1.Long non-coding RNA MALAT1 in hematological malignancies and its clinical applications

Metastasis-associated lung adenocarcinoma transcript 1 ( MALAT1) is a well-established oncogenic long non-coding RNA, the higher expression of which is strongly correlated with cancer events such as tumorigenesis, progression, metastasis, drug resistance, and treatment outcome in solid cancers. Recently, a series of studies has highlighted its potential role in hematological malignancies in terms of these events. Similar to solid cancers, MALAT1 can regulate various target genes via sponging and epigenetic mechanisms, but the miRNAs sponged by MALAT1 differ from those identified in solid cancers. In this review, we systematically describe the role and underlying mechanisms of MALAT1 in multiple types of hematological malignancies, including regulation of cell proliferation, metastasis, stress response, and glycolysis. Clinically, MALAT1 expression is related to poor treatment outcome and drug resistance, therefore exhibiting potential prognostic value in multiple myeloma, lymphoma, and leukemia. Finally, we discuss the evaluation of MALAT1 as a novel therapeutic target against cancer in preclinical studies.
2.Population pharmacokinetics of Ainuovirine and exposure-response analysis in human immunodeficiency virus-infected individuals

Background::Ainuovirine (ANV) is a new generation of non-nucleoside reverse transcriptase inhibitor for the treatment of human immunodeficiency virus (HIV) type 1 infection. This study aimed to evaluate the population pharmacokinetic (PopPK) profile and exposure-response relationship of ANV among people living with HIV.Methods::Plasma concentration-time data from phase 1 and phase 3 clinical trials of ANV were pooled for developing the PopPK model. Exposure estimates obtained from the final model were used in exposure-response analysis for virologic responses and safety responses.Results::ANV exhibited a nonlinear pharmacokinetic profile, which was best described by a two-compartment model with first-order elimination. There were no significant covariates correlated to the pharmacokinetic parameters of ANV. The PopPK parameter estimate (relative standard error [%]) for clearance adjusted for bioavailability (CL/F) was 6.46 (15.00) L/h, and the clearance of ANV increased after multiple doses. The exposure-response model revealed no significant correlation between the virologic response (HIV-RNA <50 copies/mL) at 48 weeks and the exposure, but the incidence of adverse events increased with the increasing exposure ( P value of steady-state trough concentration and area under the steady-state curve were 0.0177 and 0.0141, respectively). Conclusions::Our PopPK model supported ANV 150 mg once daily as the recommended dose for people living with HIV, requiring no dose adjustment for the studied factors. Optimization of ANV dose may be warranted in clinical practice due to an increasing trend in adverse reactions with increasing exposure.Trial registration::Chinese Clinical Trial Registry https://www.chictr.org.cn (Nos. Objective:We aimed to develop a novel visualized model based on nomogram to predict postoperative overall survival.Methods:This was a multicenter, retrospective, observational cohort study, including participants with histologically confirmed gastric and colorectal cancer who underwent radical surgery from 11 medical centers in China from August 1, 2015 to June 30, 2018. Baseline characteristics, histopathological data and nutritional status, as assessed using Nutrition Risk Screening 2002 (NRS 2002) score and the scored Patient-Generated Subjective Global Assessment, were collected. The least absolute shrinkage and selection operator regression and Cox regression were used to identify variables to be included in the predictive model. Internal and external validations were performed.Results:There were 681 and 127 patients in the training and validation cohorts, respectively. A total of 188 deaths were observed over a median follow-up period of 59 (range: 58 to 60) months. Two independent predictors of NRS 2002 and Tumor-Node-Metastasis (TNM) stage were identified and incorporated into the prediction nomogram model together with the factor of age. The model's concordance index for 1-, 3- and 5-year overall survival was 0.696, 0.724, and 0.738 in the training cohort and 0.801, 0.812, and 0.793 in the validation cohort, respectively.Conclusions:In this study, a new nomogram prediction model based on NRS 2002 score was developed and validated for predicting the overall postoperative survival of patients with gastric colorectal cancer. This model has good differentiation, calibration and clinical practicability in predicting the long-term survival rate of patients with gastrointestinal cancer after radical surgery.
4.Clinical Efficacy and Safety of Flumatinib in the Treatment of Patients with Newly Diagnosed Chronic Myeloid Leukemia in Chronic Phase

Objective:To explore the clinical efficacy and safety of flumatinib mesylate produced in China in patients with newly diagnosed chronic myeloid leukemia in chronic phase(CML-CP).Methods:32 newly diagnosed CML-CP patients admitted to the Hematology Department of the Affiliated Hospital of Southwest Medical University from March 1,2020 to March 31,2022,who had never received any tyrosine kinase inhibitor(TKI)were included in the study.The patients were treated by flumatinib mesylate 600mg once daily.The hematologic,cytogenetic and molecular responses were assessed at 3-,6-and 12-month,and adverse effects of the drug were evaluated.Results:31 patients were treated with flumatinib for≥3 months,of which 24 patients were treated for ≥ 6 months and 14 patients were treated for ≥ 12 months.At 3rd month of treatment,30 out of 31 patients achieved complete hematologic response(CHR);24 patients underwent cytogenetic testing and 22 cases achieved major cytogenetic response(MCyR),of which 21 cases achieved complete cytogenetic response(CCyR);Among 25 patients who underwent molecular testing,22 patients had BCR-ABLIS ≤ 10%,including 10 patients with BCR-ABLIS ≤ 0.1%,and 6 patients with BCR-ABLIS≤0.01%.At 6th month of treatment,23 out of 24 patients achieved CHR;17 patients underwent cytogenetic testing and all achieved CCyR;Among 23 patients who underwent molecular testing,20 patients had BCR-ABLIS ≤1%,including 16 patients with BCR-ABL1S≤0.1%and 12 patients with BCR-ABLIS ≤ 0.01%.At 12nd month of treatment,all 14 patients achieved CHR and CCyR;Among them,10 patients had BCR-ABLIS ≤ 0.1%,including 9 patients with BCR-ABLIS ≤ 0.01%.The grade Ⅲ/Ⅳ leukopenia,thrombocytopenia and anemia rates in the patients were 13.3%,20.0%and 3.3%,respectively.One patient stopped flumatinib therapy due to severe and persistent hematologic toxicity.The major non-hematologic adverse events were abnormal liver function(20%),diarrhea(10%),bone/joint pain(10%),muscle spasm(10%),rash(6.7%),acute kidney injury(6.7%)and nausea(3.3%),most of which were grade Ⅰ-Ⅱ.No patient experienced grade Ⅳnon-hematologic adverse events.No drug toxicity-related death occurred.Conclusion:Flumatinib mesglate,as the first-line treatment for newly diagnosed CML-CP,can enable the patients to achieve early and deep molecular and cytogenetic responses,and shows good safety.
5.Effect of obesity factor on myocardial ischemia-reperfusion injury in mice and role of TRPA1 expression

Objective:To evaluate the effect of obesity factor on myocardial ischemia-reperfusion injury and the role of transient receptor potential ankyrin 1 (TRPA1) expression in mice.Methods:Twenty-four SPF healthy male C57BL/6J mice, aged 6-7 weeks, weighing 18-21 g, were divided into 2 groups ( n=12 each) using a random number table method: common diet group (CD group) and high fat diet group (HFD group). CD group was fed a common diet and HFD group was fed a high-fat diet supplied with 60% fat for 12 weeks, and the body weight of mice was recorded every week. The mice in CD group were then divided into 2 groups ( n=6 each) using a random number table method: CD+ solvent group (CD+ C group) and CD+ TRPA1 agonist allyl isothiocyanate (AITC) group (CD+ A group). The mice in group HFD were divided into 2 groups ( n=6 each) using a random number table method: HFD+ solvent group (HFD+ C group) and HFD+ AITC group (HFD+ A group). At 13th week, AITC 25 mg·kg -1·d -1 was intragastrically administered based on the original diet in CD+ A group and HFD+ A group, and the equal volume of solvent was given in CD+ C and HFD+ C groups. At 14th week, myocardial I/R injury was established by occlusion of the left coronary artery for 30 min followed by 120-min reperfusion. The body weight, epididymal fat, mesenteric fat and perirenal fat were recorded, and the levels of LDH, triglyceride and cholesterol in serum were determined. The percentage of myocardial infarct size was calculated. The expression of TRPA1, Bax and Bcl-2 was detected by Western blot, and the ratio of Bax/Bcl-2 was calculated. Results:Compared with CD+ C group, the expression of TRPA1 in myocardial tissues was significantly up-regulated ( P<0.05), and no significant differences were found in the other parameters in CD+ A group ( P>0.05), and the concentrations of serum triglyceride and cholesterol, body weight, weight of epididymal fat, mesenteric fat, perirenal fat and percentage of myocardial infarct size were significantly increased, the expression of TRPA1 in myocardial tissues was down-regulated, and the concentration of LDH and ratio of Bax/Bcl-2 were increased in HFD+ C group ( P<0.05). Compared with HFD+ C group, the concentrations of serum triglyceride and cholesterol, body weight, weight of epididymal fat, mesenteric fat, perirenal fat and percentage of myocardial infarct size were significantly decreased, the expression of TRPA1 in myocardial tissues was up-regulated, and the concentration of LDH and ratio of Bax/Bcl-2 were decreased in HFD+ A group ( P<0.05). Conclusions:Obesity can aggravate myocardial ischemia-reperfusion injury in mice, and the down-regulated myocardial TRPA1 expression is involved in this process.
6.Effects of Sanshiliudang Kange Powder on airway inflammation in bronchial asthma remission of mice

AIM To investigate the effects of Sanshiliudang Kange Powder on airway inflammation in bronchial asthma remission of mice.METHODS Seventy-eight BALB/c mice were randomly divided into the blank group,the model group,the dexamethasone group(0.5 mg/kg)and the low-dose,medium-dose and high-dose Sanshiliudang Kange Powder groups(9.3,18.6 and 37.2 g/kg).Except for those of the blank group,the mice of the other groups were sensitized by intraperitoneal injection of ovalbumin(OVA)and aluminum hydroxide mixture,and induced into models of asthma remission followed by 28 days consecutive corresponding drug administration which was initiated on the 77th day of modeling.The mice had their pulmonary pathological changes observed by HE staining;their levels of IL-1β,IL-6,TNF-α,HMGB1 and NF-κB in serum and alveolar fluid determined by ELISA;and their pulmonary protein expressions of TLR4,MyD88,NF-κB p65 and p-NF-κB p65 determined by Western blot.RESULTS Compared with the model group,each group intervened with a drug displayed significantly reduced pulmonary pathological injury and inflammation;decreased levels of IL-1β,IL-6,TNF-α,HMGB1,NF-κB in serum and BALF(P<0.01);and decreased pulmonary protein expressions of TLR4,MyD88,p-NF-κB p65/NF-κB p65(P<0.01).Generally,the medium-dose Sanshiliudang Kange Powder group demonstrated a very good efficacy,which was only inferior to that of the dexamethasone group.CONCLUSION Sanshiliudang Kange Powder has a good inhibitory effect on airway inflammation in bronchial asthma remission of mice,and its mechanism may be related to the inhibited activation of HMGB1/TLR4/MyD88 signal pathway.

Objective To investigate the risk factors for bronchopulmonary dysplasia(BPD)in twin preterm infants with a gestational age of<34 weeks,and to provide a basis for early identification of BPD in twin preterm infants in clinical practice.Methods A retrospective analysis was performed for the twin preterm infants with a gestational age of<34 weeks who were admitted to 22 hospitals nationwide from January 2018 to December 2020.According to their conditions,they were divided into group A(both twins had BPD),group B(only one twin had BPD),and group C(neither twin had BPD).The risk factors for BPD in twin preterm infants were analyzed.Further analysis was conducted on group B to investigate the postnatal risk factors for BPD within twins.Results A total of 904 pairs of twins with a gestational age of<34 weeks were included in this study.The multivariate logistic regression analysis showed that compared with group C,birth weight discordance of>25%between the twins was an independent risk factor for BPD in one of the twins(OR=3.370,95%CI:1.500-7.568,P<0.05),and high gestational age at birth was a protective factor against BPD(P<0.05).The conditional logistic regression analysis of group B showed that small-for-gestational-age(SGA)birth was an independent risk factor for BPD in individual twins(OR=5.017,95%CI:1.040-24.190,P<0.05).Conclusions The development of BPD in twin preterm infants is associated with gestational age,birth weight discordance between the twins,and SGA birth.
8.Application of exchange transfusion in neonates with severe pertussis and hyperleukocytosis

Objective To investigate the efficacy and safety of exchange transfusion in neonates with severe pertussis. Methods A retrospective analysis was performed for the medical data of five neonates with severe pertussis who underwent exchange transfusion in the Department of Neonatology,Hunan Children's Hospital,from August 2019 to March 2024. The clinical characteristics of the patients were summarized,and the efficacy and adverse reactions of exchange transfusion were analyzed. Results All five neonates had the symptoms of hypoxemia,recurrent apnea,and heart failure and required invasive mechanical ventilation. Two cases of pulmonary hypertension were observed,one of which was complicated by decompensated shock. Before exchange transfusion,the five children had a median leukocyte count of 82.60×109/L,a median absolute lymphocyte count of 28.20×109/L,and a median absolute neutrophil count of 43.10×109/L,and reexamination at 4 hours after exchange transfusion showed that these values decreased to 28.40×109/L,7.60×109/L,and 15.40×109/L,respectively. The four children who underwent exchange transfusion in the early stage of cardiopulmonary failure showed varying degrees of improvement in oxygenation and a reduction in the partial pressure of carbon dioxide,and they were discharged after improvement;the one child who underwent exchange transfusion in the late stage of cardiopulmonary failure ultimately died. No child experienced severe adverse reactions related to exchange transfusion. Conclusions For neonates with severe pertussis,initiating exchange transfusion in the early stages of cardiopulmonary failure can effectively reduce leukocyte levels,potentially improve survival rates,and is relatively safe.
9.Effects of Modified Ditan Decoction in Regulating miR-149 on Liver Injury in Rats with Chronic Intermittent Hypoxia

Objective To investigate the effects and mechanism of modified Ditan Decoction in regulating miR-149 on liver injury in rats with chronic intermittent hypoxia.Methods Totally 18 male SD rats were randomly divided into control group,model group,and modified Ditan Decoction,with 6 rats in each group.The model group and the modified Ditan Decoction group were modeled in a low oxygen chamber.During the modeling period,the modified Ditan Decoction group was given gavage of the modified Ditan Decoction for 12 consecutive weeks.HE staining was used to observe the pathological changes of liver tissue;the ultrastructural changes of liver tissue were observed by transmission electron microscopy,TUNEL staining was used to observe hepatocyte apoptosis,the contents of ALT,AST,SOD and MDA in liver tissue were detected,the mRNA and protein expression of miR-149,ATF6,GRP78 and CHOP in liver tissue were detected by RT-qPCR and Western blot.The correlation between the mRNA transcription levels of miR-149 and ATF6 was analyzed.Results Compared with the control group,a small amount of inflammatory cell infiltration in liver tissue of the model group,with irregular arrangement of liver cells,varying degrees of edema,expansion of endoplasmic reticulum,ribosome shedding,mitochondrial membrane rupture,and apoptosis rate of liver cells increased(P<0.05),the contents of ALT,AST and MDA significantly increased(P<0.05),while the content of SOD significantly decreased(P<0.05),the expression of miR-149 mRNA in liver tissue decreased(P<0.05),while the mRNA and protein expression of ATF6,GRP78 and CHOP increased(P<0.05).Compared with the model group,no obvious edema or inflammatory cell infiltration was observed in liver tissue of rats in the modified Ditan Decoction group,the liver cells were arranged in a regular manner,with slightly expanded endoplasmic reticulum and more uniform distribution of ribosomes,the mitochondrial membrane was relatively intact,and the apoptosis rate of liver cells decreased(P<0.05),the contents of ALT,AST and MDA decreased(P<0.05),while the content of SOD increased(P<0.05),the expression of miR-149 mRNA in liver tissue significantly increased(P<0.05),while the mRNA and protein expression of ATF6,GRP78 and CHOP significantly decreased(P<0.05).The level of miR-149 mRNA was significantly negatively correlated with that of ATF6 mRNA(r=-0.766).Conclusion Modified Ditan Decoction may inhibit liver oxidative stress response in chronic intermittent hypoxia rats and improve liver injury by regulating the miR-149/ATF6 pathway.
10.Prediction of pathological complete response to neoadjuvant chemoradiotherapy in locally advanced rectal cancer using contrast-enhanced ultrasound radiomics

Objective:To evaluate the diagnostic performance of radiomics model based on contrast-enhanced ultrasound(CEUS) in predicting pathological complete response(pCR) after neoadjuvant chemoradiotherapy(nCRT) in patients with locally advanced rectal cancer(LARC).Methods:One hundred and six patients with LARC who underwent total mesorectal excision after nCRT between April 2018 and April 2023 in the First Affiliated Hospital of Guangxi Medical University were retrospectively included, the patients were randomly divided into a training set of 63(14 pCR patients) and a validation set of 43(12 pCR patients) in a 6∶4 ratios. Radiomics features were extracted from the tumors′ region of interest of CEUS images based on PyRadiomics. Intra-class correlation coefficient(ICC), Mann-Whitney U test, and least absolute shrinkage and selection operator(LASSO) algorithms were used to reduce features dimension. Finally, 7 radiomics features relevanted to pCR were selected to construct an ultrasomics model using elastic network regression, based on the R language. A combined model was constructed by jointing clinical feature. The performance of the models was assessed with the area under the ROC curve(AUC). Results:The AUC of the ultrasomics model and the combined model was 0.695(95% CI=0.532-0.859) and 0.726(95% CI=0.584-0.868) respectively in the training set. The AUC of the ultrasomics model and the combined model was 0.763(95% CI=0.625-0.902) and 0.790(95% CI=0.653-0.928) respectively in the validation set. Both univariate and multivariate Logistic regression analyses showed that CA199( P<0.05) and ultrasomics score( P<0.001) could be an independent predictor of pCR after nCRT in patients with LARC. Conclusions:The CEUS-based radiomics scores has certain predictive value for whether LARC patients achieve pCR after nCRT, and may provide a non-invasive imaging biomarker for predicting LARC patients achieve pCR after nCRT.
