1.Research progress on the comorbidity mechanism of dry eye and depression

Dry eye disease(DED)and depression(DEP), though anatomically and clinically distinct, show significant epidemiological, pathophysiological, and prognostic interplay. Their co-occurrence has risen sharply in recent years, yet the mechanisms driving this comorbidity remain under-investigated. This review systematically synthesizes current evidence, highlighting that pro-inflammatory cytokines, neuro-regulatory imbalance, mitochondrial dysfunction, and gut-microbiota dysbiosis constitute a shared molecular network, while sleep deprivation and antidepressant use further amplify the vicious cycle. By identifying limitations in existing studies, this review proposes future research directions to offer new theoretical and clinical insights for managing DED-DEP comorbidity.
2.Reconsideraton of Stroke with Syndrome of Combined Blood Stasis and Toxin from the Perspective of Xiang Thinking

Xiang thinking is a cognitive approach that reflects the relationships between phenomena and their underlying principles by analyzing their external manifestations through methods such as analogy, reasoning, deduction, and symbolism. This article applied xiang thinking to analyze the etiology and pathogenesis of "wind, fire, phlegm, and blood stasis" in stroke, thereby exploring its impact on the principles of syndrome differentiation and treatment of this condition. Meanwhile, the article traced the construction process of xiang thinking, and interpreted the concept of "toxin pathogen" in traditional Chinese medicine from four perspectives, state, attribute, origin, and law. Furthermore, the relationship between the process of constructing xiang thinking and the origin of etiology, identification methods, pathogenesis evolution, and treatment strategies for stroke with syndrome of combined blood stasis and toxin was explored, so as to provide insights into research on the etiology and pathogenesis of stroke, as well as clinical diagnosis and treatment approaches.
3.Exploring the Pathogenesis and Treatment of Multiple Sclerosis Based on the Theory of Collateral Diseases

[Objective]To explore the etiology,pathogenesis and treatment principles of multiple sclerosis(MS)based on the theory of collateral diseases in traditional Chinese medicine(TCM).[Methods]By analyzing ancient and modern literature on TCM concepts such as"collateral diseases"and"atrophy syndromes",and integrating modern medical research on the biological mechanisms of MS treatment,this study summarizes the approach to diagnose and treat MS based on the theory of collateral diseases,supported by a case example.[Results]The collaterals are the units for the distribution of blood and Qi,which,through the coordination of meridian Qi flow and the balance of nutrient and defensive Qi,plays a role in distributing blood,Qi and body fluids,and nourishing the meridians and viscera.The physiological characteristics of the collaterals determine that they become sites for the invasion,concealment and development of pathogenic factors.Analysis based on the theory of collateral diseases suggests that damp-heat stagnation,essence deficiency and blood stasis in the collaterals are the underlying mechanisms for the complex clinical manifestations of MS.Treatment principles proposed include strengthening the spleen and promoting diuresis to clear dampness,harmonizing the liver and kidneys to nourish the collaterals,and promoting the circulation of Qi and blood to unblock the collaterals.The attached test case belonged to the impotence of liver and kidney deficiency,treated by supplementing the liver and kidney,warm and smooth meridians,and achieved better curative effect.[Conclusion]The theory of collateral diseases,as an important part of the meridian and collateral system in TCM,holds broad prospects in the treatment of neurological diseases.It can provide methods and ideas for the theoretical research and clinical treatment of MS with TCM.
4.Severity of SARS-CoV-2 infection in children with kidney disease undergoing immunosuppressive therapy

Objective:To investigate the impact of immunosuppressive therapy on the severity of SARS-CoV-2 infection and cytokine levels in pediatric patients with kidney diseases.Methods:A retrospective analysis was conducted on the clinical data of 40 hospitalized pediatric patients who were diagnosed with SARS-CoV-2 infection at the 900th Hospital of PLA Joint Logistic Support Force from December 2022 to February 2023. Based on their immunosuppressive status prior to SARS-CoV-2 infection, these patients were categorized into immunosuppressive group and non-immunosuppressive group. Independent sample t-tests, Mann-Whitney U tests, and χ2 test were employed to compare the clinical baseline characteristics and laboratory data, the severity of SARS-CoV-2 infection, and the levels of cytokines between the 2 groups. Results:Among the 40 patients, 11 were in the immunosuppressive group (aged 13 (8, 14) years, 9 males and 2 females) and 29 in the non-immunosuppressive group (aged 2 (1, 4) years, 15 males and 14 females). In the immunosuppressive group, 2 were asymptomatic cases, 8 were mild cases, and 1 was moderate case, and there was no severe or critical cases. In the non-immunosuppressive group, 8 were mild cases, 5 were moderate, 15 were severe cases, 1 was critical case, and no asymptomatic cases. The underlying diseases in the immunosuppressive group included nephrotic syndrome (6 cases), IgA vasculitis nephritis (2 cases), lupus nephritis (1 case), post-renal transplantation (1 case), and renal failure (1 case), with a mean total immunosuppression score (TIS) of (3.6±1.4) points. In the non-immunosuppressive group, 2 patients had a history of epilepsy, and the remaining 27 cases had no underlying conditions, all with TIS scores of 0. Compared to the children in the non-immunosuppressive group, those in the immunosuppressive group were more likely to exhibit asymptomatic or mild infection, with lower risks of severe disease, cytokine storm, fever, and cough, but a higher risk of fatigue ( OR=1.22, 2.66, 0.48, 0.12, 0.12, 0.13, 1.22; 95% CI 0.93-1.62, 0.99-7.15, 0.33-0.70, 0.03-0.57, 0.03-0.57, 0.03-0.65, 0.93-1.62; all P<0.05). The levels of cytokine IL-6, interferon-α and interferon-γ in the immunosuppressive group were all lower than those in the non-immunosuppressive group ( Z=2.23, 2.51, 2.92, respectively; all P<0.05). Conclusion:Pediatric patients with kidney diseases receiving appropriate immunosuppressive therapy may mitigate the severity of SARS-CoV-2 infection by suppressing the expression of cytokines.

Fasting plasma glucose(FPG)has been widely used in the diagnosis of diabetes mellitus(DM)for its advantages of rapidity,economy,simplicity and acceptability.Hemoglobin A1c(HbA1c)has the advantages of being simple,stable,accurate and unaffected by related factors,and has been used in the diagnosis of DM since it was standardized.However,the value of FPG and HbA1c in the early diagnosis of diabetes have been controversial.The purpose of this paper is to explore the value of the two in the early diagnosis of diabetes.
6.Research progress in the mechanism of Chinese materia medica in the treatment of viral pneumonia based on signaling pathway

The research on the mechanism of Chinese materia medica in the treatment of viral pneumonia (VP) is mainly based on the monomer components of Chinese materia medica and TCM compounds. Among them, the monomer components are mainly polyphenols, flavonoids and anthraquinones, which have anti-inflammatory, antiviral, immunomodulatory and antioxidant pharmacological effects. The efficacy of TCM compounds is mainly based on clearing heat, and it has the functions of removing phlegm, removing blood stasis, removing dampness, moistening lung and so on. The intervention of Chinese materia medica in VP mainly involves NF-κB, MAPK, JAK/STAT, PI3K/Akt and other signaling pathways. The mechanism includes regulating oxidative stress, apoptosis, regulating immune function, inhibiting inflammatory response, etc., which can reduce the pathological damage of inflammatory cell infiltration and edema in lung tissue, and achieve the protective effect on lung tissue. The current research models exhibit unclear patterns of syndrome differentiation, and the mechanisms of Chinese materia medica involving multiple targets and pathways are poorly understood. Future research should integrate disease-syndrome combination models to further explore the mechanisms by which TCM regulates multiple targets and pathways, thereby providing insights and methodologies for the treatment of viral pneumonia with Chinese materia medica.
7.Experience in Treating Ischemic Stroke with Triple-Medicinal Combinations

This paper summarized the clinical experience in treating ischemic stroke with jiao (角) medicine (triple-medicinal combinations). Clinically, the combination of Roucongrong (Cistanche deserticola)-Shanyurou (Cornus officinalis)-Guijia (Testudinis Carapax et Plastrum) is used to nourish the kidneys and liver for disease mechanism of liver-kidney depletion, and foundation deficiency due to insufficient essence and blood; the combination of Xixiancao (Sigesbeckia orientalis)-Tianma (Gastrodia elata)-Gouteng (Uncaria rhynchophylla) is used to extinguish wind and eliminate dampness for treating numbness and swelling of limbs caused by ischemic stroke; the combination of Shichangpu (Acorus Tatarinowii)-Yuanzhi (Polygala tenuifolia)-Yujin (Curcumae Radix) is used to improve intelligence, refresh the brain, and clear the mind for treating cognitive impairment, memory loss, or speech difficulties caused by ischemic stroke; the combination of Banxia (Pinellia ternata)-Baizhu (Atractylodes macrocephala)- Tianma (Gastrodiae rhizoma) is used to dissolve phlegm and extinguish wind, unblock meridians and relieve dizziness for treating dizziness or headache caused by ischemic stroke; the combination of Danggui (Angelica sinensis radix)- Chuanxiong (Ligusticum chuanxiong)-Guijianyu (Euonymus alatus) is used to nourish and activate blood circulation, remove blood stasis and unblock meridians for treating weak limbs and activiry difficulty caused by ischemic stroke; the combination of Chaihu (Bupleurum chinense)-Zhiziz (Gardenia jasminoides)-Guanye Jinsitao (Hypericum perforatum) is used to soothe the liver and resolve constraint, cool the blood and calm the mind for treating emotional complications.
8.Research progress in osteoradionecrosis of the jaws

Osteoradionecrosis of the jaws (ORNJ) is among the most severe oral complications following radiotherapy for head and neck tumors. It is essentially a form of pathological necrosis of the jawbone induced by radiation therapy. ORNJ is defined as bone damage, primarily characterized by inflammation and necrosis, occurring in the jawbone within the irradiated area and accompanied by soft tissue injury, persisting for more than 3 months without spontaneous healing. Diagnosis requires exclusion of other potential etiologies, including primary tumor recurrence, medication-related osteonecrosis, and radiation-induced neoplasms of the jawbone, etc. In this review, recent advances in the epidemiology, risk factors, diagnosis, classification and staging, dosimetric parameters, pathogenesis, treatment, and prevention of radiation-induced osteonecrosis of the jaws were summarized.

The incidence of apoplexy remains persistently high among the older population.Based on the traditional Chinese medicine principle of"treating the elderly by focusing on the fu",this paper explores the holistic connotation of"fu".It proposes that the onset of apoplexy in the elderly is characterized by obstruction,stagnation,depression,and sluggishness,which should be treated from six fu viscera,xuanfu,and collaterals.The interconnected hierarchical network of these three systems,serving as macro-and micro-channels for qi and fluid metabolism,plays a central role in both disease development and treatment.The failure of the six fu viscera to descend and the upward invasion of turbid yin are identified as prerequisites for apoplexy onset,whereas yang qi stagnation and xuanfu blockage act as key pathogenic drivers,and the core mechanisms involve phlegm-stasis-toxin accumulation,and dysfunction of the collaterals and fu.In the treatment,acute phase requires unblocking fu viscera and restoring xuanfu patency;chronic phase focuses on dredging collaterals and opening xuanfu;and recovery phase emphasizes tonifying combined with regulating xuanfu.The understanding of"treating the elderly by focusing on the fu"emphasizes that the pathogenesis of the disease should be changed to individual conditions;"six fu viscera-xuanfu-collaterals"exhibits a pathological mechanism characterized by the transmission and reception of pathogenic factors,as well as progressive mutual involvement;in clinical practice,treatment should meticulously assess the severity and nuances of the condition,prioritizing method that emphasizes unobstructed flow;additionally,therapy should be essential to protect stomach qi and integrate therapeutic attacks within a framework of supplementation.These principles offer valuable reference for the differentiation and treatment of apoplexy.
