months was measured by the three ear thermometers,and the difference was statistically significant(F=3.868,P<0.05).there was no difference in children aged over 3 months.Conclusion:For children of different ages,medical institutions can make choices based on the comprehensive consideration of the use cost,the speed of measurement time and other factors.For children aged over 3 months,choose product B with lower cost and faster measurement time,for children aged 0-3 months,it is recommended to choose product A with more accurate temperature measurement values.
4.Research on Procurement Classifcation of Medical Equipment Based on Kraljic Model.

Based on the Kraljic model,this paper established the scoring standard system for medical equipment procurement,and determined the weight coefficients of each evaluation factor by the analytic hierarchy process (AHP),then the procurement classification model of medical equipment was successfully established.It is proved that this model can classify medical equipment effectively,which enables the purchasing personnel to adopt corresponding purchasing strategies according to different classification.This model can be extended to all medical equipment procurement,realizing the delicacy and individualized procurement management of the equipment,which has strong operability and guiding significance.

5.Hospital Networked Medical Equipment Safety Management.

By applying the ISO 80001 and the MDS(Manufacturer Disclosore Statement for Medical Device Security), the paper puts forward the measures and suggestions for the security management of networked medical equipment.

6.Research on the Clinical Alarm Management Mechanism Based on Closed-loop Control Theory.

This paper proposes a clinical alarm management system based on the theory of the closed loop control. The alarm management mechanism can be divided into the expected standard, improving execution rule, rule execution, medical devices with alarm functions, results analysis strategy and the output link. And, we make relevant application and discussion. Results showed that the mechanism can be operable and effective.

7.Long-term survival outcome and failure pattern after intensity-modulated radiotherapy for nasopharyngeal carcinoma

Objective To analyze the 10-year survival outcome and failure patterns for patients with nasopharyngeal carcinoma (NPC) after intensity-modulated radiotherapy (IMRT),aiming to provide reference for optimized treatment for NPC.Methods Clinical data of 866 patients with NPC receiving IMRT from January 2001 to December 2008 were retrospectively analyzed.Survival analysis was performed using the Kaplan-Meier estimator.Univariate analysis was carried out by log-rank test and multivariate analysis was performed using Cox proportional hazards model.Results The median follow-up time was 132 months.The 10-year local recurrence-free survival (LRFS),distant metastasis-free survival (DMFS),progression-free survival (PFS) and disease specific survival (DSS) were 92.0%,83.4%,75.7% and 78.6%,respectively.A total of 210 patients died including 124 patients (59.0%) from distant metastasis,which was the primary cause of death,and 47 (22.3%) from local regional recurrence.Independent negative factors of DSS included age>50 years (P=0.00),LDH ≥ 245 IU/L (P=0.00),Hb< 120 g/L (P=0.01),T2-T4 staging (P=0.00),N1-N3 staging (P=0.00) and GTV-nx>20 cm3(P=0.00).The 10-year LRFS,DMFS and DSS of stage Ⅱ NPC patients did not significantly differ after IMRT alone and chemoradiotherapy (P=0.83,0.22,0.23).For patients with stage Ⅲ NPC,the 10-year LRFS and DSS in the chemoradiotherapy arm were significantly higher than those in the IMRT alone (P=0.01,0.01),whereas no statistical significance was observed in the DMFS between two groups (P=0.14).The overall survival of stage Ⅳa+Ⅳb NPC patients is relatively poor.Conclusions IMRT can improve the long-term survival of NPC patients.Distant metastasis is the primary failure pattern.Patients with stage Ⅰ-Ⅱ NPC can obtain satisfactory survival outcomes after IMRT alone.The addition of chemotherapy can further enhance the LRFS and DSS of stage Ⅲ NPC patients.However,the optimal therapeutic strategy remains to be urgently investigated for stage a+ Ⅳb NPC patients.
8.Survey and Discussion on VentiIator AIarm Status in ICU

Objective The article aims at survey and analysis on ventilator alarm status in pediatric hospital. Methods First,we design the statistical table of ventilator, and analyze the reliability of statistical table. Then we evaluate the effectiveness of the alarm. Finally, we evaluate the alarm information synthetically through investigation and statistics.Results The ventilator alarm is ineffective, a large number of nonsensical alarms, leading to lower alert sensitivity of medical staff. Conclusion According to the survey results, we should analyze the cause of the problem, and propose the corresponding alarm management method.

As the dressings currently used in clinic settings unflat shape in general, they can't be fitted completely on occiput, heel, elbow, knee and other body parts unflat. This paper developed one kind of foam dressing of special shape based on local anatomy. The foam dressing is waterproof and air permeable, it can cover the wound closely enough to prevent bacteria from invasion and infection. With a saturated absorption ratio of 1: 8 or higher, it can keep the wound clean and moisture by absorbing large amounts of wound inflammatory secretions and is almost completely permeable to oxygen and carbon dioxide. Assuring safety and effect meanwhile, it has better outcomes than common dressings in the same application settings.
