1.Pathogenic Mechanisms of Spleen Deficiency-Phlegm Dampness in Obesity and Traditional Chinese Medicine Prevention and Treatment Strategies:from the Perspective of Immune Inflammation

Based on spleen deficiency-phlegm dampness as the core pathogenesis of obesity, and integrating recent advances in modern medicine regarding the key role of immune inflammation in obesity, this paper proposes a multidimensional pathogenic network of "obesity-spleen deficiency-phlegm dampness-immune imbalance". Various traditional Chinese medicine (TCM) herbs that strengthen the spleen, regulate qi, and resolve phlegm and dampness can treat obesity by improving spleen-stomach transport and transformation, promoting water-damp metabolism, and regulating immune homeostasis. This highlights immune inflammation as an important entry point to elucidate the TCM concepts of "spleen deficiency-phlegm dampness" and the therapeutic principle of "strengthening the spleen and eliminating dampness to treat obesity". By systematically analyzing the intrinsic connection between "spleen deficiency generating dampness, internal accumulation of phlegm dampness" and immune dysregulation in obesity, this paper aims to provide theoretical support for TCM treatment of obesity based on dampness.

Pulpotomy, which belongs to vital pulp therapy, has become a strategy for managing pulpitis in recent decades. This minimally invasive treatment reflects the recognition of preserving healthy dental pulp and optimizing long-term patient-centered outcomes. Pulpotomy is categorized into partial pulpotomy (PP), the removal of a partial segment of the coronal pulp tissue, and full pulpotomy (FP), the removal of whole coronal pulp, which is followed by applying the biomaterials onto the remaining pulp tissue and ultimately restoring the tooth. Procedural decisions for the amount of pulp tissue removal or retention depend on the diagnostic of pulp vitality, the overall treatment plan, the patient's general health status, and pulp inflammation reassessment during operation. This statement represents the consensus of an expert committee convened by the Society of Cariology and Endodontics, Chinese Stomatological Association. It addresses the current evidence to support the application of pulpotomy as a potential alternative to root canal treatment (RCT) on mature permanent teeth with pulpitis from a biological basis, the development of capping biomaterial, and the diagnostic considerations to evidence-based medicine. This expert statement intends to provide a clinical protocol of pulpotomy, which facilitates practitioners in choosing the optimal procedure and increasing their confidence in this rapidly evolving field.

3.A meta-analysis of the association between estimated glomerular filtration rate and the onset and progression of type 2 diabetic retinopathy

Objective:To investigate the association between estimated glomerular filtration rate (eGFR) and the onset and progression of type 2 diabetic retinopathy (DR).Methods:Observational studies on the relationship between eGFR and the progression of DR were searched in the databases of PubMed, Web of Science, Foreign Medical Literature Retrieval Service (FMRS), China National Knowledge Infrastructure and Wanfang data. The search period was from the inception of the databases to January 20, 2024. Meta-analysis of the association between eGFR and the onset and progression of DR with the research data was conducted by using Review Manager 5.3 and Stata 15.0, the weighted mean difference (WMD) and 95% confidence interval (CI) were calculated. Sensitivity analysis and Egger′s test were performed to assess the result stability and publication bias.Results:A total of 30 studies involving 119 142 patients with type 2 diabetes were included in the analysis. The eGFR in the DR group was significantly lower than that in the non-DR group (WMD=8.11, 95% CI: 5.97-10.25, P<0.001). Subgroup analysis by DR type revealed that patients with diabetic macular edema (WMD=7.61, 95% CI: 3.82-11.40) and proliferative DR (WMD=17.40, 95% CI: 10.13-24.66) had significantly lower eGFR when compared to that in non-DR group (both P<0.001). The subgroup analysis results according to different DR diagnostic criteria showed that both the 2003 International DR Grading Standard Group (WMD=8.55, 95% CI: 5.29-11.81) and the 2002 International Clinical DR Severity Grading Standard Group (WMD=10.70, 95% CI: 7.99-13.41) indicated the statistically significant differences in eGFR in relation to the occurrence and progression of DR. Conclusion:The decrease of eGFR is closely related to the occurrence and progression of DR.
4.Application of key point nursing from perspective of cluster analysis in preventing complications among patients with acute myocardial infarction undergoing percutaneous coronary intervention

Objective To explore the application effect of key point nursing from the perspective of cluster analysis in preventing complications among patients with acute myocardial infarction(AMI)undergoing percutaneous coronary intervention(PCI).Methods A total of 97 AMI patients who un-derwent PCI in the cardiology department of our hospital from March 2022 to October 2023 were se-lected and divided into control group with 51 patients(March to December 2022)and observation group with 46 patients(January to October 2023)based on their admission time.The control group received routine nursing,while the observation group received key point nursing from the perspective of cluster analysis.The emergency nursing conditions,occurrence of complications during hospitaliza-tion,major adverse cardiovascular events(MACE),and satisfaction of patients were compared be-tween the two groups.Results The door-to-balloon(D2B)time,results of time to myocardial injury markers,and length of hospital stay were shorter in the observation group than those in the control group(P<0.05).The incidence rates of reperfusion arrhythmia during surgery,total vascular com-plications after surgery,unplanned extubation,and total MACE within 6 months after surgery were lower in the observation group than those in the control group(P<0.05).The overall satisfaction rate with nursing in the observation group was 95.65%(44/46),which was higher than 82.35%(42/51)in the control group(P<0.05).Conclusion Key point nursing from the perspective of cluster analysis can shorten the rescue time and length of hospital stay for patients undergoing PCI,reduce the incidence of vascular complications and MACE during hospitalization,and improve satis-faction of patients.
5.Clinical effects of moxibustion regulating the different phenotypic polarization of macrophages on the treatment of RA synovitis

Objective To investigate the effect of moxibustion on different phenotypic expression of macrophages in rheumatoid arthritis(RA).Methods Thirty rats were randomly divided into the blank group,the model group and the moxibustion group,with 10 rats in each group.Rats in the model group and the moxibustion group were injected with complete Freund's adjuvant(CFA)into right foot pad to prepare RA model.The moxibustion group received moxibustion treatment.The blank group was injected with the same amount of normal saline.The thickness of foot pad was measure and arthritis index score was performed.Pathological changes of joints were observed by hematoxylin eosin staining(HE),and the expressions of CD80,CD163 and SPP1 in macrophages of synovial tissue were detected by immunofluorescence.Results Compared with the blank group,arthritis index score and thickness of foot pad were increased in the model group and the moxibustion group(P<0.05).After moxibustion treatment,compared with the model group,the score of arthritis index and thickness of foot pad were decreased in the moxibustion group(P<0.05).HE staining showed joint space narrowing,subchondral bone destruction and inflammatory cell infiltration in the model group.The degree of joint space narrowing,subchondral bone destruction and inflammatory cell infiltration were less in the moxibustion group than those in the model group.Compared with the blank group,the scores of synovial tissue hyperplasia,synovial fibrous tissue hyperplasia and inflammatory cell infiltration increased in the model group.Compared with the model group,scores of synovial tissue hyperplasia,fibrous tissue hyperplasia and inflammatory cell infiltration decreased in the moxibustion group(P<0.05).Results of immunofluorescence detection showed that compared with the blank group,the expressions of CD80 and SPP1 in synovial tissue were up-regulated in the model group,while the expression of CD163 was down-regulated.Compared with the model group,the expressions of CD80 and SPP1 were down-regulated,and the expression of CD163 was up-regulated in the moxibustion group(P<0.05).Conclusion Moxibustion can regulate the expression of different phenotypes of macrophages and reduce synovial inflammation of RA.The mechanism may be related to regulating the expression of different phenotypes of macrophages.
6.Correlation analysis of glycemic index and peripheral neuropathy in patients with type 2 diabetes mellitus under continuous glucose monitoring

Objective:To analyze the correlation between blood glucose index and peripheral neuropathy in type 2 diabetes patients under continuous blood glucose monitoring.Methods:A total of 110 patients with type 2 diabetes admitted to the Lu′an Hospital Affiliated to Anhui Medical University from November 2022 to January 2024 were retrospectively observed, and the blood glucose indexes of patients were monitored by continuous glucose meter [glucose time in range (TIR), mean blood glucose (MBG), estimated hemoglobin A 1c (eHbA 1c), blood glucose standard deviation (SD), coefficient of variation (CV), largest amplitude of glycemic excursions (LAGE), mean amplitude of glycemic excursions (MAGE), means of daily differences (MODD), etc ]. All patients with type 2 diabetes were grouped according to whether they had peripheral neuropathy or not, and were divided into the developing group and the non-developing group. General data and blood glucose indexes of the two groups were collected and sorted out. Receiver operating characteristic (ROC) curve was used to analyze the value of various blood glucose indicators in predicting peripheral neuropathy in patients with type 2 diabetes. Spearman correlation analysis was also used to evaluate the correlation between blood glucose index and peripheral neuropathy. Results:The disease course, TIR, MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD of the occurrence group were significantly different from those of the non-occurrence group (all P<0.05). By plotting the ROC curve, It was found that the area under the curve (AUC) of TIR, MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD predicting peripheral neuropathy in patients with type 2 diabetes were 0.942, 0.840, 0.705, 0.759, 0.819, 0.813, 0.857 and 0.677, respectively. The AUC of the combined prediction was 0.971(95% CI: 0.946-0.997), which was higher than that of the single indicator (all P<0.05). Spearman correlation assessment showed a negative correlation between TIR and peripheral neuropathy ( r=-0.738, P<0.05). MBG, eHbA 1c, SD, CV, LAGE, MAGE and MODD were positively correlated with peripheral neuropathy ( r=0.554, 0.376, 0.452, 0.490, 0.527, 0.625, 0.272, all P<0.05). Conclusions:Based on continuous blood glucose monitoring, the blood glucose index of type 2 diabetes patients is closely related to peripheral neuropathy, and the above blood glucose index can accurately predict peripheral neuropathy, providing a reference for reducing or preventing the occurrence of peripheral neuropathy.

Objective To investigate the effect of human umbilical vein endothelial cells(HUVECs)on the proliferation and stem-ness of human dental pulp stem cells(hDPSCs)silencing with integrin α6(ITGA6).Methods ITGA6 silencing lentivirus was used to interfere the ITGA6 expression of hDPSCs,and its silencing efficiency was detected.Then HUVECs were added to the chambers to co-culture,and the experiments were divided into four groups(sh-NC,sh-ITGA6,sh-NC+HUVECs and sh-ITGA6+HUVECs).hDP-SCs in the sh-NC and sh-ITGA6 groups were transfected with sh-NC and sh-ITGA6 respectively.hDPSCs transfected with sh-NC and sh-ITGA6 were co-cultured with HUVECs in the sh-NC+HUVECs group and sh-ITGA6+HUVECs group respectively.The proliferation capacity of hDPSCs of each group was examined by CCK-8 and EdU on day 7.Immunofluorescence detected the expression of Stro-1,and Real-time PCR was used to detect the expression of Oct4 and Nanog.Results ①Fluorescence microscopy showed that the trans-fection efficiency was about 80%.Real-time PCR and Western blot results showed that sh-ITGA6 lentivirus effectively interfered with ITGA6 expression in hDPSCs.②CCK-8 results showed that on day 5 of co-culture,the proliferation ability of the sh-ITGA6+HUVECs group was superior to that of the sh-ITGA6 group(P<0.05);on day 7,the proliferation ability of the sh-NC+HUVECs and sh-ITGA6+HUVECs group was superior to that of the sh-NC and sh-ITGA6 group(P<0.05).EdU results showed that the DNA synthesis ability of hDPSCs in the co-culture group was significantly stronger than that in the single-culture group(P<0.05).③Immunofluorescence stai-ning revealed that the expression of Stro-1 in the co-culture group was significantly stronger than that in the single-culture group.④Re-al-time PCR results showed that the expression of Oct4 in the co-culture group was higher than that in the single-culture group(P<0.05);the expression of Nanog in hDPSCs with sh-ITGA6 was elevated by the addition of HUVECs co-culture(P<0.05).Conclusion HUVECs significantly enhance the proliferation and stemness of hDPSCs silencing integrin α6.
