1.Validation of the Chinese version of the DSM-5 Social Anxiety Disorder Severity Scale in adults

Objective:To examine the validity and reliability of the DSM-5 Social Anxiety Disorder Severity Scale(SAD-D)in a Chinese adult population.Methods:The Chinese version of the DSM-5 Social Anxiety Disor-der Severity Scale was administered via online data collection platform Credamo to 300 adults(Sample 1,for item analysis,exploratory factor analysis and item selection of brief version of SAD-D)and 528 adults(Sample 2,for confirmatory factor analysis,criterion validity test,measurement invariance analysis and internal consistency reliabil-ity analysis for both SAD-D and its brief version).Criterion validity was tested with the Social Phobia Scale(SPIN)and Personal Report of Confidence as a Speaker(PRCS).A brief version of the scale was developed by u-sing the Ant Colony Optimization(ACO).A retest was conducted with 152 participants from Sample 2 after three weeks.Results:Exploratory factor analysis indicated that the SAD-D was a unidimensional scale with factor load-ings ranging from 0.49 to 0.82,and the results of the confirmatory factor analysis also supported the unidimension-al structure(x2/df=3.49,RMSEA=0.069,CFI=0.971,TLI=0.962,SRMR=0.028).The scores of Chinese version of the SAD-D were positively correlated with the SPIN scores(ICC=0.70,P<0.001)and PRCS scores(ICC=0.73,P<0.001).The Cronbach'α of the scale was 0.92,and the retest reliability was 0.85.The scale dem-onstrated cross-gender measurement invariance(△CFI<0.01,△RMSEA<0.01).The brief version of the SAD-D was selected as items 2,5,and 6,and its Cronbach'α coefficient was 0.86.Conclusion:The Chinese version of the SAD-D has satisfactoryvalidity andreliability,making it suitable for the assessment of social anxiety symptoms with Chinese adults.
2.Mechanism of A20 in rheumatoid arthritis and progress of Chinese medicine intervention

Rheumatoid arthritis(RA)is an autoimmune disease characterized by chronic,aggressive joint inflammation,which seriously harms mental and physical health of patients.Efficacy of clinical drugs for RA is limited,so it is a hot issue to seek new therapeutic targets and drugs.Zinc finger protein A20,a cytoplasmic ubiquitin modifier,is closely related to negative regulation of nuclear factor-kappaB(NF-κB),an inflammatory signaling pathway,and has anti-inflammatory and anti-apoptotic effects.A20 is involved in pathogenesis of RA and can improve inflammatory response and bone destruction of RA.Treatment of RA by traditional Chinese medicine has unique advantages of multi-target and multi-pathway,and studies have found that traditional Chinese medicine has a regulatory effect on A20.This paper expounds mechanism of A20 in RA and research progress of traditional Chinese medicine intervention in A20,in order to provide references for treatment and drug development of RA.

5.A multi-constraint optimal puncture path planning algorithm for percutaneous interventional radiofrequency thermal fusion of the L5/S1 segments

Objective To minimize variations in treatment outcomes of L5/S1 percutaneous intervertebral radiofrequency thermocoagulation(PIRFT)arising from physician proficiency and achieve precise quantitative risk assessment of the puncture paths.Methods We used a self-developed deep neural network DWT-UNet for automatic segmentation of the magnetic resonance(MR)images of the L5/S1 segments into 7 key structures:L5,S1,Ilium,Disc,N5,Dura mater,and Skin,based on which a needle insertion path planning environment was modeled.Six hard constraints and 6 soft constraints were proposed based on clinical criteria for needle insertion,and the physician's experience was quantified into weights using the analytic hierarchy process and incorporated into the risk function for needle insertion paths to enhance individual case adaptability.By leveraging the proposed skin entry point sampling sub-algorithm and Kambin's triangle projection area sub-algorithm in conjunction with the analytic hierarchy process,and employing various technologies such as ray tracing,CPU multi-threading,and GPU parallel computing,a puncture path was calculated that not only met clinical hard constraints but also optimized the overall soft constraints.Results A surgical team conducted a subjective evaluation of the 21 needle puncture paths planned by the algorithm,and all the paths met the clinical requirements,with 95.24%of them rated excellent or good.Compared with the physician's planning results,the plans generated by the algorithm showed inferior DIlium,DS1,and Depth(P<0.05)but much better DDura,DL5,DN5,and AKambin(P<0.05).In the 21 cases,the planning time of the algorithm averaged 7.97±3.73 s,much shorter than that by the physicians(typically beyond 10 min).Conclusion The multi-constraint optimal puncture path planning algorithm offers an efficient automated solution for PIRFT of the L5/S1 segments with great potentials for clinical application.
6.Study on the Mechanism of Jianpi Yangzheng Formula Regulating Tumor-Associated Macrophage Exosomes to Induce Anoikis in Gastric Cancer Cells

OBJECTIVE To investigate the effect of Jianpi Yangzheng(JPYZ)formula regulating tumor-associated macrophage(TAM)exosomes on anoikis in gastric cancer and its mechanism.METHODS TAM model was established by inducing human mononuclear THP-1 cells in vitro;M0,TAM and TAM+JPYZ formula exosomes were extracted by ultracentrifugation and co-incuba-ted with gastric cancer cells.Flow cytometry was used to detect the effect of exosomes in each group on anoikis in gastric cancer cells.A BALB/c transplanted tumor mouse model was constructed,and the expression level of apoptotic proteins in transplanted tumors was detected by Western blot.Label-free mass spectrometry proteomics and bioinformatics were used to analyze the differential proteins in gastric cancer cells before and after intervention;Western blot and qPCR experiments were used to detect the expression level of dif-ferential protein isocitrate dehydrogenase 1(IDH1),and ubiquitination experiments were used to detect the ubiquitination level of IDH1;kits were used to detect the levels of α-ketoglutarate(α-KG),NADPH/NADP+,glutathione(GSH/GSSG)and reactive oxy-gen species(ROS)content.RESULTS The anoikis rate of gastric cancer cells was reduced after TAM exosomes intervention(P<0.05,P<0.01),while it was significantly increased after TAM+JPYZ exosomes intervention(P<0.05,P<0.01).In the in vivo mouse experiment,the ratio of Cleaved Caspase-3/Caspase-3 in the tumor of the TAM exosome group was reduced(P<0.05),while the ra-tio was significantly increased after TAM+JPYZ exosome intervention(P<0.05).Proteomic analysis showed that IDH1 was significant-ly different after intervention,and was related to tricarboxylic acid cycle metabolism.Compared with the M0 group,the IDH1 ubiquiti-nation level of gastric cancer cells in the TAM group with exosome intervention was increased,the levels of α-KG,NADPH/NADP+and GSH/GSSG were significantly increased,and the ROS content was reduced(P<0.05),while TAM+JPYZ exosomes could reverse the above phenomenon.CONCLUSION JPYZ Formula can regulate TAM exosomes to cause ubiquitin degradation of IDH1 in gas-tric cancer cells,reduce the level of tricarboxylic acid cycle metabolism in gastric cancer cells,promote ROS accumulation,induce anoikis in gastric cancer,and thus inhibit the development of gastric cancer.

8.Correlation between the mutation spectrum of the UGT1A1 gene and clinical phenotype in patients with inherited hyperunconjugated bilirubinemia

Objective:To analyze the distribution characteristics of UGT1A1 mutant genes (including enhancers, promoters, and exons 1-5) and further explore the correlation between UGT1A1 genotype and clinical phenotypes in patients with inherited hyperunconjugated bilirubinemia.Methods:Patients diagnosed with hereditary hyperunconjugated bilirubinemia at Nanjing Second Hospital from June 2015 to December 2022 were retrospectively analyzed. The UGT1A1 gene was examined using Sanger sequencing in all patients. Complete blood count, liver function, and abdominal imaging examinations were performed. Comparison of categorical variable data using χ2 testor Fisher percision tests. Comparison of continaous veriable data with normal distribution using t-test. Results:112 cases (male:female ratio 81:31, aged 9-70 years) had inherited hyperunconjugated bilirubinemia, with a total of 14 mutation sites identified, of which seven were confirmed mutations, and the frequency ranged from high to low: (TA)n accounted for 50%, c.211G>A (p.G71R) accounted for 49.10%, 1456T>G (p.Y486D) accounted for 16.96%, c.686C>A (p.R229W) accounted for 12.5%, 1091C>T (p.P364L) accounted for 8.04%, and c- 3279T>G accounted for 0.982%. Simultaneously, all patients had one to four mutations, of which only one mutation was the most common (55.36%), followed by two mutations (37.5%), and rare three and four mutations (5.36% and 1.78%). There was no statistical significance in total bilirubin (TBil) levels among the four groups ( F=0.652, P=0.583). One mutation was most common in (TA)n and c.211G>A (p.G71R), among which TA6/TA7 ( n=10) and TA7/TA7 ( n=14) mutations were statistically significant in TBil ( t=2.143, P=0.043). The c.211G>A (p.G71R) heterozygous ( n=9) and isolated ( n=15) mutation had no statistical significance in TBil ( t=0.382, P=0.706). The GS group accounted for 75%, the intermediate group accounted for 16.9%, and the CNS-Ⅱ group accounted for 8%. TBil was statistically significant among the three groups ( F=270.992, P<0.001). There was no statistically significant difference ( χ2=3.317, P=0.19) between mutation 1 (44 cases, 14 cases, and 4 cases, respectively) and mutations ≥ 2 (40 cases, 5 cases, and 5 cases, respectively) in the GS group, intermediate group, and CNS-II group. Conclusion:The number of UGT1A1 gene mutation sites may have no synergistic effect on TBil levels in patients with inherited hyperunconjugated bilirubinemia. TA7/TA7 mutations are not uncommon, and TBil levels are relatively high.
9.Mechanism of rosuvastatin attenuating high glucose-induced vascular en-dothelial injury based on bioinformatics

AIM:This study investigated the protective effects and underlying mechanisms of rosuvastatin(RST)in mitigating high glucose(HG)-induced damage in human umbilical vein endothelial cells(HUVECs),comple-mented by bioinformatics analysis.METHODS:Network pharmacology was employed to identify the potential targets and signaling pathways of RST in treating HG-induced vascular endothelial dysfunction.Molecular docking techniques were used to evaluate the binding affinity of RST to these core targets.The HUVECs were cultured in vitro and assigned into control,HG,and HG+RST(0.01,0.1,1,2,5 and 10 μmol/L)groups.Cell viability was determined using the MTS as-say.Levels of lactate dehydrogenase(LDH)and nitric oxide(NO)were quantified using chemical colorimetric assays.The mRNA levels of endothelial nitric oxide synthase(eNOS),claudin-1(CLDN-1),occludin(OCLN),zonula oc-cludens-1(ZO-1),aldose reductase(AR),Janus kinase 2(JAK2),mitogen-activated protein kinase 1(MAPK1),Ras homologous gene family member A(RHOA)and heat shock proteins 90AB1(HSP90AB1)were assessed by RT-qPCR.Western blot analysis was used to evaluate protein levels of eNOS,OCLN and ZO-1.RESULTS:Network pharmacology analysis suggested that RST may improve HG-induced vascular endothelial dysfunction by influencing the chemokine sig-naling pathway,tyrosine metabolism,MAPK signaling pathway,and hypoxia-inducible factor 1 alpha signaling pathway.The MTS assay indicated that RST significantly enhanced cell viability in an HG environment(P<0.01)and reduced HG-induced damage in HUVECs.Compared with the control group,the HG group showed a significant increase in LDH levels(P<0.01)and decreases in NO,eNOS,CLDN-1,OCLN and ZO-1 levels(P<0.05).Additionally,the mRNA levels of AR,JAK2,MAPK1 and RHOA were elevated(P<0.01),and HSP90AB1 was reduced in the HG group(P<0.05).Rel-ative to the HG group,RST treatment significantly decreased LDH levels(P<0.01)and increased the levels of NO,eNOS,CLDN-1,OCLN and ZO-1(P<0.01).Moreover,the mRNA levels of AR,JAK2,MAPK1 and RHOA were re-duced(P<0.01),and HSP90AB1 expression was increased in the HG+RST group(P<0.01).CONCLUSION:RST ef-fectively attenuates HG-induced endothelial injury.This protective effect is potentially mediated by downregulating AR,JAK2,MAPK1,and RHOA expression and upregulating HSP90AB1 expression.
10.Minimally Invasive Transthoracic Intramyocardial Cellular Transplantation Under Echocardiographic Guidance for Myocardial Impairment

Objective::To explore the approach of minimally invasive transthoracic intramyocardial cellular transplantation under echocardiographic guidance to promote ischemic myocardial repair in a preclinical big-animal study.Methods::Female Guangxi Bama miniature pigs (weight: 25–30 kg) were randomly allocated into the sham group, untreated myocardial infarction (MI) group (MI group), the MI and surgical intramyocardial injection (SIM) group (MI-SIM group), and the MI and transthoracic echocardiography-guided percutaneous intramyocardial injection (TTEPIM) group (MI-TTEPIM group) ( n = 4 each) using a lottery method. A swine MI model was established in the 3 groups excluding the sham group, and human induced pluripotent stem cell-derived cardiomyocytes (hiPS-CM) labeled with the herpes simplex virus type-1 thymidine kinase reporter gene (hiPS-CM TK+) were transplanted by SIM in MI-SIM group and TTEPIM in MI-TTEPIM group. The operation time, postoperative recovery time of animals and volume of blood loss were collected for comparison between MI-SIM group and MI-TTEPIM group. 9-(4-[ 18F] fluoro-3-(hydroxymethyl) butyl) guanine positron emission tomography/computed tomography imaging was performed to track the hiPS-CM TK+in vivo. Cardiac function and morphology were evaluated by echocardiography. Results::The operation time and postoperative recovery time of MI-TTEPIM group were significantly shorter than those of MI-SIM group ((28.3 ± 3.6) min vs. (97.0 ± 6.7) min, P < 0.001; (1.3 ± 0.3) d vs. (7.5 ± 0.9) d, P < 0.001). MI-TTEPIM also showed significantly lesser volume of blood loss during cell transplantation than MI-SIM group ((4.3 ± 0.8) mL vs. (47.0 ± 4.1) mL, P < 0.001). The transplanted cells could be traced more accurately in vivo in MI-TTEPIM than in MI-SIM. The circumferential strain of intervention region in the MI-TTEPIM group (–25.07% ± 0.27%) was significantly higher than that of the MI-SIM (–20.39% ± 0.67%) and MI groups (–19.68% ± 0.67%), respectively ( P < 0.01). Conclusion::A minimally invasive TTEPIM protocol with stem cells for treating the ischemic myocardium was established in this study. Transplantation of hiPS-CM TK+ with this method could promote the recovery of the circumferential strain of the ischemic myocardium. The findings of this study lay a foundation for the clinical transformation of this auxiliary means of treatment in the future.
