1.Danzhi Jiangtang Capsule improves renal vascular endothelial function in rats with diabetic nephropathy by downregulating the Notch1/NICD/MAML1 signaling pathway

Objective To investigate the therapeutic mechanism of Danzhi Jiangtang Capsule(DZJTC)for repairing renal vascular endothelial injury in rats with diabetic nephropathy(DN).Methods Fifty male SD rat models of DN,established by left nephrectomy,high-sugar and high-fat diet and streptozotocin injection,were randomized into DN model group,low-,medium-,and high-dose DZJTC treatment groups,and DAPT(a γ-secretase inhibitor)treatment group,with 10 rats with normal feeding as the control group.DZJTC was administered by daily gavage at 0.315,0.63,or 1.26 g/kg,and DAPT(20 mg/kg,dissolved in 50%CMC-Na solution)was given by gavage every other day for 4 weeks;normal saline was given in the control and model groups.After treatment,the levels of creatinine(CRE),blood urea nitrogen(BUN),and microalbuminuria(mALB)were detected with ELISA,and renal pathologies were observed by transmission electron microscopy.Renal expressions of vascular endothelial growth factor(VEGF)and endothelin-1(ET-1)were measured by immunohistochemistry,and the protein expressions of CD31 and Notch signaling pathway components were detected using Western blotting.Results The rat models of DN showed significantly increased CRE,BUN,and mALB levels,obvious renal pathologies under electron microscopy,increased renal VEGF,ET-1 and CD31 expressions,and upregulated Notch1,NICD,and MAML1 protein levels.Treatment with DZJTC at the 3 doses and DAPT significantly reduced CRE,BUN,and mALB levels,improved renal pathology,decreased VEGF,ET-1 and CD31 expressions,and lowered Notch1,NICD and MAML1 levels,and the effects were the most pronounced with high-dose DZJTC.Conclusion DZJTC ameliorates hyperproliferation and dysfunction of renal vascular endothelium in DN rats possibly by regulating renal VEGF and ET-1 levels via inhibiting NICD-and MAML1-mediated Notch signaling pathway.
2.Expression of HER2,CA199 and LC3B in gastric cancer tissues and their relationship with recurrence after radical gastrectomy

Objective To investigate the expression of human epidermal growth factor receptor 2(HER2),carbohydrate antigen 199(CA199)and microtubule-associated protein 1 light chain 3β(LC3B)in gastric canc-er tissues and their relationship with recurrence after radical gastrectomy.Methods A total of 95 patients with gastric cancer who underwent radical gastrectomy in the hospital from January 2021 to December 2022 were retrospectively selected.The expressions of HER2,CA199 and LC3B in gastric cancer tissues and adja-cent tissues were compared,and the clinicopathological characteristics were analyzed.According to the postop-erative recurrence of gastric cancer patients,they were divided into recurrence group(n=19)and non-recur-rence group(n=76).Univariate analysis and multivariate Logistic regression analysis were used to analyze the influencing factors of recurrence in patients with gastric cancer after radical resection.Results The ex-pression levels of HER2,CA199 and LC3B in gastric cancer tissues were significantly higher than those in pa-racancerous tissues(P<0.05).The positive expression rates of HER2,CA199 and LC3B in gastric cancer tis-sues(38.95%,38.95%and 35.79%,respectively)were higher than those in adjacent tissues(9.47%,7.37%and 11.58%,respectively),and the differences were statistically significant(P<0.05).The positive expres-sion rate of HER2 in gastric cancer tissues was significantly different in patients with different TNM stage and lymph node metastasis(P<0.05).There were significant differences in the positive expression rates of CA199 and LC3B in gastric cancer patients with different tumor maximum diameter,TNM stage and lymph node metastasis(P<0.05).The maximum diameter of tumor≥5 cm,TNM stage Ⅲ/Ⅳ,lymph node metas-tasis,positive expression of HER2,positive expression of CA199 and positive expression of LC3B were inde-pendent risk factors for recurrence of gastric cancer patients after radical resection(P<0.05).Conclusion The gene expression and positive expression rates of HER2,CA199 and LC3B in gastric cancer tissues are high,which are related to the maximum tumor diameter,TNM stage and lymph node metastasis.The positive expression of HER2,CA199 and LC3B may increase the risk of recurrence of gastric cancer after radical gastrectomy.
3.Research on the current status and influencing factors of the multidimensional symptom network in maintenance hemodialysis patients

Objective To investigate the current symptom status of maintenance hemodialysis(MHD)patients,analyze the influencing factors from the perspective of multidimensional symptom burden,extract symptom groups,and construct a symptom network.Methods A total of 490 patients who received MHD at the blood purification center of a tertiary hospital in Shanghai from March to April 2025 were selected using a convenience sampling method.30 symptoms of MHD patients were investigated using a modified dialysis symptom index table.SPSS and R software were utilized to analyze the influencing factors,extract symptom groups,and construct a multidimensional symptom network.Results A total of 488 valid questionnaires were collected,with a valid questionnaire recovery rate of 99.59%.The median number of symptoms experienced by patients was 6(interquartile range:4,8),with the top 5 symptoms being dry skin(69.88%),fatigue or tiredness(56.97%),itching(56.97%),worry(50.20%),and dry mouth(46.31%).Multidimensional analysis indicated that age,duration of dialysis,education level,and complications were influencing factors of symptom burden scores across different dimensions.6 stable symptom groups were extracted,namely skin discomfort,fatigue and pain,gastrointestinal tract,sleep disorders,water-electrolyte imbalance,and uremia.The results of the symptom network analysis revealed that difficulty of falling asleep had the highest intensity centrality(rs=2.18,2.25,2.12).Conclusion The symptom association is relatively stable,and difficulty of falling asleep is the core symptom.This finding can assist clinical medical staff in achieving efficiency and precision in symptom group management.
4.Study on Functional Substance Basis of Jinhong Tablet Based on GES-1 Cell Model and Mouse Gastric Organoid Model

OBJECTIVE To explore the functional substance basis of Jinhong Tablet in the treatment of chronic superficial gastri-tis(CSG).METHODS Three different models were constructed to investigate the anti-inflammatory and antioxidant effects,func-tional material basis of Jinhong Tablet:inflammatory model in human gastric epithelial cells(GES-1)induced by lipopolysaccharide(LPS),LPS-induced inflammatory model in mouse gastric organoids,and ethanol-induced oxidative damage model in GES-1 cells.MTS assay was performed to detect cell proliferation activity;qPCR was applied to measure the relative mRNA expression of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),and interleukin-8(IL-8)in cells and gastric organoids;and the levels of superoxide dismutase(SOD),malondialdehyde(MDA),and reactive oxygen species(ROS)in cells were detected.RESULTS Jinhong Tablet and 10 functional components significantly reduced the relative expression of inflammation-relat-ed genes TNF-α,IL-1β,IL-6,and IL-8 in LPS-induced GES-1 cells and gastric organoids,suggesting that these 10 components are the functional substance basis for the anti-inflammatory effects of Jin Hong Tablet.Jin Hong Tablet and 11 functional components markedly decreased the levels of MDA and ROS and increased the activity of SOD,indicating that these 11 components were the func-tional substance basis of the antioxidant effects of Jinhong Tablet.CONCLUSION Through in vitro cell and gastric organoid experi-ments,it has been preliminarily determined that allocryptopine,corydaline,dehydrocorydaline,palmatine hydrochloride,chlorogenic acid,costunolide,rutin,quercitrin,dehydrocostus lactone,tetrahydrocoptisine,isochlorogenic acid B,toosendanin,protopine,and quercetin are the functional material basis of Jinhong Tablet in treating CSG,accumulating scientific evidence for the enhancement of the quality standards of Jinhong Tablet.

6.Treatment of Thyroid Nodules,Breast Nodules,and Uterine Fibroids Based on the Theory of "Liver Governing the Free Flow of Qi"

Guided by the theory of "liver governing the free flow of qi", it is believed that liver fail to govern the free flow of qi may lead to qi stagnation, phlegm coagulation, and stasis, which is the core pathogenesis of thyroid nodules, breast nodules and uterine fibroids; qi stagnation, phlegm coagulation, and stasis are not only the important pathological products, but also the obstruction to the liver's function, and the two affect each other as the cause of each other. In clinic, it is advocated that using the treatment method of soothing the liver, rectifying qi, and resolving constraint, with prescription of Tongqi Powder (通气散) as the basic formula, and modified according to symptoms. For liver depression and qi stagnation syndrome, the formula chooses modified Tongqi Powder to soothe the liver and rectify qi; for qi stagnation and phlegm coagulation syndrome, the formula chooses modified Tongqi Powder plus Shenling Baizhu Powder (参苓白术散) to soothe the wood and regulate the earth, and resolve phlegm and dissipate masses; for qi stagnation and blood stasis syndrome, the formula applies modified Tongqi Powder plus Taohong Sizu Decoction (桃红四物汤) to move qi and invigorate blood circulation, unblock the collaterals and dispel accumulation. At the same time, according to the characteristics of thyroid nodules, breast nodules, and uterine fibroids and their different disease locations, medicinals were added or subtracted according to the symptoms, so as to treat both the symptoms and the root cause of the disease simultaneously.
7.Advances in the clinical application of neoadjuvant immunotherapy for resectable locally advanced esophageal squamous cell carcinoma

Esophageal cancer cases in China account for more than 50% of the world, among which approximately 90% are histological subtypes of esophageal squamous cell carcinoma. Over 50% of esophageal cancer patients are initially diagnosed at locally advanced or advanced stages. The R0 resection rate with surgical treatment alone is relatively low, and local recurrence and distant metastasis are prone to occur, resulting in a low 5-year survival rate. Recent research has focused on neoadjuvant therapy for esophageal cancer, but the most effective form of such therapy remains undetermined. Immunotherapy is currently the most active research field in tumor treatment. Further exploration of the treatment model combing immunotherapy with neoadjuvant chemotherapy or chemoradiotherapy is expected to improve the therapeutic effect and survival benefit in patients with locally advanced resectable esophageal squamous cell carcinoma.

10.Current Situation,Existing Problems and Solving Strategies of the Assessment of Medical Ethics and Styles in the Examination of Doctors' Qualification

This paper discussed the current education status on medical ethics and styles and the assessment condition in the examination of doctors' qualification, as well as the existing problems and potential solutions by reviewing domestic and foreign literature and summarizing the practice experience. Traditionally, medical ethics and styles have always been integrated into clinical medical practice in China. However, under the modern medical education system, it is challenged to integrate traditional education on medical ethics and styles with the rules of modern medical knowledge. By summarizing the education and assessment status of medical ethics and styles in the examination of doctors' qualification, it is found that the current examination is relatively poor in the evaluation content, and the way of evaluation is not diverse, with lack of curriculum of medical humanities. The solutions suggested are enriching relevant examination content, introducing more and comprehensive evaluation method, and establishing more medical humanities-related courses.
