1.Proposal for the Guidelines on Off-label Use of Common Psychiatric Medications in China

Off-label drug use in psychiatry has long been prevelent, with common clinical practices lacking standardized guidance and associated risks warranting close attention. To address the practical needs of frontline clinicians, the Mental Health Branch of the China National Narcotic Drugs Association, in collaboration with multiple institutions, has launched a project to formulate the Guidelines on Off-label Use of Common Psychiatric Medications in China ("Guideline"). The Guidelines focuses on evaluating evidence and formulating recommendations regarding off-label use scenarios of common psychiatric medications, including indications, dosage, administration, and specific populations. It employs standard methodological frameworks, notably the GRADE-ADOLOPMENT approach (Grading of Recommendations Assessment, Development, and Evaluation Evidence to Decision frameworks for adoption, adaptation, and de?novo development of recommendations), to ensure both scientific rigor and practical applicability. This proposal systematically outlines the background, objectives, processes, organizational structure, and methodologies of the Guideline, aiming to provide psychiatrists with practical and evidence-based prescribing recommendations.

3.Effect of transient receptor potential channel 5 on myocardial pyroptosis in intermittent hypoxia

Objective To investigate the effect of TRPC5 on intermittent hypoxia-induced myocardial pyroptosis.Methods TRPC5 knock out(TRPC5-/-)and SD rats were each randomly divided into chronic intermit-tent hypoxia group(CIH group)and normoxia group(Control group)(n=6 for each group).The myocardial fibrosis in rats was visualized by Masson staining,the levels of serum inflammatory factors were measured by ELISA and the relative expression levels of TRPC 5 and pyroptosis-related proteins were determined by Western blotting.Results Masson staining showed that the collagen volume fraction in the TRPC5-/--CIH group was higher than that in the TRPC5-/--Control group and lower than that in the WT-CIH group.The ELISA results showed that the serum levels of IL-1,IL-6,and TGF-β in the TRPC5-/--CIH group were higher than those in the TRPC5-/--Control group and lower in the WT-CIH group.Western blotting results showed that the relative expression levels of pyroptosis related proteins caspase-1,NLRP3,GSDMD,and GSDMD-N in the TRPC5-/--CIH group were higher than those in the TRPC5-/--Control group and lower than those in the WT-CIH group.Conclusion TRPC5 deficiency alleviates myocardial pyroptosis and myocardial fibrosis induced by hypoxia.
4.Antimicrobial coated medical catheters and their progress in enhanced recovery after surgery nursing

Enhanced recovery after surgery has reduced the number and duration of medical catheter placements through perioperative optimization measures. However, in clinical practice, catheters are still inevitably heavily used, and the risk of catheter-related infections persists. Preventing bacterial adhesion and biofilm formation on catheter surfaces is crucial. Medical antibacterial-coated catheters introduce substances that prevent fouling and adhesion, inhibit or kill bacteria on the surface, forming an antibacterial functional surface, providing a new strategy to address catheter-related infections. This article summarizes the current research status of medical antibacterial-coated catheters, introduces the antibacterial strategies and principles of the coatings, describes their actual effects in clinical applications, and analyzes future research directions. These studies help reduce catheter-related infections, promote innovation in clinical nursing technology, and improve the quality and efficiency of nursing care.

6.Predictive value of vitamin D,sex hormones and their receptors on relapse in male patients with clinically isolated syndrome

Objective To investigate the predictive value of vitamin D,sex hormones and their receptors in the development and regression of male patients with clinical isolated syndrome(CIS).Methods A total of 40 male CIS patients were included in the CIS group and 30 healthy subjects matched in age and long-term res-idence were included in the control group.The levels of vitamin D[1,25(OH)2D2 and 1,25(OH)2D3]inpe-ripheral blood of the included individuals and the first relapsed multiple sclerosis patients(MS group)were detected by ultra-high-performance liquid chromatography-tandem mass spectrometry.The levels of testoster-one(T),progesterone(P),and estradiol(E2)in peripheral blood were detected by electrochemiluminescence.Estrogen receptor(ERα,ERβ),androgen receptor(AR)and vitamin D receptor(VDR)levels in lymphocyte supernatant were detected by enzyme-linked immunosorbent assay(ELISA).Results After a median follow-up of 31.5 months,29 patients(72.5%)with CIS were converted to multiple sclerosis.Compared with pa-tients with non-recurrent CIS patients,patients with multiple sclerosis had a younger age of onset and were prone to positive oligoclonal bands.Compared with HC group,vitamin D levels were reduced in CIS patients,and the difference was statistically significant(P<0.05).When CIS patients reverted to multiple sclerosis,se-rum levels of E2,ERα,and ERβ were elevated compared with those before relapse,and the levels of testosteroneand AR were decreased,and the difference was statistically significant(P<0.05).Correlation analysis showed no correlation between peripheral blood levels of vitamin D,sex hormones and their receptors and EDSS in CIS patients.Conclusion Male CIS patients with younger onset age and positive oligoclonal bands are prone to transition to multiple sclerosis.The decrease of vitamin D level may be associated with the onset of male CIS.Vitamin D,sex hormones,and their receptors have no predictive value in the conversion of CIS to multiple sclerosis.
7.Establishment of layered training system of ultrasound medicine based on physician and sonographer collaboration

In order to meet the social demand for ultrasound talents, a layered training system of ultrasound medical talents suitable for China's national conditions has been explored. The department of ultrasound of West China Hospital of Sichuan University has formulated and implemented a "three-layers" ultrasound education and training system in combination with its own reality. The first layer mainly refers mastering the basic ultrasound examination ability mainly through relevant ultrasound theoretical knowledge and basic operation skills training. The second layer is based on the first level to further master the ability of ultrasound diagnosis and differential diagnosis of common and frequently occurring diseases in various systems, and to be capable of engaging in basic ultrasound teaching and scientific research. The third layer means mastering the ability of ultrasound diagnosis, evaluation and interventional diagnosis and treatment of difficult and specialized diseases, grasping the latest development direction of the sub-specialty, and having the ability to engage in ultrasound teaching and clinical research. The results show that the "three-layers" ultrasound education and training system conforms to China's national conditions and is worthy of reference and promotion.
8.Association analysis of the polymorphisms of dopamine-β-hydroxylase and norepinephrine transporter genes with panic disorder

Objective:To explore the association between dopamine-β-hydroxylase (DβH), norepinephrine transporter (NET) gene polymorphisms and panic disorder(PD).Methods:The structured clinical interview for the diagnostic and statistical manual of mental disorders fourth edition (DSM-Ⅳ) axis Ⅰ disorders was administered by trained clinical psychiatrist, 139 patients with PD(PD group) and 196 healthy controls(control group) were enrolled in the study.Single nucleotide polymorphism(SNP) genotyping was performed using an improved multiplex ligation detection reaction technique.SPSS 16.0 and PLINK softwares were used to compare the allele frequency and genotype distribution.Results:(1)Compared with control group, PD group carried more G allele(76.3% vs 68.4%) and fewer A allele(23.7% vs 31.6%) in NET rs5569, and the difference was significant(χ 2=4.986, OR=0.67, 95% CI: 0.47-0.95, P<0.05). However, the correlation was no longer significant after adjusting for Bonferroni’s multiple testing( P>0.05). (2)The additive model of NET rs5569 showed a association with PD ( OR=0.68, 95% CI: 0.48-0.96, P<0.05). And the recessive model of DβH rs1611114 showed a association with PD( OR=0.42, 95% CI: 0.18-0.96, P<0.05). However, these correlations were no longer significant after adjusting for Bonferroni's multiple testing( P>0.05). (3)No matter allele or genotype, there were no significant differences in DβH (rs129882, rs1611114, rs1611115) and NET (rs2242446, rs28386840) gene polymorphisms between panic disorder group and control group(all P>0.05). Conclusion:The present study indicates that there is no significant association of DβH and NET gene polymorphisms with PD.
9.Association study of the BDNF promoter methylation status with panic disorder

Objective:To explore whether brain-derived neurotrophic factor (BDNF) promoter methylation status is associated with panic disorder(PD), and then assess the effect of the BDNF gene methylation status on the severity of clinical symptoms in PD.Methods:The methylation levels of the BDNF gene were compared between 111 patients with PD and 130 matched healthy controls using MethylTarget approach.In addition, the panic disorder severity scale(PDSS), Hamilton anxiety rating scale(HAM-A), and Hamilton depression rating scale(HAM-D) were respectively assessed to all subjects.Results:(1)The result showed that 7 CpG regions from the promoter regions of the BDNF gene were sequenced.However, there was no statistically significant differences between cases and controls in terms of BDNF DNA methylation status ( OR=1.087, 95% CI=0.849-1.391, P>0.05). (2)Spearman correlation analysis revealed that the hypermethylation of BDNF gene was significantly associated with the severity of the depressive symptoms in PD patients (all P<0.05). The methylation levels of BDNF gene was not significantly related to the severity of anxiety and panic in PD patients(all P>0.05). Conclusion:No association between BDNF promoter methylation status and panic disorder is found in Chinese Han population, but BDNF promoter methylation status may be related to the severity of depressive symptoms in patient with panic disorder.
10.Potential risk factors for mild cognitive impairment in the elderly population in communities of Shanghai

Objective To investigate the potential risk factors for mild cognitive impairment (MCI) in the elderly population in the community, and to provide a basis for the primary prevention of MCI. Methods A cross-sectional study of elderly population in communities of Shanghai, China was conducted. A total of 368 subjects including both males and females, aged 65-80 years old, were selected to complete the mini-mental state examination (MMSE), basic information questionnaires, and physical examinations. Logistic regression analysis was used to analyze the potential risk factors of MCI. Results Of the 368 subjects participating in the study, 53 were found to have MCI and the prevalence rate was 14.4%. Univariate analysis found that older age, low education, no folic acid supplementation, stroke, osteoporosis and hyperlipidemia were risk factors of MCI. Multiple logistic regression analysis showed that advanced age [OR=1.146 (95%CI: 1.052-1.249)] and osteoporosis [OR=2.371 (95%CI: 1.042-5.396)] were the independent risk factors for MCI, while higher education [OR=0.073 (95%CI: 0.011-0.478)] was a protective factor. Age influenced all the aspects of MMSE scores (all P values <0.05). In addition, the analysis of the results suggested that subjects with regular folic acid supplementation got higher MMSE scores, especially in the aspect of language and praxis (P=0.002). On the contrary, patients with osteoporosis had lower attention and computing power scores (P=0.022). Conclusion The prevalence of MCI increased with age. Low education and osteoporosis may be the independent risk factors for MCI in the elderly population. Although no association was observed between folic acid supplementation and MCI, folic acid supplementation could improve the performance of language and praxis.
