1.Medial-to-lateral approach for laparoscopic right hepatectomy of the remnant liver: a safe technique for patients with a history of posterior segment resection (with video)

Laparoscopic repeat hepatectomy (LRH) remains challenging when initial resection sites overlap with target areas. We report a safe surgical approach for LRH of the remnant right liver after prior posterior segment resection. An 83-year-old female with recurrent intrahepatic cholangiocarcinoma in segment 5, following segmentectomy 6, required right hepatectomy.Due to dense adhesions in the posterior segment making right lobe mobilization, including dissection around the inferior vena cava (IVC), difficult, we employed a medial-to-lateral approach for LRH. We initially performed parenchymal transection to expose the ventral side of the IVC, followed by a medial-to-lateral dissection around the IVC under a favorable operative field. Operative time was 469 minutes with 160 mL blood loss. The patient was discharged on postoperative day 17 without complications. In LRH after posterior segment resection, a medial-to-lateral approach using the parenchymal transection-first technique is effective for minimizing risks of collateral injury during right hepatectomy.
2.Identifying early risk factors for chronic pain development following vertebral fractures: a single-center prospective cohort study

Methods:
Hospitalized patients with acute VFs underwent assessment of vertebral morphology and paraspinal muscles. Two weeks post-admission, patients were evaluated for pain intensity (using the Verbal Rating Scale [VRS]), pain sensitivity (Pressure Pain Threshold [PPT] and Conditioned Pain Modulation), psychological factors, physical function, and activity levels. At 12 weeks, patients were categorized into CP and non-CP (NCP) groups based on VRS scores. Between-group comparisons and logistic regression analysis were performed to identify predictors of CP development.
Results:
The CP group exhibited significantly lower remote PPT and reduced low-intensity physical activity time, but higher Pain Catastrophizing Scale rumination scores and prolonged 5-Times Sit-to-Stand Test (5SST) compared to the NCP group. Logistic regression identified prolonged 5SST and reduced low-intensity physical activity as independent predictors of CP development.
Conclusions
Prolonged 5SST and reduced low-intensity physical activity may predict CP development after VFs. Early assessment of these factors may facilitate CP risk screening in hospitalized patients with VFs.
3.A Case of the Left Atrial Appendage Perforation with the WATCHMAN Device Implantation Rescued by an Open Heart Surgery

The transcatheter left atrial appendage occluding device, WATCHMAN, can be used to prevent thromboembolism in patients with nonvalvular atrial fibrillation who are at high risk of bleeding during anticoagulation. We report an emergent surgical case of cardiac tamponade due to the left atrial appendage perforation with the WATCHMAN device implantation. An 83-year-old woman with recurrent paroxysmal atrial fibrillation (CHA2DS2-VASc score 4, HAS-BLED score 3) after catheter ablation was scheduled for the WATCHMAN device implantation. During implantation, the patient developed cardiac tamponade and underwent emergent surgery with cardiopulmonary bypass for active bleeding despite pericardial drainage. The bleeding was caused by the left atrial appendage perforation, and suture exclusion of the left atrial appendage was performed.
4.Smaller cross-sectional areas of the hamstring tendon measured from preoperative ultrasonography are likely to need additional gracilis harvesting for double-bundle anterior cruciate ligament reconstructions

Methods:
We enrolled 20 patients (13 men and seven women) who underwent DB-ACLR between October 2017 and March 2019. The mean patient age was 28.5 years. The ipsilateral ST tendon was measured using ultrasonography before surgery. Measurements included the diameter and breadth of the short-axis image. The cross-sectional area (CSA) was calculated from these measurements. During surgery, when two grafts with diameters of ≥ 5.0 mm could not be made, the G tendon was also harvested. Patients were categorized into two groups: the ST group where only the ST tendon was harvested, and the semitendinosus gracilis tendon (STG) group where the ST and G tendons were both harvested. The CSA value was compared between the two groups, and the cutoff value was calculated.
Results:
In the ST group (n = 8), the mean diameter and breadth of the semitendinosus tendon were 4.21 and 2.34 mm, respectively. In the STG group (n = 12), the mean diameter and breadth of the ST tendon were 3.39 and 1.78 mm, respectively.The CSAs calculated for the ST group and the STG group were 7.74 mm 2 and 4.79 mm 2, respectively. A cutoff value of 7.0 mm 2was found to correspond to a specificity and sensitivity to harvest the G tendon of 87.5% and 75.0%, respectively.
Conclusions
The preoperative CSA of the ST tendon determined using ultrasonography can, therefore, be informative for deciding whether to harvest the G tendon for DB-ACLR. The results of this study provide valuable information for graft selection in anterior cruciate ligament reconstruction.Level of Evidence: IV (Retrospective case series design).
5.Survey of Patient Comprehension of Medication Instructions for SGLT2 Inhibitors

Objective: We evaluated patients’ degree of understanding of the effects and adverse drug reactions of SGLT2 inhibitors.Methods: We targeted 26 patients who were administered SGLT2 inhibitors during hospitalizations between April 2017 and March 2018. The survey was conducted by interviewing the patients using a questionnaire.Results: In total, 14 patients (53. 8%) were able to explain the term “efficacy.” Although 6 patients (23. 1%) understood “dehydration,” there was little understanding of “urinary tract infection” (7.7%) and “rash/erythema” (2 and 0 patients, respectively). In addition, we confirmed the details of the descriptions of adverse reactions caused by SGLT2 inhibitors with pharmacists, and found that 13 patients (50.0%) clearly received an explanation of “dehydration,” only 3 patients received an explanation of “urinary tract infection” (11.5%), and none of them comprehended “rash/erythema.” Overall, the patients’ awareness of the adverse drug reactions of SGLT2 inhibitors was low.Conclusion: Unlike common drugs for diabetes, SGLT2 inhibitors have been attracting attention as protective agents of the heart and kidneys. Therefore, it is expected that prescriptions for SGLT2 will increase in the future. Pharmacists need to explain the effects and adverse drug reactions of SGLT2 inhibitors to the patients as well as make the patients understand the pharmacological mechanisms of action of SGLT2 inhibitors.
6.Effect of a combination of astaxanthin supplementation, heat stress, and intermittent reloading on satellite cells during disuse muscle atrophy.

We examined the effect of a combination of astaxanthin (AX) supplementation, repeated heat stress, and intermittent reloading (IR) on satellite cells in unloaded rat soleus muscles. Forty-nine male Wistar rats (8-week-old) were divided into control, hind-limb unweighting (HU), IR during HU, IR with AX supplementation, IR with repeated heat stress (41.0-41.5 °C for 30 min), and IR with AX supplementation and repeated heat stress groups. After the experimental period, the antigravitational soleus muscle was analyzed using an immunohistochemical technique. Our results revealed that the combination of dietary AX supplementation and heat stress resulted in protection against disuse muscle atrophy in the soleus muscle. This protective effect may be partially due to a higher satellite cell number in the atrophied soleus muscle in the IR/AX/heat stress group compared with the numbers found in the other groups. We concluded that the combination treatment with dietary AX supplementation and repeated heat stress attenuates soleus muscle atrophy, in part by increasing the number of satellite cells.

7.Improvement Effects of Dentifrice Containing Plant Extracts on Periodontal Disease

Effects of three plant extracts (Hordeum vulgare L., Apocynum Venetum L., Brasenia schreberi J.F.G mel.) on human gingival fibroblasts were examined. As a result, we observed the promoting effect of the extract of Hordeum vulgare L. and the extract of Apocynum Venetum L. respectively on FGF2 and FGF7 production. Moreover, the mixture of the three plant extracts showed the effect of improving the changes in type I collagen gene expression and matrix metalloproteinase 1 gene expression by LPS addition. Next, a dentifrice containing the three plant extracts was subjected to human efficacy trials. We measured periodontal pocket depth, attachment level, bleeding on probing and saliva TNFα as an indicator of periodontal disease. The results suggest that the dentifrice formulated with the three plant extracts were effective for the improvement of periodontal disease.
8.Factors Relevant to an Independent Resident Group’s Initiated and Continued Attendance in a Preventive Measures Health Program

The purpose of study was to clarify the factors involved in the popularization of a voluntary group in several locations within Shitara Town. A questionnaire survey was administered to participants of a voluntary group concerning its founding, their participation in the group, and continuity within the group. Responses were obtained from 96 participants. Regarding the founding and participation aspects of the voluntary group, participants believed that the presence of others (i.e., the “existence of peers”) had significant effects. They also believed that voluntary participation that was driven by “one’s own health” was more likely to lead to continued participation. It is important to consider regional factors and interpersonal linkages to enhance the relationship between resident-led self-help and mutual assistance measures aimed at improving residents’ health awareness.
9.Effects of Our Locomotive Syndrome Prevention Program

[Purpose] This study was carried out to examine the effects of the locomotive syndrome prevention program implemented by our hospital. [Subjects] Thirty-four individuals (3 males and 31 females; mean age, 72.2±6.9 years) who participated in the exercise class held under the program from 2010 to 2011 were enrolled in the study. [Methods] We examined their bodily functions and the self-assessment checklists for locomotion and exercise habit at the beginning of, at the end of, and six months after the class, and made a comparison between the results obtained on the three occasions. The bodily function examinations included tests of grip strength, 10-m fast-paced walk, tandem gait, 30-sec chair-stand (CS-30), timed “up and go” and one-leg stance with their eyes open. The lower limb length to height ratio was calculated in percentage. [Results] A good deal of improvement was observed in all of the bodily functions tested except for the grip strength. The follow-up survey which was taken six years after the class ended showed a significant improvement in the 10-m fastest walk, lower limb length ratio, and CS-30. The average number of locomotion-syndrome-positive items on the locomotion checklist decreased from 2.5 at the beginning of the exercise class to 1.7 at the end of the exercise class, though the difference was not so significant. The number further decreased six months after the class ended. It was also shown that an increasing number of subjects took exercise more frequently. [Conclusion] The abovementioned results suggested that our program was effective in preventing locomotive syndrome.
10.EFFECT OF HEAT STRESS ON DESMIN EXPRESSION IN ATROPHIED SOLEUS MUSCLE

In the present study, we investigated the effect of heat stress on disuse atrophy from changes in the muscle protein levels of desmin and calpain. Wistar strain female rats (6-8 months old) were randomly assigned to two experimental groups: control (C) and heat stress (H). One hindlimb of all animals was immobilized in plantar flexion with plaster. Before immobilization, animals in H group were placed in a heat chamber (42°C for 60 min). Following 6 days of immobilization, the soleus muscles were removed and analyzed. Although immobilization resulted in significant muscle atrophy in all experimental animals, the soleus weight-to-body weight ratio in immobilized limbs of H group was significantly higher compared to that of C group. Expression of desmin and HSP72 in the atrophied soleus muscle from C group was significantly lower compared with the contralateral muscle; but this was not the case in H group. Further, in C group, the ratio of autolyzed calpains I increased significantly in the atrophied muscle compared to the contralateral muscle. These results show that the effect of heat stress on disuse skeletal muscle atrophy is attributed to the decreasing degradation of desmin by suppressing the activation of calpain.
