1.A Case of Corneal Perforation Following Intravitreal Faricimab Injection: Case Report

Purpose:
We report a case of corneal perforation during intravitreal Faricimab injection for neovascular age-related macular degeneration (nAMD) in a patient with a history of herpetic keratitis.Case summary: A 78-year-old male with a history of bilateral herpetic keratitis presented with corneal opacity in the right eye and a history of penetrating keratoplasty in the left eye. During follow-up, he reported sudden visual deterioration in the right eye. Although the corneal status remained stable, optical coherence tomography and indocyanine green angiography revealed nAMD in the right eye.The patient was treated with intravitreal Faricimab injections. Approximately one month after the second injection, the patient presented again with sudden visual loss in the right eye. Slit-lamp examination demonstrated a geographic corneal ulcer, corneal perforation, and anterior chamber collapse, necessitating permanent amniotic membrane transplantation.
Conclusions
This case suggests that intravitreal Faricimab triggered herpes virus reactivation in a patient with a history of herpetic keratitis, resulting in corneal perforation. Careful monitoring is essential when administering anti-VEGF therapy in patients with prior viral keratitis, particularly those with pre-existing corneal thinning. As no previous cases of herpes reactivation and corneal perforation related to Faricimab have been reported, this highlights the need for further investigation and preventive strategies in high-risk patients.
2.Short-term Efficacy and Safety of Faricimab Switching in Neovascular Age-related Macular Degeneration Previously Treated with Anti-vascular Endothelial Growth Factor

Purpose:
To evaluate the short-term efficacy and safety of intravitreal faricimab injection (IFI) in patients with neovascular age-related macular degeneration (nAMD) previously treated with another anti-vascular endothelial growth factor (anti-VEGF).
Methods:
We retrospectively reviewed the medical records of patients with nAMD refractory to previous anti-VEGF treatment who were switched to faricimab from June 2023 to July 2024. A good responder was defined as a case with complete absence of subretinal fluid (SRF) or intraretinal fluid (IRF) 8 weeks after IFI, whereas a poor responder was defined as having residual SRF or IRF 8 weeks after IFI. We evaluated best-corrected visual acuity (BCVA), central subfield retinal thickness (CST), sub-foveal choroidal thickness (SFCT), maximum height of pigment epithelial detachment (PED) and the presence of SRF or IRF, and the presence of optical coherence tomography parameters of nAMD at baseline (before the injection of faricimab) and 8 weeks and 16 weeks after faricimab switching. All adverse events following faricimab injection were analyzed.
Results:
Seventy-two eyes of 69 patients were included. There was no significant BCVA change after IFI. CST, SFCT and maximum PED height were significantly decreased 8 weeks and 16 weeks after faricimab switching compared to baseline (all p < 0.05). A fluid-free state was reached in 39% at 16 weeks. The poor responder group had a higher number of brolucizumab injections before switching compared to the good responder group (1.1 ± 1.9 vs. 2.4 ± 3.3, p = 0.04). The good responder group had more cases without SRF before switching and lower baseline CST than the poor responder group (all p < 0.05). There were no severe adverse events after IFI.
Conclusions
Faricimab was safe and anatomically effective, though it did not produce functional improvements. IFI could serve as another option in the treatment of patients with nAMD treated with previous anti-VEGF.

4.Transurethral Resection of Prostate under Local Anesthesia in Patients with Benign Prostatic Hyperplasia.

This study was performed for the patients with benign prostatic hyperplasia who had underlying diseases which increase the risk of spinal or general anesthesia. Standard transurethral resections of prostate were done on patients with prostatic hyperplasia under local anesthesia. The selection criteria were urinary retention or below 10 ml/sec of maximum flow rate, coexist with medical problems which increase the risk of spinal and general anesthesia. Local infiltrations of 1% lidocaine were done at penoscrotal junction on each side of the corpus spongiosum, the lateral portion of the prostate and the bladder neck. This anesthesia was supplemented usually by modest dose of intravenous tranquilizers and analgesics under the continuous monitoring by an anesthesiologist. The Visual Pain Analogue Scale(VAS) was used for the evaluation of intraoperative and postoperative pain. The mean operating time, amount of the resected tissues and intraoperative and postoperative visual analogue scale were 50.2 min, 16 grams. 2.7 and 2.0. We thought that this type of anesthesia was a safe, simple and effective procedure.

5.A Case of Incidental Testicular Teratoma Found during Hydrocelectomy.

Testicular teratoma contains more than one germ cell layer in various stages of maturation and differentiation. The teratoma occurs at all ages but is most common in the 25-30 year age group. It is relatively infrequently seen in childhood and infancy. In these age group, teratoma is a benign tumor. Recently, we experienced one case of incidental testicular teratoma during hydrocelectomy in 22 month-old child, The hydrocele is frequently associated with teratoma under age 4 years and is the most common misdiagnosis. So we report this one case of testicular teratoma with review of related literatures.

7.A Case of 47XYY Syndrome with Azoospermia.

The XYY syndrome is a rare entity, which is a phenotypic man with a 47 XYY It was first reported by Sandberg and associates in 1961. The XYY individual is seldom detected during childhood or even in adult because the features of XYY syndrome are often subtle and not overtly suggestive of a chromosomal abnormalities. We have reported a case of XYY syndrome associated with absence of pubic and axillary hair, no voice mutation and azoospermia. Clinical, endocrinologic and genetic studies were presented and theories regarding the etiology of the XYY syndrome were discussed with review of the literature.
