1.Research advances in Infantile liver failure syndrome.

Pediatric acute liver failure (PALF) is a rare and critical clinical syndrome with a poor prognosis. Its etiology is complex, with a significant proportion of cases having remained classified as indeterminate or cryptogenic PALF. With the application of high-throughput sequencing technologies, a spectrum of disorders caused by specific genetic metabolic defects and characterized by stress-sensitive Recurrent acute liver failure (RALF) has been gradually unveiled, collectively termed Infantile liver failure syndrome (ILFS). Although the molecular mechanisms underlying the subtypes ILFS1, ILFS2, and ILFS3 differ by involving aminoacyl-tRNA synthetase defects, vesicular transport disorders, and autophagy abnormalities, respectively, they share a common clinical phenotype of RALF triggered by fever or infection. This article has systematically reviewed the clinical phenotypic spectrum, molecular genetic characteristics, differential diagnosis strategies, and therapeutic advances of the three ILFS subtypes, with the goal of improving early clinical recognition and precise intervention, and providing an important reference for evaluating the prognosis of different subtypes.

2.Clinical efficacy of the traditional Mongolian medicine Zadi-5 pills combined with escitalopram oxalate in the treatment of first-episode depression

BackgroundThe global prevalence of depressive disorders is high, with a significant treatment gap remaining. Escitalopram oxalate, as a commonly used antidepressant in clinical practice, has a single target and is prone to cause adverse reactions such as gastrointestinal discomfort. The traditional Mongolian medicine Zadi-5 pills has a multi-target anti-depression mechanism and unique advantages such as improving the intestinal flora. However, there is currently a lack of randomized controlled trial evidence for the combined treatment of first-episode depression, along with insufficient research on its metabolic safety and potential drug interactions. ObjectiveTo explore the efficacy and safety of Zadi-5 pills combined with escitalopram oxalate in the treatment of the first-episode depression, so as to provide references for Mongolian medicine in depression management. MethodsA randomized controlled trial was conducted. A total of 103 patients with first-episode depression who were treated in the outpatient or inpatient departments of the Inner Mongolia Autonomous Region Mental Health Center from January 2023 to December 2024 and met the diagnostic criteria for depressive episode as defined in International Classification of Diseases, tenth edition (ICD-10) were included. Random numbers were generated using the RAND function in Excel software, and the patients were divided into the study group (n=52) and the control group (n=51). Both groups received escitalopram oxalate treatment, starting from 5 mg/d and gradually increasing to 20 mg/d. The study group additionally received Zadi-5 pills, 13 pills per day. The treatment duration for both groups was 6 weeks. Before treatment and at the 2nd, 4th, and 6th weeks of treatment, the severity of depressive symptoms was evaluated using the Hamilton Depression Scale-24 item (HAMD-24). At these time points, blood was collected on an empty stomach at 7∶00 for the detection of liver function (aspartate aminotransferase, alanine aminotransferase), triglycerides, and the blood concentration of escitalopram oxalate. ResultsA total of 100 patients completed the study, with 50 cases in the study group and the control group. The results of the repeated measures ANOVA showed that the interaction effect between group and time in the HAMD-24 score was statistically significant (F interaction= 18.070, P<0.01). At the end of the 4th and 6th weeks of treatment, the HAMD-24 score of the study group was lower than those of the control group (P<0.05). At each time point after treatment, the reduction rate of the HAMD-24 score in the study group was higher than those of the control group, and the differences were all statistically significant (P<0.05). The interaction effect between the triglyceride level groups and time was statistically significant (Finteraction=3.381, P<0.05).After treatment, the triglyceride levels in the study group showed a downward trend, while those in the control group showed an upward trend. There was no statistically significant interaction effect between the groups and time for liver function (aspartate aminotransferase and alanine aminotransferase) and the blood concentration of escitalopram oxalate (P>0.05). ConclusionCompared with escitalopram oxalate treatment alone, the combination of Zadi-5 pills and escitalopram oxalate is more effective in treating the first-episode depression, without increasing the risk of liver damage. Pulpotomy, which belongs to vital pulp therapy, has become a strategy for managing pulpitis in recent decades. This minimally invasive treatment reflects the recognition of preserving healthy dental pulp and optimizing long-term patient-centered outcomes. Pulpotomy is categorized into partial pulpotomy (PP), the removal of a partial segment of the coronal pulp tissue, and full pulpotomy (FP), the removal of whole coronal pulp, which is followed by applying the biomaterials onto the remaining pulp tissue and ultimately restoring the tooth. Procedural decisions for the amount of pulp tissue removal or retention depend on the diagnostic of pulp vitality, the overall treatment plan, the patient's general health status, and pulp inflammation reassessment during operation. This statement represents the consensus of an expert committee convened by the Society of Cariology and Endodontics, Chinese Stomatological Association. It addresses the current evidence to support the application of pulpotomy as a potential alternative to root canal treatment (RCT) on mature permanent teeth with pulpitis from a biological basis, the development of capping biomaterial, and the diagnostic considerations to evidence-based medicine. This expert statement intends to provide a clinical protocol of pulpotomy, which facilitates practitioners in choosing the optimal procedure and increasing their confidence in this rapidly evolving field.

4.Analysis of risk factors for parenteral nutrition related water-soluble vitamin accumulation

Objective To explore the influencing factors of abnormal increase of plasma water-soluble vitamins in patients with parenteral nutrition(PN).Methods A total of 862 hospitalized patients receiving nutritional diagnosis and treatment were enrolled.Daily water-soluble vitamin supply and total urine output were monitored.If patients underwent renal replacement therapy(RRT),the ultrafiltration volume was recorded.Plasma concentrations of nine water-soluble vitamins(Vit-C,B1,B2,B3,B5,B6,B7,B9 and B12)were measured.According to the test results,patients were divided into the accumulation group and the non-accumulation group.Renal function(BUN,Cr,UA),inflammatory indicators(CRP,WBC,NEU,SII)and nutritional indicators(ALB,PA,Hb,LYM)were compared between the two groups.Logistic regression was used to analyze the independent risk factors for the accumulation of water-soluble vitamins.Results There was a high incidence of abnormal plasma water-soluble vitamin levels,including Vit-B2(36.08%),Vit-B6(16.01%)and Vit-B5(13.81%).Compared with the non-accumulation group,the Vit-B2 accumulation group had higher levels of age,BUN,Cr,UA and NEU,and lower levels of ALB,PA and urine volume.The Vit-B5 accumulation group had higher levels of BUN,Cr,UA and NEU,and lower levels of LYM,Hb,ALB,PA and urine volume.Patients in the Vit-B6 accumulation group was older,with increased levels of BUN,Cr and UA,and decreased levels of LYM,Hb,ALB,urine volume and vitamin intake(P<0.05).Multivariate Logistic regression analysis showed that the elevated level of BUN was independent risk factors for Vit-B2 accumulation,the elevated BUN and reduced urine volume were independent risk factors for Vit-B5 accumulation,and elevated BUN and Cr,as well as reduced urine volume,were independent risk factors for Vit-B6 accumulation.Receiver operating characteristic curve analysis showed that the area under the curve for BUN in predicting Vit-B2,B5 and B6 accumulation were 0.659(95%CI:0.620-0.697),0.728(95%CI:0.675-0.781)and 0.785(95%CI:0.738-0.831),respectively,with high specificity but low sensitivity.Conclusion The serum levels of water-soluble vitamins are affected by renal function.When implementing parenteral nutrition water-soluble vitamins supplementation,the patient's renal function status should be evaluated,and the risk of abnormal elevation of Vit-B2,B5 and B6 should be vigilantly monitored.
5.Correlation analysis of glycemic index and peripheral neuropathy in patients with type 2 diabetes mellitus under continuous glucose monitoring

Objective:To analyze the correlation between blood glucose index and peripheral neuropathy in type 2 diabetes patients under continuous blood glucose monitoring.Methods:A total of 110 patients with type 2 diabetes admitted to the Lu′an Hospital Affiliated to Anhui Medical University from November 2022 to January 2024 were retrospectively observed, and the blood glucose indexes of patients were monitored by continuous glucose meter [glucose time in range (TIR), mean blood glucose (MBG), estimated hemoglobin A 1c (eHbA 1c), blood glucose standard deviation (SD), coefficient of variation (CV), largest amplitude of glycemic excursions (LAGE), mean amplitude of glycemic excursions (MAGE), means of daily differences (MODD), etc ]. All patients with type 2 diabetes were grouped according to whether they had peripheral neuropathy or not, and were divided into the developing group and the non-developing group. General data and blood glucose indexes of the two groups were collected and sorted out. Receiver operating characteristic (ROC) curve was used to analyze the value of various blood glucose indicators in predicting peripheral neuropathy in patients with type 2 diabetes. Spearman correlation analysis was also used to evaluate the correlation between blood glucose index and peripheral neuropathy. Results:The disease course, TIR, MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD of the occurrence group were significantly different from those of the non-occurrence group (all P<0.05). By plotting the ROC curve, It was found that the area under the curve (AUC) of TIR, MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD predicting peripheral neuropathy in patients with type 2 diabetes were 0.942, 0.840, 0.705, 0.759, 0.819, 0.813, 0.857 and 0.677, respectively. The AUC of the combined prediction was 0.971(95% CI: 0.946-0.997), which was higher than that of the single indicator (all P<0.05). Spearman correlation assessment showed a negative correlation between TIR and peripheral neuropathy ( r=-0.738, P<0.05). MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD were positively correlated with peripheral neuropathy ( r=0.554, 0.376, 0.452, 0.490, 0.527, 0.625, 0.272, all P<0.05). Conclusions:Based on continuous blood glucose monitoring, the blood glucose index of type 2 diabetes patients is closely related to peripheral neuropathy, and the above blood glucose index can accurately predict peripheral neuropathy, providing a reference for reducing or preventing the occurrence of peripheral neuropathy.
6.Study on the application of autologous umbilical cord blood priming of CPB circuit in cardiac surgery for congenital heart disease

Objective:To explore the clinical value of autologous umbilical cord whole blood(UCB) priming of the cardiopulmonary bypass(CPB) circuit in neonatal cardiac surgery for congenital heart disease(CHD).Methods:This prospective non-randomized controlled trial included neonates undergoing CHD surgery at Guangdong Provincial People’s Hospital from August 2024 to January 2025. The experimental group used autologous UCB for CPB circuit priming, while the control group used adult allogeneic blood(AAB) priming when UCB was unavailable. Preoperative characteristics, intraoperative CPB and aortic cross-clamping(ACC) times, postoperative ICU stay duration, mechanical ventilation time, and hospitalization length were compared.Results:There were no significant differences in preoperative baseline characteristics between the two groups( P>0.05). At the end of surgery, red blood cell count(RBC), hemoglobin level(Hb), and creatine kinase(CK) showed no significant differences between the groups( P> 0.05). Additionally, perioperative left ventricular ejection fraction(LVEF) demonstrated no statistically significant variations( P>0.05). At surgery completion, the UCB group exhibited lower hematocrit(HCT) and higher blood lactic acid(Lac) levels but these differences resolved by 6 hours postoperatively( P>0.05). The UCB group had higher maximum vasoactive-inotropic scores(VISmax) within 48 hours and longer ICU stays, though total hospitalization and mechanical ventilation durations showed no significant differences( P>0.05). Conclusion:Autologous UCB priming reduces AAB requirements and has minimal impact on postoperative cardiac and pulmonary function recovery, or homeostasis., which is safe and feasible. This study provides evidence supporting the clinical application of UCB priming in CPB circuits.
7.Clinical observation of enteral immune microecological nutrition combined with ω-3 fish oil fat emulsion for postoperative hepatocellular carcinoma resection

OBJECTIVE To study the effects of enteral immune microecological nutrition combined with ω-3 fish oil fat emulsion on postoperative recovery,immune function,liver function and inflammation level in patients with hepatocellular carcinoma resection,as well as the safety of the medication.METHODS A total of 106 patients with hepatocellular carcinoma admitted to our Hospital from June 2020 to December 2023 were selected and divided into control group and study group according to the random number table method,with 53 cases in each group.After undergoing hepatocellular carcinoma resection,control group was given Intacted protein enteral nutrition solution+Enhanced enteral immune microecological nutrition,and the study group was given ω-3 fish oil fat emulsion injection based on the control group.The clinical indicators(postoperative exhaust time,defecation time,postoperative ambulation,and hospital stay),liver function indicators[alanine transaminase(ALT),lactate dehydrogenase(LDH),aspartate transaminase(AST)],immune function indexes(CD3+,CD4+,CD8+,CD4+/CD8+),inflammatory factor indexes[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),IL-6,IL-8],and indicators of intestinal mucosal barrier[D-lactic acid,intestinal fatty acid binding protein(I-FABP)]were compared between 2 groups,and the occurrence of ADR was recorded.RESULTS Compared with the control group,the postoperative exhaust time,postoperative defecation time,and hospital stay of the study group were shortened significantly,and postoperative ambulation increased significantly(P<0.05).After treatment,ALT,LDH,AST,CD8+,inflammatory factors,D-lactic acid and I-FABP of 2 groups were significantly lower than before treatment,and the study group was significantly lower than the control group(P<0.05);CD4+,CD3+,and CD4+/CD8+of two groups were significantly higher than before treatment,and the study group was significantly higher than the control group(P<0.05).There was no significant difference in the incidence of ADR between 2 groups(P>0.05).CONCLUSIONS Enteral immune microecological nutrition combined with ω-3 fish oil fat emulsion injection can shorten the recovery time of patients after hepatocellular carcinoma resection,improve immune function,reduce inflammatory response,and improve liver function with good safety.
8.Effect of HUVECs on proliferation and sternness of hDPSCs silencing integrin a 6

Objective To investigate the effect of human umbilical vein endothelial cells(HUVECs)on the proliferation and stem-ness of human dental pulp stem cells(hDPSCs)silencing with integrin α6(ITGA6).Methods ITGA6 silencing lentivirus was used to interfere the ITGA6 expression of hDPSCs,and its silencing efficiency was detected.Then HUVECs were added to the chambers to co-culture,and the experiments were divided into four groups(sh-NC,sh-ITGA6,sh-NC+HUVECs and sh-ITGA6+HUVECs).hDP-SCs in the sh-NC and sh-ITGA6 groups were transfected with sh-NC and sh-ITGA6 respectively.hDPSCs transfected with sh-NC and sh-ITGA6 were co-cultured with HUVECs in the sh-NC+HUVECs group and sh-ITGA6+HUVECs group respectively.The proliferation capacity of hDPSCs of each group was examined by CCK-8 and EdU on day 7.Immunofluorescence detected the expression of Stro-1,and Real-time PCR was used to detect the expression of Oct4 and Nanog.Results ①Fluorescence microscopy showed that the trans-fection efficiency was about 80%.Real-time PCR and Western blot results showed that sh-ITGA6 lentivirus effectively interfered with ITGA6 expression in hDPSCs.②CCK-8 results showed that on day 5 of co-culture,the proliferation ability of the sh-ITGA6+HUVECs group was superior to that of the sh-ITGA6 group(P<0.05);on day 7,the proliferation ability of the sh-NC+HUVECs and sh-ITGA6+HUVECs group was superior to that of the sh-NC and sh-ITGA6 group(P<0.05).EdU results showed that the DNA synthesis ability of hDPSCs in the co-culture group was significantly stronger than that in the single-culture group(P<0.05).③Immunofluorescence stai-ning revealed that the expression of Stro-1 in the co-culture group was significantly stronger than that in the single-culture group.④Re-al-time PCR results showed that the expression of Oct4 in the co-culture group was higher than that in the single-culture group(P<0.05);the expression of Nanog in hDPSCs with sh-ITGA6 was elevated by the addition of HUVECs co-culture(P<0.05).Conclusion HUVECs significantly enhance the proliferation and stemness of hDPSCs silencing integrin α6.
