1.Study on the Improvement of Glucose-Lipid Metabolism in Type 2 Diabetes Mellitus by Compounding Seven-Flower Granules

Objective To investigate the ameliorative effect of Qihuakeli,a Hani formula,on glycolipid metabolism in type 2 diabetes mellitus by in vivo and in vitro experiments.Methods Rat liver mesenchymal stromal cells(BRL-3A)were inoculated in six-well plates and divided into blank,palmitic acid,fenofibrate,and Qihuakeli serum-containing 5.4,10.8,and 21.6 g/kg groups.Except for the blank group,the remaining groups were intervened with 0.2 mmol/L palmitic acid(PA)for 24 hour,and then added with drug-containing serum,and then continued to incubate for 24 hour.The proliferation rate of BRL-3A cells in each group was determined.Total cholesterol(T-CHO),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)concentrations in the supernatant of each cell group were measured,cell culture medium was aspirated and discarded,triglyceride(TG)concentration in the cell lysate.The lipid content of the cells was determined by measuring and staining with red oil.Meanwhile,45 rats were taken and divided into blank group,model group,fenofibrate group(0.225 g/kg),Qihuakeli compound 5.4 g/kg group,and Qihuakeli compound 10.8 g/kg group,the blank group was given normal feed and the rest of the groups were given high-fat feed for 42 day.Beginning on the 43rd day,each group,except the blank group,was injected with a single intraperitoneal injection of Starting from the 43rd day,except the blank group,each group was given a one-time intraperitoneal injection of 0.25%streptozotocin(STZ)solution,and at the same time,the corresponding drugs were given by gavage for 14 day.The rats'weight gain and liver index were measured.Serum fasting blood glucose(FBG)and fasting insulin(FINS)were detected,and the insulin resistance index(ISI)was calculated.Serum free fatty acid(FFA)levels and tumor necrosis factor-α(TNF-α)in liver tissue were also detected.HE staining was used to detect pathological changes in the pancreas.Pathological changes were observed in the tissues,and islet α and β cell expression was detected by immunohistochemistry.Results Compared to the PA group,the accumulation rate of BRL-3A cells was significantly higher(P<0.01)in the 10.8 and 21.6 g/kg Qihuakeli-containing serum groups.The levels of T-CHO,LDL-C and TG in the 5.4 and 21.6 g/kg serum groups were significantly lower(P<0.05),and HDL-C levels significantly increased(P<0.05).Oil red staining results showed that lipids in the cytoplasm of the 5.4,10.8 and 21.6 g/kg.Qihuacel-containing groups significantly reduced.Compared to the model group,the body weight of the 10.8 g/kg group containing Qihuakeli granules increased significantly(P<0.05).The liver index of the 5.4 g/kg group containing Qihuakeli decreased significantly(P<0.05).The serum indices of FBG,FINS,FFA and insulin resistance of the 5 g/kg group containing Qihuakeli decreased significantly(P<0.05).In the 5.4,10.8 g/kg groups,all serum FBG,FINS,FFA and insulin resistance indices significantly reduced in the 5.4 and 10.8 g/kg Qihuakeli groups(P<0.05 or P<0.01).TNF-α levels were significantly reduced(P<0.01).HE staining showed that a small number of lymphocytes were scattered in the pancreatic ducts and perivascular area of the rats in the Qihuakeli 5.4 and 10.8 g/kg groups,the local vasodilatation was observed,the number of pancreatic islet cells and the area of islet cells significantly increased.Immunohistochemical study was further used.The results of immunohistochemistry showed that the area of pancreatic islet α-cells significantly reduced and the area of pancreatic islet β-cells significantly increased in Qihuakeli 5.4 and 10.8 g/kg groups.Conclusion Qihuakeli compound improved glucose-lipid metabolism in T2DM,probably by improving the function of pancreatic islet cells,increasing the sensitivity of insulin to blood glucose,improving insulin resistance,decreasing the secretion of insulin and glucagon,and thus lowering the level of fasting blood glucose.Meanwhile,by decreasing the content of TNF-α,inhibiting lipolysis in the body,and promoting the uptake of FFA by adipocytes,and further lowering the FFA.Thus,it regulates the levels of TG,T-CHO,HDL-C and LDL-C,improves the abnormalities of glucose and lipid metabolism,and alleviates T2DM.
2.Observation on the therapeutic effect of snake-like moxibustion combined with aconitum ointment in treating ankylosing spondylitis and its influence on ESR and serum inflammatory factors

Objective:To observe the clinical efficacy of snake-like moxibustion combined with aconitum ointment based on routine Western medication in the treatment of ankylosing spondylitis(AS)of cold-dampness blockage pattern,and to evaluate its effects on traditional Chinese medicine(TCM)symptoms,serum Dickkopf-1 protein(DKK-1)level,and disease activity.Methods:A total of 120 AS patients were selected as research subjects and randomly divided into an observation group and a control group according to the random number table method,with 60 cases in each group.The control group was treated with oral sulfasalazine enteric-coated tablets as conventional therapy;the observation group was additionally treated with snake-like moxibustion combined with external application of aconitum ointment based on the intervention in the control group.After 4 weeks of consecutive treatment in both groups,the total effective rate was compared.Changes in TCM symptom scores,spinal pain visual analog scale(VAS)score,overall evaluation VAS score,Bath ankylosing spondylitis disease activity index(BASDAI)score,Bath ankylosing spondylitis functional index(BASFI)score,erythrocyte sedimentation rate(ESR),and serum levels of C-reactive protein(CRP),tumor necrosis factor(TNF)-α,and DKK-1 were observed.Results:The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of TCM symptoms,spinal pain VAS,overall evaluation VAS,BASDAI,and BASFI,as well as ESR,serum levels of CRP,TNF-α,and DKK-1 in both groups were lower than those before treatment,with statistically significant differences within groups(P<0.05);all indicators in the observation group were lower than those in the control group,with statistically significant differences between groups(P<0.05).Conclusion:Based on routine Western medication treatment,the addition of snake-like moxibustion combined with external application of aconitum ointment yields better therapeutic efficacy than Western medication alone in the treatment of AS.It can significantly improve clinical symptoms,control inflammation,and down-regulate the serum DKK-1 level.
3.Application of Modified Percutaneous Endoscopic Gastrostomy in Patients With Amyotrophic Lateral Sclerosis

Objective To evaluate the value of modified percutaneous endoscopic gastrostomy(PEG)for patients with amyotrophic lateral sclerosis(ALS)and dysphagia.Methods From January 2018 to December 2023,47 patients with ALS were treated with modified PEG in our hospital.The surgery was performed under local anesthesia.The patients were placed in a semi-seated position,and the surgeon performed surgery with an ultra-fine endoscope through oral entrance to the stomach.Results The modified PEG were successfully completed in all the 47 cases.The operation time was5-20 min[mean,(10.3±1.7)min].The postoperative hospitalization lasted for2-8 d(mean,2.8 d).Postoperative complications occurred in6 cases(12.8%),including aspiration pneumonia in4 cases(8.5%)and incision infection in2 cases(4.3%).The body mass index was17.06±0.89 before PEG and 18.15±0.81 at 3 months after PEG,with significant difference(t=-10.373,P=0.000).Conclusion For ALS patients with dysphagia,modified PEG is feasible,which significantly improves nutritional status of patients.
4.Safety of percutaneous endoscopic gastrostomy in older patients with amyotrophic lateral sclerosis

Objective:A retrospective study was conducted to investigate the safety of Percutaneous Endoscopic Gastrostomy(PEG)in elderly patients with Amyotrophic Lateral Sclerosis(ALS)and to identify the factors influencing major complications.The aim is to establish a more robust foundation for optimizing both the procedure and its timing, thereby enhancing the evidence base for refining the surgical approach.Methods:We retrospectively collected clinical data from elderly ALS patients(≥60 years old)who underwent PEG at Peking University Third Hospital between January 2006 and January 2024.This dataset includes information on general health conditions, the progression of ALS, comorbidities, surgical details, and related complications.Additionally, we analyzed the postoperative complications experienced by the included patients, focusing specifically on the risk factors associated with aspiration pneumonia.Results:A total of 140 patients were included in the study, with a mean age of 68.2±5.9 years.Among these, 69 were male(49.3%)with a mean age of 68.4±6.5 years, and 71 were female(50.7%)with a mean age of 67.7±6.0 years.Successful outcomes were observed in 139 cases(99.3%).One case was not completed due to respiratory arrest caused by intraoperative aspiration.Postoperative complications occurred in 36 cases(25.7%), which included 9 cases of wound infection(6.4%), 1 case of abdominal infection(0.7%), 21 cases of aspiration pneumonia(15.0%), 1 case of local fistula leakage(0.7%), 2 cases of transient fever(1.4%), and 2 cases of death during hospitalization(1.4%).Both univariate and multivariate analyses indicated that an onset duration of ≤1 year( P=0.020)and a half-sitting position( P=0.022)significantly influenced the occurrence of aspiration pneumonia, acting as protective factors( β<0, OR<1). Conclusions:PEG is a safe method for providing enteral nutrition to elderly patients with ALS.While most complications associated with the procedure are mild and can be managed, Aspiration pneumonia remains a common and serious complication.However, early surgical intervention and the use of a semi-seated position during the procedure can help reduce the incidence of aspiration pneumonia.
5.Event-related potential assessment of cognitive function in children with obstructive sleep-disordered breathing

Objective:To assess cognitive impairment in children with obstructive sleep-disordered breathing (OSDB) using event-related potentials (ERPs).Methods:This case-control study analyzed data from 143 OSDB children[94 males, 49 females, aged 9.0(7.0-11.0) years] scheduled for adenotonsillectomy at the Department of Otolaryngology-Head and Neck Surgery, Beijing Tsinghua Changgung Hospital, Tsinghua University, between June 2023 and September 2024, along with 17 healthy controls [control group: 10 males, 7 females, aged 10.0 (7.5-12.0) years]. Based on polysomnography results, OSDB children were divided into a mild group [obstructive apnea-hypopnea index (OAHI)≤5 events/hour, 49 males, 29 females, aged 9.0 (7.0-10.0) years] and a moderate-to-severe group [OAHI>5 events/hour, 45 males, 20 females, aged 9.0 (8.0-10.0) years]. All children completed a face perception integration task. The occipital P100 and parietal, central and frontal P300 components of incomplete face stimuli (S1) and complete face stimuli (S2) were recorded. Amplitude and latency differences across groups were analyzed. Intergroup comparisons were performed using ANOVA, while independent samples t-tests were used for pairwise comparisons. Non-normally distributed data were analyzed using the Mann-Whitney U test. Results:(1) P100: Both the mild group [occipital P100 amplitude: O1-S1(12.44±5.96) μV, O2-S1(14.19±6.39) μV, O2-S2(30.34±11.30) μV] and moderate-to-severe group [O1-S1 (12.12±5.58) μV, O2-S1 (14.08±5.48) μV, O2-S2(29.12±10.89) μV] showed significantly higher amplitudes than the control group [O1-S1(8.46±4.74) μV,O2-S1(9.68±3.70) μV,O2-S2(23.09±9.16) μV] ( F=3.501, 4.486, 3.072; all P<0.05). No significant differences were found between the two OSDB subgroups ( P>0.05), suggesting compensatory neuronal hyperactivity maintaining normal perceptual function. The moderate-to-severe group exhibited significantly prolonged P100 latency [O2-S1 (134.52±13.42) ms] compared to controls [O2-S1 (125.18±15.31) ms] ( F=3.156 , P<0.05), while no significant difference was observed between the mild group and either the control or moderate-to-severe groups ( P>0.05), indicating delayed visual processing in severely affected children. (2) P300: The mild group exhibited significantly higher P300 amplitudes in parietal regions [P4-S1(8.22±4.32) μV, P4-S2(17.67±9.42) μV] compared to controls [P4-S1 (4.84±2.89) μV, P4-S2 (13.19±7.23) μV] ( F=7.19, 4.771; both P<0.05), whereas no significant differences were observed between the moderate-to-severe group and either the control or mild groups ( P>0.05), indicating mild group reduced alertness. The latency of P300 in the central region showed an increase in the mild group, although not significantly ( P>0.05), indicating a potential decrease in attentional response speed. However, the moderate-to-severe group demonstrated significantly shorter P300 latencies [CZ-S1(394.18±89.12) ms] compared to the mild group [CZ-S1 (433.33±100.33) ms] ( F=3.145, P<0.05), possibly reflecting compensatory enhancement of attentional engagement in more severe cases. Conclusion:Children with OSDB exhibit impairments in primary visual processing and attentional regulation, as evidenced by altered ERP components such as P100 and P300. These findings suggest that OSDB may affect neural mechanisms underlying sensory integration and executive functioning.
6.Status quo and influencing factors of fertility information need of breast cancer patients at childbearing age

Objective:To explore the status quo of fertility information need of breast cancer patients at childbearing age, and analyze its influencing factors.Methods:From April to October 2023, 400 cases of breast cancer patients of childbearing age in two Class Ⅲ Grade A hospitals in Zhengzhou City were selected by convenience sampling. Patents were investigated with the General Information Questionnaire and the Fertility Information Needs Assessment Questionnaire for Breast Cancer Patients of Childbearing Age. Multiple linear regression was used to analyze the influencing factors of fertility information need of breast cancer patients of childbearing age. A total of 400 questionnaires were distributed, and 385 valid questionnaires were collected, with an effective response rate of 96.25% (385/400) .Results:The 385 breast cancer patients of childbearing age had a high level of need for fertility information with an average score of items (3.17±0.73). The scores for each dimension in descending order of the mean score of items were disease impact information, treatment impact information, psychosocial information, fertility management information, sexual health information, and fertility protection information. Age, marital status, education level, per capita monthly family income and duration of illness were the factors influencing the fertility information need ( P<0.05) . Conclusions:Breast cancer patients of childbearing age have a high level of need for fertility information, and their need is influenced by a number of factors. Medical and nursing staff should improve the fertility information support program based on the real needs of breast cancer patients, and provide personalized fertility information support for breast cancer patients of childbearing age with different characteristics.

A total of 400 questionnaires were distributed, and 385 valid questionnaires were collected, with an effective response rate of 96.25% (385/400) .Results:The 385 breast cancer patients of childbearing age had a high level of need for fertility information with an average score of items (3.17±0.73). The scores for each dimension in descending order of the mean score of items were disease impact information, treatment impact information, psychosocial information, fertility management information, sexual health information, and fertility protection information. Age, marital status, education level, per capita monthly family income and duration of illness were the factors influencing the fertility information need ( P<0.05) . Conclusions:Breast cancer patients of childbearing age have a high level of need for fertility information, and their need is influenced by a number of factors. Medical and nursing staff should improve the fertility information support program based on the real needs of breast cancer patients, and provide personalized fertility information support for breast cancer patients of childbearing age with different characteristics.
