Objective To observe the value of contrast-enhanced ultrasound(CEUS)for diagnosing splenic lymphoma.Methods Eighteen patients with pathologically confirmed splenic lymphoma were retrospectively collected.The routine ultrasound and CEUS manifestations of lesions were observed,and the diagnostic accuracy of these two methods for diagnosing splenic lymphoma were compared.Results Among 18 cases,conventional ultrasound showed single lesion in 8(8/18,44.44％)and multiple lesions in 10 cases(10/18,55.56％).The maximum diameter of lesions was(5.43±2.03)cm.For the largest lesion(target lesion)in 18 cases,5(5/18,27.78％)were found with very low echo,11(11/18,61.11％)with low echo and 2(2/18,11.11％)with mixed echo.The internal echo was uneven in 10 lesions(10/18,55.56％)while uniform in 8 lesions(8/18,44.44％),8(8/18,44.44％)shaped regularly but 10(10/18,55.56％)irregularly,15(15/18,83.33％)with clear and 3(3/18,16.67％)with unclear boundaries.CDFI showed blood flow signals in 10(10/18,55.56％)but not in 8 lesions(8/18,44.44％).During CEUS of 18 target lesions,no enhancement was detected in 1 lesion(1/18,5.56％),peripheral to central enhancement,central to peripheral enhancement and synchronous enhancement was observed in 12(12/18,66.67％),2(2/18,11.11％)and 3 lesions(3/18,16.67％),respectively,which at the peak mainly presented as low(8/18,44.44％)or high(7/18,38.89％)enhancement,while no enhancement or equal enhancement was noticed in 1(1/18,5.56％)and 2(2/18,11.11％)lesions,respectively.Uniform and uneven distribution of contrast agents was found in 5(5/18,27.78％)and 12(12/18,66.67％)lesions,respectively,while 1 lesion(1/18,5.56％)without distribution of contrast agents.Annular and dendritic enhancement in peripheral area was observed in 7(7/18,38.89％)and 5 lesions(5/18,27.78％),while nodular enhancement(3/18,16.67％)or no enhancement(3/18,16.67％)was noticed each in 3 lesions.The scope enlarged after enhancement in 1 lesion(1/18,5.56％),while 17 lesions(17/18,94.44％)did not increase significantly.In the late stage of enhancement,16 lesions(16/18,88.89％)showed low enhancement,while 2(2/18 11.11％)showed no enhancement.Among 18 target lesions,CEUS showed blood supply in 17 lesions(17/18,94.44％),while no blood supply was found in 1 lesion(1/18,5.56％).CEUS correctly diagnosed 11 cases of splenic lymphoma,with diagnostic accuracy of 61.11％(11/18),higher than that of conventional ultrasound([4/18,22.22％],P=0.020).Conclusion CEUS had important clinical value for diagnosing splenic lymphoma.

Objective This study aimed to investigate the antimicrobial resistance profiles of bacterial strains isolated from pediatric intensive care units(PICU)in China for better antimicrobial therapy.Methods Clinical isolates were collected from 17 institutions,including tertiary care children's hospitals and pediatric department of tertiary general hospitals in China from January 1,2020 to December 31,2022.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were interpreted according to the breakpoints released by the Clinical and Laboratory Standards Institute(CLSI)in 2020.Results A total of 10 688 isolates were collected,including gram-positive organisms(39.2％)and gram-negative organisms(60.8％).The top three organisms were S.aureus(13.6％,1 453/10 688),A.baumannii(10.0％,1 067/10 688),and coagulase-negative Staphylococcus(9.9％,1 058/10 688).Multi-drug resistant organisms(MDROs)were very common in children.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant E.coli,carbapenem-resistant K.pneumoniae(CRKP),carbapenem-resistant A.baumannii(CRAB),and carbapenem-resistant P.aeruginosa(CRPA)was 41.1％,19.4％,8.8％,30.9％,67.4％,and 28.8％,respectively.Overall,more than 50％of Enterobacteriales isolates were resistant to cephalosporins,while nearly 25％of Enterobacteriales isolates were resistant to carbapenems.MDROs were highly resistant to commonly used antibiotics.More than 80％of CRE and CRAB strains were resistant to all beta-lactam antibiotics.CRE and CRAB showed low resistance rates to tigecycline and polymyxin.CRPA showed lower resistance rates to piperacillin,beta-lactamase inhibitor combinations than the resistance rates to third and fourth generation cephalosporins.All of the Staphylococcus and Enterococcus isolates were susceptible to vancomycin and tigecycline.None of PRSP strains isolated from meningitis and nonmeningitis samples were resistant to rifampicin,vancomycin,or linezolid.The prevalence of β-lactamase-negative ampicillin-resistant(BLNAR)strains was 43.3％in Haemophilus influenzae.Conclusions MDROs were prevalent in PICU.It is necessary to establish an effective multidisciplinary team(MDT)to control the antimicrobial resistance.

Objective: To explore the intervention effects of high intensity interval training (HIIT) on glucose metabolism, cortisol (Cor), and sleep quality among college students with comorbid depressive symptoms and obesity, so as to provide a reference for improving sleep quality among college students with comorbid depressive symptoms and obesity. Methods: In March 2023, 45 college students with comorbid depressive symptoms and obesity were recruited and randomly assigned to an exercise group ( n =23) and a control group ( n =22) by random number table method. The exercise group received HIIT intervention for 12 weeks, three times a week, while the control group received no intervention. Blood samples were collected from participants to measure fasting insulin (FINS), fasting blood glucose (FBG), homeostatic model assessment of insulin resistance (HOMA-IR), Cor, and Pittsburgh Sleep Quality Index (PSQI) before and after intervention. Statistical analysis was performed using t-test, repeated measures analysis of variance (ANOVA), simple effect analysis. Results: The repeated measures ANOVA revealed statistically significant time×group interaction effects for body composition (weight, body mass index, percentage of body fat, fat mass, waist to hip ratio), depressive symptoms, PSQI scores and its subdimensions (subjective sleep quality, sleep onset time, sleep efficiency, sleep disorders, daytime dysfunction), as well as FBG, FINS, and HOMA-IR between the exercise group and control group before and after intervention ( F =7.10-53.38, all P <0.05). Simple effect analysis showed that compared to the control group, the exercise group demonstrated significant improvements in body composition (body mass index, fat mass, waist to hip ratio), depressive symptoms, PSQI scores and its sub dimensions (subjective sleep quality, sleep onset time, sleep efficiency, sleep disorders, daytime dysfunction), FBG, FINS, HOMA-IR, and Cor (all P <0.05). Conclusion HIIT can improve the sleep quality of college students with comorbid depressive symptoms and obesity by enhancing glucose metabolism and regulating Cor levels.

The rapid development of generative large language models(GLLM)has attracted considerable attention in research of imaging medicine,which showed promise in terms of potential applications in ultrasound medicine.At present,GLLM possessed the theoretical level of a medical practitioner and had the capacity to enhance productivity through generating reliable,structured ultrasound reports and assisted diagnoses,hence being significant valuable for reducing time costs and improving effectiveness of physician-patient communication.The research status of GLLM in ultrasound medical knowledge quizzes,structured ultrasound report generation,as well as diagnosis and management of disease were reviewed in this article.

Objective To analyze the metabolic changes of myocardial tissue in rats under acute exposure to macleaya cordata by gas chromatography mass spectrometry(GC-MS),explore forensic identifications of its characteristic metabolites,and verify its toxicological mechanism in poisoning cases.Methods The rats in the exposure group were given 382 mg/kg macleaya extract solution by gavage,and the rats in the control group were given the same dose of solvent.The myocardial samples were analyzed by GC-MS,and pattern recognition was conducted through partial least squares discriminant analysis(PLSDA).The differential metabolites with characteristic changes were identified by variable importance projection(VIP value＞1)and Student's t test(P＜0.01).Results Compared with the control group,21 potential characteristic metabolites were identified.Through KEGG pathway enrichment analysis,it was found that these metabolites were mainly involved in the pathways of glycine,serine and threonine metabolism;pyruvate metabolism and glycerolipid metabolism.Conclusion Through the study of myocardial metabolism in rats exposed to macleaya cordata,we found the information on metabolites closely related to poisoning,which provides new insight and reference for studies on the mechanisms of macleaya cordata poisoning in the field of forensic medicine.

Objective:To analyze the prevalence and disease burden of male and female infertility in China from 1990 to 2019.Methods:The global disease burden database of 2019 was used to analyze the prevalence of male and female infertility and disability adjusted life years (DALY) in China from 1990 to 2019 among the total Chinese male and female population and different age groups.Results:In 2019, the number, prevalence, DALY and DALY rate of male infertility in China were 14 577 432, 2 011.18/100 000, 77 983 person years, 1 076/100 000, respectively. The number, prevalence, DALY and DALY rate of female infertility were 37 884 300, 5 431.22/100 000, 197 046 person years, 28.25/100 000, respectively. The prevalence and disease burden of female infertility were higher than those of male infertility. The age group with the highest male prevalence rate and DALY rate were 35-39 and 30-34 years old respectively, the age group with the highest female prevalence rate and DALY rate were 40-44 and 35-39 years old respectively, and the age group with the highest female prevalence rate and DALY rate was 5 years later than the male age group. From 1990 to 2019, the prevalence rate and DALY rate of male and female infertility increased in the first 20 years or so, and decreased in the last 10 years or so. The prevalence rate and DALY rate of male and female infertility in China were significantly higher than those in the whole world and different socio-demographic index (SDI) regions.Conclusion:Although the prevalence rate and DALY rate of male and female infertility in China have decreased in recent years, the population base of China is large, and the disease burden of male and female infertility in the world is still large. Active prevention and treatment measures should be taken to reduce the disease burden of male and female infertility.
