1.Prediction Model of Large for Gestational Age Infants in Pregnant Women with Gestational Diabetes Mellitus

Objective:To establish a prediction model for larger for gestational age(LGA)infants in pregnant women with gestational diabetes mellitus(GDM)in order to improve pregnancy outcomes.Methods:A retro-spective analysis was performed on the clinical data of 338 pregnant women with GDM who underwent routine prenatal examinations and were hospitalized for delivery in the First Affiliated Hospital of Nanjing Medical Universi-ty from January 1,2018 to December 31,2023.Pregnant women with complete HbAlc data during pregnancy were divided into a training set of 241 cases and a validation set of 97 cases.Lasso and Logistic regression analysis and variable screening combined with previous clinical experience were used to construct a nomogram model,and its degree of differentiation and calibration were evaluated.Result:①By Lasso regression analysis,age,family histo-ry of type 2 diabetes,body mass index(BMI),gestational weight gain(GWG),fasting blood glucose(FBG),postprandial 1-hour blood glucose(1h PBG),HbAlc,free triiodothyronine(FT3),free thyroxine(FT4)and insulin treatment were important predictors of LGA.②Multivariate Logistic regression analysis showed that GWG and HbAlc were independent risk factors for LGA in pregnant women with GDM(OR>1,P<0.05).③Combined with Lasso and Logistic regression analysis,previous literature reports and clinical experience,BMI,GWG,FBG,1h PBG,HbAlc and FT3 were selected as independent variables,and LGA as dependent variable.A nomogram pre-diction model was constructed in the training set,and the C-index of 0.71.ROC curve analysis showed that the AUC values of the training set and the validation set were 0.709 and 0.700,respectively,and the discriminative a-bility of the model was acceptable.The calibration curve of the model was close to the ideal curve,and the clinical decision curve suggested that the model showed a positive net benefit at the threshold of 10%to 50%.Conclu-sion:The predictive model has certain value in predicting the occurrence of LGA in pregnant women with GDM,and provides help for early diagnosis,treatment and clinical intervention of GDM and its complications,in order to improve perinatal and long-term adverse outcomes.

It is believed that Shengyang Yiwei Decoction (升阳益胃汤, SYD) is effective in regulating the flow of Qi (气), and can treat various diseases caused by the disorder of the spleen and stomach Qi. In clinical practice, based on the pathological characteristics of children often having insufficient spleen, and adhering to the principle of treating different diseases with the same method, the focus is placed on the core pathogenesis of spleen and stomach Qi disharmony. We use SYD in various pediatric conditions such as allergic rhinitis, post COVID-19 condition, urethral syndrome, and dysfunctional uterine bleeding in adolescence, and emphasize the treatment is flexibly tailored to the symptoms.

Gastric cancer(GC)is one of the most common and high mortality tumors in the world,and the clinical staging during diagnosis is closely related to the prognosis of patients.Therefore,finding sensitive diagnostic biomarkers to improve the early diagnosis rate has become an urgent issue.Exosomes are nanoscale extracellular vesicles secreted by various cells,which play an important role in cellular communication by transporting bioactive substances in tumor microenvironment(TME).CircRNA is a type of non-coding RNA with a specific structure.Because it is difficulty in degradation,it can be enriched in extracellular vesicles and participate in various pathophysiological processes mediated by it.Exo-circRNA can affect the proliferation,invasion,drug resistance and tissue metastasis of tumor cells,and has more significant diagnostic specificity than conventional diagnostic biomarkers in clinical practice.It may become a supplement to biomarker for the diagnosis and prognosis of GC.This article briefly introduces the characteristics,formation mechanism and role of extracellular vesicles and circRNA in GC.It discusses in detail the role of exo-circRNA in the occurrence and development of GC,and discusses their potential clinical application value and challenges in GC.

6.Association between P2Y12 Genetic Polymorphisms and Clinical Efficacy of Clopidogrel in the Patients with Cardiovascular and Cerebrovascular Diseases:a Meta-analysis

Objective:To systematically review the association between P2Y12 genetic polymorphisms and the clinical safety of clopidogrel in the patients with cardiovascular and cerebrovascular diseases. Methods:Retrieved from MEDLINE,Embase,CNKI,Si-noMed and Wanfang Database (from January 1995 to December 2016),array researches about the association between P2Y12 genetic polymorphisms and the clinical safety of clopidogrel were collected including the studies of patients taking clopidogrel with cardiovascu-lar and cerebrovascular diseases and excluding animal experimental studies. The bias of recruited studies was assessed and meta-analy-sis was performed by RevMan 5.1 software. Results:Totally 10 array researches(3 in English and 7 in Chinese) were enrolled invol-ving 5 223 patients. There were no statistical differences between T allele gene carriers and CC genetype patients of C34T in the inci-dence of adverse cardiovascular events (RR=0.95,95% CI:0.82-1.09,P=0.46). The incidence of adverse cardiovascular events in T allele gene carriers of G52T was higher than that in GG genetype patients(RR=1.99,95% CI:1.63-2.44,P<0.000 01). The incidence of clopidogrel resistance in T allele gene carriers of C34T was higher than that in CC genetype patients(RR=2.02,95% CI:1.37-2.96,P=0.000 4). The incidence of clopidogrel resistance in T allele gene carriers of G52T was higher than that in GG gene-type patients (RR=1.56,95% CI:1.04-2.34,P=0.03). There was no statistical difference in the risk of clopidogrel resistance be-tween C allele gene carriers of i-T744c and T allele gene no-carriers(RR=0.99,95% CI:0.78-1.25,P=0.92). Conclusion:T al-lele gene carriers of C34T might be a risk factor of the occurrence of clopidogrel resistance,T allele gene carriers of G52T might be a risk factor of the occurrence of cardiovascular events and clopidogrel resistance,and C allele gene carriers of i-T744c might not increase the danger of the occurrence of clopidogrel resistance.
7.Relationship between Wnt/β-catenin signaling pathway and pancreatic cancer

Wnt signaling pathway is one of canonical pathways that can regulate diverse processes such as cell proliferation,differentiation,migration.Interaction between Wnt ligands and Wnt receptors results in an activation ofdifferent intracellular signaling pathways,and Wnt/β-catenin signaling pathway has been proved to have a close relationship with pancreatic cancer.This review presents a brief overview of Wnt/β-catenin signaling pathway in the mechanism of the tumorigenesis of the pancreatic cancer at deregulation of cell cycle,apoptosis,cancer stem cells and angiogenesis.
