BACKGROUND:
China is seeing a growing demand for rehabilitation treatments for post-stroke upper limb spastic paresis (PSSP-UL). Although acupuncture is known to be effective for PSSP-UL, there is room to enhance its efficacy.
OBJECTIVE:
This study explored a semi-personalized acupuncture approach for PSSP-UL that used three-dimensional kinematic analysis (3DKA) results to select additional acupoints, and investigated the feasibility, efficacy and safety of this approach.
DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS:
This single-blind, single-center, randomized, controlled trial involved 74 participants who experienced a first-ever ischemic or hemorrhagic stroke with spastic upper limb paresis. The participants were then randomly assigned to the intervention group or the control group in a 1:1 ratio. Both groups received conventional treatments and acupuncture treatment 5 days a week for 4 weeks. The main acupoints in both groups were the same, while participants in the intervention group received additional acupoints selected on the basis of 3DKA results. Follow-up assessments were conducted for 8 weeks after the treatment.
MAIN OUTCOME MEASURES:
The primary outcome was the Fugl-Meyer Assessment for Upper Extremity (FMA-UE) response rate (≥ 6-point change) at week 4. Secondary outcomes included changes in motor function (FMA-UE), Brunnstrom recovery stage (BRS), manual muscle test (MMT), spasticity (Modified Ashworth Scale, MAS), and activities of daily life (Modified Barthel Index, MBI) at week 4 and week 12.
RESULTS:
Sixty-four participants completed the trial and underwent analyses. Compared with control group, the intervention group exhibited a significantly higher FMA-UE response rate at week 4 (χ2 = 5.479, P = 0.019) and greater improvements in FMA-UE at both week 4 and week 12 (both P < 0.001). The intervention group also showed bigger improvements from baseline in the MMT grades for shoulder adduction and elbow flexion at weeks 4 and 12 as well as thumb adduction at week 4 (P = 0.007, P = 0.049, P = 0.019, P = 0.008, P = 0.029, respectively). The intervention group showed a better change in the MBI at both week 4 and week 12 (P = 0.004 and P = 0.010, respectively). Although the intervention group had a higher BRS for the hand at week 12 (P = 0.041), no intergroup differences were observed at week 4 (all P > 0.05). The two groups showed no differences in MAS grades as well as in BRS for the arm at weeks 4 and 12 (all P > 0.05).
CONCLUSION:
Semi-personalized acupuncture prescription based on 3DKA results significantly improved motor function, muscle strength, and activities of daily living in patients with PSSP-UL.

This study aimed to characterize and identify the non-volatile components in aqueous and ethanolic extracts of the stems and leaves of Rhododendron tomentosum by using sensitive and efficient ultra-performance liquid chromatography-quadrupole-time of flight mass spectrometry(UHPLC-Q-TOF-MS) combined with a self-built information database. By comparing with reference compounds, analyzing fragment ion information, searching relevant literature, and using a self-built information database, 118 compounds were identified from the aqueous and ethanolic extracts of R. tomentosum, including 35 flavonoid glycosides, 15 phenolic glycosides, 12 flavonoids, 7 phenolic acids, 7 phenylethanol glycosides, 6 tannins, 6 phospholipids, 5 coumarins, 5 monoterpene glycosides, 6 triterpenes, 3 fatty acids, and 11 other types of compounds. Among them, 102 compounds were reported in R. tomentosum for the first time, and 36 compounds were identified by comparing them with reference compounds. The chemical components in the ethanolic and aqueous extracts of R. tomentosum leaves and stems showed slight differences, with 84 common chemical components accounting for 71.2% of the total 118 compounds. This study systematically characterized and identified the non-volatile chemical components in the ethanolic and aqueous extracts of R. tomentosum for the first time. The findings provide a reference for active ingredient research, quality control, and product development of R. tomentosum.

In recent years, gastrointestinal stromal tumors (GIST) have increased incidence and mortality, and most GIST is caused by the activation mutation of the c-KIT gene. Therefore, c-KIT has become a promising therapeutic target of GIST. At present, the drugs approved for the treatment of GIST including imatinib, sunitinib, regorafenib and ripretinib, are mostly prone to developing resistance and accompanied by various degrees of adverse reactions. Therefore, there is an urgent need to develop new c-KIT inhibitors to solve the problem of resistance. In this study, we investigated the anti-tumor effect of a novel c-KIT inhibitor PN17-1 on gastrointestinal stromal tumor GIST-882 cells

Objective:To investigate the present situation and influencing factors of hearing loss among noise exposed workers in a gas production plant, and to provide basis and guidance for enterprises to prevent hearing loss. Methods:In November 2023, a cross-sectional survey was conducted among personnel exposed to noise in a gas production plant. A total of 380 questionnaires were collected, among which 319 were valid resulting in an effective recovery rate of 83.9%. Ultimately, 292 were included in the study. The questionnaire covered demographic characteristics, work conditions, behavioral and lifestyle habits, hearing loss, and medical history. The noise exposure levels of the research subjects were determined based on the occupational disease hazard factor detection reports provided by the gas production plant for 2022 and 2023. Whether hearing loss occurred was determined based on the pure tone audiometry results from the occupational health examination. Chi-square tests were used to analyze the influence of demographic characteristics, work conditions, and behavioral and lifestyle habits on hearing loss. A multivariate logistic regression model was used to analyze the relationship between hearing loss and related influencing factors. Results:Among the noise-exposed workers in the gas production plant, the incidence of high-frequency hearing loss was 14.4% (42/292) . The rate of hearing loss among men was higher than that among women ( P=0.010) , and the rate of hearing loss among those with a history of smoking was higher than that among those who had never smoked ( P=0.015) . Conclusion:Gas production enterprises should prioritize targeted interventions for workers with prolonged employment histories and smoking habits. Training programs should be implemented to cultivate proper ear protection practices, enhance knowledge of hearing conservation, and promote healthier lifestyle behaviors among employees.

Objective To explore the correlation between fasting blood glucose(FBG)level and fractional flow reserve(FFR)in patients with borderline coronary artery disease, and to clarify its potential influence on FFR measurement. Methods From August 2020 to August 2023,the data of 135 patients with coronary atherosclerotic heart disease who received coronary angiography and FFR evaluation in the Fourth Affiliated Hospital of Harbin Medical University were retrospectively collected.According to the exclusion and inclusion criteria,85 cases of borderline diseased vessels of single coronary artery with stenosis degree of 50%-80%were screened out,and they were divided into FBG≥6.1 mmol/L group(47 cases)and FBG<6.1 mmol/L group(38 cases). The baseline data, angiographic and functional indexes of the two groups were compared, and the correlation between FBG and FFR was analyzed. Results Compared with the FBG<6.1 mmol/L group, the FBG≥6.1 mmol/L group had a higher proportion of FFR negative results(72.3%vs.23.7%,P<0.001),and the FFR measurement values were generally increased[0.84(0.80,0.90)vs.0.75(0.68,0.80),P<0.001],with statistically significant differences.Pearson correlation analysis was performed on all lesions, and FFR>0.80(negative result)was positively correlated with FBG≥6.1 mmol/L(r=0.484,P<0.001).Conclusions Among the patients with borderline coronary artery disease(50%-80%stenosis)included in this study,FBG≥6.1 mmol/L is significantly correlated with FFR>0.80.For patients with borderline coronary lesions with elevated FBG,the influence of blood glucose factors should be carefully considered in clinical interpretation of FFR results.

This study aimed to address the limitations of current diagnostic methods for well leg compartment syndrome(WLCS),including invasiveness, high costs, and insufficient accuracy,by proposing a solution based on electrical impedance tomography(EIT)technology. The electrical response characteristics of the human calf muscle to changes in compartment pressure using EIT were investigated, aiming to visualize the effects of pressure variations on the electrical properties within the compartment and to provide technical support for early non-invasive detection of WLCS.EIT sensors were placed on the right calf of the experimental subjects, with pressure applied externally to the right thigh.Measurements were conducted in two phases:pre-pressure(pre)and post-pressure(post).Pre-pressure,the conductivity distribution image σpre was measured when the calf was placed horizontally.Post-pressure,the calf was raised at an angle of approximately 30°,and pressures of 0,40,80,and 120 mmHg were applied to the right thigh, and the corresponding conductivity distribution images σP=0,σP=40,σP=80,andσP=120were recorded. To quantitatively analyze the pressure effects on the compartment response, paired sample t-test was used to assess the spatial-mean conductivity((σ))from the EIT reconstructed images.Compared to the horizontal position of the right calf, raising the calf at approximately 30° resulted in a significant increase in the spatial-mean conductivity(σ)of the M1 compartment.Furthermore, when pressure was applied to the right thigh while the calf remained at a 30° angle, the spatial-mean conductivity of the M1 compartment σM1 showed an increasing trend with rising pressure. The results indicated that as compartment pressure increased, the volume of extracellular fluid and ion concentration significantly increased, leading to an increase in conductivity, which reflected ischemia and hypoxia in muscle tissue and the related pathophysiological changes.EIT,due to its high sensitivity to conductivity changes, offered a potential effective diagnostic method for non-invasively monitoring the onset and progression of muscle compartment syndrome.

Aim To investigate the effect of N6-methy-ladenosine(m6A)demethylase ALKBH5 on the prolif-eration and migration of cardiac fibroblasts(CFs)in-duced by high glucose. Methods Primary CFs were isolated from neonatal mouse hearts and identified u-sing optical and confocal microscopy.Cell activation was induced using a high-glucose medium(33 mmol·L-1 glucose). An ALKBH5 overexpression model was established by transfecting CFs with an ALKBH5 ex-pression vector in a high-glucose medium. The expres-sion of ALKBH5 in CFs was assessed through immuno-fluorescence staining,Western blot and RT-qPCR.Changes in m6A levels were evaluated using Dot blot a-nalysis.Additionally,Alterations in the expression of proliferating cell nuclear antigen(PCNA)and collagenⅠ,a pivotal fibrosis indicator, were measured using Western blot. The proliferation and migration ability of CFs were assessed through EdU staining and Transwell migration assay, respectively. Results Following treatment with high glucose, the expression of ALKBH5 in CFs notably decreased, while m6A level increased. This was accompanied by a significant increase in the expression of the proliferation marker PCNA and the fi-brosis marker collagen Ⅰ.Additionally, there was a sig-nificant improvement in the ability of proliferation and migration.Overexpression of ALKBH5 resulted in a significant decrease in the expressions of PCNA and collagen Ⅰ,leading to the inhibition of both proliferation and migration in CFs.Conclusion Overexpression of ALKBH5 suppresses the expression of PCNA and colla-gen Ⅰ,consequently reducing the proliferation and mi-gration of CFs,potentially through m6A methylation modification.
