1.A Case of Closure of the Foramen Ovale by Minimally Invasive Cardiac Surgery through Right Thoracotomy for the Treatment of the Platypnea-Orthodeoxia Syndrome Caused by a Patent Foramen Ovale

The patient was a 78-year-old man who presented with dyspnea and decreased SpO₂ in the sitting and standing positions. Transthoracic echocardiography revealed a patent foramen ovale (PFO), and a microbubble test showed Grade 1 in the supine position and Grade 4 in the sitting position, suggesting an increase in shunt volume when seated. Chest CT revealed age-related kyphosis and right atrial compression by the ascending aorta. The patient was diagnosed with platypnea-orthodeoxia syndrome (POS) due to PFO, and a PFO closure was performed using minimally invasive cardiac surgery (MICS). The postoperative course was favorable, and the patient was discharged on the 20th postoperative day. Although percutaneous device closure is generally performed for PFO closure, MICS was useful in this case due to the anatomical unsuitability caused by an atrial septal aneurysm.
2.Therapeutic Impact on Quality of Life in Adult Patients With Chronic Intestinal Pseudo-obstruction: A Multicenter, Cohort Study

Background/Aims:
To evaluate the quality of life (QOL) of patients with chronic intestinal pseudo-obstruction (CIPO) before and after treatment.
Methods:
This study enrolled 50 adult patients diagnosed with CIPO at 4 institutions, of whom 42 underwent therapy.Patient background, body mass index, treatment, palliative care intervention, numerical rating scale for abdominal pain or bloating, the medical outcomes study 36-item short-form health survey (SF-36) and summary scores (physical component summary, mental component summary, and role/social component summary), Japanese version of the Patient Assessment of Constipation Quality of Life (JPAC-QOL) and subscales (physical discomfort, psychosocial discomfort, worries and concerns, and satisfaction), and Patient Assessment of Constipation Symptoms (PAC-SYM) and subscales (stool, rectal, and abdominal symptoms) were prospectively sampled and collected.
Results:
The patients (n = 50; 35 females and 15 males) had an average age of onset of 44.7 years and, body mass index of 17.3 kg/m², and 38 (76%) required palliative care intervention. Effective therapeutic interventions included intestinal sterilization (19/42), decompression therapy with percutaneous endoscopic gastrojejunostomy (PEG-J) (6/42), and dietary restriction (6/42). The numerical rating scale for abdominal distension and pain decreased significantly after treatment. All SF-36 summary scores showed improvements. The JPAC-QOL and PAC-SYM both showed improvement overall and in all subscales.
Conclusions
Patients with CIPO have a lower physical, psychological and social QOL, which is equivalent to or lower than that of adult patients with inflammatory bowel disease or psychiatric disorders. Sterilization of the intestinal tract and PEG-J decompression effectively improve the QOL of patients.
3.Emergency medical team coordination management following the 2024 Vanuatu earthquake

Problem: On 17 December 2024, a magnitude 7.3 earthquake struck the Pacific Island country of Vanuatu, affecting over 80 000 people and causing 14 deaths and 265 injuries. Facing limited specialist clinical capacity, increased patient load and strained facilities, Vanuatu’s Ministry of Health requested support from emergency medical teams.
Context: Vanuatu, highly disaster-prone and geographically dispersed, has a health system constrained by limited human resources and recent cyclone-related damage. The country’s national emergency medical team, the Vanuatu Medical Assistance Team, was established in 2017 and plays a central role in health emergency response in the country.
Action: Within 24 hours of the earthquake, the Vanuatu Medical Assistance Team was activated and established an Emergency Medical Team Coordination Cell within the national health emergency operations centre. National and international emergency medical teams from Australia, Fiji, Indonesia, Japan and New Zealand were deployed over the subsequent weeks and provided clinical, mental health and information management support. The World Health Organization’s emergency medical team Minimum Data Set was implemented to monitor health trends and inform response actions.
Outcome: Eight national and international emergency medical teams deployed alongside local health services. Together, they reported more than 5500 consultations to the Emergency Medical Team Coordination Cell, and a specialized team provided mental health and psychosocial support to 521 people. Minimum Data Set analysis showed a rapid decline in trauma-related presentations and detected increases in influenza-like illness, guiding the transition from emergency to recovery.
Discussion: This response underscored the value of emergency medical teams following disasters, the importance of national leadership and coordination, and the significance of regional solidarity and partnerships. It also highlighted the importance of information management during emergencies and demonstrated potential innovations in this space.
4.Evaluation of simultaneous tandem drainage of intra-abdominal abscesses caused by gastrointestinal anastomotic failure

Background:
This study aimed to demonstrate the effectiveness of a percutaneous method for treating intra-abdominal abscesses caused by gastrointestinal anastomotic leakage using tandem catheter placement for simultaneous drainage.
Methods:
This retrospective study involved 12 patients with intra-abdominal abscesses from anastomotic leakage who underwent image-guided percutaneous drainage. In the tandem drainage technique, one or more catheters were inserted into the abscess while an additional catheter was advanced into the gastrointestinal tract through the anastomotic dehiscence to decompress the tract and prevent further leakage. The study evaluated the technical and clinical success rates of this approach.
Results:
Technical success was achieved in nine of 12 patients (75.0%), with clinical success in eight of these nine patients (88.9%). The mean overall drainage duration after tandem placement was 65.0 days (range, 6–154 days), and the mean hospitalization duration for patients with clinical success was 66.7 days (range, 10–156 days).
Conclusion
Simultaneous tandem drainage is a feasible treatment for intra-abdominal abscesses caused by anastomotic leakage and may be a viable option for patients who are not candidates for surgery.

Postoperative improvements in Knee Injury and Osteoarthritis Outcome Score were evaluated. The patients performed squats under single-fluoroscopic surveillance in the sagittal plane for biomechanical evaluation. A two-dimensional/three-dimensional registration technique was employed to measure the tibiofemoral kinematics. The axial rotation of the femoral component relative to the tibial component and the anteroposterior translation of the medial and lateral femorotibial contact points were measured.
Results:
Group H showed significantly higher pain scores than Group L at 12 and 36 months postoperatively and a significantly higher symptom score at 36 months postoperatively. The kinematic comparison revealed that the axial external rotation in Group L was larger than that in Group H from 70° to 80° with flexion. Moreover, in the medial anteroposterior translation, Group L was more anteriorly located than Group H, with flexion beyond 30°.
Conclusion
The results suggest that a high postoperative nutritional status significantly improved pain and other symptoms and was associated with better knee biomechanics following total knee arthroplasty.
