1.Allogeneic intrastromal lenticule implantation combined with corneal collagen cross-linking for moderate to advanced keratoconus

AIM: To evaluate the safety and efficacy of allogeneic intrastromal lenticule implantation combined with corneal collagen cross-linking(CXL)in patients with moderate to advanced keratoconus.METHODS: A retrospective case series analysis was conducted. A total of 19 patients(20 eyes)with moderate to advanced keratoconus who underwent combined allogeneic intrastromal lenticule implantation and CXL at the Jinan Mingshui Eye Hospital from June 2021 to December 2023 were included. The uncorrected distance visual acuity(UCVA), thinnest corneal thickness, central corneal epithelial thickness, anterior corneal flat keratometry(Kf), steep keratometry(Ks), and mean keratometry(Km), as well as the first applanation time(A1T), the first applanation length(A1L), the velocity during the first applanation moment(VIN), the second applanation time(A2T), the second applanation length(A2L), the velocity during the second applanation moment(VOUT), highest concavity time(HCT), highest concavity radius(HCR), peak distance(PD), deformation amplitude(DA), stiffness parameter at first applanation(SP-A1), integrated radius(IR), central corneal thickness(CCT), intraocular pressure(IOP), corneal thickness-corrected IOP, biomechanically intraocular pressure IOP(bIOP), and corneal thickness variation rate(ARTH)were compared between the two groups before surgery and at 1 wk, 1, 3 and 6 mo after surgery.RESULTS: All patients successfully completed the surgery without any intraoperative complications. No significant differences were observed between pre-operative and post-operative measurements for UCVA or the corneal biomechanical parameters, including A1L, A2L, PD, A1T, A2T, VIN, VOUT, DA, IOP, and bIOP(all P>0.05). Significant differences were found between pre-operative and post-operative values for corneal thinnest point thickness, central corneal epithelial thickness, Kf, Ks, Km, and the corneal biomechanical parameters, including HCT, HCR, SP-A1, ARTH, IR, and CCT(all P<0.05). The anterior corneal curvature demonstrated an initial increase followed by a decrease post-operatively. Furthermore, significant differences were observed between pre-operative and post-operative values for HCT, HCR, SP-A1, ARTH, IR, and CCT(all P<0.005).CONCLUSION: Allogenic intrastromal lenticule implantation combined with corneal collagen cross-linking demonstrates favorable safety and stability in treating moderate-to-advanced keratoconus. This combined procedure effectively increases corneal thickness and rigidity, resulting in corneas that are more resistant to deformation postoperatively.
2.Progress of different cyclotorsion compensation methods and theirs effects on small incision lenticule extraction

Cyclotorsion of the eye is a factor that affects the accuracy of astigmatic correction after small incision lenticule extraction(SMILE), potentially leading to unexpected outcomes such as astigmatic undercorrection and an increase in higher-order aberrations. One possible explanation for these issues is the lack of active rotational tracking software in the VisuMax femtosecond laser system. Accurately measuring ocular cyclotorsion can effectively guide the design of the surgery and improve the postoperative visual quality. Several methods have been developed to measure and compensate for cyclotorsion during SMILE. This article reviews the characteristics of cyclotorsion in refractive surgery, the methods of cyclotorsion compensation, and the impact of cyclotorsion compensation on the outcomes of SMILE.
3.Transjugular intrahepatic portosystemic shunt for hepatic sinusoidal obstruction syndrome caused by Gynura segetum:analysis of the mid-to-long-term survival

Objective To explore the effect of transjugular intrahepatic portosystemic shunt(TIPS)in treating patients with hepatic sinusoidal obstruction syndrome(HSOS)caused by gynura segetum,and to analyze the mid-to-long-term survival.Methods The clinical data of 44 patients with HSOS caused by gynura segetum,who were admitted to the First Affiliated Hospital of University of Science and Technology of China from April 2016 to September 2022 to receive TIPS treatment,were retrospectively analyzed.The preoperative and postoperative liver and kidney functions,ascites,and portal vein pressure were compared.Ultrasonography examination was performed to check the patency of stent and the thickness of ascites,the postoperative complications were recorded.Kaplan-Meier curve was used to calculate the cumulative survival so as to evaluate the survival of patients.Results Based on the severity of the illness,the 44 patients were divided into mild group(n=5),moderate group(n=13),severe group(n=19)and very severe group(n=7).Successful TIPS operation was achieved in all the patients,with a surgical success rate of 100%.No serious complications occurred.After TIPS,the incidence of hepatic encephalopathy(HE)was 20.5%(9/44).The portal pressure decreased from preoperative(42.00±0.91)cm H2 O to postoperative(18.27±0.67)cm H2O,the difference was statistically significant(P<0.001).The postoperative blood creatinine,aminotransferase,total bilirubin and Child-Pugh score were decreased when compared with their preoperative values,while the albumin level was increased when compared with its preoperative value,the differences were statistically significant(all P<0.05).The postoperative one-year and 5-year cumulative survival rates were 90.9%and 88.6%respectively.The 1-,3-,6-and 12-month survival rates in mild group were all 100%,in moderate group were 100.0%,100.0%,92.3%and 92.3%respectively,in severe group were 94.7%,94.7%,94.7%and 89.5%respectively,and in very severe group were 85.7%,71.4%,71.4%and 71.4%respectively.Conclusion For the treatment of HSOS caused by gynura segetum,TIPS can significantly improve the patient's ascites with low overall complication rate and high mid-to-long-term survival rate.
4.Refractive reconstruction of keratoconus

Keratoconus is a progressive disease that seriously affects the eyesight of young people. Keratoconus often causes high myopia, irregular astigmatism, and some patients with advanced corneal scar, visual acuity significantly decreased. In the course of diagnosis and treatment of keratoconus, refractive reconstruction of keratoconus is the focus of clinical attention. After years of research and exploration, a variety of treatment methods in different stages of disease(non-progressive, progressive, advanced)for keratoconus patients refractive reconstruction, so that they can obtain good visual function. This paper mainly reviews the refractive reconstruction schemes of keratoconus in different periods, analyzes the advantages and disadvantages of various treatment schemes at home and abroad, and summarizes the characteristics of different treatment methods, with a view to providing theoretical reference and new treatment ideas for the diagnosis and treatment of keratoconus.
5.Carrier screening for 223 monogenic diseases in Chinese population:a multi-center study in 33 104 individuals

Objective To investigate the epidemiological characteristics and mutation spectrum of monogenic diseases in Chinese population through a large-scale,multicenter carrier screening.Methods This study was conducted among a total of 33 104 participants(16 610 females)from 12 clinical centers across China.Carrier status for 223 genes was analyzed using high-throughput sequencing and different PCR methods.Results The overall combined carrier frequency was 55.58%for 197 autosomal genes and 1.84%for 26 X-linked genes in these participants.Among the 16 669 families,874 at-risk couples(5.24%)were identified.Specifically,584 couples(3.50%)were at risk for autosomal genes,306(1.84%)for X-linked genes,and 16 for both autosomal and X-linked genes.The most frequently detected autosomal at-risk genes included GJB2(autosomal recessive deafness type 1A,393 couples),HBA1/HBA2(α-thalassemia,36 couples),PAH(phenylketonuria,14 couples),and SMN1(spinal muscular atrophy,14 couples).The most frequently detected X-linked at-risk genes were G6PD(G6PD deficiency,236 couples),DMD(Duchenne muscular dystrophy,23 couples),and FMR1(fragile X syndrome,17 couples).After excluding GJB2 c.109G>A,the detection rate of at-risk couples was 3.91%(651/16 669),which was lowered to 1.72%(287/16 669)after further excluding G6PD.The theoretical incidence rate of severe monogenic birth defects was approximately 4.35‰(72.5/16 669).Screening for a battery of the top 22 most frequent genes in the at-risk couples could detect over 95%of at-risk couples,while screening for the top 54 genes further increased the detection rate to over 99%.Conclusion This study reveals the carrier frequencies of 223 monogenic genetic disorders in the Chinese population and provides evidence for carrier screening strategy development and panel design tailored to the Chinese population.In carrier testing,genetic counseling for specific genes or gene variants can be challenging,and the couples need to be informed of these difficulties before testing and provided with options for not screening these genes or gene variants.
6.Correlation between metabolic markers and blood lipid levels in kidney transplant recipients

Objective To analyze the co-expressed genes in blood lipid metabolism,hyperlipidemia and tacrolimus metabolism and their correlation with blood lipid levels in kidney transplant recipients.Methods Co-expressed genes were screened from Comparative Toxicogenomic Database(CTD).Baseline data of 25 kidney transplant recipients were collected.The expression levels of ATP binding cassette subfamily A member 1(ABCA1),peroxisome proliferator activated receptor γ(PPAR-γ)and glycosylphosphatidylinositol anchored high density lipoprotein binding protein 1(GPIHBP1)were measured.All recipients were followed up.The concentrations of fasting blood glucose,glycosylated hemoglobin,triglyceride,total protein,albumin,globulin,cholesterol,high-density lipoprotein,low-density lipoprotein and tacrolimus blood concentration were collected at postoperative 1,3,6 and 12 months,and the incidence of hyperlipidemia in the recipients was analyzed.The correlation between ABCA1,GPIHBP1,PPAR-γ and clinical indexes was assessed.The diagnostic effiiciency of related indexes for hyperlipidemia after kidney transplantation was evaluated.Results Three co-expressed genes including ABCA1,PPAR-γ and GPIHBP1 were screened.ABC Al was positively correlated with cholesterol level at postoperative 6 months and tacrolimus blood concentration at postoperative 3 months,whereas negatively correlated with fasting blood glucose level at postoperative 3 months(all P<0.05).GPIHBP1 was negatively correlated with preoperative cholesterol and triglyceride levels,whereas positively correlated with tacrolimus blood concentration at postoperative 3 months(all P<0.05).PPAR-γ was negatively correlated with preoperative globulin and low-density lipoprotein levels(both P<0.05).ABCA1,GPIHBP1 and PPAR-γ combined with preoperative globulin and blood glucose level at postoperative 1 and 6 months after operation yielded high diagnostic effiiciency for hypertriglyceridemia after kidney transplantation(AUC=0.900).ABCA1,GPIHBP1 and PPAR-γ combined with tacrolimus blood concentrations at postoperative 1 and 6 months and blood glucose level at postoperative 6 months had high diagnostic effiiciency for hypercholesterolemia after kidney transplantation(AUC=0.931).Conclusions ABCA1,GPIHBP1 and PPAR-γ are correlated with blood lipid level and tacrolimus blood concentration after kidney transplantation to different degrees.No definite evidence has been supported for predicting hyperlipidemia after kidney transplantation.Immunity improvement and rational blood glucose management may be beneficial factors for hyperlipidemia control.

8.Effects of electroacupuncture on the integrity of myelin and the expressions of myelin-related proteins in the spinal dorsal horn of neuropathic pain model rat

Objective:To investigate the effect of electroacupuncture treatment on the integrity of myelin and the related molecules in the spinal dorsal horn of neuropathic pain model rat.Methods:Totally 32 SPF grade Sprague-Dawley rats were randomly divided into 4 groups: sham group, pain model group, electroacupuncture group and sham electroacupuncture group, with 8 rats in each group. Neuropathic pain model was established by right sciatic nerve ligation. Electroacupuncture stimulation of Huantiao point and Yanglingquan point was started from 1 day after surgery, 30 min/d, and lasted for 14 days. The paw withdraw mechanical threshold (PWMT) were tested before surgery, 3 days, 7 days and 14 days after surgery. Immunofluorescence was used to detect myelin basic protein (MBP) in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn and Western blot was used to measure the expression of β-secreatase 1(BACE1), neuregulin 1 type Ⅲ (NRG1 Ⅲ) and phosphorylated ErbB receptor tyrosine kinase 2 (p-ErbB2) at 14 days after surgery.SPSS 24.0 was used for statistical analysis.Repeated measurement ANOVA was used for behavioral analysis and one-way ANOVA followed by Bonferroni test was used for Western blot and immunofluorescence data analysis.Results:(1) The results of pain behaviors showed that the interaction effect of time and group on PWMT was significant ( F=29.817, P<0.001), and both the time main effect ( F=240.598, P<0.001) and group main effect ( F=304.291, P<0.001) were significant. There were no significant differences among 4 groups before surgery. Compared with pain model group, the PWMT of electroacupuncture group were significantly increased at 3 days ((16.87±1.82) g, (11.31±1.36) g), 7 days ((15.09±1.75) g, (10.33±0.73) g) and 14 days ((15.07±1.49) g, (9.87±0.98) g) after surgery (all P<0.01). (2)The difference of the intensity of MBP in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn staining was significant among 4 groups ( F=92.06, P<0.001). Compared with sham group, the intensity of MBP staining in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn in pain model group was significantly decreased ((13.26±1.90)%, (36.37±0.68)%) ( P<0.01) ). Compared with pain model group, the intensity of MBP staining in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn in electroacupuncture group ((28.21±3.15)%) was significantly increased after electroacupuncture treatment ( P<0.01). (3) The differences of the expressions of BACE1, NRG1 Ⅲ and p-ErbB2 in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn were significant among the 4 groups ( F=31.04, 21.20, 11.74, all P<0.01). Compared with sham group, the expressions of BACE1, NRG1 Ⅲ and p-ErbB2 in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn in pain model group were significantly downregulated (BACE1 (0.42±0.09), (1.16±0.13); NRG1 Ⅲ (0.54±0.05), (1.00±0.10); p-ErbB2 (0.73±0.06), (1.02±0.15) ) (all P<0.05). Compared with pain model group, the expression of BACE1 (0.86±0.09), NRG1 Ⅲ (0.81±0.05) and p-ErbB2 (1.12±0.04) in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn in electroacupuncture group were upregulated(all P<0.05). Conclusion:Electroacupuncture treatment could relieve neuropathic pain, improve the demyelination and upregulate the expressions of myelin-related proteins in the ipsilateral Ⅰ-Ⅲ laminae of spinal dorsal horn.
9.A case report of human infected with Thelazia callipaeda

Middle-aged women experienced foreign body sensation in both eyes without obvious causes for many days. The clinician tried to diagnose the patient according to her physical and laboratory examinations. After detecting worm bodies in the patient′s eye, and examining the worms with a microscope, the diagnosis of conjunctival sucking nematodes was confirmed. After targeted treatment, the patient′s symptoms improved significantly.
10.Comparison of the effect of two disinfectants on the independent waterway disinfection of a periodontal ultrasonic scaler

Objective :
To compare the disinfection effect of 3% (v/v) hydrogen peroxide and 500 mg/L chlorine-containing disinfectants in the independent waterway of a periodontal ultrasonic scaler to provide a reference for clinical waterway disinfection management in stomatology departments.
Methods :
The 18 ultrasonic scalers were randomly divided into 3 groups of 6 units: the control group, experimental group 1 (3% hydrogen peroxide disinfectant group), and experimental group 2 (500 mg/L chlorine-containing disinfectant group). The replaceable parts of the independent waterway pipes of the 3 groups of ultrasonic scalers were replaced, and the water supply was supplied with sterile distilled water (DW). In the control group, special treatment was not applied to the nonreplaceable pipe part. In experimental group 1, the 3% hydrogen peroxide was used to disinfect nonreplaceable pipelines. In experimental group 2, the nonreplaceable part was disinfected with the 500 mg/L chlorine-containing disinfectant. The water sample was taken from the outlet of the scaler working part in the three groups for monitoring before disinfection, immediately after disinfection and 10 consecutive days after disinfection. Bacteria in the water samples were cultured for the colony counts. Then, the bacterial culture data were compared between groups. The qualified criterion of the water sample was that the number of bacterial colonies was less than or equal to 100 CFU/mL. After disinfection, a bacterial species mass spectrometry identification analysis was carried out when the number of bacterial colonies in each group exceeded the standard for the first time. Biofilms from the inner wall of the tube in the three groups were observed under an electron microscope on the 10th day after disinfection.
Results :
There were no significant differences between the three groups before disinfection (F = 2.549, P = 0.111). The number of bacterial colonies in the spout of 6 scalers in the control group all exceeded the standard, and three kinds of bacteria were cultured: Sphingomonas melonis, Herbaspirillum huttiense, and Ralstonia pickettii. Compared with those in the control group, the number of bacterial colonies in experimental group 1 decreased significantly for 1-2 days after disinfection (P<0.05) and reached the standard. On the 3rd day after disinfection, the number of bacterial colonies of group 1 increased rapidly and exceeded the standard, and three kinds of bacteria were cultured: Sphingomonas, Herbaspirillum huttiense, and Ralstonia pickettii. For experimental group 2, the number of bacterial colonies decreased significantly compared to the control group on Days 1 to 6 after disinfection, but the number of bacterial colonies increased slightly from the 7th day after disinfection and exceeded the standard. Two kinds of bacteria were cultured: Herbaspirillum huttiense and Ralstonia pickettii. The average number of bacterial colonies 10-day after disinfection in experimental group 2 was lower than that in experimental group 1(P<0.001). Under an electron microscope, the biofilm thickness of the two experimental groups was significantly lower than that of the control group.
Conclusion
There is water pollution in the independent waterway of a periodontal ultrasound scaler. Three percent hydrogen peroxide and 500 mg/L chlorine disinfectant both have effective disinfection effects on the outlet water of scalers, and the effect of 500 mg/L chlorine disinfectant is better than that of 3% hydrogen peroxide. The use of 3% hydrogen peroxide to disinfect periodontal ultrasound scaler-independent waterways is recommended for disinfection every other day, and disinfection once a week is recommended for the use of 500 mg/L chlorine disinfectant.
