1.Research advances in mitochondrial dysfunction in the pathogenesis of hepatic fibrosis

Hepatic fibrosis refers to excessive accumulation and abnormal proliferation of fibrous connective tissue in the liver triggered by multiple pathogenic factors, and it may progress to liver cirrhosis, portal hypertension, and liver cancer. The pathological mechanisms of hepatic fibrosis involve hepatocyte injury, inflammatory cell infiltration with the release of inflammatory mediators, hepatic stellate cell activation, and extracellular matrix deposition. Recent studies have focused on mitochondrial dysfunction in disease progression, including the molecular pathways for hepatic fibrosis driven by metabolic disorders, energy deficiency, oxidative stress, mitochondrial dynamic imbalance, and autophagic dysfunction, all of which can induce liver injury. This article reviews the latest advances in hepatic fibrosis, in order to provide new therapeutic strategies for clinical management.
2.Posaconazole for prevention of invasive fungal infections:a rapid health technology assessment

OBJECTIVE To evaluate the efficacy, safety, and cost-effectiveness of posaconazole for the prevention of invasive fungal infections (IFIs). METHODS PubMed, Embase, CNKI, Wanfang data and other Chinese and English databases were searched, as well as the official websites of related health technology assessment (HTA). Systematic review (SR)/meta-analysis, pharmacoeconomic studies, and HTA reports of posaconazole for the prevention of IFIs were collected. The retrieval time limit was from the establishment of the database to November 1, 2025. After screening the literature, extracting the data, and evaluating the quality of the literature, the results of the included studies were descriptively analyzed. RESULTS A total of 45 studies were included, involving 17 SR/meta-analyses, 26 pharmacoeconomic studies, 1 SR/meta-analysis combined with a pharmacoeconomic study, and 1 HTA report. In terms of effectiveness, posaconazole was significantly superior to other antifungal drugs (eg., fluconazole, itraconazole) in reducing the incidence of IFIs, the incidence of invasive aspergillosis, all-cause mortality, and IFI-related mortality ( P <0.05). In terms of safety, posaconazole was significantly superior to other antifungal drugs in the incidence of total adverse reactions ( P <0.05), but there was no significant difference in the incidence of serious adverse reactions, the incidence of gastrointestinal advers e reactions, drug-induced liver injury, and the withdrawal rate due to adverse drug reactions ( P >0.05). In terms of cost-effectiveness, most studies have shown that posaconazole possesses more cost-effectiveness advantages. CONCLUSIONS Posaconazole demonstrates favorable efficacy, safety, and cost-effectiveness in preventing IFIs.
3.Efficacy and safety of cholesterol absorption inhibitors combined with statins in the treatment of dyslipidemia:a network meta-analysis

OBJECTIVE To evaluate the efficacy and safety of cholesterol absorption inhibitors (ezetimibe and hybutimibe) combined with statins for the treatment of dyslipidemia. METHODS Randomized controlled trials (RCTs) about above-mentioned drug combination in patients with dyslipidemia were retrieved from CNKI,PubMed and other Chinese and English databases. The retrieval time limit was from the establishment of the database to October 2025. After screening the literature,extracting the data,and evaluating the quality of the literature,network meta-analysis was performed using R software. RESULTS A total of 36 RCTs were included. Results showed that in reducing low-density lipoprotein cholesterol (LDL-C) and total cholesterol (TC), ezetimibe+statins and hybutimibe+statins were significantly superior to statins+placebo and placebo alone ( P <0.05). The top two interventions according to surface under the cumulative ranking curve (SUCRA) rankings were both ezetimibe+statins and hybutimibe+statins. In terms of triglyceride (TG) lowering, both hybutimibe+statins and ezetimibe+statins were significantly better than statins alone, statins+placebo and placebo alone ( P <0.05), with hybutimibe+statins and ezetimibe+statins ranking top two by SUCRA. For apolipoprotein B (ApoB) reduction, ezetimibe+statins was significantly superior to statins alone and placebo alone ( P <0.05), and the top two SUCRA rankings were ezetim ibe+statins and statins+placebo. In terms of high-density lipoprotein cholesterol (HDL-C) increase, placebo was significantly better than statins alone, hybutimibe+statins, statins+placebo, and ezetimibe+statins ( P <0.05), and the top two SUCRA rankings were placebo alone and statins alone.There was no statistically significant difference in the overall incidence of adverse reactions among the interventions ( P >0.05). CONCLUSIONS Ezetimibe combined with statins shows significant advantages in reducing LDL-C, TC and ApoB, while the hybutimibe combined with statins may have potential advantages in lowering TG; both combination regimens have good safety.
4.Prospective study of association between dietary macronutrients and lung function in school aged children

Objective:
To explore the longitudinal associations between dietary macronutrients and lung function in schoolaged children, so as to provide the nutritional research evidence for promoting children s lung health.
Methods:
In November 2021, two primary schools located in Chengdu, Sichuan Province were selected from the Southwest China Childhood Nutrition and Growth (SCCNG) cohort by a stratified cluster random sampling method, enrolling a total of 1 112 school aged children aged 8 to 13 years. At baseline, the dietary and sociodemographic characteristics of the children were assessed. One year later, the forced vital capacity (FVC) of the children was measured and converted into Z scores (FVC- Z ), while the vital capacity index (VCI) was also calculated. Generalized linear regression analysis was employed to examine the associations between dietary macronutrients and lung function, considering interactions with gender and age, followed by stratified analysis.
Results:
After adjusting for confounding factors, the analysis results of the generalized linear regression model showed that the carbohydrate energy ratio was negatively correlated with FVC- Z ( β =-0.02) and VCI ( β =-0.16), while the fat energy ratio showed a positive correlation with FVC- Z ( β =0.03) and VCI ( β =0.23) ( P <0.05). The protein energy ratio was positively correlated with FVC- Z ( β =0.09) and VCI ( β =0.60) specifically in girls ( P <0.05). Additionally, there was an interaction effect of age on the associations between macronutrients and lung function ( P <0.01); in children aged 8-9 and 10-11, the carbohydrate energy supply ratio was negatively correlated with FVC- Z ( β =-0.04, -0.03) and VCI ( β =-0.29, -0.21), and fat energy supply ratio was positively correlated with FVC- Z ( β =0.07, 0.05) and VCI ( β =0.46, 0.32) ( P <0.05).
Conclusions
There are age and sex differences in the association of dietary macronutrients with lung function, with a low carbohydrate, high fat diet promoting lung function in children. Additionally, protein intake appears to have a positive influence on the lung function of girls. The early school age period may represent a critical window for dietary interventions aimed at promoting lung health.
5.Catalpol regulates expression of lnc-NONHSAT071210 in salivary gland epithelial cells for therapy mechanism of Sj?gren's syndrome model mice

Objective:To observe therapeutic effect of catalpol on Sj?gren's syndrome(SS)model mice and explore further mechanism through vitro experiments.Methods:Eight-week-old NOD mice were given catalpol 100 mg/kg by gavage for 8 weeks.Serum levels of IFN-γ and IL-17 were measured.Pathological of salivary glands were observed.lnc-NONHSAT071210 shRNA trans-fected salivary gland epithelial cell lines were treated with 50 μmol/L catalpol,and expression of lnc-NONHSAT071210 was detected.Cells were intervented by 10 ng/ml IFN-γ,lnc-NONHSAT071210 shRNA and catalpol treatment for 72 h,cell proliferation and expressions of IL-17 and IFN-γ were detected.Results:Compared with control group,expressions of IFN-γ and IL-17 were decreased(P<0.05),pathology of salivary gland showed that infiltration of lymphocytes was reduced and destruction of gland structure was significantly reduced in catalpol group.Compared with IFN-γ intervention group,catalpol treatment significantly increased prolifera-tion of salivary gland epithelial cells,and decreased expressions of IFN-γ and IL-17(P<0.05).After catalpol treatment,expression of lnc-NONHSAT071210 was decreased(P<0.05).Moreover,IFN-γ and IL-17 expressions were decreased by catalpol and lnc-NONH-SAT071210 shRNA co-treatment(P<0.01).Conclusion:Catalpol can inhibit expressions of inflammatory cytokines in serum and infiltration of lymphocyte in salivary gland of SS model mice,and inhibit salivary gland ductal fine epithelial inflammatory response and progression of SS by regulating lnc-NONHSAT071210.
6.Development of evaluation indicators for care ability of caregivers in home-based palliative care based on the Farran caregiver skills mode

Objective:To develop evaluation indicators for care ability of caregivers in home-based palliative care based on the Farran caregiver skills model.Methods:Based on the Farran caregiver skills model, a preliminary evaluation index system was constructed through theoretical analysis, literature review, and semi-structured interviews. From July to October 2024, experts in the field were selected to conduct expert consultations, revise and improve the index system, and determine the weight of the indicators through the analytic hierarchy process.Results:A total of two rounds of expert consultations were conducted. The effective response rates for the first and second rounds of expert consultation questionnaires were 90.00% (18/20) and 100.00% (18/18), respectively. In the two rounds of expert consultations, 15 experts (83.33%) and 3 experts (16.67%) respectively proposed revision suggestions. In the two rounds of consultation, the expert authority coefficients were 0.931 and 0.936, respectively, and the Kendall coefficient of concordance were 0.152~0.283、0.183~0.269 ( P<0.05). The final evaluation indicators for care ability of caregivers in home-based palliative care included four first-level indicators, 12 second-level indicators, and 45 third-level indicators. Conclusions:The evaluation indicators for care ability of caregivers in home-based palliative care developed are highly scientific and reliable, and can provide a reference for the evaluation of family caregiver care ability and the formulation of training programs.
7.Dorzagliatin in the treatment of type 2 diabetes:a rapid health technology assessment

Objective To evaluate the efficacy,safety and cost-effectiveness of dorzagliatin in the treatment of type 2 diabetes mellitus using a rapid health technology assessment,and to provide medical evidence for clinical decision-making.Methods PubMed,Embase,Cochrane Library,CNKI,Wanfang Data,SinoMed and relevant health technology assessment(HTA)websites and databases were searched to collect systematic reviews/Meta-analysis,pharmacoeconomic literature and HTA reports of dorzagliatin in the treatment of type 2 diabetes from inception to July 20,2024.Two reviewers independently screened literature,extracted date and assessed quality.The results were qualitatively described and analyzed.Results A total of 6 studies were included,including 5 systematic reviews/Meta-analysis and a pharmacoeconomic study.In terms of efficacy compared with placebo,dorzagliatin significantly reduced glycated hemoglobin(HbA1c),fasting plasma glucose,2-hour postprandial blood glucose and insulin resistance index(P<0.05),improved HbA1c compliance rate in terms of safety,dorzagliatin,and enhanced homeostasis model assessment-β(P<0.05),significantly increased total cholesterol,triglyceride,body weight and body mass index(P<0.05).In terms of safety,the total incidence of adverse reactions of dorzagliatin was slightly higher than that of the placebo group(P<0.05).However,there was no significant difference in the incidence of severe adverse reactions between the two groups(P>0.05),and there was no significant increase in the incidence of hyperlipidemia and hypoglycemia(P>0.05).In terms of economy,the treatment of dorzagliatin combined with metformin has a long-term cost-utility advantage,with an economic probability of nearly 70%.Conclusion Compared with placebo,dorzagliatin has a good overall efficacy and safety in the treatment of type 2 diabetes.In terms of safety,the total incidence of adverse reactions of dorzagliatin was slightly higher than that of placebo.In terms of economy,compared with metformin alone,dorzagliatin combined with metformin has economic advantages.However,there is a lack of head-to-head comparisons of doxorubicin with other classes of glucose-lowering medications,and none of the included studies described long-term cardiac,cerebral,or renal benefits,which limits the comprehensive evaluation of the efficacy and safety of dorzagliatin.

9.Three cases of pediatric acute leukemia complicated with arterial ischemic stroke and literature review

Objective:To analyze the clinical characteristics of acute leukemia complicated with arterial ischemic stroke (AIS) in children, and to provide a reference for its diagnosis, treatment, and prognosis.Methods:Case summary.This report presents three children with acute leukemia complicated with AIS admitted to the First Affiliated Hospital of Zhengzhou University from April 2015 to August 2024, and reviews the relevant literature at home and abroad to analyze the clinical characteristics, pathogenesis, and treatment of the disease.Results:All three cases were female, aged 4-14 years; two had acute lymphoblastic leukemia (ALL) and one had acute myeloid leukemia (AML). Hemiparesis was the main presenting symptom in all cases, occurring during induction therapy.Symptoms resolved completely after anticoagulant and symptomatic treatment, with no sequelae and good prognoses.A literature search identified 8 reported cases of pediatric acute leukemia complicated with AIS.Combining these with our 3 cases yielded a total of 11 cases: 5 males and 6 females; median age 7 years (range 2-15 years); 8 with ALL and 3 with AML.Clinically, all presented with hemiparesis.Vascular imaging in 6 patients showed involvement of the middle cerebral artery.In 8 cases of ALL complicated with AIS, the event occurred during induction therapy, which was considered associated with the use of Asparaginase and intrathecal Cytarabine.Anticoagulation was the main treatment.Symptoms resolved in 10 cases, 3 had neurologic sequelae, and 1 died.Conclusions:AIS complicating acute leukemia in children is often the first clinical manifestation of hemiparesis, which mainly occurs in the process of induction therapy, and may be related to the adverse reactions of chemotherapy drugs such as hypercoagulable state of the blood caused by mendonuclease and insufficient cerebral perfusion caused by intrathecal injection of Cytarabine, etc.; once hemiplegic neurological symptoms appear in the process of induction therapy of children′s acute leukemia, it is highly suspicious of the concomitant AIS, and earlycranial magnetic resonance examination can help to clarify the diagnosis.Although most symptoms resolve with treatment, some patients may develop neurological sequelae.
10.Study on the Outcomes and Influencing Factors of Internet-based Video Medical Consultation Services

Objective Based on clinical consultation practices,it systematically evaluates the outcomes of remote video consultation services,identifies key factors influencing these outcomes,and provides empirical evidence for optimizing telemedicine services.Methods It recruited 201 patients nationwide requiring video consultations,objectively recorded diagnostic outcomes,and analyzed influencing factors using hierarchical regression.Results Among the 201 participants,62(30.84%)achieved effective diagnosis and treatment,while 139(69.16%)did not meet expected outcomes.Hierarchical regression results revealed that disease category,physician title,availability of auxiliary diagnostic information,and presence of assisting personnel significantly impacted consultation outcomes(P<0.05).Higher physician titles,richer auxiliary diagnostic information provided by patients,and involvement of assisting personnel correlated with better alignment of outcomes with expectations.Conclusion Significant variations in video consultation outcomes were observed,primarily influenced by assisting personnel,physician qualifications,auxiliary diagnostic information and disease category.It is suggested to introduce a large artificial intelligence model for pre-triage,set up standardized video consulting rooms in primary medical and health institutions and configure collaborative personnel,and build a regional patient information sharing platform,so as to improve patient satisfaction during treatment and promote the further development and optimization of video consultation.
