Although the clinical importance of esophagogastric junction (EGJ) cancer is being increasingly recognized, its definition and treatment strategies vary considerably across regions. Recently, new concepts such as the gastroesophageal junction zone have been proposed for classification of EGJ cancer. Moreover, EGJ adenocarcinoma has been shown to possess a heterogeneous molecular profile consisting of both esophageal- and gastric-like phenotypes, indicating the need for EGJ-specific therapeutic strategies.Although international clinical practice guidelines for EGJ cancer have been published, substantial regional differences remain. This review aimed to summarize recent updates on the biological characteristics and management of EGJ cancers. Trials such as TIME and MIRO have demonstrated that minimally invasive surgery (MIS) reduces postoperative complications and improves the early postoperative quality of life. More recently, the MONET trial showed that MIS is not inferior to open esophagectomy in terms of long-term oncological outcomes, and the REVATE trial demonstrated the advantages of robot-assisted surgery over thoracoscopic approaches. Perioperative treatment strategies also differ across regions. Western guidelines recommend 5-fluorouracil, leucovorin, oxaliplatin, and docetaxel (FLOT)-based perioperative chemotherapy, as supported by the FLOT4 and ESOPEC trials, whereas Asian studies, such as RESOLVE and PRODIGY, have demonstrated the efficacy of docetaxel, oxaliplatin, and S-1 or S-1 plus oxaliplatin regimens. The application of immune checkpoint inhibitors is also evolving; although KEYNOTE-585 did not show a survival advantage, the MATTERHORN trial demonstrated that durvalumab plus FLOT significantly improved event-free survival, establishing a new emerging global standard. Future directions include redefining EGJ classification, implementing molecular-guided treatment selection, advancing organ-preserving strategies, and optimizing perioperative immunochemotherapy.

Purpose: In Korea, the Health Insurance Review and Assessment Service (HIRA) mandates patient participation in outpatient multidisciplinary team (MDT) meetings as part of national cancer quality assessment. However, the necessity of involving patients and families in MDT discussions remains debatable. This study explored international expert perspectives on patient participation in gastric cancer MDTs. Methods: A cross-national expert survey was conducted in September 2021 among 15 gastric cancer specialists from Korea, China, Japan, Singapore, and the United States. The survey assessed the frequency of patient and family member attendance, perceived pros and cons, and preferences regarding mandatory versus selective involvement. Additionally, MDT structures and policies were reviewed by countries. Results: Most respondents reported that patients and families rarely or never attend MDT meetings: nine stated that patients are never included, three reported “usually not,” and three indicated “sometimes.” None consistently included patients. The most cited benefits were sharing opinions with patients and families simultaneously, followed by improved explanation of treatment and legal protection. Major concerns included hindered discussion, inefficiency, and logistical challenges. Only four respondents supported routine participation, while 11 favored case-dependent involvement. Conclusion International experts do not widely support mandatory patient participation in MDT meetings. A flexible approach that allows MDTs to operate with or without patient involvement may better reflect actual clinical practice.

Introduction: This study aimed to determine the trends in the use of medicines that can affect the recognition function of patients aged ≧70 years. Since 2015 when the “Safe Drug Treatment Guidelines for the Elderly” was revised, these medicines are dispensed at a community pharmacy located in a satellite city. Methods: We compared the dispensing results from 2014 to 2015-2019 and investigated the ratio of prescriptions containing the medicines that can affect the recognition function using the clinical department and medicine type. Results: Prescriptions containing medicines that can affect recognition function have been on a downward trend since 2015. During this period, the number of medicines per prescription did not decrease, and the medicines that can affect recognition function were replaced with other medicines. On the other hand, no decrease in the number of medicines that can affect recognition function was observed in internal medicine. In medicine type, the ratio of benzodiazepines drugs decreased; however, it increased again in 2019. Conclusion: There was a decreasing trend in the use of medicines that can affect recognition function of the elderly in departments other than internal medicine after 2015.

Esophageal cancer has one of the highest malignant potentials of any type of tumor. The 3-field lymph node dissection is the standard procedure in Japan for surgically curable esophageal cancer in the middle or upper thoracic esophagus. Minimally invasive esophagectomy is being increasingly performed in many countries, and several studies report its feasibility and curability; further, the magnifying effect of the thoracoscope is another distinct advantage. However, few studies have reported that minimally invasive esophagectomy is more beneficial than open esophagectomy. A recent meta-analysis revealed that minimally invasive esophagectomy reduces blood loss, respiratory complications, the total morbidity rate, and hospitalization duration. A randomized study reported that the pulmonary infection rate, pain score, intraoperative blood loss, hospitalization duration, and postoperative 6-week quality of life were significantly better with the minimally invasive procedure than with other procedures. In the future, sentinel lymph node mapping might play a significant role by obtaining individualized information to customize the surgical procedure for individual patients' specific needs.

Total muscle mass is thought of as one of the intrinsic limiting factors of aerobic power. But it is not yet clarified how the total muscle mass limits the magnitude of aerobic power. The purpose of this study was to clarify the relationship between total muscle mass and aerobic power and to make a further discussion conscerning its limiting factors. 17 sedentary students with a mean age of 19.3 (18-23) years, and 17 varsity footballers 19.9 (19-21) years old, were measured for lean body mass, which was an approximate indicator of the total muscle mass, by underwater-weighing method and VO₂ max by bicycle ergometer. The correlationship between lean body mass and VO₂max for sedentary students was found to be higher (r=0.821) than for footballers (r=0.595) . It was also shown that footballers had a significantly larger VO₂max per 1kg of lean body mass than sedentary students. Previous studies showed that the increment of VO₂max through endurance training was caused by the increment of such factors as mitochondria, maximum cardiac output and maximum stroke volume, and was not by lean body mass. So, it was concluded that lean body mass was a reasonable limiting factor for sedentary students, but was not a stable factor of VO₂max because of lower linearlity for footballers and the significant difference on VO₂max per 1kg of lean body mass between two groups.
