OBJECTIVES:
This study aimed to explore the clinical efficacy of severe early childhood caries (SECC) treatment combined with local anesthesia under general anesthesia.
METHODS:
A total of 108 children under 6 years old who underwent SECC dental treatment under general anesthesia at the Department of Pediatric Dentistry, Third Affiliated Hospital of Air Force Medical University from March to December 2023 were selected as the study subjects, with American Society of Anesthesiologists (ASA) classification of classⅠor Ⅱ. The study subjects were divided into a control group (n=54) and an experimental group (n=54) by retrieving intraoperative cases and postoperative follow-up records. The control group was given general anesthesia through inhalation combined with nasotracheal intubation, whereas the experimental group was given local anesthesia with 2% lidocaine on each treated tooth on the basis of general anesthesia. The basic information, preoperative anesthesia depth, hemodynamic changes during different surgical procedures, postoperative pain, and adverse reactions in the two groups were recorded and analyzed.
RESULTS:
No statistically significant difference was found in the basic information and preoperative anesthesia depth between the two groups (P>0.05). Among the three procedures (pulpotomy, root canal treatment, and tooth extraction), the three observed indicators in the experimental group were significantly lower than those in the control group (P<0.05). The proportion of patients in the experimental group who needed to take analgesic measures in accordance with the modified facial pain scale (FPS-R) score was significantly lower than that in the control group at postoperative wakefulness and 2 h after surgery (P<0.05). Meanwhile, no statistically significant difference was observed between the groups at 24 h after surgery (P>0.05). The proportion of patients in the experimental group who needed to take analgesic measures on the basis of the parent posto-perative pain measurement (PPPM) score was significantly lower than that in the control group when they were awake after surgery (P<0.05). No statistically significant difference was found between the groups at 2 and 24 h after surgery (P>0.05). Moreover, no statistically significant difference was observed in the incidence of adverse reactions between the two groups at 24 h after surgery (P>0.05).
CONCLUSIONS
The combination of local anesthesia during SECC dental treatment under general anesthesia results in minimal changes in intraoperative hemodynamics and mild postoperative pain response, hence worthy of clinical promotion.

Objective To investigate the MRI findings of duodenal papillary adenocarcinoma(DPA),and to evaluate the importance of diffusion-weighted imaging(DWI)in diagnosis of DPA.Methods A complete data of 52 patients with DPA were prospectively collected. All patients underwent surgery within 72 hours after conventional MRI,DWI and MRCP scans. Before surgery, four different MRI findings were used to calculate the sensitivity, specificity and the probability of correctness. Two experienced radiologists who were blind to the pathologic diagnosis handled the MRI findings.Thirty eight patients were pathologically diagnosed for DPA.Based on the pathological diagnosis, the detection rate of DPA by the MR sequence was recorded and the chi square test was used to do the statistical analysis. Results The accuracy rate in diagnosis of DPA with MRI was 78.8% in our study. The findings of DPA consist of thickening wall of duodenal, duodenal papilla node imaging,DWI showing high signal of duodenal papilla and"beak"sign of dilated bile duct.Corresponding sensitivities were 70.5%,66.7%,86.3% and 87.9%,and specificities were 75.0%,30.0%,50.0% and 63.1% respectively. The incidences of positive on T2WI and T1WI,MRCP,DWI scans were 60.5%,76.3% and 92.1% respectively. The detection rate of each sequence has significant difference(χ2=10.48,P<0.005).Conclusion The MRI manifestations of DPA consist of thickening wall of duodenal, duodenal papilla node imaging,DWI showing high signal of duodenal papilla and"beak"sign of dilated bile duct. The detection rate of DWI sepuence on DPA lesions is significantly higher than that of other sequences.

Objective To explore the MSCT and MRI characteristics of hydrosalpinx. Methods The CT and MRI findings of 28 patients with hydrosalpinx confirmed by surgery and pathology were retrospectively studied. Before operation, the plain and contrast enhanced CT in 25 patients and the plain and contrast enhanced MRI in 3 had been carried out. Results 1 9 patients with unilateral hydrosalpinx (12 on the left and 7 on the right side)and 9 patients with bilateral hydrosalpinx were found. Both CT and MRI images showed the cystic mass with different forms in annex area, including botuliform changes in 1 6 patients, beaded form in 9,polycystic changes in 8 and retort-like changes in 4.The low density as water on plain CT,and the low signal on T1 WI and high signal on T2 WI were showed. The cystic walls were significantly enhanced without nodules. If there is septum, mild enhancement was also found.14 patients with chronic salpingitis,2 with fascial thickening,3 with uterine fibroid and 1 7 with pelvic fluid. Conclusion CT and MRI findings of hydrosalpinx are characteristic.

Objective To study the effects of lifestyle quantilization based-weight management on overweight or obesity police officers. Methods One hundred and seven overweight or obesity police officers received lifestyle quantilization based-weight management (i.e.Jinbi weight management) and were then assigned to the excellent performance group (group A,n =50),good performance group (group B,n =42)and loss to follow-up group (group C,n =15).Dietary habits, body weight, height, waist circumference (WC),blood pressure (BP),total cholesterol (TC),triglyceride (TG),low-density lipoprotein cholesterol (LDL-C),high-density lipoprotein cholesterol (HDL-C) and fasting plasma glucose (FPG) were measured before and after the intervention. After 8 weeks' intervention, the changes of above parameters were recored.Student's t test was used for data analysis. Results Of group A and B,83 reported weight loss (group A t =13.31,group B t =5.04 ; both P < 0.05).In group A,body weight, body mass index (BMI),WC,body fat and visceral fat index were significantly decreased,in contrast to body water (t values were 13.31,13.72,10.8,8.59,6.83 and-6.62,respectively; all P < 0.05).However,there were no significant changes of BP,FPG,TC,TG,LDL-C and HDL-C in group A.Following intervention, daily dietary energy intake of group A was reduced by 74.1 k J,fat intake was decreased by 11.6 g,energy ratio of dietary fat was decreased by 1.8%,energy ratio of cereal was increased by 4.2%,and sodium chloride and cooking oil was decreased by 1.3 g and 10 g,respectively. Conclusion Lifestyle quantilization based-weight management shows effectiveness among overweight or obese police officers, and thus may be recommended for other functional communities.

Objective To study the safety and efficacy of fast track program in anorectal surgery perioperative period. Methods One hundred and sixty-nine cases of rectal cancer were divided into the study group of 86 patients with fast track program, and the control group of 83 patients with traditional programs by random digits table. Both groups were compared from the time out of bed, the first intestinal discharge time, intravenous fluids stopping time, length of hospital stay, total cost of treatment and the incidence of postoperative complications. Results The study group compared with the control group: the first intestinal discharge time [(33.6 ± 12.9) h vs. (81.7 ± 20.1) h], intravenous fluids stopping time [(4.5 ±1.3) d vs.(7.4 ± 1.6) d],and length of hospital stay [(5.6 ± 1.2) d vs.(8.9 ±2.7) d],the total cost of treatment [(15 000 ± 3000) yuan vs. (16 000 ± 4000) yuan], the differences were statistically significant (P < 0.01 or < 0.05),and had less incidence of postoperative complications in study group than that in control group [5.8% (5/86) vs. 16.9% (14/83)], the difference was statistically significant (P < 0.05).Conclusions Fast track program in anorectal surgery perioperative period is safe and effective, beneficial, conducive to rehabilitation of patients.
