Objective To introduce the causes of accidents and the diagnosis and treatment of two patients with radiation-induced skin injury admitted to our hospital in 2023, and to provide a reference for the clinical treatment of subsequent radiation-induced skin injury. Methods The clinical treatment process of two patients with acute skin injury caused by external radiation exposure were summarized and analyzed. Results The exposure history of the two patients was reconstructed, the flaw detection scenario was simulated, the biological dose and hand skin exposure dose were estimated, and the infrared thermal imaging device was used for dynamic monitoring. A comprehensive analysis was conducted based on clinical manifestations and other data. The diagnosis of “Xie” was excessive exposure combined with acute radiation-induced skin injury on both hands (Grade IV for the right hand palm, index finger, and middle finger and Grade II for the left hand little finger). The diagnosis of “Hao” was acute radiation-induced skin injury on both hands (Grade I). The two patients received different clinical treatment measures: “Xie” was treated with both local and systemic therapies, while “Hao” was mainly treated with systemic therapy. Conclusion After systematic and effective treatment, the radiation-induced skin injuries healed in both patients.

Objective:To examine the clinical effects of the SCERTS model-based peer-mediated intervention (PMI) on the social skills of children with autism spectrum disorder (ASD) and provide new approaches to the rehabilitation treatment of ASD children.Methods:A randomized controlled study.A total of 120 children with mild-to-moderate ASD diagnosed Xi′an Traditional Chinese Medicine Hospital of Encephalopathy between April 2023 and April 2024 were selected.They were assigned to either the experimental or control group using the random number table method, with 60 cases in each group.The experimental group was treated with the SCERTS model-based PMI, whereas the control group underwent conventional rehabilitation training, comprising cognitive, language and behavioural interventions, acupuncture and other techniques.Changes in Social Responsiveness Scale (SRS), Autism Social Skills Scale (ASSS) and Autism Treatment Evaluation Checklist (ATEC) scores were observed in the 2 groups of children before and after treatment.The statistical analyses were performed using a paired-samples t-test for intra-group comparisons and an independent-samples t-test for inter-group comparisons. Results:Following 12 weeks of intervention, the SRS and ATEC scores decreased while the ASSS scores increased after treatment in both groups.The differences were statistically significant (all P<0.05).The SRS [(83.25±14.56) points] and ATEC [(79.41±15.36) points] of the experimental group were lower than those of the control group [SRS(89.80±12.69) points, ATEC(85.95±16.13) points].The ASSS score of the experimental group was higher than that of the control group [(112.77±22.42) points vs.(103.80±24.13) points] ( t=2.627, 2.274, -2.109; all P<0.05). Conclusions:The SCERTS model-based PMI is an efficacious approach for the improvement of social skills in ASD children, and thus merits further investigation and application.

BACKGROUND: Previous studies have demonstrated that short-term exposure to ambient particulate matter elevates the risk of ischemic stroke in major urban areas of various countries. However, there is a notable gap in research focusing on remote areas inhabited by ethnic minorities and the cumulative effects of air pollutants. Our study conducted in the area aims to explore the potential association between ischemic stroke and air pollutants and contribute to improving health outcomes among the community. METHODS: This retrospective observational study was conducted at the Xing'an League People's Hospital in Inner Mongolia. The medical records of 4,288 patients admitted for IS between November 1, 2019, and October 31, 2020, were reviewed. Data on demographics (age and sex), air pollutants (PM₁₀, PM_2.5, NO₂, NO, CO, and O₃), and meteorological factors (daily average temperature, daily average wind speed, and daily average atmosphere pressure) were collected and analyzed. The statistical analysis included descriptive statistics, Poisson distribution analysis to evaluate the adverse effects of atmospheric pollutants on daily hospitalizations, and subgroup analysis to determine whether gender and age could modify the impact on hospitalizations. RESULTS: A substantial correlation was revealed in single-day lags model. The peak delayed effects of PM₁₀, PM_2.5, SO₂, and NO₂ were observed at lag8 (PM₁₀ (OR = 1.016, 95%CI 1.002, 1.030), PM_2.5 (OR = 1.027, 95%CI 1.007, 1.048), SO₂ (OR = 1.153, 95%CI 1.040, 279) and NO₂ (OR = 1.054, 95%CI 1.005, 1.105)) while males exhibited a consistent trend from lag0 to lag8 (PM₁₀ (OR = 1.035, 95%CI 1.018, 1.053), PM_2.5 (OR = 1.056, 95%CI 1.030, 1.082), SO₂ (OR = 1.220, 95%CI 1.072, 1.389), NO₂ (OR = 1.126, 95%CI 1.061, 1.120), CO (OR = 10.059, 95%CI 1.697, 59.638) and O₃ (OR = 0.972, 95%CI 0.946, 0.999)). When gender and age were considered, a positive impact was also observed after three days cumulative effect in males. CONCLUSIONS There is a significant cumulative effect of exposure to air pollution on IS hospital admissions, especially the males and patients under the age of 65. Our results also suggested that a notable association between CO and NO₂ in two-pollutant models.

Robot-assisted surgery with its advantages such as three-dimensional high-definition vision, dexterous robotic arms, and tremor filtration, is increasingly being applied to complex radical gastrectomy. However, the role of the surgical assistant remains crucial during the procedure. The assistant is responsible for tasks outside the console, including adjusting robotic arms, changing instruments, exposing the surgical field, and addressing unexpected situations. The technical proficiency of the assistant and their collaboration efficiency with the primary surgeon directly impact the smoothness of surgery and patients' outcomes. With the expansion of robot-assisted surgical indications, the establishment of a standardized training system and the optimization of team collaboration models have become urgent challenges to address. This article draws on the author's practical experience as an assistant in robot-assisted gastric cancer surgeries, conducting an in-depth analysis of the responsibilities and operational skills of surgical assistants in robot-assisted procedures. The aim is to develop a relatively comprehensive set of operational guidelines for surgical assistants in robot-assisted radical gastrectomy, providing valuable references for enhancing the overall efficiency of surgical teams and improving surgical outcomes.

Objective To investigate the molecular typing,virulence,and drug resistance characteristics of Staphylococcus aureus(SA)in bone and joint infections,providing basis for anti-infection treatment.Methods The SA strains isolated from inpatients with bone and joint infections in Beijing Jishuitan Hospital,Capital Medical University from January 2014 to December 2021 were collected.Multi-locus sequence typing(MLST)and Staphylococcal A protein(Spa)typing for all the strains and Staphylococcal cassette chromo-some mec(SCCmec)typing of methicillin-resistant Staphylococcus aureus(MRSA)were performed based on whole genome sequencing.The virulence genes and drug resistance genes of the strains were identified by online database.The antimicrobial susceptibility tests were carried by automatic microbiological assay system.Results MRSA accounted for 30.0％of the 100 isolated strains of SA.A total of 22 ST types and 39 Spa types were identified in the 100 strains of S.aureus,among which ST59(16.0％)and ST239(14.0％)were the dominant ST types,and t437(13.0％)and t030(10.0％)were the dominant Spa types.ST239-SCCmecⅢ-t030/t037 clone(46.7％)was the main epidemic clone in MRSA isolates.The biofilm gene(icaA,icaB,icaC,icaD,icaR),hemolysin gene(hlb,hld,hlgA,hlgB,hlgC),adhesion gene(clfA,clfB,fnbA,fnbB,ebp),and immune escape gene(adsA,sbi,scn)were widespread in all SA strains,with detection rates ranging from 89.0％to 100.0％.The detection rates of enterotoxin genes seb(43.3％),selk(93.8％)and selq(83.3％)in MRSA were significantly higher than those in methicillin-sensitive Staphylococcus aureus(MSSA)(all P＜0.05).In terms of drug-resistance characteristics,the detection rate of the resistance gene blaZ(87.0％)was the highest among all the S.aureus strains.In the isolated MRSA strains,the detection rate of resistance genes for erm(A),tet(M),aph(3')-Ⅲ,ant(6)-Ⅰa,ant(9)-Ⅰa,and aac(6')-aph(2")ranged from 43.3％to 50.0％,which were significantly higher than those in MSSA(all P＜0.05).The results of the drug-sensitivity test showed that the resistant rates of S.aureus strains to penicillin,erythromycin,and clindamycin were relatively high(89.0％,67.0％,and 51.0％,respectively).The resistant rates of MRSA to the antimicrobial agents commonly used in clinical practice were significantly higher than those of MSSA(all P＜0.05).Conclusion The molecular epidemiological char-acteristics of SA strains isolated from bone and joint infections were diversified in our hospital.ST239-SCCmecⅢ-t030/t037 was the most common epidemic clone among the strains.There were significant differences in the resistance genes and drug resistance rates be-tween MRSA and MSSA strains,for which clinical attention should be paid.

Thoracic anesthesia with spontaneous respiration represents a form of precision anesthesia meticulously customized to individual patients.Considering the more stringent requirements this anesthesia approach imposes on the regulation of respiratory function,the writing group of the"Consensus of Experts on the Management of Thoracic Anesthesia with Spontaneous Respiration"has formulated elaborate guidelines regarding indications and contraindications,preoperative evaluation,anesthesia implementation,common complications,and treatment strategies.This was accomplished by referencing relevant domestic and international literature and integrating it with actual clinical requirements.The objective is to standardize the rational application of this anesthesia method.

The surgical treatment of thoraco-abdominal aortic aneurysm (TAAA) requires a unique multidisciplinary approach. A thorough preoperative examination and evaluation are essential to determine the optimal timing for surgery and to optimize organ function as needed. During the perioperative period, excellent surgical skills and an appropriate strategy for extracorporeal circulation will be employed based on the extent of the aneurysm. Additionally, necessary measures will be taken to monitor and protect the functions of vital organs. Close monitoring and management in the postoperative stage, along with early detection of complications and effective treatment, are crucial for improving the prognosis of TAAA surgery. This article reviews the current research progress in the perioperative management of TAAA surgery.

Brain metastases are the most common intracranial tumors, and their incidence is increasing with the improvement of systemic treatments and survival rates. Optimal treatment usually requires a multidisciplinary approach, including radiotherapy, surgical resection, chemotherapy, targeted therapy, and immunotherapy. Stereotactic radiotherapy, compared to whole-brain radiotherapy, offers improved local control rates and reduced risk of neurocognitive impairment, and has become a new standard option for the treatment of brain metastases. Additionally, the widespread use of targeted and immune therapies in brain metastases has significantly improved the survival of some patients. This article reviews and integrates recent literature on the treatment of brain metastases and analyzes the role of stereotactic radiotherapy in comprehensive treatment, aiming to provide a reference for the selection of clinical treatment plans.

Cognitive dysfunction has emerged as a significant health challenge in aging societies, among which Alzheimer's disease(AD)and related dementia have become one of the leading causes of death among the elderly.These diseases are characterized by insidious onset and progressive deterioration, often reaching irreversible advanced stages at initial diagnosis, making early detection and prevention imperative.As the frontline of public health management, communities bear the responsibility for screening and managing major chronic diseases.However, community-based cognitive screening in China still remains in its nascent stages, lacking systematic management and prevention guidelines.This guideline, established under a multidisciplinary collaborative framework and adhering to evidence-based medicine principles, systematically reviews and analyzes the latest clinical evidence worldwide.Utilizing the GRADE(Grading of Recommendations Assessment, Development and Evaluation)system to evaluate evidence quality and following the RIGHT(Reporting Items for Practice Guidelines in Healthcare)statement for standardized guideline development, it ultimately presents 17 recommendations through three rounds of Delphi expert consultations.The core recommendations of this guideline is mainly to use digital cognitive scales to conduct large-scale community cognitive screening for the active individuals aged 60 or above.Stratification and staging should be performed based on screening results, and digital cognitive files with quantified risk factor grading should be established, followed by the formulation of personalized artificial intelligence(AI)prevention and control programs.This guide is the first in China and internationally to propose a quantitative grading system for low, medium, and high-risk factors associated with AD and vascular dementia(VD). It aims to provide standardized principles and practical pathways to advance China's ambitious goals: achieving ≥80% public awareness of dementia prevention knowledge, ≥80% cognitive screening rate among the elderly, ≥80% intervention guidance rate for at-risk populations, and ≥50% medical consultation rate for suspected cognitive impairment cases.
